In this dissertation, I present three essays in health economics that can be read independently from one another. Chapter 2 examines the long-run impact of teenage access to family planning services on young women's health and non-health outcomes. Using the state-level variation in Medicaid family planning expansions, I find that expanding access to family planning services negatively impacts teen births and has positive spillover effects with regards to education and labor outcomes. A back of the envelope calculation suggests that the benefits outweigh the costs of providing these services to teenagers. In Chapter 3, I exploit the state-time variation in …

However, how does belonging to a different sector of employment tied closely to changes in minimum wage and inflation relate to overall health? Access to electricity has the potential to improve the nutritional status of children by a variety of pathways such as increased wealth, reduced fertility through the change in time use, spread of information through technology such as TV, and improved health care services. Yet, the relationship between electrification and children’s nutritional status is rarely explored in the literature. We attempt to fill this lacuna by offering microeconometric evidence from rural Bangladesh, where a rapid expansion of electrification and significant improvement in children’s nutritional status were observed in the past two decades. This dissertation comprises three essays on the economics of driving and health. The first examines the effect of the ride-hailing service Uber, its launch in New York City in May 2011, and the resulting drunk-driving outcomes in the counties that received service compared with those that did not. The difference-in-differences estimation of this effect implies a 25 to 35 percent decrease in the alcohol-related collision rate for the affected New York City boroughs, or about 40 collisions per month. With differentiated treatment effects for each affected county, the difference-in-differences effect is higher for Manhattan, middling for the Bronx and Brooklyn, …

This work attempts to highlight the global magnitude of the obesity and examining the evolutionary perspective with focus on the correlates and temporal trend. It is based on a wide literature survey following PRISMA. The data on country wise prevalence was obtained after systematic review of over 500 articles and websites on the prevalence of obesity. A total of 301 entries/data points were found for 84 countries. Regression analysis was computed for 177 countries based on prediction made by WHO to identify the correlates. The Effect Of Expansions In Medicaid Eligibility On Hospital Financial Performance: Evidence From Medicare Cost Reports, Kevin Wu

Expansions in Medicaid eligibility and effects on profitability for different hospitals are examined using data from Medicare Cost Reports and difference-in-differences analysis. Critical access and disproportionate share hospitals are associated with the largest profitability gains from these expansions. Results imply that vulnerable hospitals are affected most by changes to Medicaid.

Background: As rural hospitals in Kentucky face insolvency, stakeholders must assess the value of rural hospitals as well as alternatives such as rural clinics and private physician offices.

Objective: To identify the value of attributes of healthcare facilities based on Kentucky’s rural residents’ willingness-to-pay (WTP).

Methods: A survey instrument was created and distributed to ten counties in rural Kentucky. A conditional logit model was used to obtain baseline values and then a mixed logit model was used to address heterogeneity among consumers in estimating WTP. This thesis investigates how proximity to parks, historic district designations, and water quality are valued at residential housing prices. The first essay argues that the negative influences of parks and historic districts, if not noticed, could promote negative externalities and unincentivized investments. I find a negative impact on housing values for a close proximity to a park, suggesting disamenities in park features. When the boundary discontinuity and park amenities are considered, I find a positive valuation for a park. Overall, these results imply a mixed influence of parks on homeowners. The primary aim of this study was to cross-validate the Revised Fit, Stigma, & Value (FSV) Scale, a questionnaire for measuring barriers to counseling, using a stratified random sample of adults in the United States. Researchers also investigated the percentage of adults living in the United States that had previously attended counseling and examined demographic differences in participants’ sensitivity to barriers to counseling. The results of a confirmatory factor analysis supported the factorial validity of the three-dimensional FSV model. Discrete choice models (DCMs) are applied in statistical modelling of consumer behavior. Such models are used in many areas including social sciences, health economics, transportation research, and health systems research and they are time dependent. In this manuscript, we review references on the study of such models, develop DCMs with emphasis on time dependent best-worst choice and discrimination between choice attributes. Referenced measurements of the dynamic DCMs are simulated. Expected utilities over time are derived using Markov decision processes. We study attributes and attribute-levels associated with the quality of life of seniors, report the estimation results, and discuss our findings.

To examine racial and geographic disparities in the use of-and outcomes associated with-Medicare observation stays versus short-stay hospitalizations.\nWe used 2007-2010 fee-for-service Medicare claims, including 3,555,994 observation and short-stay hospitalizations for individuals over age 65. We estimated linear probability models with hospital fixed effects to identify within-facility disparities in observation stay use, and estimated in-hospital mortality, and 30- and 90-day post-discharge mortality, return ED visits, and hospital readmissions as a function of placement in observation using linear probability models, propensity-score matching, and interaction terms.\nWe identified racial and geographic disparities in the likelihood of observation stay use within hospitals (blacks 3.9 percentage …

This dissertation consists of three chapters on the economics of social insurance. Each chapter explores an aspect of the evaluation and design of social insurance in terms of nutrition, healthcare and unemployment.

The first chapter, Kim, Fleisher and Sun (2016) report evidence of long-term adverse health impacts of fetal malnutrition exposure of middle-aged survivors of the 1959-1961 China Famine using data from the China Health and Retirement Longitudinal Study. We find that fetal exposure to malnutrition has large and long lasting impacts on both physical health and cognitive abilities, including the risks of suffering a stroke, physical disabilities in speech, …
