One key to increasing aviation safety and airport efficiency is the proper management of aircraft energy states. Aircraft accident statistics are replete with examples of aircraft that crossed the runway threshold with excessive energy or maintained insufficient energy to reach the runway. This trend has resulted in aircrews that are trained in conservative energy management that trades efficiency for simplicity. Additionally, traditional aircraft flight displays provide limited aircraft energy state cues that do not enable efficient management of aircraft energy states. The Human Factors Analysis and Classification System (HFACS) is a theoretically based tool for investigating and analyzing human error associated with accidents and incidents. Previous research has shown that HFACS can be reliably used to identify general trends in the human factors associated with military and general aviation accidents. The aim of this study was to extend previous examinations of aviation accidents to include specific aircrew, environmental, supervisory, and organizational factors associated with 14 CFR Part 121 (Air Carrier) and 14 CFR Part 135 (Commuter) accidents using HFACS. Flight Operational Quality Assurance (FOQA), or Flight Data Monitoring (FDM), has benefited flight safety in both fixed-wing and helicopter operations. The relative youth of FOQA programs has resulted in their minimal application among the helicopter fleets of the world; thus, Helicopter FOQA (HFOQA) has merited consolidation and expansion. This mixed methods design developed HFOQA analysis software via a blend of the qualitative data from helicopter and FOQA experts with quantitative data represented by a sample of de-identified digital flight data from 1,014 helicopter flights. “Due to the requirement to maintain and improve the safety record of commercial air transportation in the United States (U.S.) despite increasing traffic, several proactive safety programs have been introduced in recent years. Among these proactive safety programs is a form of Flight Data Monitoring (FDM) known in the U.S. as Flight Operational Quality Assurance (FOQA). FOQA is a program utilizing quantifiable, objective data collected from the air carrier aircraft’s data recording system. This thesis addresses the development of a new micro-scale interrupter mechanism for a safe and arm device used in modern weapon systems. The interrupter mechanism often consists of a physical barrier that prevents an initial source of energy, in an explosive train, from being transferred to subsequent charges. In general, when the physical barrier is removed, the weapon is considered armed, and the charge is allowed to propagate. Historically, general aviation (GA) accidents have been overlooked and their impact under-appreciated when compared with those in the commercial or military sector. Recently however, the Federal Aviation Administration and other governmental and civilian organizations have focused their attention on one piece of this proverbial “iceberg,” that being GA accidents occurring in Alaska. This study examines more than 17,000 GA accidents using the Human Factors Analysis and Classification System. Comparisons of Alaska to the rest of the U.S. (RoUS) included traditional demographic and environmental variables, as well as the human errors committed by aircrews. The Human Factors Analysis and Classification System (HFACS) is a theoretically based tool for investigating and analyzing human error associated with accidents and incidents. Previous research performed at both the University of Illinois and the Civil Aerospace Medical Institute has successfully shown that HFACS can be reliably used to analyze the underlying human causes of both commercial and general aviation (GA) accidents. These analyses have helped identify general trends in the types of human factors issues and aircrew errors that have contributed to civil aviation accidents. The SHEL Model has been used to explore relationships between liveware, environmental, hardware and software factors. This study attempts to integrate Flight Operational Quality Assurance (FOQA) program data with the SHEL model. Aircraft record data that can be used to monitor the human interface within the entire system, plus identify faults and potential failures within the system before a major accident or incident occurs. These data have existed for over four decades, and FOQA offers a way to both analyze and act upon them. The Influence Of Visibility, Cloud Ceiling, Financial Incentive, And Personality Factors On General Aviation Pilots' Willingness To Take Off Into Marginal Weather, Part I: The Data And Preliminary Conclusions, William Knecht, Howard Harris, Scott Shappell

Adverse weather is the leading cause of fatalities in general aviation (GA). In this research, influences of ground visibility, cloud ceiling height, financial incentive, and personality were tested on 60 GA pilots' willingness to take off into simulated adverse weather. Results suggested that pilots do not see "weather" as a monolithic cognitive construct but, rather, as an interaction between its separate factors. This is supported by the finding that the multiplicative statistical effect of visibility and ceiling could better predict takeoff than could the linear effect of either variable considered separately. This study investigates the effect of security measures implemented as a result of the terrorist attacks of September 11, 2001 on aviation higher education and how the measures affected aviation higher education students' attitudes toward the United States. The underlying premise of the study is that international persistence represents the ultimate indicator of economic and cultural effect on higher education while international student attitudes provide insight into possible causes of eroding international relationships that lead to terrorism. Surveys and interviews reveal that general aviation manufacturers rely on user[s] to identify problems in maintenance documentation with corrections typically initiated in response to users-reported problems found in the manual;. This investigation compares techniques that manufacturers can use to improve the quality of the maintenance documentation developed by technical writing groups. The techniques, User Performance and Cognitive Walkthrough, were used to identify problems in aircraft maintenance documentation.
