การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลอง มีวัตถุประสงค์เพื่อเปรียบเทียบผลของรูปแบบการจัดการดูแลผู้ป่วยโรคหลอดเลือดสมองตีบร่วมกับการใช้โมบายแอปพลิเคชัน ต่อ 1) ความสามารถในการดูแลของผู้ดูแลหลักผู้ป่วยโรคหลอดเลือดสมองตีบ และ 2) ความพึงพอใจของพยาบาลวิชาชีพต่อรูปแบบการดูแล โดยใช้กรอบแนวคิด IMB Model ของ Fisher & Fisher (1992) หลักการจัดการดูแลของ DMAHS (2023) และแนวทางการดูแลผู้ป่วยโรคหลอดเลือดสมองสำหรับผู้ดูแลของสถาบันประสาทวิทยา (2563) กลุ่มตัวอย่าง คือ ผู้ดูแลหลักของผู้ป่วยโรคหลอดเลือดสมองตีบจำนวน 36 คน แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม 18 คน และพยาบาลวิชาชีพจำนวน 17 คน เครื่องมือวิจัย ได้แก่ รูปแบบการจัดการดูแลผู้ป่วยโรคหลอดเลือดสมองตีบร่วมกับการใช้โมบายแอปพลิเคชัน (Stroke Link) ที่ออกแบบให้ใช้งานสะดวก ครอบคลุมเนื้อหา 10 ด้านสำคัญต่อการดูแลผู้ป่วยหลังจำหน่าย ได้แก่ ด้านร่างกาย ด้านจิตใจ และการป้องกันภาวะแทรกซ้อน โดยมีฟังก์ชันการแจ้งเตือน การบันทึกข้อมูล และการสื่อสารกับทีมพยาบาล กลุ่มผู้ดูแลได้รับการประเมินความสามารถในการดูแล 3 ครั้ง ได้แก่ ก่อนเริ่มใช้ หลังสิ้นสุด และ 4 สัปดาห์หลังจำหน่าย พยาบาลวิชาชีพได้รับการประเมินความพึงพอใจหลังจากการนำโปรแกรมไปใช้จริง เครื่องมือที่ใช้เก็บรวบรวมข้อมูล ได้แก่ แบบประเมินความพึงพอใจในรูปแบบการดูแลของพยาบาล วิเคราะห์ข้อมูลโดยใช้ Wilcoxon signed-ranks test และ Mann-Whitney U test โดยกำหนดระดับนัยสำคัญทางสถิติที่ .05 ผลการวิจัยสรุปดังนี้ 1. ความสามารถในการดูแลของผู้ดูแลหลักหลังได้รับรูปแบบการจัดการดูแลผู้ป่วยโรคหลอดเลือดสมองตีบร่วมกับการใช้โมบายแอปพลิเคชัน มีคะแนนสูงกว่าก่อนการใช้โปรแกรม และสูงกว่ากลุ่มที่ได้รับการดูแลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. ความพึงพอใจของพยาบาลวิชาชีพต่อรูปแบบการดูแลผู้ป่วยโรคหลอดเลือดสมองตีบหลังจากใช้รูปแบบการจัดการดูแลผู้ป่วยโรคหลอดเลือดสมองตีบร่วมกับการใช้โมบายแอปพลิเคชัน มีคะแนนสูงกว่าการดูแลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05

การวิจัยเชิงพรรณนาแบบความสัมพันธ์เชิงทำนาย (Predictive Correlational Research) นี้มีวัตถุประสงค์เพื่อ ศึกษาคุณภาพชีวิตด้านสุขภาพของผู้ป่วยมะเร็งเต้านมที่ได้รับการรักษาด้วยยาเคมีบำบัด และ ศึกษาอำนาจการทำนายของอายุ จำนวนครั้งที่ได้รับเคมีบำบัด แรงสนับสนุนทางสังคม อาการข้างเคียงจากการได้รับเคมีบำบัด และพฤติกรรมส่งเสริมสุขภาพที่มีต่อคุณภาพชีวิตด้านสุขภาพของผู้ป่วยกลุ่มดังกล่าว กลุ่มตัวอย่างคือผู้ป่วยมะเร็งเต้านมที่ได้รับเคมีบำบัด โดยคัดเลือกด้วยวิธีการแบบเจาะจง (Purposive Sampling) ขนาดกลุ่มตัวอย่างจำนวน 