Oral vaccines present an attractive alternative to injectable vaccines for enteric diseases due to ease of delivery and the induction of intestinal immunity at the site of infection. However, susceptibility to gastrointestinal proteolysis, limited transepithelial uptake and a lack of clinically acceptable adjuvants present significant challenges. A further challenge to mass vaccination in developing countries is the very expensive requirement to maintain the cold chain. Oral delivery is the most preferred route for drug administration. Oral drug delivery is limited by poor physicochemical properties of drugs and physiological barriers in the gastrointestinal tract. The field of salvinorin chemistry represents a novel and emerging field of opioid research. The novelty is derived from the lead pharmacophore: salvinorin A — a neoclerodane diterpenoid natural product isolated from Salvia divinorum. Salvinorin A represents a pharmacologically unique compound in that it is the first known non-nitrogenous KOR subtype-selective agonist, exhibits a comparatively safe physiological profile with no reports of toxicological effects in clinical trials, and, most importantly, has a steadily growing body of literature indicating potentially useful clinical applications (e.g. antinociceptive, anti-addictive, antipruritic, neuroprotective, etc.). Neurological disorders include disorders such as migraine, tension-type headaches, Parkinson’s disease (PD), epilepsy, dementia, and Alzheimer’s disease (AD). Globally, PD is the second most comneurodegenerative disorder following AD. In developed countries, PD affects 1% of all population over 60 years of age. The prevalence of PD is increasing over the years and is expected to double by 2030. The prevention and cure of malaria is difficult as it thrives under poor socio- and pharmacoeconomic conditions in third world, tropical and subtropical regions. It is a parasitic disease spread through the bite of an infected Anopheles mosquito. In humans, malaria can be caused by several Plasmodium species that include: Plasmodium falciparum, Plasmodium vivax, Plasmodium ovale, Plasmodium malariae and Plasmodium knowlesi. The natural product artemisinin is a sesquiterpene lactone with an endoperoxide bridge isolated from the Chinese annual herb qinghao (Artemisia annua L or sweet wormwood). Lipid based systems and topical ocular inserts of various drugs were developed for the intervention/treatment of posterior segment ocular complications. Indomethacin (IN) was developed into solid lipid nanoparticles (SLNs) and Nanostructured lipid carriers (NLCs). Effect of surface functionalization using chitosan (CS) on lipid nanocarriers was tested to evaluate corneal penetration. Surface modification of SLNs with CS increased ocular penetration of IN. NLCs maintained significantly higher IN concentrations in all ocular tissues tested, compared to the other formulations evaluated in vivo. Objectives: To examine adherence to ADHD stimulant medications and determined the condition-related, therapy-related, patient-related, health-system related, and social and economic related predictors of medication nonadherence in children/adolescents (4 – 18 years) with ADHD enrolled in Mississippi Medicaid Program. Methods: This was a retrospective cohort study of the Mississippi Medicaid administrative claims data. Data was analyzed from March 01, 2014 to February 29, 2016. Glycosaminoglycans (GAGs) are complex linear chain carbohydrate molecules found on virtually all animal cell surfaces. Owing to their negatively charged nature, GAGs interact with a number of different proteins. Thus, although they have great potential as therapeutic agents, their apparent promiscuous interactions increase their side effect risk. GAG mimetics, including GAG oligosaccharides and non-saccharide GAG mimetics (NSGMs) are viable approaches to address this. This work discusses sulfated benzofuran thrombin inhibitors with submaximal protease inhibition, sulfated diflavonoid inhibitors of plasmin and GAG oligosaccharides with selectivity for human neutrophil elastase (HNE).

The purpose of this study was to evaluate the use of semi-structured interviews as an outcome measure in a clinical trial for children with fragile X syndrome (FXS) ages 2-6. This qualitative approach was used to analyze twenty-five interviews of parents with children in a double-blind medication trial - sertraline or placebo. The aim was to assess occupational improvements in their child that may not have been detected with the use quantitative outcome measures alone. Results showed greater improvements in the sertraline group in areas of behavior, social participation, sensory-related behaviors, receptive language, education, family impact and therapeutic strategies. In December 2016, a panel of experts in microbiology, nutrition and clinical research was convened by the International Scientific Association for Probiotics and Prebiotics to review the definition and scope of prebiotics. Consistent with the original embodiment of prebiotics, but aware of the latest scientific and clinical developments, the panel updated the definition of a prebiotic: a substrate that is selectively utilized by host microorganisms conferring a health benefit. This definition expands the concept of prebiotics to possibly include non-carbohydrate substances, applications to body sites other than the gastrointestinal tract, and diverse categories other than food. The purpose of this scholarly project was to evaluate an evidence-based quality improvement program implemented in 2016 in a clinical practice setting to curtail overprescribing of opioids for noncancer pain management. In 2001, the National Pharmaceutical Council and The Joint Commission on Accreditation and Hospital Accreditation initiated a standard of practice for opioid use in noncancer pain management that resulted in opioid overprescribing and a 200% increase in opioid-related deaths and incalculable societal costs. การประเมินประสิทธิศักย์ของการให้ยาปฏิชีวนะเพื่อป้องกันการติดเชื้อในแผลสุนัขและแมวกัด, พรชนิตว์ หมื่นหน้า

