ยาที่กระตุ้นให้เกิดรอยโรคคล้ายไลเคน พลานัส, กอบกาญจน์ ทองประสม, กนิษฐ์ วงศ์รุ่งโรจน์กิจ, มนวิภา ศรีวงศ์จรรยา

ไลเคน พลานัส เป็นรอยโรคเรื้อรังที่ยังไม่ทราบสาเหตุแน่นอน การใช้ยารักษาโรคทางระบบหลายชนิด สามารถกระตุ้นให้เกิดรอยโรคในช่องปากและผิวหนัง ซึ่งมีลักษณะทางคลินิกและทางพยาธิวิทยาคล้ายไลเคน พลานัสได้ จากการศึกษาผู้ป่วยไลเคน พลานัส ในจํานวน 81 ราย โดยแบ่งผู้ป่วยออกเป็น 2 กลุ่ม คือ กลุ่มที่ได้รับยาและไม่ได้รับยารักษาโรคทางระบบ พบว่าผู้ป่วยส่วนใหญ่เป็นเพศหญิง ช่วงอายุที่พบบ่อยประมาณ 41-60 ปี บริเวณที่ตรวจพบรอยโรคมากที่สุดคือบริเวณกระพุ้งแก้ม รองลงมาคือ เหงือกกับบริเวณรอยต่อระหว่างกระพุ้งแก้มกับเหงือกริมฝีปาก เพดานปาก ลิ้น และพื้นช่องปากตามลําดับ จากการเปรียบเทียบกลุ่มผู้ป่วยที่ได้รับยาและไม่ได้รับยารักษาโรค พบว่าผู้ป่วยที่ใช้ยารักษาโรคมีความสัมพันธ์กับชนิดของการเกิดไลเคน พลานัสในช่องปากอย่างมีนัยสําคัญทางสถิติ (P<0.025) คือผู้ป่วยที่ได้รับยารักษาโรคจะมีโอกาสเป็น ไลเคน พลานัส ชนิดอีโรซีฟ มากกว่าชนิดไม่อีโรซีฟ โดยพบว่าผู้ป่วยมักใช้ยากลุ่ม antihypertensive มากที่สุด รองลงมาคือ NSAIDS, hypoglycemic และยาอื่น ๆ ตามลําดับ

จากการศึกษาต่าง ๆ ที่ผ่านมาพบว่า ปริมาณและระยะเวลาที่น้ำตาลตกค้างในช่องปากมีความสัมพันธ์กับอุบัติการณ์การเกิดโรคฟันผุ การศึกษาส่วนใหญ่กระทําโดยการหาปริมาณน้ำตาลในน้ำลายภายหลังการอมสาร ละลายกลูโคสแล้วบ้วนทิ้งด้วยวิธีเทียบสีซึ่งเป็นวิธีที่ยอมรับโดยทั่วไปว่ามีความถูกต้อง แม่นยําสูง อย่างไรก็ตามวิธี เทียบสีจําเป็นต้องใช้เครื่องมือยุ่งยาก คือ เครื่องเทียบสีและต้องมีวัสดุอุปกรณ์อื่นอีกหลายอย่าง ทําให้ไม่สะดวกในการนําไปศึกษาภาคสนาม การทดลองครั้งนี้จึงนําแผ่นพลาสติกตรวจน้ำตาลซึ่งสามารถบอกปริมาณวิเคราะห์ของน้ำตาลในน้ำลายได้ โดยสังเกตการเปลี่ยนสีด้วยตาเปล่ามาทดลองใช้ จากการหาปริมาณกลูโคสในน้ำลายที่เวลา 3,5,7,9 และ 11 นาที หลังการอมสารละลายกลูโคสความเข้มข้น 50% นาน 2 นาที พบว่า ปริมาณน้ำตาลในน้ำลายที่ตรวจวิเคราะห์ได้ด้วยแผ่นพลาสติกตรวจน้ำตาลไม่แตกต่างจากปริมาณที่วิเคราะห์ได้ด้วยวิธีเทียบสีอย่างมีนัยสําคัญทางสถิติ (P = 0.05) จากข้อมูลครั้งนี้กล่าวได้ว่า แผ่นพลาสติกตรวจน้ำตาลใช้แทนวิธีเทียบสีได้ สามารถนําไปใช้ศึกษาการ กําจัดน้ำตาลจากน้ำลายภาคสนามเป็นประโยชน์ในการพยากรณ์อุบัติการณ์การเกิดโรคฟันผุในชุมชน และการให้ทันตกรรมป้องกันต่อไป

