The Effect Of Multimedia Information About Computer-Assisted And Freehand Dental Implant Surgery On Patients’ Knowledge, Anxiety, And Decision-Making, Veerawit Lukkanasomboon

This study aimed to evaluate the impact of multimedia and verbal information on patients' knowledge, anxiety, and decision-making regarding dental implant surgery, focusing on comparing computer-assisted and freehand techniques. This randomized controlled trial involved 100 patients divided into two groups: one receiving multimedia information (n=50) and the other receiving verbal information (n=50). Data were collected through self-administered questionnaires assessing patients' baseline information, knowledge, anxiety, and decision-making before and after receiving information about dental implant surgery techniques. Results showed significant increases in knowledge scores for both groups, with the multimedia group improving from 1.96 to 3.90 (p<0.001) and the verbal group from 2.0 to 3.98 (p<0.001). Anxiety levels decreased significantly in both groups, with reductions from 12.76 to 11.48 (p=0.001) in the multimedia group and from 13.92 to 12.16 (p<0.001) in the verbal group. The proportion of patients who decided to proceed with dental implant surgery increased from 70% to 78% in the multimedia group and from 54% to 66% in the verbal group. After receiving the information, more patients in both groups opted for computer-assisted implant surgery, though treatment costs remained a significant concern for some, leading to declines in surgery acceptance. Despite these findings, no significant differences were observed between the multimedia and verbal groups regarding knowledge improvements, anxiety reduction, or decision-making. In conclusion, multimedia and verbal information effectively enhanced patient knowledge and reduced anxiety about dental implant surgery, and both methods may influence patients' decisions regarding their preferred implant technique, with no significant differences between the two methods.

ที่มาและความสำคัญ: ภาวะกล้ามเนื้อลีบจากอายุที่เพิ่มขึ้น (Age-related sarcopenia) ส่งผลให้มวลกล้ามเนื้อและความแข็งแรงของลิ้นในผู้สูงอายุลดลง ซึ่งนำไปสู่ความบกพร่องในการกลืน (dysphagia) และเพิ่มความเสี่ยงต่อภาวะทุพโภชนาการ การสำลักปอดอักเสบ และการเสื่อมถอยของสมอง การออกกำลังกายเพิ่มความแข็งแรงของลิ้น (Tongue-strengthening exercises - TSEs) สามารถช่วยเพิ่มความแข็งแรง ความหนาของลิ้น และการทำงานของการกลืนในผู้สูงอายุที่สุขภาพดีได้ วัตถุประสงค์: เพื่อศึกษาผลของการออกกำลังกายลิ้นรูปแบบต่างๆ ต่อการเพิ่มความแข็งแรง ความทนทานของลิ้นและการกลืนในผู้สูงอายุวิธีการ: ทำการสุ่มอาสาสมัครผู้สูงอายุสุขภาพดีจำนวน 36 คน ที่มีอายุ 65 ปีขึ้นไป ซึ่งเข้ารับการถอนฟันแบบง่ายเพียงซี่เดียวที่ภาควิชาศัลยศาสตร์ช่องปากและแม็กซิลโลเฟเชียล ที่คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย จากนั้นสุ่มแบ่งเป็น 3 กลุ่ม ได้แก่ กลุ่มลิ้นห่อ (Tongue wrap) กลุ่มลิ้นมุมปาก (Tongue corner) และกลุ่มทาสีลิ้น (Tongue paint) โดยประเมินความแข็งแรงของลิ้น (Tongue strength) ความทนทานของลิ้น (Tongue endurance) และทดสอบความสามารถในการกลืนโดยการกลืนด้วยวิธี Repetitive Saliva Swallowing Test (RSST) ใน 2 ช่วงเวลา คือก่อนเริ่มฝึก (T0) และสัปดาห์ที่ 4 หลังการฝึก (T1) ผลการศึกษา: ในกลุ่มลิ้นทาสีพบว่าความแข็งแรงของลิ้นส่วนหน้า (Anterior Tongue Strength - ATS) และความแข็งแรงของลิ้นส่วนหลัง (Posterior Tongue Strength - PTS) เพิ่มขึ้นอย่างมีนัยสำคัญหลังการฝึก 4 สัปดาห์ (p < 0.05) โดยความแข็งแรงของลิ้นทั้งส่วนหน้าและส่วนหลังในเพศชายของกลุ่มทาสีลิ้นเพิ่มขึ้นอย่างมีนัยสำคัญ (p < 0.05) ส่วนความแข็งแรงของลิ้นทั้งส่วนหน้า ส่วนหลัง และความทนทานของลิ้นส่วนหน้าในกลุ่มลิ้นห่อและกลุ่มลิ้นมุมปากเพิ่มขึ้นจากค่าเริ่มต้น แต่ไม่มีนัยสำคัญทางสถิติ สรุป: การออกกำลังกายลิ้นด้วยการทาสีลิ้นอาจเป็นวิธีที่มีประสิทธิภาพสำหรับการเพิ่มความแข็งแรงของลิ้น

Extractions are generally discouraged in these patients due to the risk of medication-related osteonecrosis of the jaw (MRONJ). This case study presents an 89-year-old female with metastatic breast cancer on Xgeva, facing a non-restorable tooth (#30). A collaborative approach involving the general dentist, endodontist, and oral surgeon employed endodontic therapy, crown amputation, and soft tissue advancement, avoiding extraction.
