Purpose: The purpose of this work was identify key factors associated with inpatient physical therapy utilization and length of stay for patients with low back pain (LBP) in Florida hospitals.
Rationale: Little is known about factors associated with inpatient physical therapy or length of hospitalization for patients with LBP. This group of works identified the key factors associated with inpatient physical therapy and long lengths of hospitalization for this patient population. Since physical therapy and reduced length of stay are known cost-reducers, identifying key factors may represent significant cost savings to the health care system.

Many symptoms of concussions can substantially interfere with the cognitive abilities and skills students use in school. Although students may have similar symptoms they experience concussions differently. Students returning to the classroom from a concussion often have different needs and abilities.
Research shows that a formal, individualized protocol to treat suspected concussions, both academically and physically, is vital to the successful recovery of each student. Much research is available on concussions for professional athletes, specifically related to recovery and returning to their sport. Problem: Most Iowans die from chronic diseases, or diseases developed over time, often influenced through behavioral choice. The top three, heart disease, cancer, and lower respiratory diseases account for over half of all death in Iowa and in the nation. However, most public health funding focuses on infectious disease and treatment, leaving little or nothing for primary prevention of chronic diseases.
Method: The 2014 Local Governmental Public Health Survey, annual reports, and Health Improvement projects were analyzed for primary prevention of chronic diseases at county health departments. This was compared to a review of federal and state funding for chronic …

Underwater weighing with full lung expiration has been established as a valid method to assess body composition. Many people do not prefer this method due to various reasons. Hydrostatic weighing with head above water at full lung capacity has been tested although not accepted as an alternative method. Purpose: The main purpose of this study is to examine if hydrostatic weighing with the head above water could be a better method to measure body composition. Whether healthcare system or health care provider or health-care reform, the singular mission is patient care. The motto for healthcare is Cum scientia caritas (Latin for scientific knowledge applied with care). The only challenge to this most rudimentary tenet for caring depends on how we define care.

The notochord is a rod-like embryological structure, which plays a vital role in the development of the vertebrate. Though embryological, remnants of this structure have been observed in the nucleus pulposus of the intervertebral discs of normal adults. Pathologically, these remnants can give rise to slow-growing and recurrent notochord-derived tumors called chordomas. Using standard search engines, the literature was reviewed regarding the anatomy, embryology, molecular development, and pathology of the human notochord. Clinicians who interpret imaging or treat patients with pathologies linked to the notochord should have a good working knowledge of its development and pathology.

The auditory temporal processing deficits associated with age-dependent hearing decline have been increasingly attributed to issues beyond peripheral hearing loss. Age-related hearing loss (ARHL), also known as presbycusis, is linked with changes in the expression of both excitatory and inhibitory neurotransmitters in the central auditory system. There are also age-related changes in the expression and function of the ion channels which mediate action potential firing. The slow, Ca2+ activated, K+ channels of the BK-type are essential in controlling both neurotransmitter release and neural communication via alteration of action potential durations, firing frequency, and neural adaptation. Objective: To develop a novel software method (AR2) for reducing muscle contamination of ictal scalp electroencephalogram (EEG), and validate this method on the basis of its performance in comparison to a commercially available software method (AR1) to accurately depict seizure-onset location. Methods: A blinded investigation used 23 EEG recordings of seizures from 8 patients. Each recording was uninterpretable with digital filtering because of muscle artifact and processed using AR1 and AR2 and reviewed by 26 EEG specialists. EEG readers assessed seizure-onset time, lateralization, and region, and specified confidence for each determination. The two methods were validated on the basis of …

A Movement For Change: Horatio Robinson Storer And Physicians’ Crusade Against Abortion, Ryan Johnson
James Madison Undergraduate Research Journal (JMURJ)
Abortion has not always been a controversial topic in American politics. The modern debate can be traced back to physicians’ crusade against abortion in the second half of the 19th century, led by Harvard-educated and New England-based Horatio Robinson Storer. Storer launched the crusade in 1857, in part to criminalize abortion and in part to bring respect to the medical field in a time when doctors were not highly esteemed. This paper surveys Storer’s publications and correspondence and analyzes the motives and results of his campaign.

