Cemented and cementless fixation in total knee arthroplasty (TKA) have been successfully used for decades. As the number of younger and more active patients treated with TKA continues to increase, long-term implant survivorship is of increasing importance. One of the most common complications and hence the reason for revision is mechanical loosening (23.1% of all revised TKA). The loosening mechanisms have been proposed for different fixation types. For cemented fixation, bone remodeling after surgery is regulated by the changes in strain energy density (SED). The recruitment of osteoclasts and osteoblasts is controlled by SED-related signals. PURPOSE: Injuries to the hamstring complex are one of the most commonly seen lower extremity injuries in athletic populations. It is currently unknown how psychological or sociological factors affect an athlete after the recovery process has ended and if these factors play a role in re-injury rates relating to the hamstring. It is also unknown if athlete confidence level changes while in a fatigued state after an athlete returns to play from a hamstring injury. Maintaining effective performance under stress can be challenging, especially in the dangerous environments encountered by the police and military personnel. This document reviews the impact of stress on performance, discusses breath interventions as a means of stress mitigation, suggests an approach for exploring the value of a breath intervention in police cadets, tests, analyzes, and discusses a test of this method and results. Biofeedback training can be used to produce resonance breathing that is synchronized with heart rate and optimizes heart rate variability (HRV). Work-related low back disorders are commonly associated with handling tasks. The objective of this study was to determine the combined influence of distance, pace, handled mass and height, on back loading and posture during free box transfer. Kinematics and kinetics of 17 handlers were recorded during a box transfer task between two pallets. Four-way repeated measures ANOVA were conducted on four lift-deposit height conditions (from lift and deposit of 0.16 or 1.16 m), three distances between pallets (1.5, 1.0 and 0.5 m), two handled masses (10 and 20 kg) and two paces (free and faster). Recently remodeling of lumbar soft tissues has received increased research attention. However, the major determinants that influence remodeling need to be elucidated in order to understand the impact of different rehabilitation modalities on tissue remodeling. The main aim of this study was to explore the between-subject variance of different measures of lumbar soft tissues quantified with rehabilitative ultrasound imaging (RUSI). Background
Wearing a lumbosacral orthosis (LSO) is known to influence spine mechanics, but less is known about how LSOs affect motor control. Whether the use of a LSO can negatively affect motor control of the lumbar spine is still under debate.
Objective
The current study examined the immediate effects of two flexible LSOs (extensible and non-extensible) on the anticipatory postural adjustments that prepare the spine for a predictable perturbation.

The primary purpose of this study was to explore students’ perceptions of a servicelearning course at a small, rural, state college. Throughout the semester-long course, the students learned about how to improve afterschool programs and childhood obesity in rural areas similar to the community that encompassed them. The class format consisted of both online discussions over articles, along with in-person meetings. Students then had the option to participate in a paid service opportunity to apply their knowledge in an after-school program; however, no students took part in this component of the class. Background: Chronic stroke patients (i.e., > six months since the onset of stroke) continue to experience persistent gait complications. Once formal physical therapy concludes, exercise professionals can implement exercise interventions designed to improve quality of life and reduce risk of secondary stroke. This systematic review aimed to evaluate whether lower-body aerobic exercise transferred to gait improvements in chronic unilateral stroke. Two studies are discussed in this manuscript each preceded by a literature review of the topic. The first review and study explore agility movements and the effect that alternative upper designs in shoes might have on ground reaction force measures of performance. The second review and study evaluate methods of predicting ground reaction forces without the use of a force platform. Minimal information is available with regard to the effects of backpacks on the biomechanics of gait in individuals with clinical gait abnormalities, such as those seen in cerebral palsy. Introduction: Intervertebral disc (IVD) herniation is characterized by an expulsion of nucleus pulposus (NP) material through the annulus fibrosus (AF). The AF contains two major adhesive structures, the intralamellar matrix and the interlamellar matrix, which act to maintain the strength of the AF and prevent NP material migration. As a herniation occurs, clefts form within the intralamellar matrix, pushing the NP between adjacent collagen fibres; meanwhile, delamination of the interlamellar matrix causes the NP to pool between layers of the AF. Effects of barefoot and minimal footwear conditions on performance during jumping (i.e., jump displacement) are unclear with traditional group-level studies because of intra- and interindividual variability. We compared barefoot, minimal, and conventional athletic footwear conditions relative to countermovement vertical jump (CMVJ) performance and muscle activation using a single-subject approach. Fifteen men (1.8 ± 0.6 m; 84.5 ± 8.5 kg; 23.8 ± 2.3 y) performed three CMVJ trials in barefoot, minimal, and conventional footwear conditions while ground reaction forces (GRF) and electromyograms of eight lower extremity muscles were recorded. The aim of the 2019 Student Survey was to inform the Physiology Majors Interest Group (PMIG) of characteristics of undergraduates enrolled in physiology courses or degree programs from across the United States, to be used as one input source for development of program-level guidelines. 1389 participants from seven universities completed the 2019 P-MIG Student Survey. 37% reported enrollment in a physiology/human physiology major; allied health related programs were the second most common (24%). 61% of respondents reported attending a community college, the majority of whom enrolled in one or more courses at a community college while in high school (44%). …

