Thrombolytic Therapy In Acute Myocardial Infarction In Pakistan., M. Asad Karim, Ata Ata-Ur- Rehman Qureshi, Syed Faisal Mahmood, J. Akhter

To characterize features of acute myocardial infarction (AMI) in a Pakistani population, and assess the role of thrombolysis in our country, we studied 194 consecutive admitted patients with enzyme positive AMI. Males were affected three times more frequently; women, although affected less, had a higher incidence of complications than men. Premature coronary artery disease (CAD) was present in 50% patients presenting with AMI. An unusually high incidence of anterior wall myocardial infarction (39%) was seen. Complications were frequent with a predominance of LV failure symptoms. Cardiogenic shock was associated with a very high mortality, in excess of 93%. Streptokinase (SK) …

Feasability Of Preventing Encrustation Of Urinary Catheters, J. M. Bibby, A. J. Cox, D. W. L. Hukins

Colonization of urinary catheters by bacteria which produce urease leads to an increase in urine pH, followed by deposition of the minerals struvite and hydroxyapatite. Adhesion of these encrusting deposits can be reduced, but not prevented, by using catheters with a smooth surface finish. Chemical methods for preventing encrustation are not completely satisfactory. A better way of preventing encrustation would be to prevent colonization of the catheter by bacteria. This might be achieved by controlled release of antimicrobial agents directly into the urine from the catheter itself. Preliminary experiments have demonstrated the feasibility of controlled release from solid silicone. However, …

Bone Absorptiometry In Metabolic Bone Disease: Baseline Values And Long-Term Treatment With Calcitriol (Post-Menopausal Osteoporosis Versus Osteomalacia), A. Caniggia, B. Frediani

Total body bone absorptiometry reveals low mineral density in both postmenopausal osteoporosis and osteomalacia patients. The method was used to investigate the effect of calcitriol administration on patients suffering from one of these conditions. In osteomalacia, the administration of calcitriol resulted in a dramatic improvement in bone mineral density (sometimes up to 50% in 12 months), indicating the rapid mineralization of previously uncalcified bone tissue as a result of the normalization of the Ca x P product. In osteoporosis a similar treatment was seen to halt the progressive decrease in bone mineral levels and sometimes resulted in minor increases in …

Bone chamber intravital microscopy combines the control volume of in vitro models and the chemical complexity of in vivo models to optimize the study of bone physiology in situ. As an optical tool it provides a window to dynamic events at the tissue level of magnification. In particular, it allows measures of microvascular events: (1) in space by magnifying local perfused vasculature and microcirculation at any instant, and (2) in time by providing the same volume of tissue for weekly viewing of an evolving process such as bone healing. Some Observations On The Structure Of Encrusting Biofilms Of Proteus Mirabilis On Urethral Catheters, C. Winters, D. J. Stickler, N. S. Howe, T. J. Williams, N. Wilkinson, C. J. Buckley

A physical model of the bladder has been used to study the process of urinary catheter encrustation. Techniques have been devised for the preparation of sections through these encrustations in situ on the luminal surfaces of catheters and for mapping the distribution of calcium and magnesium in the biofilms. Transmission electron microscopy on these sections showed struvite-like crystals lying in the matrix and in direct contact with the catheter surface. Calcium phosphate ("bioapatite") was distributed throughout the film and many of these amorphous particles appear to have cells at their cores. This essay surveys the need for a clear and objective definition of medical futility. It is urged that once agreement is obtained for structuring operational guidelines for determining futility, a three-tier decisional structure can be developed for testing whether a given treatment falls within the scope of these guidelines.

Under the first tier, the treating physician would be given the primary responsibility for the making the determination to withhold treatment on the grounds of futility. While the physician would be under a duty not to prescribe treatment deemed futile, he would be obliged to inform the patient and his family …

A Logistic Model Of Periodic Chemotherapy, J. C. Panetta

A logistic differential equation with a time-varying periodic parameter is used to model the growth of cells, in particular cancer cells, in the presences of chemotherapeutic drugs. The chemotherapeutic effects are modeled by a periodic parameter that modifies the growth rate of the cell tissue. A negative growth rate represents the detrimental effects of the drugs. A simple criterion is obtained for the behavior of the chemotherapy.

