Artificial Intelligence And The Challenge For Rural Medicine, James Denvir
Marshall Journal of Medicine
Recent advances in artificial intelligence, machine learning, and deep learning are beginning to have an impact on everyday experiences, from natural language processing used in automated telephone call centers to semi-autonomous vehicles. These techniques have also been applied to medical care. In this editorial we discuss applications of AI to medicine and argue for a proactive approach to include rural medicine in this paradigm shift.

Lymphangioma is a benign congenital malformation of the lymphatic system that is relatively common in juvenile population. Rarely, it can be seen in adults and majority of the cases are reported in the head and neck region (70-75%). Typically, these malformations are asymptomatic. In this report, we present a rare case of splenic lymphangioma presenting with severe, intractable abdominal pain.

•Spontaneous fungal peritonitis (SFP) is an infection defined as a neutrophil count (> 250 cells/mL) in the ascitic fluid with the evidence of positive fungal culture (1) while excluding other intra-abdominal infections. SFP is not as common as spontaneous bacterial peritonitis and has higher mortality rates due to late recognition and difficulty in differentiation between SFP and SBP. (1,2). The common risk factors that have shown to increase the mortality for SFP include hepatorenal syndrome, patient on SBP prophylaxis, elevated APACHE II scores on admission, and elevated lactate levels (2,3). Candida albicans and Candida glabrata are the two …

Amyotrophic Lateral Sclerosis, 2019 Gettysburg College
Amyotrophic Lateral Sclerosis, Anastasia M. Georgetson
Student Publications
The word amyotrophic is derived from Greek, and means “without nourishment to muscles”, lateral means to the sides and sclerosis means hardened (“What is ALS?,” n.d.). First described by Jean-Martin Charcot in the 1800s, Amyotrophic Lateral Sclerosis (ALS) is a progressive degenerative motor neuron disease. Motor neurons are very important cells, and extremely unique since they can be very long with some motor neurons having a length of over a meter (“Disease Mechanisms,” n.d.).
About 5-10% of the cases of ALS are inherited, which is known as familial ALS or fALS, and it is known as autosomal dominant in these …

Gastroenterology
A Gastric Bezoar is defined as an accumulation of indigestible foreign material found in an individual’s GI tract. The incidence of gastric bezoar is 0.3% as reported by endoscopy studies. These formations usually consist of insoluble vegetable matter (phytobezoars), hair (trichobezoars), or medications (pharmacobezoar). In general, they can be asymptomatic for many years or cause nonspecific symptoms including abdominal pain, early satiety, and weight loss. However, in recent years, these have become increasingly problematic for the general population attributing to a variety of GI disturbances. We are reporting a case which revolves around a female patient with no history of …

In non-Hodgkin's lymphoma the findings of hypercalcemia as an initial presentation is extremely rare (<3% of cases). Here we report a 67 year-old man who presented with ST changes secondary to hypercalcemia in the setting of B-cell lymphoma.

A Comparison Of Opioid Abuse Screening Tools Of Detection Of High Risk Patients In The Inpatient Setting, Grace Gu Md, Albert Cheng Md, Darren Rahaman Md, Shyh-Jeun Wang Md, Maureen Strohm Md, Napatkamon Ayutyanont

- The United States is currently experiencing an opioid epidemic, which claims 115 lives daily by overdose.
- Tools for identifying patients at increased risk for abuse include: Screener and Opioid Assessment for Patients with Pain (SOAPP-R) and Opioid Risk Tool (ORT).
- However, data is lacking for the comparison of the two most commonly used opioid screen tools (SOAPP-R and ORT).
- We are comparing the effectiveness of these screening tools for patients being discharged from the hospital who have no "red flags" for abuse.
Predicting Premature Birth Risk With Cfrna, 2019 Southern Methodist University
Predicting Premature Birth Risk With Cfrna, Jason Lin, Jonathan Marin, John Santerre
SMU Data Science Review
Identifying which genes are early indicators for preterm births using cell-free ribonucleic acid (cfRNA) from non-invasive blood tests provided by pregnant women can improve prenatal care. Currently, there are no medical tests for early detection of preterm birth risk in routine checkups for pregnant women. Recent studies have shown potential genes that can predict preterm birth. Machine learning techniques are utilized to see if the Area Under the Curve (AUC) can be improved upon when evaluating the prediction accuracy for chosen genes sequences and concentrations. Using cell-free RNA data from non-invasive blood tests in conjunction with machine learning, we improve …

Endovascular Management Of Free Floating Common Carotid Thrombi, Kevin Carr Md, Zachary Thwing Md, Ravishankar Shivashankar Md, Yafell Serulle Md
Radiology
- The incidence of symptomatic free-floating thrombi of the common carotid artery (CCA) is as low as 0.24-5% incidence in stroke victims. Patients are predisposed to a prothrombotic state
- Historical management options were limited to surgical and medical options both with individual attendant risk profiles
- Large thrombi can present with various degrees of clinical severity
- The endovascular approach is a viable tool to manage these patients

Robustness Of A Newly Proposed Risk Schema For Lymphatic Dissemination In Endometrioid Endometrial Cancer, Danielle M. Greer, Jessica J.F. Kram, Callie M. Cox Bauer, Scott A. Kamelle

Background: Surgical management for endometrioid endometrial cancer (EEC) includes complete lymph node dissection for all patients at risk of lymphatic dissemination. The standard risk schema, defined by Mayo Clinic, identifies low-risk patients as those with grade 1/2 EEC, myometrial invasion (MI) ≤ 50%, and tumor diameter (TD) ≤ 2 cm. We recently proposed (and published) a risk schema containing modified forms of grade, MI and TD that suggests a significant decrease in false-negative rate and need for lymphadenectomy in low-risk women.
Purpose: Evaluate robustness of our proposed schema for lymphatic dissemination risk stratification in a subsequent EEC patient cohort.

Impact Of Diagnostic Terminology On Parental Attitudes Toward Concussion And Brain Injury, 2019 The University of Western Ontario
Impact Of Diagnostic Terminology On Parental Attitudes Toward Concussion And Brain Injury, Naomi Sanderson
Electronic Thesis and Dissertation Repository
Although some have argued that the terms concussion and mild traumatic brain injury (mTBI) should not be used interchangeably, any differences between the diagnoses are subtle. In this study, we recruited 98 individuals through Amazon Mechanical Turk who self-identified as having children between the ages of 13 and 18 (inclusive). Participants were asked questions about a hypothetical injury to their child, related to symptoms, participation in school and physical activity, and recovery prognosis. Roughly half (n = 51) were asked these questions in reference to a “concussion”, and the remainder (n = 47) were asked questions in reference to an …
