Purpose: Interprofessional collaboration can help prevent adverse pregnancy outcomes related to poor oral health. This study was conducted to see if an educational module provided by a dental hygienist (DH) could increase the knowledge and confidence of physician assistant (PA) students with preventive oral care for the pregnant patient.

แปรงสีฟันสำหรับเด็กที่มีจำหน่ายในท้องตลาดในปัจจุบันมีหลายยี่ห้อซึ่งมีคุณสมบัติและลักษณะที่แตกต่างกันออกไป การศึกษานี้ต้องการทดสอบคุณสมบัติทางกายภาพของแปรงสีฟันสำหรับเด็กที่มีจำหน่ายในท้องตลาดโดยเปรียบเทียบการสึกของขนแปรงและปลายขนแปรงสีฟัน และการสึกของผิวเคลือบฟันน้ำนม เมื่อถูกแปรงโดยเครื่องแปรงฟันในห้องปฏิบัติการ นำชิ้นฟันที่ตัดจากด้านแก้มของฟันกรามน้ำนมขนาด 2 x 2 ตารางมิลลิเมตร จำนวน 42 ชิ้นมาฝังในอะคริลิกแล้วนำมาแบ่งเป็น 6 กลุ่ม ตามยี่ห้อของแปรงสีฟัน 1) CUdent (7mm), 2) CUdent (8mm), 3) Berman®, 4) Colgate®, 5) Fluocaril® และ 6) Kodomo® ทดสอบโดยการแปรงฟันร่วมกับสารละลายยาสีฟันด้วยเครื่อง V8 cross – brushing machine ทั้งหมด 100,000 รอบ จากนั้นนำหัวแปรงสีฟันมาหาค่าดัชนีการสึกของแปรงสีฟัน ตัดขนแปรงมาเพื่อพิจารณาลักษณะปลายขนแปรงสีฟันตามมาตรฐาน มอก. และนำผิวเคลือบฟันน้ำนมมาหาค่าเฉลี่ยความขรุขระและค่าเฉลี่ยความลึกการสึกของผิวเคลือบฟันน้ำนม ผลการศึกษาพบว่าค่าเฉลี่ยดัชนีการสึกของขนแปรงสีฟันทุกยี่ห้อก่อนการทดสอบอยู่ในช่วง 0.035±0.003 ถึง 0.038±0.004 โดยไม่พบความแตกต่างอย่างมีนัยสำคัญทางสถิติ และมีค่าเฉลี่ยดัชนีการสึกของขนแปรงสีฟันหลังการทดสอบอยู่ในช่วง 0.088±0.014 ถึง 0.245±0.028 โดยพบว่าแปรงสีฟันยี่ห้อ Fluocaril® และ CUdent (8mm) มีค่าเฉลี่ยดัชนีการสึกของขนแปรงสีฟันสูงกว่ายี่ห้ออื่นอย่างมีนัยสำคัญ (p < 0.001) ร้อยละของลักษณะปลายขนแปรงสีฟันที่ยอมรับได้ก่อนการทดสอบอยู่ในช่วงร้อยละ 90.67 ถึง 94.67 หลังการทดสอบอยู่ในช่วงร้อยละ 78.29 ถึง 96.00 และค่าเฉลี่ยความขรุขระและค่าเฉลี่ยความลึกการสึกของผิวเคลือบฟันน้ำนมก่อนการทดสอบใกล้เคียงกันอยู่ในช่วง 9.054±2.006 ถึง 10.563±3.342 และ 0.021±0.008 ถึง 0.028±0.010 ตามลำดับ และหลังการทดสอบยังคงมีค่าใกล้เคียงกันอยู่ในช่วง 9.638±1.694 ถึง 10.792±0.889 และ 0.032±0.012 ถึง 0.058±0.042 ตามลำดับ โดยไม่พบความแตกต่างอย่างมีนัยสำคัญทางสถิติทั้งก่อนและหลังทดสอบ จากการศึกษานี้สรุปว่าแปรงสีฟันที่มีจำหน่ายในท้องตลาดมีคุณสมบัติทางกายภาพใกล้เคียงกัน โดยแปรงสีฟันยี่ห้อ Fluocaril® และ CUdent (8mm) มีการสึกของขนแปรงสีฟันหลังการทดสอบมากกว่ายี่ห้ออื่น ร้อยละของลักษณะของปลายขนแปรงที่ยอมรับได้ก่อนและหลังการทดสอบผ่านเกณฑ์มาตรฐานทุกยี่ห้อที่นำมาทดสอบ และแปรงสีฟันทั้งหมดที่ทดสอบทำให้ผิวเคลือบฟันน้ำนมสึกได้ไม่แตกต่างกัน

Purpose: Compromised tooth structure on permanent and deciduous teeth in the pediatric population is increasingly being restored with tooth-colored prefabricated zirconia crowns. These restorations may need to be removed or replaced with permanent crowns. The purpose of this in vitro study was to explore the use of an Er,Cr:YSGG laser for removal of prefabricated zirconia crowns cemented with RMGI as a non-invasive alternative to rotary instruments.