การศึกษานี้มีวัตถุประสงค์ เพื่อสํารวจความพึงพอใจของประชาชนที่มีต่อการบริการทันตกรรมของโรงพยาบาลระยอง และต้องการทราบข้อมูลพื้นฐานสําหรับเปิดคลินิคผู้ป่วยทันตกรรมนอกเวลาราชการของโรงพยาบาลระยอง โดยเลือกกลุ่มตัวอย่างที่มารับบริการและประชาชนทั่วไป โดยใช้แบบสอบถาม ถามกลุ่มตัวอย่าง จํานวน 336 ราย ในช่วงระยะเวลา มกราคม-พฤษภาคม 2537 ผลการศึกษาพบว่ากลุ่มตัวอย่างมีทัศนคติในแง่บวก ต่อการบริการทันตกรรมของโรงพยาบาลระยองสูงถึง ร้อยละ 81.3 ซึ่งกลุ่มตัวอย่างประมาณครึ่งหนึ่งเคยไปใช้บริการทั้งภาครัฐและเอกชน เมื่อศึกษาหัวข้อของความพึงพอใจของกลุ่มตัวอย่างที่มีต่อการบริการทันตกรรมของโรงพยาบาลระยอง ส่วนใหญ่มีความพึงพอใจระดับดี แต่มีข้อที่ความพึงพอใจระดับต่ำ คือ ความยุ่งยากในขั้นตอนของการมารับบริการ และเวลาที่สูญเสียในการรอรับบริการ เมื่อเปรียบเทียบคะแนนความพึงพอใจของปัจจัย ที่มีระดับนัยสําคัญ (alpha = 0.05) คือ กลุ่มตัวอย่างมีระดับการศึกษาน้อย มีรายได้ต่ำ นอกจากนี้ยังพบว่า กลุ่มตัวอย่างที่มีรายได้ต่ำจะเลือก เข้ารับบริการของรัฐมากกว่า และกลุ่มตัวอย่างที่มีรายได้สูง จะเลือกรับบริการทั้งภาครัฐและเอกชน ผลจากการศึกษาครั้งนี้ สามารถนํามาเป็นแนวทางที่จะนํามาเปิดการบริการทันตกรรมนอกเวลาของโรงพยาบาลระยองได้ ผู้ที่คาดว่าจะมาใช้บริการส่วนใหญ่เป็นข้าราชการเพศหญิง

ถุงน้ำเดนเจอรัสเป็นถุงน้ำที่มีจุดกําเนิดที่เกี่ยวกับการเกิดของฟันโดยลักษณะของถุงน้ำจะคลุมอยู่รอบตัวฟันที่ยังไม่ขึ้น หรือในฟันที่กําลังจะโผล่ขึ้นมาในช่องปาก โดยที่ตัวฟันนั้นเจริญเติบโตเต็มที่เรียบร้อยแล้ว บทความนี้เป็นรายงานผู้ป่วย 2 ราย ที่มีถุงน้ำขนาดใหญ่ โดยผู้ป่วยรายแรกมีถุงน้ำเดนเจอรัสที่เกิดจากฟันเกินเบียดลึกเข้าไปในโพรงอากาศแม็กซิลล่า ข้างขวาบริเวณฟันกรามน้อยซี่ที่หนึ่งทําให้ใบหน้า 2 ข้างไม่เท่ากัน ส่วนผู้ป่วยรายที่ 2 มีถุงน้ำเดนติเจอรัสขนาดใหญ่ที่เกิดจากฟันคุดกรามล่างซี่สุดท้ายในกระดูกขากรรไกรล่างข้างขวาทําให้มีอาการชาบริเวณริมฝีปากล่างข้างขวา นอกจากนั้นยังได้รายงานถึงลักษณะอาการทางคลีนิก ลักษณะภาพถ่ายรังสี ลักษณะทางจุลพยาธิวิทยา การรักษาและการพยากรณ์ของถุงน้ำชนิดนี้ด้วย