Clinical skills are often measured using objective structured clinical examinations (OSCEs) in healthcare professions education programs. As with assessment centers, it is challenging to provide learners with effective feedback due to burdensome human capital demands. The aim of this dissertation was to evaluate the viability of using a crowdsourced system to gather OSCE ratings and feedback. Aggregating evaluations of student performance from a crowd of patient proxies has the potential to mitigate biases associated with single-rater evaluations, allow the patient a voice as the consumer of physician behavior, improve reliability, reduce costs, improve feedback latency, and help learners develop a …

In recent years, there has been exponential growth in the interest in microvesicles, which is reflected by the number of publications. Initially referred to as "platelet dust" by Peter Wolf in 1967, platelet microvesicles (PMV) are now recognized as important mediators of intercellular communication. There are examples of PMV exerting physiological effects on almost all hematological and vascular cell types, including monocytes, macrophages, neutrophils, T-cells, endothelium cells, and smooth muscle cells (SMCs). PMV can exert these effects by multiple methods: extracellular signaling through receptors, transfer of surface molecules, and delivery of intracellular contents including miRNA. Disruption of the epithelium in the female reproductive tract (FRT) is hypothesized to increase HIV infection risk by interfering with barrier protection and facilitating HIV-target cell recruitment. Here we determined whether Tenofovir (TFV), used vaginally in HIV prevention trials, and Tenofovir alafenamide (TAF), an improved prodrug of TFV, interfere with wound healing in the human FRT. TFV treatment of primary epithelial cells and fibroblasts from the endometrium (EM), endocervix (CX) and ectocervix (ECX) significantly delayed wound closure. Reestablishment of tight junctions was compromised in EM and CX epithelial cells even after wound closure occurred. In contrast, TAF had no inhibitory …

No indigenous cases of poliomyelitis have occurred in the US since 1979; however the risk of importation persists until global eradication is achieved. The seropositivity rate for different age cohorts with exposures to different poliovirus vaccine types and wild virus in the US are not presently known. A convenience sample was conducted in the Kansas City metropolitan area during 2012-2103 with approximately 100 participants enrolled for each of 5 age cohorts categorized based on vaccine policy changes over time in the US. This research project involves synthesizing cyanostilbenes and testing their anticancer properties in breast cancer cells. Breast cancer is a global issue, one that great improvement has been found in the last few decades, but also one that remains a large killer. The components of the cyanostilbene hybrids have all been shown to have anticancer properties on their own, and we would like to see if we can increase these properties by combining multiple into a single hybrid. In the past decade, an emerging process named “autophagy” has generated intense interest in many chronic lung diseases. Tissue remodeling and fibrosis is a common feature of many airway diseases, and current therapies do not prevent or reverse these structural changes. Autophagy has evolved as a conserved process for bulk degradation and recycling of cytoplasmic components to maintain basal cellular homeostasis and healthy organelle populations in the cell. Introduction: Monocarboxylate transporter 1 (MCT1) is an importer of monocarboxylates such as lactate and pyruvate and a marker of mitochondrial metabolism. MCT1 is highly expressed in a subgroup of cancer cells to allow for catabolite uptake from the tumor microenvironment to support mitochondrial metabolism. We studied the protein expression of MCT1 in a broad group of breast invasive ductal carcinoma specimens to determine its association with breast cancer subtypes and outcomes. Methods: MCT1 expression was evaluated by immunohistochemistry on tissue micro-arrays (TMA) obtained through our tumor bank. Using the lens of a community-based childhood obesity intervention, it is possible to examine the role of non-profit organizations in community development and to deconstruct the “community” in community-based research and identify the many competing interests within a community. This contextual understanding includes how the community is formed, how a community’s agenda is set, and who will complete the tasks outlined in that agenda.
