Purpose

To evaluate the safety and tolerability and exploratory efficacy of OTO-413, an intratympanic-administered, sustained release formulation of brain-derived neurotrophic factor (BDNF) in a thermoreversible gel, in participants with speech-in-noise hearing difficulties.

Methods

This was a single dose, dose-ascending, randomized, double-blind, Placebo-controlled Phase 1/2 study conducted at 7 enrolling clinical sites in the U.S. 110 participants (56% female) enrolled had self-reported speech-in-noise (SIN) difficulties that were confirmed by a SIN test and were randomized to OTO-413 or Placebo. OTO-413 dose range was escalated from 0.01 mg to 1.5 mg within 7 Cohorts. Safety evaluations included monitoring for treatment-emergent adverse events, …

Introduction

Running speech sampling is an essential component of a paediatric voice evaluation, in that it should provide the examiner with a representative vocal sample of the child's everyday voice use outside of the clinic setting. Current speech sampling practices, consisting of reading tasks, informal conversation sampling and the voice question ('Tell me about your voice problem'), may not elicit consistent samples or samples of sufficient length to allow the examiner to make a reliable and valid judgement about vocal quality. Background

Spatiotemporal mapping of neural activity during continuous speech production has been traditionally approached using correlation coefficient (CC) analysis between cortical signals and speech recordings. A prior study employed this approach using electrocorticography (ECoG) data from participants who underwent invasive intracranial monitoring for epilepsy. However, CC cannot detect nonlinear relationships and is dominated by the correspondence between periods of silence and of non-silence.

New Method

We introduce the mutual information (MI) measure, which can capture both linear and nonlinear dependencies. We validated CC and MI on the sub-second spatiotemporal brain activity recorded during continuous speech tasks. To refine the results, …

Although research supports the feasibility and reliability of health-related physical fitness (HRPF) tests in typically developing children and adolescents, little is known regarding the feasibility and reliability of these tests for those with hearing impairments (HI). The aim of this study was to evaluate the feasibility and reliability of a HRPF test battery for children and adolescents with HI. Individuals using augmentative and alternative communication (AAC) are supported by many AAC stakeholders and communication partners. Clinical decision making and AAC assessment models consider the capabilities and access needs of the individual using AAC, as well as opportunity barriers and supports imposed by communication partners and environments. The purpose of this research was to investigate opportunity barriers and supports identified by two critical AAC stakeholders: speechlanguage pathologists and caregivers. The first study investigated characteristics of AAC service provision reported by post-professional speech-language pathologists. The concept of accessible reading for deaf students is new and worthy of exploration. In the face of the reading difficulties often experienced by deaf students, the lack of a specialized reading methodology that works for them must be addressed. Central to the paper is a research case study undertaken with two young deaf students, proficient in American Sign Language (ASL) and learning to read. The students participated in a tutorial with a tutor knowledgeable in a specialized reading methodology called ASL Gloss. The participating students demonstrated progress in reading skills over time. Purpose: Listening in noise challenges listeners with auditory comprehension impairments in aphasia. We examined the effects of Trivia Game, a computerized program with questions spoken in increasing levels of background noise with success in the game.

Methods: We piloted Trivia Game in four individuals with chronic aphasia and mild auditory comprehension impairments. Participants played Trivia Game for 12 twenty-minute sessions. In addition to the Western Aphasia Battery (WAB), we measured outcomes on Quick Speech in Noise (QSIN), a sentence repetition test, administered in auditory (AUD) and auditory+visual (AV) conditions as signal-to-noise ratio varied from 25 to 0 dB.

How the human brain plans, executes, and monitors continuous and fluent speech has remained largely elusive. For example, previous research has defined the cortical locations most important for different aspects of speech function, but has not yet yielded a definition of the temporal progression of involvement of those locations as speech progresses either overtly or covertly. In this paper, we uncovered the spatio-temporal evolution of neuronal population-level activity related to continuous overt speech, and identified those locations that shared activity characteristics across overt and covert speech. The purpose of this study was to create a "behavioral treatment engine" for future use in research on physiological adjuvants in aphasia rehabilitation. We chose the behavioral target anomia, which is a feature displayed by many persons who have aphasia. Auditory processing of speech is influenced by internal (i.e., attention, working memory) and external factors (i.e., background noise, visual information). This study examined the interplay among these factors in individuals with and without ADHD. All participants completed a listening in noise task, two working memory capacity tasks, and two short-term memory tasks. The listening in noise task had both an auditory and an audiovisual condition. Participants included 38 young adults between the ages of 18-35 without ADHD and 25 young adults between the ages of 18-35 with ADHD. Children with profound congenital hearing loss often do not have the same prelinguistic opportunities for social and verbal interaction as their peers with typical hearing [14]. Consequently, language and social skills may be challenging for this group, even after they are provided with amplification or a cochlear implant. This pilot study examined the effectiveness of using a parallel talk intervention to increase the language and interactional skills of three preschoolers with deafnesss. Links between verbs and gesture knowledge suggest that verb retrieval may be particularly amenable to gesture+verbal training (GVT) in aphasia compared to noun retrieval. This study examines effects of GVT for noun and verb retrieval in nine individuals with aphasia subsequent to left hemisphere stroke. Participants presented an array of noun and verb retrieval deficits, including impairments of semantic and/or phonologic processing. In a single-participant experimental design, we investigated effects of GVT for noun and verb retrieval in two counterbalanced treatment phases. Word-retrieval treatment studies in aphasia have reported the greatest influences on picture naming for trained words. To increase treatment effects to untrained words and sentence contexts, we investigated a sentence-reading treatment hierarchy that moves from errorless to generative production of sentences incorporating target nouns and verbs. In an individual with nonfluent aphasia, treatment resulted in improved picture naming for nouns and verbs and generalized increases in numbers of grammatical sentences and content words following noun therapy. A second individual with fluent aphasia improved little in picture-naming and sentence-generation tasks for both nouns and verbs. This sentence-based word-retrieval training, in which …

The purpose of this study was to further the research of human figure drawings as useful assessment tools and add to the understanding of children’s human figure drawings from a cross-cultural perspective.

