Classifying diabetes at diagnosis is crucial for disease management but increasingly difficult due to overlaps in characteristics between the commonly encountered diabetes types. We evaluated the prevalence and characteristics of youth with diabetes type that was unknown at diagnosis or was revised over time. We studied 2073 youth with new-onset diabetes (median age [IQR] = 11.4 [6.2] years; 50% male; 75% White, 21% Black, 4% other race; overall, 37% Hispanic) and compared youth with unknown versus known diabetes type, per pediatric endocrinologist diagnosis. Photoreceptors are electrically coupled to one another, and the spatiotemporal properties of electrical synapses in a two-dimensional retinal network are still not well studied, because of the limitation of the single electrode or pair recording techniques which do not allow simultaneously measuring responses of multiple photoreceptors at various locations in the retina. Abstract

The healing of a bone fracture is a complex, multistage process consisting of inflammation in the local tissue, angiogenesis, callus formation, and eventually, remodeling and restoration of the bone to its original morphology. It is estimated that 5-10% of fractures do not heal properly and exhibit non-union of the fractured bone segments, with long-term complications compared to properly healed fractures. There is great clinical benefit in the ability to have early detection of impending non-union fractures, and diagnostic ultrasound can be used to image formation of the callus in a healing fracture through the use of different imaging modes. …

Current diagnostic criteria and treatment options for bacterial vaginosis (BV) commonly result in the misdiagnosis and/or mistreatment of the active infection, as well as subsequent reinfection with BV. Background and Purpose

Healing in bone fractures is a complex process involving local inflammation, angiogenesis (i.e., the creation of new blood vessels), and formation of a bone-building callus, all of which leads to returning the bone nearly to its pre-fracture state. This is the process in a bone where the fracture is clean and the fractured ends oppose each other. Five to 10 percent of fractures are described as non-union, which can lead to poor healing and long-term complications. In this review, we cover the current understanding of BRAF mutations and associated clinical characteristics in patients with metastatic NSCLC, approved and emerging treatment options, BRAF sequencing approaches, and unmet needs. The BRAFV600E mutation confers constitutive activity of the MAPK pathway, leading to enhanced growth, proliferation, and survival of tumor cells. Testing for BRAF mutations enables patients to be treated with therapies that directly target BRAFV600E and the MAPK pathway, but BRAF testing lags behind other oncogene testing in metastatic NSCLC. Additional therapies targeting BRAFV600E mutations provide options for patients with metastatic NSCLC. Emerging therapies and combinations under investigation could …

Relief In Gastrointestinal Symptoms With Medical Marijuana Over 1 Year, Matthew P. Wallingford, Erin L. Kelly, Allison Herens, Daniel Hanna, Emily Hajjar, Brooke Worster

INTRODUCTION: Subjective improvement in gastrointestinal (GI) symptoms was assessed among patients using medical marijuana (MMJ).

METHODS: Participants completed surveys at 0 days, 30 days, 6 months, and 12 months with questions about the severity of their GI symptoms on a scale from 1 (mild) to 3 (severe).

RESULTS: In each survey, participants reported a significant decrease in GI symptom severity when using MMJ versus when not using MMJ (p < 0.05). The most common self-reported side effects from using MMJ were increased appetite (12-21.4%), fatigue (6-16.7%), anxiety (4-11.9%), cough (4-11.9%), headache (6-7.9%), and dry mouth (4-7.1%).

CONCLUSION: In patients with chronic GI symptoms, MMJ may provide persistent symptom severity improvement. Limited product availability and mild to moderate side effects are factors to consider before trialing MMJ.

Chylothorax after cardiac surgery is a rare complication associated with severe morbidity and mortality. This report documents successful treatment with percutaneous thoracic duct embolization for chylothorax after total arch replacement. A 69-year-old man underwent replacement of the aortic arch to treat a ruptured aortic aneurysm. After surgery, the left thoracic drain discharged 2,000 to 3,000 mL serosanguineous fluid per day, even though the patient took nothing orally and was administered subcutaneous octreotide therapy. On postoperative day 9, percutaneous thoracic duct embolization was performed, and the drain could be removed. The chylothorax did not recur, and the patient was discharged on …

