For safe flight in the National Airspace System (NAS), either under the current interim rules or under anticipated longer-term regulatory guidelines facilitating unmanned aircraft system (UAS) access to the NAS, the UAS must incorporate technologies and flight procedures to ensure that neither people nor property in the air, on the ground, or on or in the water are endangered by the failure of an onboard component, by inappropriate unmanned aircraft (UA) response to pilot commands, or by inadvertent entry by the UA into prohibited airspace. In this age of satellite navigation (GPS), Inertial Reference Systems (IRS), and Flight Management Systems (FMS), why would anyone bother with the fundamentals, right? Wrong! A well educated and trained pilot is still the best asset in any aircraft. Unfortunately, we see a trend in pilot training where the basic science and concepts are increasingly disregarded. Ignoring the best in human nature – intellectual curiosity and the quest for knowledge – and inviting the worst, boredom and the feeling of inferiority, will certainly take its toll sooner or later.

Several FOQA parameters that could enhance air carrier safety have not been recorded and/or analyzed. Airlines have traditionally chosen a limited number of parameters for analysis (thousands have been available for recording). No study in the U.S. had addressed FOQA parameters, leading to an exploratory mixed methods study. A conference call comprising FOQA industry experts was planned and completed to (a increase researcher knowledge and (b) discuss a list of improved safety FOQA parameters. A questionnaire, derived from the suggested list, was electronically sent to 40 FOQA professionals. The respondents represented 75% of the airlines with an FAA-approved FOQA program. …

Runway incursion is a persistent problem that has resulted in some of the most devastating accidents in aviation history. Fog and other such weather conditions hamper the visibility of runway surfaces and any obstacles present on the runway, creating a situation where a pilot may not be able to safely land the aircraft. Assisting the pilot to land the aircraft safely in such conditions is an active area of research. A new method is being investigated that combines non-linear image enhancement with classification of runway edges to detect objects on the runway. The need to detect unauthorized usage of intentionally transmitting portable electronic devices (PEDs) onboard commercial aircraft is growing while still allowing passengers to use selected unintentionally transmitting devices, such as laptop computers and CD players, during non-critical stages of flight. This thesis presents the development and testing of an installed system for detecting PEDs over multiple frequency bands. Additionally, the advantages of a fixed versus mobile system are discussed.

It can be dangerous for a pilot to attempt to land an aircraft safely in poor visibility conditions such as rain, fog, haze, snow and low light, but external imagery of a runway can be enhanced to provide increased situational awareness. Objects that are detected in a scene may or may not be a hazard, so interpretation is left to the pilot. Previous research has identified maintenance information as one of the primary causal factors of maintenance error. Incorrect maintenance information has also been cited as a contributing factor in a number of recent aircraft mishaps. To date no one has studied the types of errors found in aircraft maintenance manuals published by manufacturers. The purpose of this research is to analyze Publication Change Requests (PCRs) to document the most frequently reported types of errors found in aircraft maintenance manual, to identify how errors vary across Air Transport Association (ATA) chapters, and identify the corrective actions required to address the cited problem. …

Although effective, the United States Army's Shadow 200 Unmanned Aerial Vehicle has suffered an unacceptably high accident rate. Errors committed by operators have significantly contributed to this accident rate. The voluntary and confidential Aviation Safety Reporting System and Aviation Safety Action Program have been successful in identifying and addressing errors committed by air carrier pilots. This study has explored the implementation of voluntary and confidential reporting systems in Shadow 200 flight operations. Mixed methods research combined quantitative survey data and qualitative interview data as a means of determining attitudes relevant to the implementation of such systems. New technologies are being developed to prevent the continuing problem of Controlled Flight into Terrain (CFIT) accidents. One such technology, the Synthetic Vision System, seems to hold the solution. While it is readily adapted to the commercial aviation instrument environment, it may not be intuitive enough for the General Aviation private pilot with minimal instrument training. This experiment attempted to examine how pilots of every experience level use and interpret the depiction of synthetic terrain to recognize and avoid potential CFIT situations. The outcome of CFIT was measured against pilot experience and display condition.

"In a previous study, multiple regression techniques were applied to Flight Operations Quality Assurance-derived data to develop parsimonious model(s) for fuel consumption on the Boeing 757 airplane. The present study examined several data mining algorithms, including neural networks, on the fuel consumption problem and compared them to the multiple regression results obtained earlier. Using regression methods, parsimonious models were obtained that explained approximately 85% of the variation in fuel flow. In general data mining methods were more effective in predicting fuel consumption. Classification and Regression Tree methods reported correlation coefficients of .91 to .92, and General Linear Models and Multilayer …

There is a need to develop an effective methodology for generating comprehensive intervention strategies that map current and proposed safety programs onto well-established types of human error. Two separate studies were conducted using recommendations from NTSB accident investigations and several joint FAA and industry working groups. The goal of the studies was to validate a proposed framework for developing and examining safety initiatives that target human error in aviation. The results suggest five approaches to reducing human factors associated with aviation accidents. When combined with the Human Factors Analysis and Classification System, the resulting Human Factors Intervention Matrix will provide …