The Relationship Between Diet, Activity, And Other Factors, And Postpartum Weight Change By Race, Debra J. Boardley, Roger G. Sargent, Ann L. Coker, James R. Hussey, Patricia A. Sharpe

Objective: To identify the impact of dietary intake and activity level on postpartum weight change.

Methods: White (n = 121) and black (n = 224) women, 7-12 months postpartum, participating in the Special Supplemental Feeding Program for Women, Infants, and Children were assessed for dietary intake, activity level, body weight, and other maternal characteristics.

Results: For both black and white women, the most important variables in predicting postpartum weight loss were pre-pregnancy weight, gestational weight gain, parity, and prenatal exercise. After these factors were controlled, race predicted that black women retained 6.4 lb more than white women. These results may …

In vitro neurovirological studies of viral infectivity or viral gene expression may be confounded by the mulHple neural cell types and/or fibrob last contamination present in early passage cultures prepared from dissociated human central nervous system (eNS) tissue. We have developed highly enriched astrocyte cultures for neurovirological study by culturing in a serum-free defined medium, 816, supplemented with basic fibroblast growth factor (FGF-2). Subculture in this medium selects against fibroblast proliferation and favors sustained proliferation of a highly enriched glial fibrillary acidic protein (GFAP)-positive cell population. Objective: The purpose of this study was to evaluate the possible association between Lewis phenotype status in pregnant women and preterm labor (PTL) or preterm rupture of the membranes (PROM).Methods: Red blood cell (RBC) Lewis phenotype was determined in 113 pregnant women admitted for PTL or PROM and in 121 controls. The results were controlled for the influence of race on Lewis phenotype.Results: Pregnancy was associated with a higher frequency in women with the a–b– phenotype. There was no association between RBC Lewis phenotype and the occurrence of PTL or PROM.Conclusions: A susceptibility to PTL or PROM …

Objective: Numerous published reports have linked various disease states and pregnancy-related conditions with meteorologic factors such as weather, humidity, and temperature. The purpose of this study was to determine if temperature and dew point affect the incidence of pyelonephritis during pregnancy.Methods: A retrospective chart review of a 4-year period from 1989 to 1992 was performed. The records of women who were diagnosed with pyelonephritis during pregnancy were abstracted for the dates of admission. Effectiveness Of Percutaneous Nephrostomy In Reversing Obstructive Renal Failure, I Ahmed, I H. Rizvi, A Aslam, M N. Ahmed, Y A. Hussain

A retrospective analysis of 34 patients of obstructive renal failure, initially managed by percutaneous nephrostomy was performed. Pre procedure blood biochemical profile was compared with upto one week follow-up of blood chemistry. There was a decline of 71.1% and 56.08% (P-values 0.0001 and 0.0028) in the mean values of serum creatinine and blood urea nitrogen respectively at 7 days after the procedure. Improvement in blood biochemical profile was solely dependent on performance of percutaneous nephrostomy.

More importantly, this study has shown that prophylactic mastectomy surgery is a viable and worthwhile treatment to prevent breast cancer recurrence. It is hoped that this information will increase the knowledge of breast cancer. For the same reason, this paper has attempted to explain what cancer is while describing how the genetics of a person cause or contribute to its development. Most of all, it is hoped that this paper will emphasize the severity of this disease in the United States today and tomorrow .

A cerebral vascular accident (CVA) is commonly referred to as a stroke. Stroke is the third leading cause of death and the number one cause of serious disability in the United States. Approximately 550,000 people suffer a stroke each year; 150,000 of these result in death. Each stroke is unique. After a stroke, some victims will suffer severe deficits and/or permanent disability for the remainder of their lives.