148 ราย เครื่องมือที่ใช้ในการวิจัยแบบสอบถามจำนวน 1 ชุด ประกอบด้วย 5 ส่วน ประกอบไปด้วย 1) แบบสอบถามข้อมูลส่วนบุคคล 2) แบบสอบถามแรงสนับสนุนทางสังคม 3) แบบสอบถามอาการข้างเคียงจากเคมีบำบัด 4) แบบสอบถามพฤติกรรมส่งเสริมสุขภาพ และ 5) แบบสอบถามคุณภาพชีวิตผู้ป่วยมะเร็งเต้านม มีค่าความเชื่อมั่น (Cronbach’s alpha) แบบสอบถามแรงสนับสนุนทางสังคม แบบสอบถามอาการข้างเคียงจากเคมีบำบัด และแบบสอบถามพฤติกรรมส่งเสริมสุขภาพ เท่ากับ .88, .80, .84 และผ่านการตรวจสอบความตรงเชิงเนื้อหา (Content Validity Index: CVI) เท่ากับ .90, 1.0, .90 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา ได้แก่ ความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติวิเคราะห์ถดถอยพหุคูณ ผลการวิจัยพบว่า คุณภาพชีวิตด้านสุขภาพของผู้ที่เป็นมะเร็งเต้านมที่ได้รับเคมีบำบัดอยู่ในระดับปานกลาง ( = 63.45, S.D. = 14.94) พฤติกรรมส่งเสริมสุขภาพ (β= .74, p < .001) แรงสนับสนุนทางสังคม (β=.42, p

การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาความผาสุก และศึกษาความสัมพันธ์ระหว่างการมีอิสระแห่งตน ภาวะผู้นำที่เอาใจใส่ผู้ปฏิบัติงาน และบรรยากาศความปลอดภัยในการปฏิบัติงาน กับความผาสุกของพยาบาลวิชาชีพแผนกอุบัติเหตุและฉุกเฉินในโรงพยาบาลตติยภูมิ กรุงเทพมหานคร กลุ่มตัวอย่างคือ พยาบาลวิชาชีพที่ปฏิบัติงานแผนกอุบัติเหตุและฉุกเฉิน 1 ปีขึ้นไป 3 โรงพยาบาล โดยใช้วิธีการสุ่มแบบหลายขั้นตอน จำนวน 163 คน เครื่องมือที่ใช้ในการวิจัย คือ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามความผาสุกของพยาบาล แบบสอบถามการมีอิสระแห่งตน แบบสอบถามพฤติกรรมภาวะผู้นำที่เอาใจใส่ผู้ปฏิบัติงานของหัวหน้างาน และแบบสอบถามบรรยากาศความปลอดภัยในการปฏิบัติงาน ผ่านการตรวจสอบความตรงเชิงเนื้อหาจากผู้ทรงคุณวุฒิ 5 คน ได้ค่า CVI เท่ากับ .97, .88, .96 และ .99 ตามลำดับ และทดสอบค่าความเที่ยง โดยคำนวณหาค่าสัมประสิทธิ์แอลฟ่า ได้ค่าความเที่ยง เท่ากับ .93, .80, .97 และ .93 ตามลำดับ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และสัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน ผลการวิจัยพบว่า พยาบาลวิชาชีพแผนกอุบัติเหตุและฉุกเฉินในโรงพยาบาลตติยภูมิ กรุงเทพมหานคร มีความผาสุกโดยรวมอยู่ในระดับมาก (M = 3.79, S.D. = .41) โดยบรรยากาศความปลอดภัยในการปฏิบัติงาน การมีอิสระแห่งตน และภาวะผู้นำที่เอาใจใส่ผู้ปฏิบัติงานของหัวหน้างาน มีความสัมพันธ์ทางบวกระดับปานกลางกับความผาสุกของพยาบาลวิชาชีพ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r = .504, .493 และ .426 ตามลำดับ) ผลการวิจัยครั้งนี้ แสดงให้เห็นถึงปัจจัยที่สัมพันธ์กับความผาสุกของพยาบาลวิชาชีพ ดังนั้นผู้บริหารทางการพยาบาลควรส่งเสริมความผาสุกกับพยาบาลวิชาชีพให้สอดคล้องกับปัจจัยที่สัมพันธ์ เพื่อส่งเสริมและธำรงรักษาให้พยาบาลวิชาชีพมีความผาสุกที่ดี อันจะส่งผลถึงคุณภาพการบริการพยาบาลที่ดี

