We propose to enhance the security and transparency of aircraft maintenance records in the aviation industry through the use of blockchain technology. A physical aircraft maintenance logbook is susceptible to being lost or destroyed. A nonexistent aircraft maintenance logbook hurts the confidence in integrity and reputation of the aircraft. Furthermore, fraud can occur through forgery of FAA personnel signatures and the installation of non-official aircraft parts. The scope of this work is to develop a secure blockchain that can store aircraft service records and information in a digital distributed ledger. Previous research has revealed that wrong identification of a failed engine during flight is not an uncommon event in an aircraft cockpit. A number of fatal accidents in the past, including the recent TransAsia Flight 235 accident, resulted from engine misidentification. Most accidents of this type happened on takeoff when pilot workload was at its highest level. This thesis consists of two research studies. For Research Study 1, a survey was created to gather opinions of airline pilots who operate twin-engine turboprop aircraft. The operation of small unmanned aerial systems (sUAS) in the National Airspace System requires a careful consideration of the operating philosophies to ensure a safe outcome for all users. Small UAS operations are explored while the associated risks and benefits from the operations are reviewed. The operation of sUAS in self-separation airspace is discussed by an examination of the concept of autonomous flight rules (AFR). The proliferation of commercial off-the-shelf unmanned aircraft technology has resulted in a growing number of illicit or hazardous UAS activities, highlighting the growing need for effective counter-UAS mitigations. The purpose of this exploratory research is to develop a better understanding of the existing tactics, techniques, procedures involved in counter-UAS operations. Using a critical paradigm approach, the authors conducted a qualitative, unstructured interview with counter-UAS professionals. The authors sought to identify mission planning considerations, counter-UAS engagement tasks, and unanticipated conditions associated with performing a commercial [non-military] counter-UAS mission. Studies examining aviation accidents have not found differences in accident rates by gender, though there may be gender differences in the types of accident. Baker, Lamb, Grabowski, and Rebok (2001) examined fixed-wing aviation accident rates of male and female private pilots and found that males were more likely to have accidents related to inattention or poor planning while female pilots were more likely to have accidents due to mishandling the aircraft. This research analyzed the National Transportation Safety Board’s aviation accident database system to examine the severity of injury and aircraft damage in rotary-wing (helicopter) accidents by gender. The growth of unmanned aircraft operations within the National Airspace System (NAS) reveal an uptick of alarming safety indicators that suggest unmanned aircraft system (UAS) regulatory policy may require adjustment. Recent reports of aircraft collisions with UAS, an increase in pilot-reported near-misses, and research findings that indicate UAS platforms are capable of causing structural damage to aircraft suggest safe and effective UAS integration may not be proceeding as planned. The authors examine several safety indicators within the context of Heinrich’s Triangle to reveal the UAS industry’s susceptibility to evolving safety-related problems.

The purpose of this qualitative study was to examine the phenomenon of enhanced, risk-based Federal Aviation Administration (FAA) oversight of Part 145 repair stations in Oklahoma that performed aircraft maintenance for Part 121 air carriers. Specifically, this research was utilized to explore the lived (personal) experiences of Part 145 repair station managers concerning operational changes in air carrier maintenance practices. The researchers’ intent was not to examine the effectiveness of changes implemented by the FAA; instead, to explore how management has experienced the enhanced changes in their Oklahoma-based Part 145 repair stations. To date, a few operational efforts have been launched by the FAA to mitigate wildlife strikes away from the airport jurisdiction, and those do not include actions by general aviation (GA) pilots. This presentation explores the safety training of GA pilots using aeronautical decision-making concepts as a strategy to mitigate the risk of mishaps due to birds. Previous research, the FAA Serial Report #22, and final reports of GA aircraft accidents due to birds will be covered.

The purpose of this study was to investigate the human factors reported as contributing to operational incidents of unstabilized approaches and landings in United States-based commercial aviation. While previous aviation safety studies have analyzed aviation incident data when investigating the human factor influences during commercial aviation operations and incidents, unstabilized approaches and landings have not been explicitly examined using similar methods. Inter-rater reliability can be regarded as the degree of agreement among raters on a given item or a circumstance. Multiple approaches have been taken to estimate and improve inter-rater reliability of the United States Department of Defense Human Factors Analysis and Classification System used by trained accident investigators. In this study, three trained instructor pilots used the DoD-HFACS to classify 347 U.S. Air Force Accident Investigation Board (AIB) Class-A reports between the years of 2000 and 2013. In this paper, an approach to accelerate search operations for a missing combat support aircraft using a portable waterproof autonomous vertical takeoff and landing unmanned aerial system called “flying locator beacon” is described. The latter is connected with both flight data and cockpit voice recorders with a parallel bus and may be deployed from the empennage during extreme emergency scenarios, which is detected when few flight parameters are overrun leading to an air crash stimulating behavior. This study was conducted to better understand the performance of the XCOR Lynx vehicle. Because the Lynx development was halted, the best knowledge of vehicle dynamics can only be found through simulator flights. X-Plane 10 was chosen for its robust applications and accurate portrayal of dynamics on a vehicle in flight. The Suborbital Space Flight Simulator (SSFS) and Mission Control Center (MCC) were brought to the Applied Aviation Sciences department in fall 2015 at Embry-Riddle Aeronautical University, Daytona Beach campus. Acknowledging the SHELL human factors model, authors examine interfaces among components and assess problems created when the model is aligned with modern digitized flight deck systems. Complacency and overreliance on automated systems are evaluated, and cognitive load and potential for degraded situational awareness are examined. Authors present a SHELL overlay demonstrating where particular digitized functions and operations present challenges to operators and markedly influence effective SHELL interactions in highly complex flight deck systems. Introduction: General aviation missions related to furtherance of business potentially carry higher risk (operations in adverse weather, at night, for longer distances) than those undertaken for recreation. The study herein was undertaken to determine fatal accident rates, proportions, and risk factors/causes. Method: The National Transportation Safety Board aviation accident database was queried for accidents (1996–2015). Annual fleet times were from the general aviation annual activity survey. Statistical analyses used Poisson distributions, proportion testing, and Cohen’s Kappa tests. General aviation rules of thumb (ROTs) for density altitude and true altitude are examined and developed. Both ROTs originate from the same basic principle of hydrostatic balance, but differ significantly in the assumptions made regarding the atmospheric temperature profile. While the ROT for DA assumes a standard atmospheric vertical temperature lapse rate, the ROT for true altitude requires information regarding the observed layer-mean temperature of the atmosphere. Since the layer-mean temperature between the aircraft and the surface is typically unknown, it must be inferred from the temperature at a single level by again assuming a linear lapse rate. This study examined the individual and combined effects of two potential antecedents to aviation-related safety performance: safety climate and occupational callings. Research exploring the importance of occupational callings to the safety domain is in its nascent stages. The extent that someone is living a calling may explain variance in actual safety performance above that which can be explained by safety climate alone. This research utilized the Human Factors Analysis and Classification System (HFACS) to understand the errors, preconditions and violations that contribute to these accidents. The purpose of this project was to research characteristics of a Lithium-ion battery fire to determine if trends exist in measured fire characteristics that can help predict different fire scenarios. These experiments will ultimately aid in developing an alarm threshold for a spacecraft smoke detector, specifically for Orion, a new, multi-purpose crew vehicle under development by NASA. Orion is the next-generation of spacecraft designed to replace the space shuttle and will play an important role in NASA’s journey to Mars.
