When language abilities in aphasia are assessed in clinical and research settings, the standard practice is to examine each language of a multilingual person separately. But many multilingual individuals, with and without aphasia, mix their languages regularly when they communicate with other speakers who share their languages. We applied a novel approach to scoring language production of a multilingual person with aphasia. Studies on the efficacy of language treatment for multilingual people with post-stroke aphasia and its generalization to untreated languages have produced mixed results. We conducted a systematic review and a meta- analysis to examine within- and cross-language treatment effects and the variables that affect them. We searched PubMed, PsycINFO, CINAHL, and Google Scholar (February 2020; January 2023), identifying 40 studies reporting on 1573 effect sizes from 85 individuals. We synthesized effect sizes for treatment outcomes using a multi-level model to correct for multiple observations from the same individuals. Bilingualism has been linked with improved function regarding certain aspects of linguistic processing, e.g., novel word acquisition and learning unfamiliar sound patterns. Two non mutually-exclusive approaches might explain these results. One is related to executive function, speculating that more effective learning is achieved through actively choosing relevant information while inhibiting potentially interfering information. While still controversial, executive function enhancements attributed to bilingual experience have been reported for decades. The other approach, understudied to date, emphasizes the role of sensory mechanisms, specifically auditory sensory memory. Recent neurolinguistic theories converge on the hypothesis that the languages of multilingual people are processed as one system in the brain. One system for the multiple languages is also at the core of a translanguaging framework of multilingualism—a framework that focuses on each speaker’s complete linguistic repertoire rather than on the separate languages they know. However, evidence from neuroimaging studies suggests at least some nonoverlapping activations of the first-acquired language (L1) and other (non-L1) languages of multilingual people, especially when the age of acquisition and/or levels of proficiency differ across the languages. Aphasia is an acquired language disorder resulting from damage to the brain, most typically a stroke in the left cerebral hemisphere. The degree and type of impairment vary depending on the site and size of the lesion, and the extent to which people with aphasia recover – with time and therapeutic intervention – their pre-stroke linguistic skills varies, depending on exogenous and endogenous variables. In multilingual people, semantic knowledge is predominantly shared across languages.
Providing semantic-focused treatment to people with aphasia has been posited to strengthen
connectivity within association cortices that subserve semantic knowledge. In multilingual people, such treatment should result in within- and cross-language generalisation to all languages, although not equally. We investigated treatment effects in two multilingual participants with aphasia who received verb-based semantic treatment in two pre-stroke highly
proficient languages. We compared within- and cross-language generalisation patterns across languages, finding within- and cross-language generalisation after treatment in the less-impaired, pre-morbidly more-proficient first-acquired language (L1). We summarize recent published work concerning assessment and treatment of aphasia in bilingual and multilingual people and review current related models of treatment outcomes. As well, we discuss studies that address the recently debated topic of cognitive processes in bilingual individuals with aphasia, with a focus on the effects of bilingualism on aphasia recovery and its potential protective effects.

Background: Despite substantial literature exploring language treatment effects in multilingual people with aphasia (PWA), inconsistent results reported across studies make it difficult to draw firm conclusions. Methods: We highlight and illustrate variables that have been implicated in effecting cross-language treatment effects in multilingual PWA. Background: Articulatory excursion and vocal intensity are reduced in many children with dysarthria due to cerebral palsy (CP), contributing to the children’s intelligibility deficits and negatively affecting their social participation. However, the effects of speech-treatment strategies for improving intelligibility in this population are understudied, especially for children who speak languages other than English. Objectives: To examine maturation of neural discriminative responses to an English vowel contrast from infancy to 4 years of age and to determine how biological factors (age and sex) and an experiential factor (amount of Spanish versus English input) modulate neural discrimination of speech.

Design: Event-related potential (ERP) mismatch responses (MMRs) were used as indices of discrimination of the American English vowels [ε] versus [I] in infants and children between 3 months and 47 months of age. The National Aphasia Association defines aphasia as an impairment of language, affecting the production or comprehension of speech and ability to write. This communication disorder is typically acquired after a brain injury resulting from an accident or stroke. Nearly 180,000 Americans acquire aphasia each year. Despite efforts to increase awareness about aphasia, via organizations such as the National Aphasia Association, general knowledge is still limited. In 2000 Elman and colleagues collected data on the number of news articles that mentioned the term “aphasia” and compared it to similar health conditions with comparable or lower prevalence rates. Increasing numbers of Hispanic immigrants are entering the US and learning American–English (AE) as a second–language (L2). Previous studies investigating the relationship between AE and Spanish vowels have revealed an advantage for early L2 learners for their accuracy of L2 vowel perception. Replicating and extending such previous research, this study examined the patterns with which early and late Spanish–English bilingual adults assimilated naturally-produced AE vowels to their native vowel-inventory and the accuracy with which they discriminated the vowels. Twelve early Spanish–English bilingual, 12 late Spanish–English bilingual, and 10 monolingual listeners performed perceptual-assimilation and categorical-discrimination tasks involving AE /i,ɪ,ɛ,ʌ,æ,ɑ,o/. This is the first study to examine the effect of phonetic contexts on children’s lexical tone production. Mandarin tones in disyllabic words produced by forty-four 2- to 6-year-old children and twelve mothers were low-pass filtered to eliminate lexical information. Native Mandarin speaking adults categorized the tones based on the pitch information in the filtered stimuli. All mothers’ tones were categorized with ceiling accuracy. Counter to the findings in most previous studies on children’s tone acquisition and the prevailing assumption in models of speech development that children acquire suprasegmental features much earlier than segmental features, this study found that children as …

The question of interest in this study was whether bilingual individuals show superior executive control compared to monolingual participants. Findings are mixed, with studies showing advantage, disadvantage, or no difference between bilingual and monolingual speakers. In this study, we used different experimental conditions to examine implicit learning, resistance to interference, monitoring, and switching, independently. In addition, we matched our monolingual and bilingual participants on baseline response time. Bilingual participants demonstrated faster implicit learning, greater resistance to interference, more efficient switching compared to monolingual participants. Functionally distinct dorsal and ventral auditory pathways for sound localization (WHERE) and sound object recognition (WHAT) have been described in non-human primates. A handful of studies have explored differential processing within these streams in humans, with highly inconsistent findings. Stimuli employed have included simple tones, noise bursts, and speech sounds, with simulated left–right spatial manipulations, and in some cases participants were not required to actively discriminate the stimuli. The general goal of this project is to study the processes and outcomes of speech perception training in postlingually deafened adults fitted with cochlear implants. As part of this work we need to measure speech perception performance, using materials that place different relative emphases on the several components of the speech perception process.