การวิจัยเชิงพรรณนานี้ มีวัตถุประสงค์เพื่อศึกษาบทบาทพยาบาลบริหารทรัพยากรสุขภาพในหน่วยงานสังกัดสำนักงานปลัดกระทรวงสาธารณสุข โดยใช้เทคนิคการวิจัยแบบเดลฟาย กลุ่มผู้ให้ข้อมูลคือ ผู้เชี่ยวชาญ จำนวน 20 คน แบ่งเป็น 5 กลุ่ม ได้แก่ กลุ่มผู้กำหนดนโยบายจากกระทรวงสาธารณสุข/กองการพยาบาล จำนวน 3 คน กลุ่มผู้อำนวยการโรงพยาบาลรัฐ/ผู้บริหารการเงินการคลังของโรงพยาบาลรัฐ จำนวน 4 คน กลุ่มหัวหน้ากลุ่มงานประกันสุขภาพโรงพยาบาลรัฐ จำนวน 5 คน กลุ่มอาจารย์พยาบาลที่มีประสบการณ์การสอนที่เกี่ยวข้องกับการบริหารทรัพยากรสุขภาพ จำนวน 3 คน และ กลุ่มผู้ปฏิบัติการพยาบาลบริหารทรัพยากรสุขภาพภาครัฐ จำนวน 5 คน ดำเนินการวิจัย 3 ขั้นตอน ได้แก่ 1) สัมภาษณ์ผู้เชี่ยวชาญเกี่ยวกับบทบาทพยาบาลบริหารทรัพยากรสุขภาพในหน่วยงานสังกัดสำนักงานปลัดกระทรวงสาธารณสุข ซึ่งประกอบด้วย กลุ่มพยาบาลที่ปฏิบัติงานในสำนักงานสาธารณสุขจังหวัดกลุ่มพยาบาลที่ปฏิบัติงานในโรงพยาบาลศูนย์/โรงพยาบาลทั่วไป และกลุ่มพยาบาลที่ปฏิบัติงานในโรงพยาบาลชุมชน 2) วิเคราะห์ข้อมูล และสร้างแบบสอบถามเพื่อให้ผู้เชี่ยวชาญประเมินระดับความสำคัญ และ 3) คำนวณค่ามัธยฐานและค่าพิสัยระหว่างควอไทล์เพื่อสรุปผล ผลการศึกษาพบว่า บทบาทพยาบาลบริหารทรัพยากรสุขภาพในหน่วยงานสังกัดสำนักงานปลัดกระทรวงสาธารณสุข ประกอบด้วยบทบาท 6 ด้าน ได้แก่ 1) ผู้พิทักษ์สิทธิประโยชน์ของผู้ป่วย 2) ผู้บริหารจัดการข้อมูลประกันสุขภาพ 3) ผู้ประสานงาน 4) ผู้ทวนสอบการใช้ทรัพยากรสุขภาพ 5) ผู้วิเคราะห์ทรัพยากรสุขภาพ และ 6) ผู้จัดการรายกรณี พยาบาลบริหารทรัพยากรสุขภาพในหน่วยงานสังกัดสำนักงานปลัดกระทรวงสาธารณสุขทุกระดับ มีบทบาททั้ง 6 ด้านเหมือนกัน แต่แตกต่างกันในบทบาทย่อย ดังนี้ 1) พยาบาลที่ปฏิบัติงานในสำนักงานสาธารณสุขจังหวัดเน้นบทบาทเชิงนโยบาย กำหนดทิศทาง วางแผน กำกับ ติดตาม และประเมินผลการบริหารทรัพยากรสุขภาพในระดับจังหวัด โดยเน้นการวิเคราะห์ข้อมูลเชิงระบบและการประสานงานระหว่างเครือข่ายบริการ 2) พยาบาลที่ปฏิบัติงานในโรงพยาบาลศูนย์/โรงพยาบาลทั่วไป บริหารจัดการระบบบริการในภาพกว้าง จัดการกรณีซับซ้อน เป็นศูนย์ข้อมูลด้านประกันสุขภาพ และเชื่อมโยงการดูแลผู้ป่วยระหว่างระดับบริการ และ 3) พยาบาลที่ปฏิบัติงานในโรงพยาบาลชุมชน มีบทบาทตรงในพื้นที่ ดูแลผู้ป่วยรายกรณี ใช้และควบคุมทรัพยากรอย่างคุ้มค่า ประสานกับชุมชน และ ปกป้องสิทธิประโยชน์ของประชาชนในระดับปฐมภูมิ ผลการศึกษาครั้งนี้ สามารถนำไปใช้เป็นแนวทางในการพัฒนาหลักสูตรหรือสมรรถนะพยาบาลบริหารทรัพยากรสุขภาพในหน่วยงานสังกัดสำนักงานปลัดกระทรวงสาธารณสุขในการปฏิบัติงานต่อไป และผู้บริหารสามารถนำไปใช้ในการส่งเสริมให้พยาบาลบริหารทรัพยากรสุขภาพปฏิบัติงานตามบทบาท และ …

