Purpose Statement:
NICU patients have a decreased risk of CLABSI infections when two-person sterile central line changes are implemented into central line maintenance bundles compared to bundles that only use one-person clean line change techniques.
Background:
Neonates admitted to the neonatal care intensive care unit (NICU) often require the placement of central lines for the administration of medications, fluids, parental nutrition, and hemodynamic monitoring. Despite their many advantages, central lines can lead to serious infections, known as cental line associated bloodstream infections (CLABSI). This project aimed to demonstrate that in-patient smoking cessation education would prepare the patient for a serious attempt to quit smoking. Thus, this project explored in-patient smoking-cessation strategies to mitigate smoking-related hospitalizations.

Pediatric mental and behavioral health care needs continue to increase across the nation. Approximately ten percent of pediatric hospitalizations are for a primary mental or behavioral health condition (Canvara & Johnson, 2020). With the increase in hospitalizations for mental health (MH) needs, pediatric psychiatric units are at capacity, and MH patients are admitted to pediatric general medical/surgical units for care. Background/Context: Falls remain a substantial risk for patients and is reported that 1,000,000 patients fall each year and one-third result in injury to the patient.
Problem/Purpose: To evaluate the implementation of a fall prevention toolkit to determine if standards of care were followed and outcomes were met using the Context, Input, Process, and Product model.
Outcomes/Results: Fall risk assessment was occurring on admission (n=33) with opportunity for reassessment throughout stay. Required post-fall documentation was not completed per policy. 26% of staff responded to the feedback survey. The facility’s fall rate target was not met but reducing the …

Can you imagine not sleeping well for weeks or months at a time? Insomnia impacts approximately one-third of adults and is considered the most prevalent of all sleep disorders. With a lack of consistent, quality sleep, insomnia has significant negative consequences that impact a person’s overall mental well-being, like increased anxiety, depression, chronic pain, and daily coping. Cognitive Behavioral Therapy for Insomnia (CBT-I) allows patients to reduce or eliminate insomnia or related symptoms without needing medication to obtain quality and consistent sleep. March 2020 changed the healthcare field entirely. When the COVID-19 pandemic swept through the United States in early 2020, it was recommended that surgical departments nationwide shutdown altogether or at least decrease their workloads to exclusively emergent procedures. This step was taken both to save surgical equipment for more critical patients and to offload those staff members from surgical departments to more critical areas in the hospital systems.

Breastfeeding is important to promote the health of both mothers and babies (Centers for Disease Control and Prevention [CDC], 2021). Many mothers experience workplace barriers and stress which negatively impact breastfeeding duration (McCardel & Padilla, 2020; Nagel et al., 2022). However, a gap in the literature exists surrounding specific barriers and the impact of stress on U.S. nurses who breastfeed. Specifically, workplace challenges and stress among nurses resulting from the COVID-19 pandemic lack exploration.

Purpose and rationale: Deep Sternal Wound Infections (DSWI) are associated with morbidity rates and mortality rates as high as 10% -15% as well as significantly increased hospital costs. The purpose of this project is to decrease current rates of DSWIs in adults undergoing cardiac surgery at a large medical institution.
Synthesis of evidence: The literature shows a strong relationship between elevated HbA1C and DSWI. All articles reviewed for this proposal which implement a sternal vest intervention support its use to prevent DSWI.

Problem Description: First defined in 1984, as a nurse knowing the correct course of action but perceiving barriers that constrain the action, moral distress has evolved to include challenging situations associated with moral uncertainty, moral conflict, and ethical or moral dilemmas (Campbell et al., 2016; Fourie, 2015, 2016; Jameton, 1984). Surgical site infections are an avoidable complication in surgical patients and one of the most common hospital-acquired infections. Adverse effects of surgical site infections include increased hospital length of stay, increased costs to patient and healthcare system, disability, morbidity, and mortality. The purpose of this quality improvement project was to design a protocol with the objective of decreasing surgical site infections in abdominal and spinal surgery patients. The protocol developed used targeted depilation and antisepsis with a chlorhexidine wipe to mitigate infection risk. High blood pressure (HBP), also known as Hypertension (HTN), is a common diagnosis for primary care office visits in the United States; however, a persistent gap remains between the stated public health targets and achieved blood pressure (BP) control rates in the United States (US). In a small sample of patients with essential HTN (ICD 10: I10) receiving primary care at a mid-western Federally Qualified Health Center (FQHC), a chart audit tool revealed 10 out of 10 patients achieved BP control during the measurement period (defined as a BP reading <140/90). Quality improvement (QI) analysis revealed the bigger picture: Achieved BP control rates of patients does not reflect the 2017 American College of Cardiology/American Heart Association HBP guidelines. Additionally, the author found potential care gaps for patients in elevated BP (120-129/>80; n=34) and Stage 1 HTN (130-139/80-89; n=44) categories (who …140/90).>

The opioid epidemic has been ravishing communities in the United States for nearly two decades, resulting in opioid misuse, dependence, and overdose-related deaths. Healthcare providers have inadvertently fueled the epidemic by overprescribing opioid medications contributing to 40% of the opioid overdose-related deaths. One in every 48 patients who receives an opioid medication for the first time will become a chronic user of these medications. Furthermore, 6% of all surgical patients will continue chronic use of opioid medications 90 days after their surgeries regardless of absence of pain. While it is too soon to fully quantify the impact of Covid-19 on nurse satisfaction, engagement, and turnover, evidence is emerging that high chronic fatigue and persistent feelings of being overwhelmed are causing nurses to either leave the bedside or quit nursing altogether, exacerbating existing nurse staffing challenges caused by a decades long nursing shortage that, pre-pandemic, predicted a national deficit of over one-half million nurses by 2030. Our plan is to improve compliance and documentation of stroke education by improving staff awareness of patient's stroke education status. by enhancing communication and improving the documentation process, nursing practice can be supported, and best patient care delivered.