วัตถุประสงค์: เปรียบเทียบอุบัติการณ์การเกิดแผลติดเชื้อในผู้ที่ได้รับยา amoxicillin และ amoxicillin-clavulanic acid เพื่อป้องการการติดเชื้อจากสุนัขและแมวกัด/ข่วน และศึกษาปัจจัยที่เกี่ยวข้องกับการเกิดแผลติดเชื้อ
วิธีการดำเนินการวิจัย: การวิจัยเชิงทดลองแท้จริงและใช้วิธีการปกปิดแบบทางเดียว ศึกษาในผู้ป่วยที่เกิดแผลจากสุนัขหรือแมวกัด/ข่วนที่มีระดับการสัมผัสโรคพิษสุนัขบ้าระดับ 3 และเข้ารับการรักษาภายใน 24 ชั่วโมง ณ แผนกฉุกเฉิน โรงพยาบาลบ้านบึง จังหวัดชลบุรี ระหว่างเดือนกุมภาพันธ์ พ.ศ. 2561 ถึงเดือนพฤษภาคม พ.ศ. 2561 โดยการสุ่มแบบบล็อกให้ผู้ป่วยได้รับยา amoxicillin หรือยา amoxicillin-clavulanic acid เป็นระยะเวลา 5 วัน และประเมินผลการเกิดแผลติดเชื้อในวันที่ 3 และ 7 หลังจากได้รับยาปฏิชีวนะ
ผลการวิจัย: ผู้ป่วยทั้งหมด 236 คน เข้ารับบริการที่โรงพยาบาลด้วยสาเหตุสุนัขกัด แมวกัดและแมวข่วนร้อยละ 75, 18.2 และ 6.8 ตามลำดับ อุบัติการณ์การเกิดแผลติดเชื้อระหว่างกลุ่มที่ได้รับยา amoxicillin (ร้อยละ 1.7) กับยา amoxicillin-clavulanic acid (ร้อยละ 4.2) ไม่มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p=0.447) ผู้ป่วยที่ได้รับยา amoxicillin-clavulanic acid เกิดอาการไม่พึงประสงค์มากกว่ากลุ่มที่ได้รับยา amoxicillin ร้อยละ 17.5 และ 10.3 ตามลำดับ (p=0.113) แผลบริเวณเท้าจากสุนัขกัดและแผลเจาะจากแมวกัดมีความสัมพันธ์กับการติดเชื้ออย่างมีนัยสำคัญทางสถิติ (p=0.048 และ p=0.029 ตามลำดับ)
สรุปผลการวิจัย : ยา amoxicillin สามารถใช้เป็นยาปฏิชีวนะเพื่อป้องกันการเกิดแผลติดเชื้อจากสุนัขและแมวกัดได้ โดยไม่มีความแตกต่างของอุบัติการณ์การเกิดแผลติดเชื้อระหว่างกลุ่มที่ได้รับยา amoxicillin กับยา amoxicillin-clavulanic acid โดยการเกิดแผลบริเวณเท้าจากสุนัขกัดและการมีแผลเจาะจากแมวกัดอาจเพิ่มความเสี่ยงต่อการเกิดแผลติดเชื้อ

Silver nanoparticles (AgNPs) and nanoparticles in the form of silver-metal alloy, such as silver-copper (AgCuNPs) and silver-tin (AgSnNPs) have been applied in various industries and medical applications. To assess effects of these nanoparticles on human immune system, this study aimed to assess toxicity of AgNPs, AgCuNPs and AgSnNPs on human monocytes, THP-1 cells, and THP-1 differentiated macrophage cells. The nanoparticles were characterized and assessed for cytotoxicity and generation of intracellular ROS. Introduction: High dose insulin (HDI) therapy, defined as > 0.5 units/kg/hour, has been postulated to improve hemodynamics in both calcium channel blocker (CCB) and beta-blocker overdose. Proposed benefits of HDI include coronary and systemic vasodilation due to enhancement of nitric oxide synthase activity leading to improved contractility and decreased systemic vascular resistance, respectively. HDI also directly improves cardiac contractility by increasing glucose uptake and improving calcium handling within cardiac cells. Background and purpose: It is still inconclusive which method of iontophoresis delivers the most medication deepest through the skin, and therefore most effective in treating lateral epicondylitis. The purpose of this systematic review is to analyze the efficacy of treatments for lateral epicondylitis using iontophoresis.

Method: The review included articles from peer-reviewed journals with sufficient data related to the purpose and focus of the study. Inclusion criteria included randomized control trials, cohort studies, case studies, systematic reviews, meta-analyses, and pilot studies published since 2000.

The misassembly of soluble proteins into toxic aggregates, including amyloid fibrils, underlies a large number of human degenerative diseases. Cardiac amyloidosis, which is most commonly, caused by aggregation of Immunoglobulin (Ig) light chains or transthyretin (TTR) in the cardiac muscle, represent an important and often underdiagnosed cause of heart failure. TTR-mediated amyloid cardiomyopathies are chronic and progressive conditions that lead to arrhythmias, biventricular heart failure, and death. The synthesis and evaluation of structure-activity relationships of saturated nitrogen heterocycles is the focal point of various pharmaceutical companies thanks to the high biological activity of previously isolated azacycles. Here, we describe an operationally simple and highly efficient approach to macrocyclic lactams bearing vicinal stereocenters and a challenging cycloalkyne motif. The outcomes are achieved through a novel [4 + 2] cycloaddition reaction between an N-iodoarylated-1,3-azadiene and cyclic anhydrides, followed by interception of the cycloadducts in cross-coupling manifolds (e.g., Sonogashira coupling) and concomitant lithiation-cyclization of the tethered alkyne.
