Is Higher Viral Load In The Upper Respiratory Tract Associated With Severe Pneumonia? Findings From The Perch Study., Daniel R Feikin, Wei Fu, Daniel E Park, Qiyuan Shi, Melissa M Higdon, Henry C Baggett, +Several Additional Authors
Epidemiology Faculty Publications
Background.
The etiologic inference of identifying a pathogen in the upper respiratory tract (URT) of children with pneumonia is unclear. To determine if viral load could provide evidence of causality of pneumonia, we compared viral load in the URT of children with World Health Organization–defined severe and very severe pneumonia and age-matched community controls.
Methods.
In the 9 developing country sites, nasopharyngeal/oropharyngeal swabs from children with and without pneumonia were tested using quantitative real-time polymerase chain reaction for 17 viruses. The association of viral load with case status was evaluated using logistic regression. Receiver operating characteristic (ROC) curves were constructed …

Background.
Lack of a gold standard for identifying bacterial and viral etiologies of pneumonia has limited evaluation of C-reactive protein (CRP) for identifying bacterial pneumonia. We evaluated the sensitivity and specificity of CRP for identifying bacterial vs respiratory syncytial virus (RSV) pneumonia in the Pneumonia Etiology Research for Child Health (PERCH) multicenter case-control study. Methods.
We measured serum CRP levels in cases with World Health Organization–defined severe or very severe pneumonia and a subset of community controls. We evaluated the sensitivity and specificity of elevated CRP for “confirmed” bacterial pneumonia (positive blood culture or positive lung aspirate or pleural fluid …

Background.
Sputum microscopy and culture are commonly used for diagnosing the cause of pneumonia in adults but are rarely performed in children due to difficulties in obtaining specimens. Induced sputum is occasionally used to investigate lower respiratory infections in children but has not been widely used in pneumonia etiology studies. Methods.
We evaluated the diagnostic utility of induced sputum microscopy and culture in patients enrolled in the Pneumonia Etiology Research for Child Health (PERCH) study, a large study of community-acquired pneumonia in children aged 1–59 months. Comparisons were made between induced sputum samples from hospitalized children with radiographically confirmed pneumonia …

When pediatric, adolescent, and young adult patients present with a bone sarcoma, treatment decisions, especially after relapse, are complex and require a multidisciplinary approach. This review presents scenarios commonly encountered in the therapy of bone sarcomas with the goal of objectively presenting a consensus, multidisciplinary management approach. Little variation was found in the authors' group with respect to local control or systemic therapy. Clinical trials were universally prioritized in all settings. Decisions regarding relapse therapies in the absence of a clinical trial had very minor variations initially, but a consensus was reached after a literature review and discussion. This review …
Identifying Risk Factors For Interstate Crashes Using Spatial Statistics, Kaitlin Gibson, Matthew Heaton
Identifying Risk Factors For Interstate Crashes Using Spatial Statistics, Kaitlin Gibson, Matthew Heaton
Journal of Undergraduate Research
The goal of systemic highway safety improvement is to identify road characteristics, called risk factors, associated with a higher prevalence of crashes, so that the roads can be modified to avoid these characteristics. However, the statistical analyses these improvement projects are based on generally use methods which are not appropriate for the data. Our project implements novel spatial statistical methodology which is appropriate and state-of-the-art to identify these risk factors.
An Algorithm For Multiple Regression Variable Selection For Biochemical Oxygen Demand, Zach White, Dr. William Christensen
An Algorithm For Multiple Regression Variable Selection For Biochemical Oxygen Demand, Zach White, Dr. William Christensen
Journal of Undergraduate Research
Biochemical Oxygen Demand (BOD) is used to measure of the amount of oxygen required by aerobic bacteria and other microorganisms to stabilize decomposable organic matter. It is run as a laboratory based biodegradation test and relies upon the presence of a thriving microbial community that may be naturally present in the sample or artificially introduced by addition of a seed, commonly a known volume of sewage effluent of known BOD. A standard BOD test is run in the dark at a temperature of 20 degrees Celsius for 5 days. This is defined as a five-day BOD (BOD5), which is the …

