Analysis Of Mesh Characteristics In Mesh-Based Phase Imaging, Desiree Caryl D'Moore Jan 2023

Analysis Of Mesh Characteristics In Mesh-Based Phase Imaging, Desiree Caryl D'Moore

Legacy Theses & Dissertations (2009 - 2024)

Mammographic screening has played a pivotal role in reducing mortality rates associated with breast cancer. Despite an overall decline of breast cancer death rates of 43% from 1989 through 2020, breast cancer remains a predominant form of invasive cancer among women. Conventional X-ray imaging used in mammography can yield limited contrast when it comes to substances with similar X-ray attenuation, such as that between benign and cancerous tissue. Mesh-based X-ray phase imaging allows for higher contrast and resolution in similar-density materials. This thesis aims to explore characteristics of the mesh used in this kind of system. To investigate the significance of …

BACKGROUND: Long term outcomes of lung transplantation are impacted by the occurrence of chronic lung allograft dysfunction (CLAD). Recent evidence suggests a role for the lung microbiome in the occurrence of CLAD, but the exact mechanisms are not well defined. We hypothesize that the lung microbiome inhibits epithelial autophagic clearance of pro-fibrotic proteins in an IL-33 dependent manner, thereby augmenting fibrogenesis and risk for CLAD.

METHODS: Autopsy derived CLAD and non-CLAD lungs were collected. IL-33, P62 and LC3 immunofluorescence was performed and assessed using confocal microscopy. Pseudomonas aeruginosa (PsA), Streptococcus Pneumoniae (SP), Prevotella Melaninogenica (PM), recombinant IL-33 or PsA-lipopolysaccharide was …

A Meta-Analysis Of Standard Versus Ultrasound-Assisted Catheter-Directed Thrombolysis In The Management Of Acute Pulmonary Embolism, Elizabeth S. Bruno, Mark Terence P. Mujer, Parth V. Desai, Yevgeniy Brailovsky, Amir Darki Jan 2023

A Meta-Analysis Of Standard Versus Ultrasound-Assisted Catheter-Directed Thrombolysis In The Management Of Acute Pulmonary Embolism, Elizabeth S. Bruno, Mark Terence P. Mujer, Parth V. Desai, Yevgeniy Brailovsky, Amir Darki

Division of Cardiology Faculty Papers

Background: Standard catheter-directed thrombolysis (SCDT) harnesses the therapeutic benefit of systemic thrombolytics while minimizing bleeding complications in patients presenting with pulmonary embolism (PE). Ultrasound-assisted catheter-directed thrombolysis (USAT) theoretically improves upon SCDT by disrupting fibrin and increasing the surface area exposed to thrombolytic agent. However, it is unclear if this translates into improved outcomes.

Methods: A systematic search of prior publications comparing SCDT and USAT in patients with intermediate or high-risk PE was conducted. Primary outcomes of interest were bleeding events, ICU and hospital length of stay. Secondary outcomes included changes in pulmonary artery systolic pressure (PASP), mean pulmonary artery pressure …

The Utility Of Thromboelastography In Acute Perioperative Trauma Resuscitation Of The Adult Coagulopathic Patient, Alexandra Sofia Keokosky Jan 2023

The Utility Of Thromboelastography In Acute Perioperative Trauma Resuscitation Of The Adult Coagulopathic Patient, Alexandra Sofia Keokosky

Nurse Anesthesia MSNA Student Capstones

One quarter of all patients admitted to level I trauma centers receive transfused blood, and approximately 25% of trauma transfusion recipients are diagnosed with coagulopathies during the resuscitation process (Hess et al., 2008; Kutcher & Cohen, 2021; Maegele et al., 2007). Such pathologies have been associated with negative clinical outcomes such as increased transfusion requirements, organ failure, sepsis, and death. (Barash et al., 2013; Cole et al., 2019; Hess et al., 2008; Sayce et al., 2020). Current laboratory standards of care to diagnose coagulopathies such as prothrombin time (PT), international normalized ratio (INR), and activated partial thromboplastin time (aPTT) are …

Effectiveness Of Ondansetron Prior To Spinal Anesthesia In Obese Parturients Undergoing Caesarean Delivery, Peter O. Lind Jan 2023

Effectiveness Of Ondansetron Prior To Spinal Anesthesia In Obese Parturients Undergoing Caesarean Delivery, Peter O. Lind

