The perception of temporal speech cues, lexical knowledge, and their interactions were examined in children (6;0-9;6) with specific language impairment (SLI). An identification task was used to test four 12-step speech continua: word-word (FEET—FEED), nonword-nonword (ZEST—ZEED), word-nonword (CHEAT—CHEED) and nonword-word (REAT—READ). The stimuli were naturally recorded and digitally edited. The vowel steady state, which varied in duration from 110 to 350 milliseconds in 20-millisecond steps, was the acoustic cue to the voicing characteristic of the final consonant in each stimulus. The analyses revealed that both the TLD and SLI groups used vowel duration as a perceptual cue. For the word-word …

The concept of self-determination or "acting as the primary casual agent in one's life and making choices and decisions regarding one's quality of life free from undue external influence or interference" (Wehmeyer, 1996, p.24) has long been a focus of educating adults with disabilities. Recently, researchers and educators have begun to examine teaching foundational skills of self-determination to young children with disabilities. It is important to evaluate programs and methodologies that are said to enhance self-determination in order to ensure their efficacy. Background: Communication about end of life, including advance care planning, life-sustaining therapies, palliative care, and end-of-life options, is critical for the clinical management of amyotrophic lateral sclerosis patients. The empirical evidence base for this communication has not been systematically examined.
Objective: To support evidence-based communication guidance by (1) analyzing the scope and nature of research on health communication about end of life for amyotrophic lateral sclerosis; and (2) summarizing resultant recommendations.
Methods: A scoping review of empirical literature was conducted following recommended practices. Fifteen health-related and three legal databases were searched; 296 articles were screened for inclusion/exclusion criteria; and quantitative …

Spectral feature computations continue to be a very difficult problem for accurate machine recognition of speech. In this work, which focuses on vowels, a new spectral peak envelope method for vowel classification is developed, based on a missing frequency components model of speech recognition. According to the missing frequency components model, vowel recognition depends only on the spectral (harmonic) peaks. Smoothing and interpolation of the spectra, performed in the standard cepstral analysis method commonly used in automatic speech recognition, actually loses valuable information and results in reduced recognition accuracy. The purpose of the present study was to compare feature retention patterns between children developing speech typically (TD) and children with phonological impairment (PI) and to discuss these findings in terms of characteristics, severity, and implication for the identification of developmental apraxia of speech (DAS). A second purpose was to determine if a relationship exists between phonological knowledge and feature retention.

This study consisted of a PI group and a TD group of children, ages four to six. A 245-item speech sample was collected from each subject. Feature retention percentages as well as percent correct underlying representation (PCUR) were calculated …

The aim of the study was to determine if Transient Evoked Otoacoustic Emissions (TEOAEs) can be used as a method of Screening for hearing impairment and to study the effects of External and Middle ear conditions on the outcome of TEOAEs. TEOAE and Tympanometry tests were performed on 193 infants (386 ears). External and Middle ear abnormalities were evaluated by ear examinations and Tympanometry, and assigned to two groups based on the tympanogram results. Speech intelligibility is the term used to describe the degree to which an individual speaker is understood by both familiar and unfamiliar listeners. There are many factors that influence an individual's intelligibility, including (but not limited to) variations in phonology, grammar, rate of speech, and vocal features. Most research has focused on the articulation and phonological influences that impact speech intelligibility. Perhaps for this reason, treatment methods for improving speech intelligibility generally have targeted an individual's phonological production skills. Studies of early-developing consonants (stops, nasals, and glides) in babbling have shown that most of the variance in consonants and their associated vowels, both within and between syllables, is due to a "frame" produced by mandibular oscillation, with very little active contribution from intrasyllabic or intersyllabic tongue movements. In a study of four babbling infants, the prediction that this apparently basic "frame dominance" would also apply to late-developing consonants (fricatives, affricates, and liquids) was tested. With minor exceptions, confirming evidence for both the predicted intrasyllabic and intersyllabic patterns was obtained. Results provide further evidence for the frame dominance conception, but …

Because there are many variables and much missing information when fitting young children with hearing instruments, it is necessary to rely on prescriptive procedures such as the Desired Sensation Level (DSL) Method to create target 2cc gain values for ordering and fitting hearing instruments. Inherent in creating 2cc targets is a conversion process known as a real-ear to coupler difference (RECD) measurement. The DSL algorithms make allowances for averaged data to be used in lieu of this measurement.

Earobics Pro PLUS Step I (Earobics I) is a software program designed to increase the auditory processing and phonological awareness skills of children when used in conjunction with traditional intervention. Phonological awareness is the ability to manipulate the sounds of a language and has been shown in previous studies to have a strong relationship with reading ability. Phonological awareness can be taught, and studies have been conducted in an effort to determine effective methods for teaching phonological awareness. With the loss of fluent speech, one may also lose the capacity to share in the casual conversation that enriches our lives. A rehabilitative course that seeks to stimulate restoration of verbal abilities can be lengthy with uncertain outcomes. Fortunately, Augmentative and Alternative Communication (AAC) interventions have shown promise in treating aphasia by providing non-verbal means of communicating. The present study investigated the nature of conversational discourse data at the point of breakdown in specific neurologically compromised patients. Data were obtained during the observation and video recording of informal conversational situations (i.e., clinical settings or the patient’s natural environment). Three adult patients who had suffered neurological damage, at the site of the basal ganglia were assessed. Due to the inappropriateness of two of the patients, a case study was conducted on the remaining patient. Brief bursts of third-octave bands of noise (center frequencies at 0.5, 1.0, 2.0 and 4.0 kHz) and band pass noises with different degrees of low-frequency content (0.5 to 4.0 kHz, 1.0 to 4.0 kHz and 2.0 to 4.0 kHz) were recorded binaurally from 17 different horizontal locations (90 degrees on the left to 90 degrees on the right in 11.25 degree steps) one meter from the ears of an anthropomorphic mannequin (KEMAR) in an anechoic room and a reverberant room. In this research, the variability of Discrete Cosine Transform Coefficient (DCTC) features was investigated. Additionally, a new pitch-synchronous processing method was explored to increase the stability of features and to reduce window effects when compared to the regular method. The noise sources that lead to feature variability were analyzed, and different smoothing methods were tested. It was found that longer frames, frequency warping, time smoothing of the log spectrum, and DCS level time smoothing, all help reduce DCTC variability and increase classification performance.
