The reported work is part of a 3 phased effort to identify human factors issues in the development of aviation technical manuals, and make recommendations for the improvement of those documents. Phase 1 of this research effort surveyed the procedures used by five manufacturers to develop maintenance documentation. Several human factors issues were identified in the development process employed by these manufacturers. They included the reactive rather than proactive use of user evaluations, the limited use of user input and procedure validation, no systematic attempts to track error, and the lack of standards for measuring document quality. This report contains the results from Phase 1 of a three-phase research effort. Phase 1 examines aviation industry procedures for developing maintenance technical data. Phase 2 will document user problems with maintenance technical data. Phase 3 will identify maintenance technical data development improvements by applying human factors principles. Five aircraft manufacturers were surveyed regarding company policy, communication, data tracking, user feedback, and error reduction efforts. The five industry participants represent both regional and large commercial transport manufacturers. Aircraft accidents resulting from attempted visual flight rules (VFR) flight into instrument meteorological conditions (IMC) continue to be one of the leading causes of fatalities in aviation. The aim of this study is to determine the role which two decision biases— optimistic bias and ability bias—may play in pilots’ decisions and perceptions of risk regarding VFR flight into IMC. A questionnaire designed to test for the presence of these biases was administered to 57 pilots enrolled in a university flight-training program and 103 general aviation pilots attending two FAA aviation safety seminars. The purpose of this research was to identify current disagreement among acquisition professionals in regards to their role and authority; and to identify benefits and risks associated with combining program management and contracting officer roles into a single position. The research questions were answered through a literature review and use of the Delphi technique. The literature review indicated that the DoD has been operating with multiple conflicting perceptions of authority among its key acquisition professionals. The Human Factors Analysis and Classification System (HFACS) is a general human error framework originally developed and tested within the U.S. military as a tool for investigating and analyzing the human causes of aviation accidents. Based upon Reason’s (1990) model of latent and active failures, HFACS addresses human error at all levels of the system, including the condition of aircrew and organizational factors. Forty years ago, the Office of the Secretary of Defense proposed using the Leontief input-output model to assess tradeoffs in the Department of Defense's (DoD) budget. We demonstrate that the Leontief input-output model can assess tradeoffs in the Air Force's budget. To increase one part of the Air Force's budget, we need to know the interrelationships between that budget area and the other areas. In this research, we look at different methods of how the functional areas might interact. We demonstrate our methodology on two data sets - DoD and the Air Force aggregate budget data. Maintaining safe flight operations, military and civilian, in cold weather requires the use of de-icing fluids. Glycols, the primary component of de-icers, have been extensively studied. Water and additives such as corrosion inhibitors and other performance enhancing chemicals make up the remaining portion of de-icing fluids. Little research has been conducted on the additives, but several are suspected toxins to microorganisms, therefore the EPA regulates the disposal of spent de-icing fluids. Tolyltriazole, a corrosion inhibitor found in de-icing fluids, is a suspected human carcinogen. It is also known to be moderately toxic to microorganisms based on Microtox results. The purpose …

In the past few years, Army aviation accidents have been on the rise, due largely to increases in mission frequency and complexity, and diminishing resources. The magnitude of the resulting losses (casualties, dollars, equipment) has prompted the Commanding General of the Army Safety Center to demand a complete examination of the way safety hazards and subsequent safety controls are evaluated and selected. This project integrates value focused thinking, Monte Carlo simulation, and integer programming in response to this demand by developing and using a methodology that effectively identifies and evaluates portfolios of controls. Human error has been implicated in 70 to 80% of all civil and military aviation accidents. Yet, most accident reporting systems are not designed around any theoretical framework of human error. As a result, most accident databases are not conducive to a traditional human error analysis, making the identification of intervention strategies onerous. What is required is a general human error framework around which new investigative methods can be designed and existing accident databases restructured. The researcher collected and analyzed data on the subject of safety seats for children and infants onboard commercial aircraft.