In this quantitative study, 147 human figure drawings made by 59 hearing and 64 hearing-impaired Taiwanese children and 24 hearing-impaired American children were collected. Several improvements in the vowel articulation training aid (VATA) are described, as well as the efforts to extend the visual feedback system to operate with short words in the form of consonant, vowel and consonant (CVC). The extended version of the visual feedback system is referred to as CATA (Computer-based Articulation Training Aid); the vowel version of the aid (VATA) only operates with ten American English monopthong vowels. This thesis presents an extension of the work previously done on speaker identification using Binary Pair Partitioned (BPP) neural networks. In the previous work, a separate network was used for each pair of speakers in the speaker population. Although the basic BPP approach did perform well and had a simple underlying algorithm, it had the obvious disadvantage of requiring an extremely large number of networks for speaker identification with large speaker populations. This thesis presents a pitch detection algorithm that is extremely robust for both high quality and telephone speech. The kernel method for this algorithm is the Normalized Cross Correlation (NCCF) reported by David Talkin [16]. Major innovations include: processing of the original acoustic signal and a nonlinearly processed version of the signal to partially restore very weak F0 components; intelligent peak picking to select multiple F0 candidates and assign merit factors; and, incorporation of highly robust pitch contours obtained from smoothed versions of low frequency portions of spectrograms. Spectral feature computations continue to be a very difficult problem for accurate machine recognition of speech. In this work, which focuses on vowels, a new spectral peak envelope method for vowel classification is developed, based on a missing frequency components model of speech recognition. According to the missing frequency components model, vowel recognition depends only on the spectral (harmonic) peaks. Smoothing and interpolation of the spectra, performed in the standard cepstral analysis method commonly used in automatic speech recognition, actually loses valuable information and results in reduced recognition accuracy. In this research, the variability of Discrete Cosine Transform Coefficient (DCTC) features was investigated. Additionally, a new pitch-synchronous processing method was explored to increase the stability of features and to reduce window effects when compared to the regular method. The noise sources that lead to feature variability were analyzed, and different smoothing methods were tested. It was found that longer frames, frequency warping, time smoothing of the log spectrum, and DCS level time smoothing, all help reduce DCTC variability and increase classification performance. A real-time visual articulation training aid has been implemented. It provides instantaneous visual feedback of vowel and stop-consonant production on a computer screen. The vowel training system corresponds to an improved floating-point implementation of a previous fixed-point system developed by Beck (1992). The new implementation provides better accuracy and an approximate five-fold increase in speed. Acoustic features computed from global short-time spectral shape are used for classification of vowels. A method is presented for the application of binary-pair partitioned neural networks to the task of speaker verification. This technique is based on a previously developed neural network classifier for speaker identification.

The main focus of this research was the development and testing of the algorithms necessary to extend the binary-pair partitioning approach from speaker identification to speaker verification. The method is based on the development of a user profile which is obtained from discriminative data provided by the binary-pair partitioned neural networks.

Formants are the natural frequencies of the human vocal tract. Existing methods for estimating formants from speech signals are computationally complex and subject to errors for certain type of speech sounds. This thesis describes a method for estimating vowel formant frequencies from Discrete Cosine Transform Coefficients (DCTC's), a form of cepstral coefficients, using a feedforward neural network with back-propagation training. Experimental results are based on a large multispeaker data base. The results are obtained for both a linear transformation and a feedforward neural network with a nonlinear hidden layer. A computer-based vowel articulation training aid has been developed. A "continuous" acoustic-phonetic transformation is performed to map speech parameters to a lower dimensionality display space. There are two possible approaches to this transformation problem. The transformation could be either linear or a combination nonlinear/linear. The nonlinear transformation is performed using a multi-layered feedforward neural network with linear output layers. Speech parameters are extracted either from an analog filter bank arrangement (band energies) or by a digital signal processing procedure (Discrete Cosine Transform Coefficients). The quality of synthetic speech from Linear Predictive (LP) vocoders is known to be degraded due to the lack of detail in the commonly used pulse/noise excitation model. In this investigation, it was hypothesized that this degradation is due to the lack of precise timing information in the pulses and to the constraint that each short-time segment of excitation be either an impulse train or white noise. This transformation is done in real-time, and the resulting color parameter are continuously displayed on a color monitor. A Karhunen-Loeve series expansion was used to block encode speech spectral principal components as a function of time. Each of ten principal components was first obtained as a linear combination of 2© speech spectral band energies. Using a fixed block length of 10 frames (0.128 s), the K-L basis vectors were computed separately for various speakers for each principal component. In all cases the resulting basis vectors were essentially a set of discrete cosine basis vectors. Synthesis of speech from the block encoded parameters showed that very little information is lost with up to 70% data reduction. The block encoding …
