The Affordable Care Act (ACA) has been under attack since before it was signed into law, culminating in its only legislative challenge under the Trump administration in 2017. In this dissertation, I examine the link between pupil transportation policy and three outcomes.

The first paper exploits idiosyncratic variation in the percentage of weekdays that are instructional school days in a month and variation in pupil transportation spending within districts over time. I build a rich, monthly, longitudinal data set for congestion, school days, and transportation policy for 51 cities from 2013 to 2019 and find congestion is significantly higher on school days and pupil transportation alleviates congestion caused by school children's travel.

Government regulators cannot mitigate the loss of wetlands and coastal erosion alone. Nonprofits, uniquely situated between coastal property owners with personal interests and governments with regulatory interests, are positioned to mediate the interests of different parties while considering local context and individual circumstances. This dissertation is comprised of three essays on education and childhood health. Each chapter examines the effect of health or health insurance coverage during childhood on schooling outcomes. Chapters 1 and 3 estimate the impact of insurance coverage for disability-related healthcare on academic and behavioral outcomes for students with disabilities. Chapter 1 exploits variation in Medicaid coverage for mental and behavioral healthcare generated by the Rosie D. vs. Patrick class action lawsuit. The resulting reforms improved outcomes for students diagnosed with an emotional disturbance and enabled them to be educated in more inclusive settings. The attendance rate for students with …

The United States' response to the COVID-19 pandemic has relied heavily on personal mitigation behaviors versus centralized governmental prevention strategies, especially early in the virus's outbreak. This study examines how family structure shapes mitigation, focusing on the intersectional effects of gender, marital status, and the presence of children while accounting for differences in worry about infection from the virus. Using data from a national survey of 10,368 United States adults early in the pandemic (March 2020), survey-weighted logistic regression models show important differences in the likelihood of personal mitigation adoption across family structures. Purpose: The early months of the COVID-19 pandemic led to reduced cancer screenings and delayed cancer surgeries. We used insurance claims data to understand how breast cancer incidence and treatment after diagnosis changed nationwide over the course of the pandemic.

Methods: Using the Optum Research Database from January 2017 to March 2021, including approximately 19 million US adults with commercial health insurance, we identified new breast cancer diagnoses and first treatment after diagnosis. Somatic copy number alterations (SCNAs) in tumors are clinically significant diagnostic, prognostic, and predictive biomarkers. SCNA detection from targeted next-generation sequencing panels is increasingly common in clinical practice; however, detailed descriptions of optimization and validation of SCNA pipelines for small targeted panels are limited. Until recently, psychotherapies, including exposure and response prevention (ERP) for obsessive-compulsive disorder (OCD), have primarily been delivered in-person. The COVID-19 pandemic required OCD providers delivering ERP to quickly transition to telehealth services. While evidence supports telehealth ERP delivery, limited research has examined OCD provider perceptions about patient characteristics that are most appropriate for this modality, as well as provider abilities to identify and address factors interfering with effective telehealth ERP. White-Sutton syndrome (WHSUS), which is caused by heterozygous pathogenic variants in POGZ, is characterized by a spectrum of intellectual disabilities and global developmental delay with or without features of autism spectrum disorder. Additional features may include hypotonia, behavioral abnormalities, ophthalmic abnormalities, hearing loss, sleep apnea, microcephaly, dysmorphic facial features, and rarely, congenital diaphragmatic hernia (CDH). We present a 6-year-old female with features of WHSUS, including CDH, but with nondiagnostic clinical trio exome sequencing. Exome sequencing reanalysis revealed a heterozygous, de novo, intronic variant in POGZ (NM_015100.3:c.2546-20T>A). RNA sequencing revealed that this intronic variant leads to skipping of exon 18. …

The QIC-WD and the Louisiana Department of Child and Family Services (DCFS) Child Welfare Division (CWD) worked together to conduct a needs assessment. A variety of information was explored to learn about the current workforce and the agency’s child welfare workforce practices, including: 1) recruitment, hiring, and retention metrics and processes; 2) organizational culture and climate; and 3) other workforce processes and initiatives, such as onboarding, mentoring, supervision, performance management, and employee recognition.

Existing approaches only enable the communication in a heuristic manner without considering the content/importance of information to be shared. UniComm embeds massive observations collected at one agent into crucial predictions of their impact on its neighbors, which improves the communication efficiency and is universal across existing methods. Urban blight has been found to cause a variety of problems, including negatively affecting the value of surrounding properties and increasing neighborhood crime rates. If the same externalities that give rise to urban sprawl also contribute to urban blight, as is suggested by Brueckner and Helsley (2011), city center vacancy rates-an indication of blight-would increase with the extent of urban sprawl. This study adds to the sparse literature on the empirical relationship between urban sprawl and blight by finding that the city-center census tract vacancy rate is higher in more sprawling cities.
