The traditional treatment protocol for young children with congenital or acquired amputations at or proximal to the knee prescribes a prosthesis without a working knee joint, based in part on the assumption that a child learning to walk cannot properly utilize a passively flexing prosthetic knee component. An alternative to this Traditional Knee (TK) protocol is an “Early Knee” (EK) protocol, which prescribes an articulating prosthetic knee in the child’s first prosthesis, during development of crawling and transitioning into and out of upright positions. Developmental Dysplasia of the hip is a condition that affects 1 to 3 of every 1000 infants born globally. It is a pathology that involves the instability, subluxation, or dislocation of the femoral head from the acetabulum. Through a rating system by the International Hip Dysplasia Institute or Graf System, hips are rated based on their level of instability or dislocation and the infant is then prescribed a treatment. While Finite element models of dysplastic hips have been developed previously, a study that seeks to understand the contribution of an abduction contracture of the hip has yet to be performed. …

Hip abduction orthosis devices (HAOD) are used to reduce the hip joint of infants affected by developmental dysplasia of the hip (DDH). HAOD have been successful for mild cases of DDH and ineffective for severe cases. Efforts in understanding the biomechanics of lower limbs have been made to improve the success rate of current treatment methods, especially for Grade IV dislocations (G4). The injury rates are four times greater in women than men. Preventative measures that help to prevent ACL injuries are limited to stretching and strengthening. The goal of this senior design projet is to develop a cutsom 3D printed orthotic shoe insert. Pressure mapping, laser scanning, and 3D modeling will be used to create a truly custom orthotic for each foot. Pressure mapping will give the weight distribution on an individual's food and the laser scanner will give the x-y coordinates of the foot. these two together will be used to generate a a 3D model which will then be 3D printed to create a custom orthotic. An Exploratory Study Of The Perceptions Of Footwear For Individuals Who Use Lower Limb Orthotics, Margaret E. Gegen, Teresa Plummer, Nancy Darr

The purpose of this study was to explore individual’s perceptions of orthotics and footwear. A survey was created by the authors with the expert opinions of physical and occupational therapists and was distributed to two therapy clinics in Tennessee that provide both occupational therapy and physical therapy as well as on several online support groups. Seventy-nine (79) caregivers completed surveys. Thematic analysis was performed using Nvivo 10 (QSR International) and descriptive statistics were generated using the Statistical Package for the Social Sciences (SPSS) IBM International). A 3D printed hand and arm prosthetic was created from the idea of adding bionic elements while keeping the cost low. It was designed based on existing models, desired functions, and materials available. A tilt sensor keeps the hand level, two motors move the wrist in two different directions, a limit switch signals the fingers to open and close, and another motor helps open and close the fingers. All sensors and motors were built on a circuit board, programmed using an Arduino, and powered by a battery. Background:
In Canada, >50% of community-dwelling lower limb amputees (LLA) fall at least once each year, a rate that is almost twice that of community-dwelling older adults. While the physical consequences of falls may be readily apparent, psychological sequelae that follow may be just as, if not more, detrimental than an actual fall itself. Current measures of balance confidence show no change in LLA following discharge from rehabilitation. The limited detectable change may be due to content validity challenges of the measures as they were not developed for the unique challenges faced by LLA.

Background: Falls in people with a lower limb amputation (PLLAs) are common and most often occur while walking, resulting in physical and psychological consequences that adversely affect quality of life. Walking is a complex motor task requiring cognitive resources. Due to walking with a prosthesis, PLLAs report focussing on every step they take, indicating greater cognitive needs. However, cognitive resources are limited, and most of our everyday activities also involve the simultaneous performance of motor and cognitive tasks, known as dual-tasking. This custom wrist orthosis project is the continuation of efforts made by students in the R.B Annis School of Engineering during the 2017-2018 academic year. Throughout this project, we have implemented the Design for Six Sigma (DFSS) methodology to guide the design process. Within the R.B. Annis School of Engineering, this methodology has been adapted into guide the DesignSpine process. We have designed a new orthosis that is custom fabricated for enhanced patient comfort. Objective: The main objective of this study was to determine functional and neuromuscular outcomes of stroke patients using their non-preferred hand with and without a 3D printed passive exoskeleton compared to controls using their non-preferred hand with and without the passive exoskeleton. Methods: Adults at least six months post stroke (Stroke, n = 5) and age- and sex- matched healthy controls (Control, n = 5) performed nine trials of a gross motor task while having their brain activity measured. Background: Foot orthoses have proven to be effective for conservative management of various pathologies. Pathologies of the lower limb can be caused by abnormal biomechanics such as abnormal foot structure and alignment, leading to inadequate support. Nearly 17 million people in the United States alone crossed finish lines in road races in 2017.1 Unfortunately, there is an extremely high rate of running related injuries (RRI) that prevent or hinder individuals from training and competing. Introduction: The alignment of a prosthesis is clinically determined based on observations by clinicians and the subjective perception of amputees during gait. However, this process has been reported to be unreliable. Socket reaction moment has been reported to be significantly impacted by alignment changes, but the impact of these alignment changes on other gait parameters is unclear. The aim of this study was to investigate the effects of coronal alignment changes of a transtibial prosthesis on socket reaction moment, spatiotemporal parameters, and perceptions of alignment during gait in amputees.

Ligament repair is a common surgical practice with a significant lack of viable replacements. The current gold standard for repair is the use of tendon grafts from cadavers or from another place in the patient’s body; however, these often cause more problems than they solve including immune responses or a lack of mobility in another place in the body. 3D bioprinting is an additive manufacturing technique that can utilize a range of bioactive materials to construct specific architectures that mimic native tissue. Near-field electrospinning (NFE) offers precise alignment control to create non-woven mats with high tensile strengths. We built a custom E-spin printer that enables layer-by-layer alternating deposition between 3D bioprinting and NFE to create composite scaffolds for the bone-ligament interface. This complex region is difficult to simulate due to its functionally graded mechanical and biochemical properties. In this commentary, we reflect on some of these challenges and provide some simple tips on strategies that can be used by allied health professionals to overcome these challenges. Background: Although many libraries have offered 3D printing as a service or available technology, there is a lack of information on course-integrated programs for 3D printing in which the library played a primary role. Therefore, librarians at the Touro College School of Health Sciences began exploring 3D printing for inclusion in the occupational and physical therapy curriculum.
