Several FOQA parameters that could enhance air carrier safety have not been recorded and/or analyzed. Airlines have traditionally chosen a limited number of parameters for analysis (thousands have been available for recording). No study in the U.S. had addressed FOQA parameters, leading to an exploratory mixed methods study. A conference call comprising FOQA industry experts was planned and completed to (a increase researcher knowledge and (b) discuss a list of improved safety FOQA parameters. A questionnaire, derived from the suggested list, was electronically sent to 40 FOQA professionals. The respondents represented 75% of the airlines with an FAA-approved FOQA program. …

Although effective, the United States Army's Shadow 200 Unmanned Aerial Vehicle has suffered an unacceptably high accident rate. Errors committed by operators have significantly contributed to this accident rate. The voluntary and confidential Aviation Safety Reporting System and Aviation Safety Action Program have been successful in identifying and addressing errors committed by air carrier pilots. This study has explored the implementation of voluntary and confidential reporting systems in Shadow 200 flight operations. Mixed methods research combined quantitative survey data and qualitative interview data as a means of determining attitudes relevant to the implementation of such systems. Flight Operational Quality Assurance (FOQA), or Flight Data Monitoring (FDM), has benefited flight safety in both fixed-wing and helicopter operations. The relative youth of FOQA programs has resulted in their minimal application among the helicopter fleets of the world; thus, Helicopter FOQA (HFOQA) has merited consolidation and expansion. This mixed methods design developed HFOQA analysis software via a blend of the qualitative data from helicopter and FOQA experts with quantitative data represented by a sample of de-identified digital flight data from 1,014 helicopter flights. This study analyzed the causes of regional airline pilot fatigue and the impact on pilot performance. An instrument was developed and refined through a field study validation by a panel of experts. The final instrument was distributed to three groups of regional airline pilots. The data supported the literature review, which revealed many causes of pilot fatigue, including flight crew schedules that are not compatible with human limitations; Federal Aviation Regulations (FARs) that have not evolved with the advancement of aircraft performance and the public demand for 24 hour availability of flights; and poor personal habits. The purpose of this study was to evaluate the potential effects of simulation gaming technique on the decision-making skills of airport emergency response technicians. Through the use of descriptive research methods this study has examined the benefits of using simulation training to enhance operational readiness. A specialized questionnaire was developed using a Likert scale format to attain the opinion of 20 airport directors--or their representatives. Findings did support the research hypothesis that the introduction of simulation training (Table Top) exercises would increase the operational readiness of airport emergency response crews. This study investigated the impact of birthdate biorhythm on aviation accidents involving Hellenic Air Force fighter pilots. A list of names, birthdates, and the dates of accidents was compiled from accidents involving pilots who had flown fighter aircraft and have reacted improperly even though they had sufficient time to implement the proper procedures. The list was acquired from the History Office of the Hellenic Air Force. A computer program was acquired for the specific purpose of tracking the pilot's birthdate biorhythm cycles.
