This two-case report presents the use of the Individual Patient Solutions (IPS) Implants® Preprosthetic system in two patients with distinct clinical needs.

Case #1 involves a 69-year-old male with significant unilateral maxillary bone loss and a residual oroantral communication (OAC) following the failure of both conventional implants and sinus lift procedures. Case #2 involves a patient with a severely atrophic, edentulous mandible. These cases present two distinct and challenging clinical scenarios where both traditional endosseous and zygomatic implants were not feasible. Both cases were successfully managed with a custom-designed IPS subperiosteal implant.

BACKGROUND: Despite its purported analgesic effects, controversy exists regarding dexmedetomidine's efficacy as an opioid-sparing anesthetic adjunct.

PURPOSE: The purpose of this study is to measure the association between dexmedetomidine (DEX) administration during orthognathic surgery and postoperative opioid consumption.

STUDY DESIGN, SAMPLE, AND SETTING: A single-blind randomized prospective cohort study was implemented. Patients consenting for orthognathic surgery at Thomas Jefferson University from January 2022 to October 2024 were screened. American Society of Anesthesiologists physical status I to III individuals aged ≥14 years were included. Exclusion criteria were revision surgery, recreational drug use, opioid prescription within 2 years, and chronic pain.

Nasal polyps are soft, droplet-like lumps that develop on the inner lining of the nose or sinuses due to long-term inflammation in those areas. Objectives: Chronic non-bacterial osteomyelitis (CNO) is a rare autoinflammatory bone disorder predominantly affecting pediatric and adolescent populations. The clinical presentation is heterogeneous, with persistent pain and swelling representing the most frequent and often initial symptoms. These manifestations are commonly localized to the mandible, although involvement of other skeletal sites, including long bones, clavicle, and vertebrae, may occur in more advanced cases. This study determined the prevalence of bruxism among dental students and explored the potential contributing factors. Periapical inflammation due to bacterial infection or pulpal necrosis can result in granulation tissue and may activate epithelial rests of Malassez, leading to a periapical cyst. Background: Maxillary sinus augmentation is a common implant placement procedure when residual bone height (RBH) in the posterior maxilla is limited, especially in subantral type IV cases. A less invasive alternative to the traditional lateral window procedure is the crestal approach using the CMC (Crestal approach with membrane Control) technology. Objective: To evaluate the differences between the CMC technique and the conventional lateral window approach in terms of clinical performance, bone density results, surgical time, and complication rates for sinus lifting in subantral type IV cases. Methods: Twenty patients with posterior maxillary edentulism and RBH < 5 mm were divided into two groups at random: Group A (n = 10) received direct sinus lift using the lateral window approach, and Group B (n = 10) received indirect sinus lift using the CMC technique. Four sites (crestal, apical, buccal, and palatal) in Hounsfield Units (HU) were evaluated for bone density using CBCT scans both before and four months after surgery. Cervicofacial subcutaneous emphysema is an uncommon but potentially life-threatening complication of dental procedures. While classically associated with surgical extractions performed with dental high-speed handpieces instead of surgical rear-exhaust handpieces, it has been documented following other dental procedures, including restorative and endodontic treatments. As cervicofacial air emphysema spreads, it may reach the retropharyngeal space, which connects to the mediastinum, leading to pneumomediastinum and a risk for mediastinitis, which carries a high mortality rate. We present a case of cervicofacial subcutaneous emphysema leading to pneumomediastinum following a Class V restoration on tooth #28 to highlight the importance of recognizing this underdiagnosed complication.

Objective: In this work, Hyflex CM, TruNatomy, and Plex V heat-treated nickel titanium rotary instruments were evaluated at body temperature (36 °C ± 1 °C) with and without a cooling protocol to assess the cyclic fatigue resistance.