ศึกษาผลของยาไอบูโปรเฟน 800 มิลลิกรัม เปรียบเทียบกับยาพาราเซทตามอล 1000 มิลลิกรัม เกี่ยวกับ การระงับอาการปวด บวม และการอ้าปากจํากัดภายหลังการผ่าตัดฟันคุดของฟันกรามล่างซี่ที่สาม โดยทําการวิจัยในผู้ป่วย 26 ราย เป็นชาย 7 คน หญิง 19 คน อายุอยู่ระหว่าง 17 - 21 ปี แบ่งออกเป็น 2 กลุ่ม ในกลุ่มที่หนึ่ง ผู้ป่วย 12 คน ได้รับยาไอบูโปรเฟน มีระดับความยากในการถอนฟันเท่ากับ 1.94 ± 0.50 ใช้เวลาในการถอนฟัน เฉลี่ย 17.66 ± 4.80 นาที ในกลุ่มที่สอง ผู้ป่วย 14 คน ได้รับยาพาราเซทตามอล มีระดับความยากของการ ถอนฟันเท่ากับ 1.67 ± 0.41 ใช้เวลาในการถอนฟันเฉลี่ย 15.5 ± 5.06 นาที ระดับความยาก และระยะเวลา เฉลี่ยที่ใช้ในการถอนฟันทั้ง 2 กลุ่ม ไม่แตกต่างกันอย่างมีนัยสําคัญทางสถิติ (P > 0.01) จากผลการวิจัยพบว่า อาการบวมและการอ้าปากจํากัดของทั้ง 2 กลุ่ม ไม่มีความแตกต่างกันอย่างมีนัยสําคัญทางสถิติ (P > 0.01) แต่พบว่ายาไอบูโปรเฟน สามารถลดอาการปวดภายใน 24 ชั่วโมง หลังการผ่าตัด ได้ดีกว่ายาพาราเซทตามอลอย่างมีนัยสําคัญทางสถิติ (P < 0.01) ไม่พบอาการแทรกซ้อนของยาเกิดขึ้นตลอดการวิจัย

The aerospace, energy, and chemical industries have benefitted from favorable applications of titanium and titanium alloys since the 1950's. Only about 15 years ago, researchers began investigating titanium as a material with the potential for various uses in the dental field, mainly because of its proven biocompatibility and other attractive properties. Mycobacterium avium complex (MAC) causes serious opportunistic infections in AIDS patients. Previous studies with MAC-infected beige mice have indicated that weekly administration of liposome-encapsulated streptomycin can reduce significantly the CFU in the liver and spleen. We examined whether streptomycin encapsulated in recently developed sterically stabilized liposomes with prolonged circulation times would have a therapeutic effect in this animal model. Two liposome types with prolonged circulation (polyethyleneglycol-distearoylphosphatidylethanolamine [PEG-DSPE]- distearoylphosphatidylcholine [DSPC]-cholesterol [chol] or phosphatidylinositol [PI]-DSPC-chol) and conventional liposomes (phosphatidylglycerol [PG]-phosphatidylcholine [PC]-chol) encapsulating streptomycin and administered twice weekly were bactericidal to MAC strain 101 in the spleen when the level of infection after …

Purpose:
To investigate the effect of a baking soda-hydrogen peroxide (0.75%) dentifrice on wound healing, plaque formation, gingival inflammation, wound healing, patient comfort, probing depth, and clinical attachment level following gingival flap surgery.
Materials and Methods:
A randomized, double-blind crossover study involving 25 patients requiring bilateral maxillary gingival flap surgery was completed. The effects of twice daily brushing with a baking soda-hydrogen peroxide dentifrice (Mentadent) or a placebo dentifrice (Crest) were observed over a 28-day post-surgical period. A murine monoclonal antibody (MAb F-91-55) raised against the complex of soluble CD4 and human immunodeficiency virus type 1 (HIV-1) gp120 had previously been found to inhibit syncytium formation without inhibiting the interaction of CD4 with gp120, and its binding site was localized within the first two domains (D1/D2) of CD4. We investigated whether this antibody inhibited the infectivity of HIV-1 in the CD4+ T cell lines A3.01, Sup-T1 and H9. We also examined the effect of the antibody on syncytium formation between these cells and chronically infected H9 cells. Syncytium formation was found to depend critically on the incubation …

Destructive Hostility: The Jeffrey Dahmer Case: A Psychiatric And Forensic Study Of A Serial Killer, Jeffrey Jentzen, George Palermo, L. Thomas Johnson, Khang-Cheng Ho, K. Alan Stormo, John Teggatz

We were involved as forensic experts in the case of the serial killer Jeffrey Dahmer. We discuss the scene and victim autopsy findings, with a brief consideration of the basic emotion of hostility. These findings support the thesis that at the basis of this serial killer's behavior were primary unconscious feelings of hate that he had channeled into a sadistic programmed destruction of 17 young men. The interview of the serial killer, the photographic scene documentation, and the autopsy findings stress the ambivalent homosexuality of the killer, his sexual sadism, his obsessive fetishism, and his possible cannibalism and necrophilia.