การศึกษานี้มีวัตถุประสงค์เพื่อเปรียบเทียบประสิทธิภาพการคืนกลับแร่ธาตุของรอยผุชั้นเนื้อฟันภายหลังการทาซิลเวอร์ไดเอมีนฟลูออไรด์ (SDF) ความเข้มข้นร้อยละ 38 ที่ใช้ระยะเวลาในการทาสาร 10 วินาที และ 60 วินาที ร่วมกับการฉายแสงหรือไม่ฉายแสง วัดความลึกรอยผุเริ่มต้น (lesion depth) และความหนาแน่นแร่ธาตุ (mineral density) ของฟันกรามน้ำนมมนุษย์ที่มีรอยผุในชั้นเนื้อฟันจำนวน 40 ชิ้น แบ่งชิ้นฟันตัวอย่างออกเป็น 4 กลุ่ม ด้วยวิธีสุ่มตัวอย่างแบบกลุ่มย่อยที่มีการเรียงลำดับ ดังนี้ กลุ่มที่ 1 ทา SDF 10 วินาที, กลุ่มที่ 2 ทา SDF 60 วินาที, กลุ่มที่ 3 ทา SDF 10 วินาที ร่วมกับฉายแสง, กลุ่มที่ 4 ทา SDF 60 วินาที ร่วมกับฉายแสง นำไปผ่านกระบวนการสลับกรด-ด่างโดยใช้เชื้อแบคทีเรียเพื่อจำลองสภาวะในช่องปากเป็นเวลา 7 วัน วัดผลการคืนกลับแร่ธาตุของรอยผุด้วยการซ้อนทับภาพรังสีดิจิทัล (digital subtraction radiography) แสดงผลเป็นค่าเฉลี่ยการเปลี่ยนแปลงความหนาแน่นแร่ธาตุ (mMDD) จากการวิเคราะห์ผลด้วยสถิติความแปรปรวนแบบสองทางด้วยโมเดลเชิงเส้นโดยนัยทั่วไป พบว่าการฉายแสงเป็นเพียงปัจจัยเดียวที่ส่งผลให้ mMDD แตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p=0.007) โดยการฉายแสงหลังทา SDF 10 วินาที และ 60 วินาที ให้ผล mMDD มากกว่าการทา SDF 10 วินาที และ 60 วินาที อย่างมีนัยสำคัญทางสถิติ (p= 0.041 และ 0.041 ตามลำดับ) และการทา SDF 60 วินาที ร่วมกับการฉายแสงให้ผล mMDD มากกว่าการทา SDF 10 วินาที อย่างมีนัยสำคัญทางสถิติ (p= 0.010) …

จุดประสงค์: เพื่อเปรียบเทียบความแข็งระดับจุลภาคที่พื้นผิวในรอยผุระยะเริ่มต้นของชิ้นฟันที่ติดกับวัสดุบูรณะอัลคาไซต์ แก้วไอโอโนเมอร์ และคอมโพสิตเรซินชนิดไม่ปลดปล่อยฟลูออไรด์ วิธีการวิจัย: นำชิ้นฟันที่ตัดจากด้านประชิดของฟันกรามน้อยแท้บน จำนวน 30 ชิ้นมาฝังในอะคริลิกแล้ววัดความแข็งผิวระดับจุลภาคที่พื้นผิวด้วยหัวกดนูป เพื่อบันทึกเป็นค่าตั้งต้นและแบ่งกลุ่ม 3 กลุ่ม (1)กลุ่มวัสดุแก้วไอโอโนเมอร์ 10 ชิ้น (2)กลุ่มวัสดุอัลคาไซต์ 10 ชิ้น (3)กลุ่มวัสดุคอมโพสิตเรซินชนิดไม่ปลดปล่อยฟลูออไรด์ 10 ชิ้น แล้วจึงนำชิ้นฟันมาสร้างรอยผุจำลอง ทำการวัดความแข็งผิวระดับจุลภาคที่พื้นผิวของรอยผุจำลอง จากนั้นนำชิ้นฟันไปติดกับบล็อกอะคริลิกที่ทำการบูรณะด้านประชิดด้วยวัสดุบูรณะตามกลุ่ม แล้วนำไปผ่านกระบวนการเลียนแบบสภาวะการเปลี่ยนแปลงความเป็นกรดด่างในช่องปากเป็นเวลา 7 วันแล้วจึงนำมาวัดความแข็งผิวระดับจุลภาคที่พื้นผิว คำนวณหาร้อยละการคืนกลับความแข็งผิวแล้วเปรียบเทียบระหว่าง 3 กลุ่มด้วยสถิติ Kruskal-Wallis Test โดยกำหนดระดับนัยสำคัญที่ 0.05 และทดสอบความแตกต่างในแต่ละคู่ด้วยสถิติ Mann-Whitney Test โดยมีระดับนัยสำคัญที่ 0.017 ซึ่งเป็นระดับนัยสำคัญที่ผ่านการทดสอบ Bonferroni multiple testing correction ผลการศึกษา: ค่าร้อยละการคืนกลับความแข็งผิวระดับจุลภาคของกลุ่มวัสดุอัลคาไซต์มีค่าเฉลี่ยอันดับมากที่สุดเป็น 25.5 ส่วนกลุ่มแก้วไอโอโนเมอร์และกลุ่มคอมโพสิตเรซินเป็น 15.5 และ 5.5 ตามลำดับ ซึ่งเมื่อทำการเปรียบเทียบค่าร้อยละการคืนกลับความแข็งผิวระดับจุลภาคระหว่าง 3 กลุ่มพบว่าทุกกลุ่มมีความแตกต่างกันอย่างมีนัยสำคัญ (p <0.017) สรุป: การบูรณะด้วยวัสดุอัลคาไซค์ให้ผลการคืนกลับแร่ธาตุในรอยผุระยะเริ่มต้นของชิ้นฟันได้มากกว่าวัสดุบูรณะแก้วไอโอโนเมอร์และคอมโพสิตเรซินอย่างมีนัยสำคัญทางสถิติในห้องปฏิบัติการ วัสดุอัลคาไซต์จึงถือเป็นวัสดุหนึ่งที่สามารถเลือกใช้ในกรณีที่ต้องบูรณะฟันคลาสทู ในกรณีที่ฟันซี่ข้างเคียงมีรอยผุระยะเริ่มต้นเพื่อยับยั้งการลุกลามของรอยโรคฟันผุ