Optimal, Two Stage, Adaptive Enrichment Designs For Randomized Trials Using Sparse Linear Programming, Michael Rosenblum, Xingyuan Fang, Han Liu
Johns Hopkins University, Dept. of Biostatistics Working Papers
Adaptive enrichment designs involve preplanned rules for modifying enrollment criteria based on accruing data in a randomized trial. We focus on designs where the overall population is partitioned into two predefined subpopulations, e.g., based on a biomarker or risk score measured at baseline. The goal is to learn which populations benefit from an experimental treatment. Two critical components of adaptive enrichment designs are the decision rule for modifying enrollment, and the multiple testing procedure. We provide a general method for simultaneously optimizing these components for two stage, adaptive enrichment designs. We minimize the expected sample size under constraints on power …
How To Estimate Statistical Characteristics Based On A Sample: Nonparametric Maximum Likelihood Approach Leads To Sample Mean, Sample Variance, Etc., Vladik Kreinovich, Thongchai Dumrongpokaphan
How To Estimate Statistical Characteristics Based On A Sample: Nonparametric Maximum Likelihood Approach Leads To Sample Mean, Sample Variance, Etc., Vladik Kreinovich, Thongchai Dumrongpokaphan
Departmental Technical Reports (CS)
In many practical situations, we need to estimate different statistical characteristics based on a sample. In some cases, we know that the corresponding probability distribution belongs to a known finite-parametric family of distributions. In such cases, a reasonable idea is to use the Maximum Likelihood method to estimate the corresponding parameters, and then to compute the value of the desired statistical characteristic for the distribution with these parameters.
In some practical situations, we do not know any family containing the unknown distribution. We show that in such nonparametric cases, the Maximum Likelihood approach leads to the use of sample mean, …

Background and objective:
Idiopathic pulmonary fibrosis (IPF) has a poor prognosis in general; however, it is heterogeneous to detect relative biomarkers for predicting the disease progression. Serum biomarkers can be conveniently collected to detect and help to differentially diagnose IPF and predict IPF prognosis. This meta-analysis aimed to evaluate the use of serum surfactant proteins A and D (SP-A and SP-D) for differential diagnosis and prognosis of IPF.
Methods:
Relevant articles were searched in PubMed, Embase, and Chinese National Knowledge Infrastructuredatabases and reviewed by 2 independent readers. Standard mean difference (SMD) and 95% confidence interval (CI) were …

BACKGROUND AND PURPOSE: Hemodynamics is thought to be an important factor for aneurysm progression and rupture. Our aim was to evaluate whether flow fields reconstructed from dynamic angiography data can be used to realistically represent the main flow structures in intracranial aneurysms. MATERIALS AND METHODS: DSA-based flow reconstructions, obtained during interventional treatment, were compared qualitatively with flow fields obtained from patient-specific computational fluid dynamics models and quantitatively with projections of the computational fluid dynamics fields (by computing a directional similarity of the vector fields) in 15 cerebral aneurysms. RESULTS: The average similarity between the DSA and the projected computational fluid …

Incorporating Place And Space: A Hierarchical Spatial Approach To Exploring Preventable Congestive Heart Failure Hospitalizations In New York City, Rachael Weiss Riley
Dissertations and Theses
Background: Faced with rising medical care costs, increasing prevalence, and widening health disparities, preventing congestive heart failure (CHF) hospitalizations is a central public health concern. Despite evidence of geographical clustering in preventable CHF admissions, there is a lack of research designed to examine spatial patterning of CHF and the local area neighborhood determinants that contribute to this variability. This study sought to assess and evaluate the importance of both space and place in analyzing preventable CHF hospitalizations and readmissions by applying appropriate statistical techniques, clarifying the assumption inherent in each method, and interpreting the findings within the context of existing …

A Direct D-Bar Method For Partial Boundary Data Electrical Impedance Tomography With A Priori Information, Melody Alsaker, Sarah J. Hamilton, Andreas Hauptmann
Mathematics, Statistics and Computer Science Faculty Research and Publications
Electrical Impedance Tomography (EIT) is a non-invasive imaging modality that uses surface electrical measurements to determine the internal conductivity of a body. The mathematical formulation of the EIT problem is a nonlinear and severely ill-posed inverse problem for which direct D-bar methods have proved useful in providing noise-robust conductivity reconstructions. Recent advances in D-bar methods allow for conductivity reconstructions using EIT measurement data from only part of the domain (e.g., a patient lying on their back could be imaged using only data gathered on the accessible part of the body). However, D-bar reconstructions suffer from a loss of sharp edges …

