Prognosis Of Hospitalized New-Onset Systolic Heart Failure In Indo-Asians--A Lethal Problem, Fahim H. Jafary, Mahesh Kumar, Irfan E. Chandna

Background: Systolic heart failure (SHF), particularly when requiring hospital admission carries a poor prognosis. There is a paucity of data in Indo-Asians on outcomes of SHF, among whom the burden of cardiovascular disease is consistently rising. The purpose of this study was to determine the frequency and predictors of mortality and morbidity amongst patients admitted with new-onset SHF at a tertiary care hospital in Pakistan.

Methods and Results: Hospital charts of 196 patients with a diagnosis of new or recent onset (!3 months) SHF (ejection fraction [EF] !40%) were reviewed. Patients who died during the admission, those with life-limiting concomitant …

For the individual with sickle cell disease (SCD), the lifespan is increasing but adults report decreased quality of life (QOL), low self-efficacy, and ineffective coping skills. The care of adult patients with SCD requires a complex multidisciplinary team approach with focus not only on physiological, psychological, and social needs, but also on spiritual needs. The purpose of the present study was to investigate the effects of an angled proto-type starting block on sprint start kinematics. The angled starting block rotates the foot medially at a 16 degree angle, which emulates the 16 degree angle of the ankle on the medial axis of the foot. This 16 degree should allow for the athlete to apply more force against the block, resulting in a higher velocity at take-off and a faster block clearance time. Identifying what stressors affect an officer and how he or she copes is essential in maintaining psychologically healthy officers. Studies have found that police officers consider the organizationally based aspects of the job to greatly influence their level of stress. The present research used the Police Stress Survey and COPE Inventory to identify common stressors and coping strategies among forty-five officers in the state of Indiana. The study attempted to determine whether or not a chronic overall stress -produced by frequent organizational stressors existed, and if this stress led to an increased use of maladaptive coping strategies. This dissertation examines disaster preparedness in the U.S. and explores the relationships between risk perception, funding from the Health Resources and Services Administration (HRSA), and preparedness. Secondary data analysis was conducted using the National Study of Rural Hospitals from Johns Hopkins University. The study, based on a random sample of rural hospitals, consisted of a mail questionnaire and a follow-up phone interview with the hospital's Chief Executive Officer (n = 134).

The study investigated the effectiveness of a time-limited, skills-based treatment in a population of female substance abusers. This project implemented a pilot clinical trial to evaluate the effectiveness of a 12-session modified Dialectical Behavior Therapy (DBT) protocol among a small sample of women, with co-morbid borderline personality disorder features, receiving concurrent outpatient treatment for methamphetamine abuse. A non-concurrent multiple baseline design was used. In addition to on-going assessment conducted during baseline and weekly over the course of treatment, a comprehensive assessment battery was administered pre-, and post-treatment as well as at 1 and 3 months following treatment. The objective of this study was to determine change in individual and aggregate attitudes of the members of two subspecialty surgery teams (cardiac and vascular) in six domains (teamwork climate, safety climate, job satisfaction, stress recognition, perception of management, and working conditions) following medical team simulation and debriefing exercises. The study was a pre-post partially randomized controlled, quasi-experimental study that took place in a 545-bed hospital with a high-volume cardiovascular surgery program. Participants included interdisciplinary team members caring for cardiac and vascular surgery patients.

Declining levels of physical activity probably contribute to the increasing prevalence of overweight in US youth. In this study, the authors examined cross-sectional and longitudinal associations between physical activity and body composition in sixth- and eighth-grade girls. In 2003, girls were recruited from six US states as part of the Trial of Activity for Adolescent Girls. Physical activity was measured using 6 days of accelerometry, and percentage of body fat was calculated using an age- and ethnicity-specific prediction equation. Treatment retention is of paramount importance in cocaine treatment research as treatment completion rates are often 50% or less. Failure to retain cocaine patients in treatment has both significant research and clinical implications. In this paper we qualitatively and quantitatively demonstrate the inconsistency found across analyses of retention predictors in order to highlight the problem. First, a qualitative review of the published literature was undertaken to identify the frequency of predictors studied and their relations to treatment retention. The hypothesis of this project is that a functionally relevant association exists between CFTR and Hsp90. In testing this hypothesis, (a) the association of CFTR and Hsp90 is confirmed via crosslinking and immunoprecipitation of CFTR from stably transfected HEK-293 cells, electrophoretic separation of cross-linked proteins, and protein identification via SDS-PAGE, western blotting, and mass spectrometry. Also, this hypothesis is tested by (b) functionally assessing the effect of Hsp90 on CFTR function via Ussing chamber analysis, two-electrode voltage clamp analysis, preliminary in vivoanimal studies, and protein phosphorylation. The role of autoreactive, antigen-specific T-cells in the development of autoimmunity has long been documented. T-cells expressing chimeric receptors are specifically redirected against such cells and have been proven to suppress autoimmune encephalomyelitis, the murine model of multiple sclerosis. We here demonstrate the ability of humanized chimeric receptors to suppress experimental autoimmune encephalomyelitis (EAE) in a humanized mouse model by redirecting T lymphocytes against autoreactive T-cells. The mesocorticolimbic pathway is critical in almost all aspects of drug abuse, including nicotine. Though many of the neurochemical and molecular effects of nicotine have been well studied, nicotine’s long-term neuroadaptive effects, specifically within the mesocorticolimbic pathway, are largely undefined. Thus, in current study, we determined the neuroadaptive changes in the mesocortical glutamatergic system during chronic nicotine self-administration (SA), which emulates important aspects of nicotine intake by humans, and after extinction. CONTEXT:Unintended pregnancy is associated with risk behaviors and increased morbidity or mortality for mothers and infants, but a woman's feelings about pregnancy may be more predictive of risk and health outcomes than her intentions.

METHODS: A sample of 1,044 black women who were at increased risk were enrolled at prenatal care clinics in the District of Columbia in 2001-2003. Bivariate and multivariate analyses assessed associations between pregnancy intentions or level of happiness about being pregnant and multiple psychosocial and behavioral risk factors, and identified correlates of happiness to be pregnant.

OBJECTIVE: The normal structure and function of articular cartilage are the result of a precisely balanced interaction between anabolic and catabolic processes. The transforming growth factor-beta (TGF-beta) family of growth factors generally exerts an anabolic or repair response; in contrast, proinflammatory cytokines such as interleukin 1 beta (IL-1beta) and tumor necrosis factor-alpha (TNF-alpha) exert a strong catabolic effect. Efficacy Of Cognitive Therapy And Pharmacotherapy In Depression: A Meta-Analysis, Daniel B. Michel

A meta-analysis comparing the long-term effectiveness of cognitive behavioral therapy (CBT) and pharmacotherapy in preventing relapse following treatment discontinuation was performed using published studies of depressed participants. Twenty-three articles met inclusionary criteria and were included in the analyses. Weighted effect sizes and moderators, treatment type, were calculated using statistical analysis software. Initially, fixed effects models were applied to the data, but due to significant between-group heterogeneity that could not be fully explained by treatment type, mixed effect models were used to account for the residual heterogeneity.