A Pilot Study Comparing The Forces Applied : Using A Chin Cup And Three Types Of Protraction Headgear, Daniel Miles Beisiegel
Loma Linda University Electronic Theses, Dissertations & Projects
A pilot study comparing force distributions of a chin cup, the Great Lakes reverse pull headgear (RPHG), Ormco's adjustable dynamic RPHG, and Grummons RPHG was done. A skull from a 69 year old female which demonstrated a Class III malocclusion was used to perform the tests of force. Forces applied to and generated on the skull were measured using a water-filled balloon system. This method was tested and found to be reliable and accurate.

การศึกษานี้มีวัตถุประสงค์เพื่อปรับปรุงวิธีการตรวจเชื้อจากคลองรากฟันให้มีประสิทธิภาพมากขึ้นกว่าวิธีเดิมที่ปฏิบัติอยู่ในคณะทันตแพทยศาสตร์ มหาวิทยาลัยมหิดล โดยศึกษาการตรวจเชื้อจากคลองรากฟันซึ่งมีพยาธิสภาพที่ตรวจได้จากภาพถ่ายรังสีขนาดเล็กกว่าหรือเท่ากับ 4 มิลลิเมตรกลุ่มหนึ่ง และอีกกลุ่มหนึ่งพยาธิสภาพมีขนาดมากกว่า 4 มิลลิเมตร การตรวจหาเชื้อจะเก็บเชื้อจากคลองรากฟันที่มีการประเมินทางคลินิกแล้วว่าพร้อมที่จะทําการอุดได้ โดยแบ่งเป็น 3 วิธี วิธีที่ 1 เป็นวิธีที่ปฏิบัติอยู่คือ ใช้แท่งกระดาษซับปลอดเชื้อเก็บเชื้อในคลองรากแล้วนําไปเพาะเลี้ยงเชื้อในบรรยากาศปรกติ วิธีที่ 2 คือใช้กระบอกฉีดทูเบอร์คูลินดูดและฉีดน้ำเกลือปริมาตร 0.1 มิลลิลิตร ประมาณ 5 ครั้ง จากนั้นดูดน้ําเกลือกลับมา 0.05 มิลลิลิตร เพื่อนําไปเพาะเลี้ยงเชื้อ วิธีที่ 3 จะใช้แท่งกระดาษซับปลอดเชื้อซับน้ำเกลือที่เหลือในคลองรากจนแห้ง แล้วนําไปเพาะเลี้ยงเชื้อ วิธีที่ 2 และวิธีที่ 3 จะนํามาเพาะเลี้ยงเชื้อ ในบรรยากาศไร้ออกซิเจน จากการศึกษาพบว่าการตรวจเชื้อโดยวิธีที่ 3 มีแนวโน้มที่จะมีความไวสูงกว่าวิธีที่ 1 และ 2 แต่ไม่พบความแตกต่างอย่างมีนัยสําคัญทางสถิติ จึงควรได้มีการศึกษาเพิ่มเติมเพื่อเป็นประโยชน์สําหรับการตรวจเชื้อในคลองรากฟันต่อไป

บทรายงานผู้ป่วยโรค เมลิออยโดสิส (Melioidosis) ซึ่งแสดงภาวะในรูปลักษณะของกระดูกขากรรไกรล่างอักเสบเฉียบพลัน (Acute Osteomyelitis of mandible) รายแรกในโรงพยาบาลมหาราชนครราชสีมา และยังไม่เคยพบรายงานของโรคนี้ที่เกิดขึ้นในช่องปากและกระดูกขากรรไกรในประเทศไทยเลย ผู้ป่วยได้รับการวินิจฉัยได้ถูกต้องรวดเร็วและสามารถให้การรักษาอย่างเร่งด่วน แม้ว่าความรุนแรงของสภาวะการติดเชื้อของโรคจะทําลาย กระดูกขากรรไกรล่างไปอย่างรวดเร็วก็ตาม รายงานนี้มีวัตถุประสงค์เพื่อเผยแพร่เป็นตัวอย่างให้ผู้ประกอบวิชาชีพทันตกรรมพึงสังวรณ์ระวังอาการ ที่ไม่พึงประสงค์หรืออาการแทรกซ้อน ทําให้เกิดอาการร้ายแรงเกินกว่าจะคาดฝันได้