Purpose
Three-fourths of patients with advanced cancer are reported to suffer from pain. A primary barrier to provision of adequate symptom treatment is failure to appreciate the intensity of the symptoms patients are experiencing. Because data on Bangladeshi and Nepalese patients’ perceptions of their symptomatic status are limited, we sought such information using a cell phone questionnaire.
Methods
At tertiary care centers in Dhaka and Kathmandu, we recruited 640 and 383 adult patients, respectively, with incurable malignancy presenting for outpatient visits and instructed them for that single visit on one-time completion of a cell phone platform 15-item survey of questions …

INTRODUCTION: The presence of cerebrovascular pathology may increase the risk of clinical diagnosis of Alzheimer's disease (AD).
METHODS: We examined excess risk of incident clinical diagnosis of AD (probable and possible AD) posed by the presence of lacunes and large infarcts beyond AD pathology using data from the Statistical Modeling of Aging and Risk of Transition study, a consortium of longitudinal cohort studies with more than 2000 autopsies. We created six mutually exclusive pathology patterns combining three levels of AD pathology (low, moderate, or high AD pathology) and two levels of vascular pathology (without lacunes and large infarcts or with …
Spatial Analysis Of Time Between Two Consecutive Dental And Two Consecutive Well-Child Visits For Foster Care Youth, Chenyang Shi
Spatial Analysis Of Time Between Two Consecutive Dental And Two Consecutive Well-Child Visits For Foster Care Youth, Chenyang Shi
Dissertations
Foster care youth is a medically vulnerable population. Poor dental health and irregular well-child visits may cause serious health-related issues, such as mental disorder, nutrition imbalance, tooth damage, etc. Michigan requires all youth in foster care to receive annual dental and well-child visits. Usually, the study of foster care well-child and dental visits include two parts: time between two consecutive visits (gap time) and number of visits. For this study, a longitudinal-spatial model that has the flexibility to analyze the well-child/dental gap times and number of visits was developed. The longitudinal data (2009-2012) on Michigan foster care youth from 10 …
Development Of Traditional And Rank-Based Algorithms For Linear Models With Autoregressive Errors And Multivariate Logistic Regression With Spatial Random Effects, Shaofeng Zhang
Dissertations
Linear models are the most commonly used statistical methods in many disciplines. One of the model assumptions is that the error terms (residuals) are independent and identically distributed. This assumption is often violated and autoregressive error terms are often encountered by researchers. The most popular technique to deal with linear models with autoregressive errors is perhaps the autoregressive integrated moving average (ARIMA). Another common approach is generalized least squares, such as Cochrane-Orcutt estimation and Prais-Winsten estimation. However, these usually have poor behaviors when fitting small samples. To address this problem, a double bootstrap method was proposed by McKnight et al. …

Mechanistic Mathematical Models: An Underused Platform For Hpv Research, Marc Ryser, Patti Gravitt, Evan R. Myers
Global Health Faculty Publications
Health economic modeling has become an invaluable methodology for the design and evaluation of clinical and public health interventions against the human papillomavirus (HPV) and associated diseases. At the same time, relatively little attention has been paid to a different yet complementary class of models, namely that of mechanistic mathematical models. The primary focus of mechanistic mathematical models is to better understand the intricate biologic mechanisms and dynamics of disease. Inspired by a long and successful history of mechanistic modeling in other biomedical fields, we highlight several areas of HPV research where mechanistic models have the potential to advance the …