Background: Athletes who play outdoor sports have multiple risk factors for the development of skin cancer. Literature highlights the need for the use of evidence-based strategies to improve the knowledge and utilization of sun protection practices in this high-risk population. Methods: Playing it Sun Safe was a multifaceted skin cancer prevention initiative implemented in a population of collegiate soccer athletes in collaboration with a university athletic department and campus health clinic. The rapid evolution of SARS-CoV-2 is driven in part by a need to evade the antibody response in the face of high levels of immunity. Here, we isolate spike (S) binding monoclonal antibodies (mAbs) from vaccinees who suffered vaccine break-through infections with Omicron sub lineages BA.4 or BA.5. Twenty eight potent antibodies are isolated and characterised functionally, and in some cases structurally. Diabetic foot ulcers (DFUs) are a prevalent and profoundly debilitating complication that afflicts individuals with diabetes mellitus (DM). These ulcers are associated with substantial morbidity, recurrence rates, disability, and mortality, imposing substantial economic, psychological, and medical burdens. Timely detection and intervention can mitigate the morbidity and disparities linked to DFU. Nevertheless, current therapeutic approaches for DFU continue to grapple with multifaceted limitations. Here, synthesis and thorough characterization of β-NaFeO2 nanoparticles utilizing a co-precipitation technique is presented. XRD analysis confirmed a hexagonal-phase structure of β-NaFeO2. SEM revealed well-dispersed spherical nanoparticles with an average diameter of 45 nm. The FTIR spectrum analysis revealed weak adsorption bands at 1054 cm-1 suggested metal-metal bond stretching (Fe-Na). UV-Visible spectroscopy indicates a 4.4 eV optical band gap. Colloidal stability of β-NaFeO2 was evidenced via Zeta potential (-28.5 mV) and Dynamic Light Scattering (DLS) measurements. BET analysis reveals a substantial 343.27 m2 g-1 surface area with mesoporous characteristics. Environmental degradation and energy constraint are important risks to long-term sustainability in the modern world. Water splitting is a vital approach for environmentally friendly and sustainable energy storage, providing a clean way to produce hydrogen without pollutants. Preparing a catalyst that is active, bifunctional, and durable for water splitting is a difficult task. We addressed the difficulty by creating a bifunctional heterogeneous catalyst, MoS2/rGO, with an ideal weight percentage of 5 wt% by a hydrothermal process. The cGAS-STING pathway plays a crucial role in innate immune activation against cancer and infections, and STING agonists based on cyclic dinucleotides (CDN) have garnered attention for their potential use in cancer immunotherapy and vaccines. However, the limited drug-like properties of CDN necessitate an efficient delivery system to the immune system. To address these challenges, we developed an immunostimulatory delivery system for STING agonists. Objective: Aspirin has been used for primary prevention of atherosclerotic cardiovascular disease (ASCVD) for decades, but this indication has become controversial with recent trial data. The 2022 US Preventive Services Task Force (USPSTF) provided a recommendation to consider aspirin use for primary prevention in adults 40-59 years with a 10-year ASCVD risk ≥10 % and not at increased risk of bleeding, yet population estimates for the impact of this recommendation are unknown. The objective of this study is to determine the prevalence and demographics of the US population who meet eligibility criteria for aspirin under the new 2022 USPSTF guidelines. …

BACKGROUND: Multilocus pathogenic variants (MPVs) are genetic changes that affect multiple gene loci or regions of the genome, collectively leading to multiple molecular diagnoses. MPVs may also contribute to intrafamilial phenotypic variability between affected individuals within a nuclear family. In this study, we aim to gain further insights into the influence of MPVs on a disease manifestation in individual research subjects and explore the complexities of the human genome within a familial context.

METHODS: We conducted a systematic reanalysis of exome sequencing data and runs of homozygosity (ROH) regions of 47 sibling pairs previously diagnosed with various neurodevelopmental disorders (NDD). …

Background: We aimed to describe the frequency and yield of genetic testing in supravalvar aortic stenosis (SVAS) following negative evaluation for Williams-Beuren syndrome (WS).

Methods and results: This retrospective cohort study included patients with SVAS at our institution who had a negative evaluation for WS from May 1991 to September 2021. SVAS was defined as (1) peak supravalvar velocity of ≥2 meters/second, (2) sinotubular junction or ascending aortic Z score < -2.0, or (3) sinotubular junction Z score < -1.5 with family history of SVAS. Patients with complex congenital heart disease, aortic valve disease as the primary condition, or only postoperative SVAS were excluded. Genetic testing and diagnoses were reported. Of 162 patients who were WS negative meeting inclusion criteria, 61 had genetic testing results available (38%). Chromosomal microarray had been performed in 44 of 61 and was nondiagnostic for non-WS causes of SVAS. Sequencing of 1 or more genes was performed in 47 of 61. Sensory functions decline with age, and the likelihood of having multiple sensory impairments increases with age, potentially leading to decreased physical activity and performance, decreased social participation, poorer quality of life, and greater health care spending. Presbyvestibulopathy (PVP), a diagnosis established in 2019, is a vestibular dysfunction or loss related to the aging process characterized by mild bilateral vestibular deficits, chronic dizziness, unsteadiness, gait disturbance, and/or recurrent falls. Background: In the United States, it was predicted that approximately 13,960 new cases of invasive cervical cancer would be diagnosed in 2023 (American Cancer Society, 2023). The research question focused on identifying changes in occupational performance for patients with cervical cancer. The areas of occupational performance considered were activities of daily living, leisure, work, education, social participation, and mental health.

Methods: The respondents completed a cross-sectional, mixed methods Qualtrics survey. Likert scale questions captured the degree of functional impact of cervical cancer on the predetermined areas of occupational performance.
