The high expenditure of healthcare in the United States (U.S.) does not translate into better quality of care. Indeed, the U.S. healthcare system is recognized by its lack of efficiency and waste (which represents about 20% of the country’s healthcare expenses). Lack of coordination is one of the most referenced causes of waste in the U.S. healthcare system, and preventable hospital readmissions have been acknowledged to be evidence of poor coordination of care. Background: Nursing education needs to provide the necessary tools for students to develop leadership skills and to practice civic agency to create meaningful change in the shifting health care field. This article focuses on facilitating a student's role in becoming a citizen nurse through curricular modifications.
Method: Through an ongoing partnership, nursing faculty and community organizers implemented a year-long pilot project to discover the deeper insights into the role of a citizen nurse and to analyze the skills students need to be effective agents of change. This research looks at the overall question of how planetary health impacts mental health and psychological welling. The central focus is eco-anxiety—mental distress caused by climate change and environmental degradation—and how it manifests in university students. In order to gather data for this research I collected 114 student survey responses, interviewed seven young adults (ages 20-25), and interviewed seven experts in the fields of psychology and environmental studies.
They survey results show high levels of general stress and anxiety, high levels of stress and anxiety related to climate change and the state of the world, and a very high level …

Congress is currently considering options to significantly reduce federal funding for the Medicaid expansion and the Marketplace subsidies implemented under the Affordable Care Act (ACA). Separately, the Health Centers Fund, which currently accounts for 70% of all federal health center grant funding, is set to expire in September 2017. These potential changes in federal funding could have a dramatic impact on health centers and the communities they serve. This report summarizes the 2006-2015 preterm deliveries among singleton gestations using birth certifi-cate data from South Dakota Vital Records. Preterm deliveries were limited to births from 24 to 36 weeks of gestation. Preterm deliveries were divided into three types: (1) spontaneous labor with intact membranes (spontaneous preterm birth), (2) preterm premature rupture of the membranes (PPROM), and (3) labor induc-tion or caesarean delivery for maternal or fetal indications (medically indicated).

In this study, we have uncovered a novel crosstalk between TGFβ and IGF-1R signaling pathways. We show for the first time that expression and activation of IRS-1, an IGF-1R adaptor protein, is decreased by TGFβ/Smad3 signaling. Loss or attenuation of TGFβ activation leads to elevated expression and phosphorylation of IRS-1 in colon cancer cells, resulting in enhanced cell proliferation, decreased apoptosis and increased tumor growth in vitro and in vivo. Downregulation of IRS-1 expression reversed Smad3 knockdown-mediated oncogenic phenotypes, indicating that TGFβ/Smad3 signaling inhibits cell proliferation and increases apoptosis at least partially through the inhibition of IRS-1 expression and activation. …

Research Round-up: Cancer survivors frequently experience cancer treatment–related functional impairments and disability. While a growing body of evidence supports the effectiveness of rehabilitation interventions for these individuals, there are significant gaps in the existing literature and no interdisciplinary agreement on the priorities for cancer rehabilitation research. In 2016, the Oncology Section revised its strategic plan and set an objective to create an oncology rehabilitation research agenda for the physical therapy (PT) profession. To achieve this goal, the scope of the agenda, a development and review process, and dissemination plan need to be developed.

Development Of A Delirium Educational Program For Hospital Medicine Providers, Lindsay Erin Stone

Hospital medicine providers were surveyed to evaluate baseline delirium attitudes and behaviors. An educational program was then shared, and a follow up survey was given to determine if their delirium attitudes and practices were impacted by the educational intervention. Follow up survey results indicated that providers perceived more of a change in their attitudes than practices, and overall found the education to be useful and felt more confident in treating delirium as a result.

Social isolation is the absence of meaningful contact with others. Older adults who perceived themselves as socially isolated are at risk of negative health outcomes. A person’s psychological wellbeing can be improved through engaging in meaningful leisure activities.
The Intergenerational Social Engagement Program, or the ISEP, was developed to connect older adults and high school students to help decrease social isolation in older adults. The ISEP incorporates concepts from the Canadian Model of Occupational Performance and is intended for implementation in senior living communities. OBJECTIVE: To evaluate the safety and efficacy of TX-004HR vaginal estradiol soft-gel capsules for moderate-to-severe dyspareunia associated with postmenopausal vulvar and vaginal atrophy.
METHODS: In this randomized, double-blind, placebo-controlled, phase 3 study, postmenopausal women with a self-identified most bothersome symptom of dyspareunia received 4, 10, or 25 μg TX-004HR or placebo for 12 weeks. Four co-primary efficacy endpoints were change from baseline to week 12 in percentages of superficial and parabasal cells, vaginal pH, and severity of dyspareunia. Secondary endpoints included severity of vaginal dryness and vulvar and/or vaginal itching or irritation. Endometrial histology and adverse events (AEs) were included …