Lisinopril Or Coreg Cr In Reducing Cardiotoxicity In Women With Breast Cancer Receiving Trastuzumab: A Rationale And Design Of A Randomized Clinical Trial, Maya Guglin, Pamela Munster, Angelina Fink, Jeffrey Krischer
Internal Medicine Faculty Publications
Background—Trastuzumab (TZB) is an established therapy for HER2 positive breast cancer. The use of TZB is commonly associated with cardiotoxicity manifesting as asymptomatic decrease in left ventricular ejection fraction (LVEF) or overt heart failure. Several studies demonstrated favorable effects of angiotensin converting enzyme (ACE) inhibitors and beta blockers (BB) in the prevention of chemotherapy-induced cardiotoxicity. We hypothesize that patients, randomized to receive an ACE inhibitor or a beta-blocker during trastuzumab therapy for breast cancer, will maintain a higher LVEF than patients randomized to placebo.
Methods and Results—We designed a prospective, multicenter, randomized, phase II placebo-controlled clinical trial to …

Measured as elapsed time from first use to dependence syndrome onset, the estimated "induction interval" for cocaine is thought to be short relative to the cannabis interval, but little is known about risk of becoming dependent during first months after onset of use. Virtually all published estimates for this facet of drug dependence epidemiology are from life histories elicited years after first use. To improve estimation, we turn to new month-wise data from nationally representative samples of newly incident drug users identified via probability sampling and confidential computer-assisted self-interviews for the United States National Surveys on Drug Use and Health, …
Exponential Chain Dual To Ratio Cum Dual To Product Estimator For Finite Population Mean In Double Sampling Scheme, Yater Tato, B. K. Singh
Exponential Chain Dual To Ratio Cum Dual To Product Estimator For Finite Population Mean In Double Sampling Scheme, Yater Tato, B. K. Singh
Applications and Applied Mathematics: An International Journal (AAM)
This paper considers an exponential chain dual to ratio cum dual to product estimator for estimating finite population mean using two auxiliary variables in double sampling scheme when the information on another additional auxiliary variable is available along with the main auxiliary variable. The expressions for bias and mean square error of the asymptotically optimum estimator are identified in two different cases. The optimum value of the first phase and second phase sample size has been obtained for the fixed cost of survey. To illustrate the results, theoretical and empirical studies have also been carried out to judge the merits …

A Semiparametric Estimation For The Nonlinear Vector Autoregressive Time Series Model, Rahman Farnoosh, Mahtab Hajebi, Seyed J. Mortazavi
Applications and Applied Mathematics: An International Journal (AAM)
In this paper, the nonlinear vector autoregressive model is considered and a semiparametric method is proposed to estimate the nonlinear vector regression function. We use Taylor series expansion up to the second order which has a parametric framework as a representation of the nonlinear vector regression function. After the parameters are estimated through the least squares method, the obtained nonlinear vector regression function is adjusted by a nonparametric diagonal matrix, and the proposed diagonal matrix is also estimated through the nonparametric smooth-kernel approach. Estimating the parameters can yield the desired estimate of the vector regression function based on the data. …

The mosaic package provides a simplified and systematic introduction to the core functionality related to descriptive statistics, visualization, modeling, and simulation-based inference required in first and second courses in statistics. This introduction to the package describes some of the guiding principles behind the design of the package and provides illustrative examples of several of the most important functions it implements. These can be combined to help students "think with data" using R in their early course work, starting with simple, yet powerful, declarative commands.

An M/G/1 Retrial Queue With Single Working Vacation, S. P. Bala Murugan, K. Santhi
Applications and Applied Mathematics: An International Journal (AAM)
We consider an M=G=1 retrial queue with general retrial times and single working vacation. During the working vacation period, customers can be served at a lower rate. Both service times in a vacation period and in a service period are generally distributed random variables. Using supplementary variable method we obtain the probability generating function for the number of customers and the average number of customers in the orbit. Furthermore, we carry out the waiting time distribution and some special cases of interest are discussed. Finally, some numerical results are presented.

Researchers in information science and related areas have developed various methods for analyzing textual data, such as survey responses. This article describes the application of analysis methods from two distinct fields, one method from interpretive social science and one method from statistical machine learning, to the same survey data. The results show that the two analyses produce some similar and some complementary insights about the phenomenon of interest, in this case, nonuse of social media. We compare both the processes of conducting these analyses and the results they produce to derive insights about each method's unique advantages and drawbacks, as …