Research indicates that a moderate dose of caffeine may enhance muscular strength and power, particularly in males. Less is known on caffeine’s impact on muscular performance in females. The purpose of this study was to determine the effects of a 200 mg dose caffeine gum on muscular strength and muscular power in recreationally active females. Nineteen women participated in 3 laboratory sessions, the first being the familiarization trial. Background and purpose
Recent findings suggest that a gait assessment at a discrete moment in a clinic or laboratory setting may not reflect functional, everyday mobility. As a step towards better understanding gait during daily life in neurological populations, we compared gait measures that best discriminated people with multiple sclerosis (MS) and people with Parkinson’s Disease (PD) from their respective, age-matched, healthy control subjects (MS-Ctl, PD-Ctl) in laboratory tests versus a week of daily life monitoring.

AbstractIndividuals with a low level spinal cord injury (SCI) at T1 or below must use a wheelchair for mobility and in order to accomplish daily activities, such as using a recreational wheelchair for sports. These individuals must transfer into and out of their wheelchair several times a day. Motion capture laboratories can measure multiple variables at high frame rates, but we can only measure the average metabolic rate of a stride using respiratory measurements. Biomechanical simulations with equations for calculating metabolic rate can estimate the time profile of metabolic rate within the stride cycle. Purpose: Altered hip strength is a risk factor for lower extremity injury but its relationship to biomechanical dysfunction is debated. Hip strength assessment methods are criticized for using unidirectional, non-weight-bearing positions which may not be representative of athletic activity and may affect comparison to biomechanical analysis of athletic tasks. A weight-bearing task may better represent hip muscle function during these movements. Purpose: Patellar fractures only account for approximately 1% of all skeletal injuries. Patellar fractures usually result from direct trauma (i.e. falling on the knee, dashboard injury, etc.), or, less frequently, occur as a combination of direct and indirect mechanism (i.e. receiving a direct blow while contracting the quadriceps). Typically, gait rehabilitation uses an invariant stimulus paradigm to improve gait related deficiencies. However, this approach may not be optimal as it does not incorporate gait complexity, or in more precise words, the variable fractal-like nature found in the gait fluctuations commonly observed in healthy populations. Pteropods (also known as sea butterflies or sea angels), are holoplanktonic marine snails which swim by flapping a pair of extremely flexible wings. The wings are modified from the molluscan foot and the wing motions are supported by the fluid pressure without any rigid support. Sea angels (gymnosome pteropods) are completely naked; in contrast, sea butterflies (thecosome pteropods) have negatively buoyant aragonite shells which vary in geometry and size among different species. Completing simultaneous tasks while standing or walking (ie, a high cognitive load situation [HCLS]) is inevitable in daily activities and can lead to interference in task performances. Movement persistency, reflected in systematic cycle to cycle fluctuations of a rhythmical task such as walking or voluntary sway, is compromised with increasing age, making older adults more susceptible to falls. In the present study, we tested whether it is possible to improve rhythmic voluntary sway persistency in old age by actively tracking the complex (i.e. persistent) motion of a visual target.
