Introduction: Corticosteroids affect numerous downstream cytokines to exert anti-inflammatory and immunosuppressive effects. Intralesional corticosteroid injection provides several advantages, including bypassing the stratum corneum to deliver a higher concentration of drug locally, while reducing systemic exposure. Intralesional corticosteroids are used in a variety of dermatologic conditions, and suboccipital steroid injection has been described as a novel treatment for cluster headaches. Case Presentation: A 52-year-old Caucasian female presented to the outpatient dermatologic clinic with indented skin lesions on the scalp and forehead for six weeks. The indentation started on her left frontal scalp and spread down gradually to her forehead and temple. …

Background: Inter-surgeon variability is a potential driver of variation in bundled care costs associated with cervical spine surgery.

Objective: This study aims to determine modifiable inter-surgeon factors that predict hospital cost variations associated with anterior cervical discectomy and fusions (ACDF).

Methods: This is a retrospective review of ACDF cases performed at a single institution by 15 surgeons from 2011 to 2017. Univariate analyses and mixed linear regression models were then used with the surgeon as a random effects variable to determine which factors were associated with surgical episode costs and surgical time. Background: Neurocytoma is an unusual intracranial neoplasm; it represents 0.1-0.5% of brain neoplasms. The first report of an extraventricular neurocytoma (EVN) was published in 1989. Suprasellar neurocytomas are classified as EVN, representing a scarce variant of neurocytoma. There are only 22 cases of sellar neurocytomas reported in the literature, including the present one.

Observations: A 48-year-old woman arrived at the emergency department (ED) with a previously made MRI, which reported a sellar neoplasm. Background: During the COVID-19 pandemic, a wide variety of neurological symptoms have appeared as part of SARS-CoV-2 infection, including cranial nerve impairments. Case Description: We describe a 33-year-old woman who developed glossopharyngeal neuralgia (GPN) and trigeminal neuralgia (TN) immediately after the curse of COVID-19. MRI demonstrated two vascular structures contacting the right trigeminal nerve and possible vascular contact on the ipsilateral glossopharyngeal nerve. Severe arachnoiditis was identified during microvascular decompression of the right glossopharyngeal and trigeminal nerves. Staphylococcus aureus is a leading cause of medical device-associated biofilm infections. This is influenced by the ability of S. aureus biofilm to evade the host immune response, which is partially driven by the anti-inflammatory cytokine interleukin-10 (IL-10). Here, we show that treatment of human monocyte-derived macrophages (HMDMs) with IL-10 enhanced biofilm formation, suggesting that macrophage anti-inflammatory programming likely plays an important role during the transition from planktonic to biofilm growth. Klebsiella pneumoniae (KLP) is a Gram-negative pathogen that can be highly antibiotic-resistant. Our group has worked with gallium-based compounds as a means of treating bacterial infections. Here the possible mechanism is investigated for dual therapy comprised of gallium nitrate (Ga(NO₃)₃) and gallium protoporphyrin (GaPP) on KLP. It is found that in vitro the combination of Ga(NO₃)₃ and GaPP is synergistic against KLP. Oxacillin (bactericidal) and tetracycline (bacteriostatic) are clinically relevant antibiotics that are routinely prescribed to treat Staphylococcus aureus infections but not conventionally used in combination. There is an urgent need for treatment regimens that can act upon biofilms during infection, associated with chronic infections on indwelling devices, as well as acute planktonic (systemic) infection. Here we show that in an in vitro model oxacillin and tetracycline act synergistically against S. aureus UAMS-1 biofilms, reducing the concentration of both antibiotics necessary to eradicate an established biofilm. CKD patients face increased CVD-related morbidity and mortality. While the exact mechanisms linking CKD and CVD are complex, traditional risk factors (e.g., diabetes, hypertension) and nontraditional factors (e.g., inflammation, oxidative stress) play a role.