การวิจัยในครั้งนี้เป็นการวิจัยกึ่งทดลอง ใช้รูปแบบการศึกษาสองกลุ่ม วัดผลก่อนและหลังการทดลอง มีวัตถุประสงค์เพื่อเปรียบเทียบ 1) ภาระการดูแลของผู้ดูแลผู้ที่เป็นโรคจิตเภทในชุมชน ก่อนและหลังได้รับโปรแกรมการสร้างพลังสำหรับผู้ดูแล และ2) ภาระการดูแลของผู้ดูแลผู้ที่เป็นโรคจิตเภทในชุมชน ระหว่างกลุ่มที่ได้รับโปรแกรมการสร้างพลังสำหรับผู้ดูแล กับกลุ่มที่ได้รับการพยาบาลตามปกติ กลุ่มตัวอย่างเป็นผู้ดูแลหลักในครอบครัวที่ให้การดูแลผู้ที่เป็นโรคจิตเภทที่อาศัยอยู่ในชุมชน ซึ่งมีคุณสมบัติครบตามเกณฑ์ จำนวน 50 คน จากนั้นได้รับการจับคู่ด้วยระยะเวลาในการดูแล แล้วสุ่มเข้ากลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 25 คน เครื่องมือที่ใช้ในการวิจัยประกอบด้วย โปรแกรมการสร้างพลังสำหรับผู้ดูแล แบบวัดภาระในการดูแลของผู้ดูแลผู้ป่วยจิตเภท เครื่องมือทุกชุดผ่านการตรวจสอบคุณภาพ โดยผู้ทรงคุณวุฒิจำนวน 5 ท่าน มีค่าสัมประสิทธิ์อัลฟ่าของครอนบาค เท่ากับ .88 วิเคราะห์ข้อมูลโดยใช้ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติทดสอบที ผลการวิจัยสามารถสรุปได้ ดังนี้ 1. ภาระการดูแลของผู้ดูแลผู้ที่เป็นโรคจิตเภทในชุมชน หลังเข้าร่วมโปรแกรมการสร้างพลังสำหรับผู้ดูแล น้อยกว่าก่อนเข้าร่วมโปรแกรมการสร้างพลังสำหรับผู้ดูแล อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. ภาระการดูแลของผู้ดูแลผู้ที่เป็นโรคจิตเภทในชุมชน ที่เข้าร่วมโปรแกรมการสร้างพลังสำหรับผู้ดูแล น้อยกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05

Cigarette smoking among adolescent is a significant public health issue in South Asia, especially in Pakistan. This randomized controlled trial with a repeated-measures design aimed to compare changes in smoking cessation (primary outcome) measured by both self-reported and carbon monoxide exhalation level quit rates, and smoking quit attempts as secondary outcome, among male adolescent smokers aged 16-19. การวิจัยเชิงคุณภาพเชิงพรรณนานี้ มีวัตถุประสงค์เพื่อบรรยายประสบการณ์การบริหารทรัพยากรสุขภาพของพยาบาลกลุ่มงานประกันสุขภาพ โรงพยาบาลศูนย์ ผู้ให้ข้อมูล คือ พยาบาลวิชาชีพที่ปฏิบัติงานบริหารทรัพยากรสุขภาพ ในกลุ่มงานประกันสุขภาพ โรงพยาบาลศูนย์ไม่น้อยกว่า 5 ปี ผ่านการอบรมหลักสูตรที่เกี่ยวข้องกับการบริหารทรัพยากรสุขภาพ และมีความยินดีในการเข้าร่วมการวิจัย จำนวนทั้งสิ้น 12 ราย เก็บรวบรวมข้อมูลโดยใช้วิธีการสัมภาษณ์เชิงลึก (In-dept interview) ร่วมกับการสังเกต (Observation) และการจดบันทึกภาคสนาม (Field notes) วิเคราะห์ข้อมูลแบบแก่นสาระตามวิธีการของ Braun and Clarke และตรวจสอบความน่าเชื่อถือของข้อมูลตามเกณฑ์ของ Guba and Lincoln ผลการวิเคราะห์ข้อมูลประสบการณ์การบริหารทรัพยากรสุขภาพของพยาบาลกลุ่มงานประกันสุขภาพ โรงพยาบาลศูนย์ ประกอบด้วย 5 ประเด็นหลัก และ 19 ประเด็นย่อย ดังต่อไปนี้ 1. การบริหารองค์ความรู้และศักยภาพบุคลากร ประกอบด้วย 4 ประเด็นย่อย ได้แก่ 1.1) การเรียนรู้และเข้าใจเกณฑ์กองทุนต่าง ๆ 1.2) การใช้ความรู้ความชำนาญด้านการพยาบาล 1.3) การพัฒนาทักษะการให้รหัสโรคและหัตถการ และ 1.4) การติดตามความรู้ด้านนโยบายและการเปลี่ยนแปลงระบบสุขภาพ 2. การทบทวนการใช้ทรัพยากรสุขภาพ ประกอบด้วย 3 ประเด็นย่อย ได้แก่ 2.1) การทบทวนความสมบูรณ์ของข้อมูลเพื่อการเบิกจ่าย 2.2) การทบทวนความสมเหตุสมผลของการใช้ทรัพยากร และ 2.3) การจัดทำรายงานและบันทึกผลการทบทวน 3. การบริหารงบประมาณและความคุ้มค่าของทรัพยากรสุขภาพ ประกอบด้วย 5 ประเด็นย่อย ได้แก่ 3.1) การวิเคราะห์ต้นทุนและผลตอบแทน 3.2) การควบคุมงบประมาณ 3.3) การบริหารมาตรฐานการดูแลผู้ป่วยให้สอดคล้องกับสิทธิ 3.4) การพัฒนากระบวนการให้เอื้อต่อการเบิกจ่าย และ 3.5) การสร้างนวัตกรรมหรือบริการใหม่ที่คุ้มค่า 4. การประสานงานเพื่อสร้างความร่วมมือใช้ทรัพยากรสุขภาพอย่างเหมาะสม ประกอบด้วย 3 ประเด็นย่อย ได้แก่ 4.1) การประสานงานภายในองค์กร 4.2) การประสานงานกับหน่วยงานภายนอก และ 4.3) การประสานงานกับผู้รับบริการ 5. …

In 2018, the US Congress enacted a policy permitting Medicare Advantage (MA) plans to cover telehealth services in a beneficiary’s home and through audio-only means as part of the basic benefit package of services, where prior to the policy change such benefits were only allowed to be covered as a supplemental benefit. MA plans were afforded 2 years of lead time for strategizing, negotiating, and capital investment prior to the start date (January 1, 2020) of the new coverage option. Background: Vaccine adherence in the pediatric population refers to receiving vaccines as per the recommended schedule, which significantly reduces vaccine-preventable diseases and enhances community immunity. Vaccine hesitancy, driven by concerns such as safety, misinformation, or cultural beliefs can affect adherence. National coverage of state-mandated vaccines among kindergarteners declined from 95% to around 93% between 2019 and 2022, varying across different vaccines.