Nurse Anesthesia MSNA Student Capstones

Spinal anesthesia is the preferred route of anesthesia for parturients undergoing casesarean section (Hawkins et al., 2011). Parturients with obesity undergo caesarean delivery and have higher rates of initial intrathecal failure comparatively to non-obese (Hood, et al, 2015). Obesity is on the rise leading to a reconsideration as to the dosing of spinal anesthetic type (Dennis, 2017). Historically, the utilization of ondansetron, as a selective 5-HT3 antagonist, has been utilized prior to spinal administration to prevent the sympathectomy that can occur (Sahoo et al., 2012). This has decreased the utilization of vasopressors which can be detrimental to the fetus during …

Effects Of Intravenous Lidocaine Infusions On Postoperative Opioid Consumption In Adults Undergoing Laparoscopic Abdominal Surgery, Kayla Richardson Jan 2023

Effects Of Intravenous Lidocaine Infusions On Postoperative Opioid Consumption In Adults Undergoing Laparoscopic Abdominal Surgery, Kayla Richardson

Nurse Anesthesia MSNA Student Capstones

Lidocaine is a local anesthetic that is often administered to surgical patients undergoing anesthesia. While it was originally utilized as an intravenous antiarrhythmic or for local infiltration, it is now frequently used for the proven opioid-sparing and anti-inflammatory effects during surgeries (Beaussier et al., 2018). A common complaint of postoperative patients is pain, and the administration of intravenous lidocaine can potentially decrease patients' perception of pain after surgical procedures (Lee & Schraag, 2022).

Opioids are commonly administered to patients to help control their pain immediately after surgical procedures. Though opioids are the typically chosen treatment for acute surgical pain, they …

Recurrence And Metastasis Of Breast Cancer After Volatile Inhalation Agents For Primary Cancer Resection, Elise Amico Jan 2023

Recurrence And Metastasis Of Breast Cancer After Volatile Inhalation Agents For Primary Cancer Resection, Elise Amico

Nurse Anesthesia MSNA Student Capstones

Surgical resection of a breast tumor is frequently a treatment option for cancer, and in some cases can be curative (Hurtado et al., 2021). Multiple factors increase the risk of metastasis or recurrence including the neuroendocrine stress response to surgery and manipulation of the tumor itself (Kim, 2018). Additional factors include depression of cell-mediated immunity including sympathetic nervous system (SNS) stimulation, hypothalamic-pituitary-adrenal (HPA) axis stimulation, and pain (Hurtado et al., 2021). Research suggests that anesthetic selection, particularly total intravenous anesthesia versus volatile inhalation anesthesia (IA), has an impact on cancer recurrence and metastasis in breast cancer patients, however it remains …

Analysis Of Biologically Effective Dose For Retroactive Yttrium-90 Trans-Arterial Radioembolization Treatment Optimization, Mj Lindsey Jan 2023

Analysis Of Biologically Effective Dose For Retroactive Yttrium-90 Trans-Arterial Radioembolization Treatment Optimization, Mj Lindsey

CMC Senior Theses

Trans-arterial radioembolization (TARE) is a protracted modality of radiation therapy where radionuclides labeled with Yttrium-90 (90Y) are inserted inside a patient's hepatic artery to treat hepatocellular carcinoma (HCC). While TARE has been shown to be a clinically effective and safe treatment, there is little understanding of the radiobiological relationship between absorbed dose and tissue response, and thus there is no dosimetric standard for treatment planning. The Biologically Effective Dose (BED) formalism, derived from the Linear-Quadratic model of radiobiology, is used to weigh the absorbed dose by the time pattern of delivery. BED is a virtual dose that can …

Visualization And Characterization Of The Immunological Synapse Between Chlorotoxin Chimeric Antigen (Cltx-Car) Redirected T Cells And Targeted Glioblastoma Tumors, Arianna Livi Jan 2023

Visualization And Characterization Of The Immunological Synapse Between Chlorotoxin Chimeric Antigen (Cltx-Car) Redirected T Cells And Targeted Glioblastoma Tumors, Arianna Livi

CMC Senior Theses

Chimeric Antigen Receptor T (CAR-T) cells have demonstrated anti-tumor activity against aggressive and invasive cancers such as glioblastoma (GBM); however, clinical response rates remain low in clinical trial studies. Tumor heterogeneity and tumor microenvironment conditions pose significant challenges for treatment of GBM, thus continuous optimization of CAR-T cell therapies and identification of novel, widely expressed, and highly specific GBM antigens are vital to better patient outcomes. A newly developed CAR-T cell construct incorporating chlorotoxin (CLTX) as the targeting domain exhibited broad GBM-targeting capabilities and elicited potent cytotoxic effects during preclinical studies and is currently being tested in a phase I …
