A Systemic Review Of Functional Outcomes And Patient Satisfaction Across Lower-Limb Prosthetic Types In Military Personnel And Veterans, Emma Ho Harris, Amir Ghafarian, Sen Gao, Khaled Saleh, Jason Pasley

Background: Military personnel and veterans are at increased risk of lower-limb amputation, often due to combat-related trauma such as improvised explosive devices (IEDs), as well as secondary factors including tobacco use, peripheral artery disease, and diabetes. This study systematically reviews and compares lower-limb prosthetic technologies in military personnel and veterans, focusing on functional outcomes, mobility, and patient satisfaction.

Methods: A comprehensive literature search was conducted using PubMed MeSH terms to identify studies examining lower-limb prosthetic use and outcomes among military personnel and veterans. The final MeSH-based search yielded 27 results.

The lower limb prosthesis is an important assistive technology for persons with limb amputations and differences. The interface (prosthetic liner) between the rigid prosthetic socket and the residual limb is often made of silicone, urethane or thermoplastic elastomer gel. This material provides cushioning and support as well as suspension for the transtibial (below knee) prosthetic leg. Unfortunately the cost of liners range from $3-$600. This cost precludes its use in resource limited settings such as Sri Lanka. As a result patients are fit with a foam liner which has less cushioning and suspension capabilities than a gel type liner. The purpose of this capstone project was to deepen my understanding of upper limb prosthetics, observe patient care, and identify gaps in occupational therapy education related to upper limb loss, prosthetic use, and rehabilitation. A key objective was to support the Hanger Clinic by increasing the number of completed Upper Limb Prosthetic User Outcome Forms, as well as to capture photos and videos—separate from this video training series—to contribute to Hanger’s educational library. Amputations can cause a lack of motivation which will hinder occupational performance and participation after limb loss (Şimsek et al., 2020). The fit of an individual’s prosthetic is crucial to allow for an increase in participation (Diment et al., 2022). Occupational therapists treat the limb loss community but there is little evidence in the literature to support social participation with the use of a prosthetic limb. There is a need for new research to better understand how the use of a prosthetic can impact an individual’s social participation.

Lower-limb amputations can cause a plethora of obstacles that lead to a lower quality of life. Implementing machine learning techniques means advanced prosthetics can contribute to facilitating the lives of those that live with lower-limb amputations. Using the publicly available HuGaDB data set, the current study investigates several classification models (random forest, neural network, and Vowpal Wabbit) to predict the locomotive intentions of individuals using lower-limb prostheses. The results of this study show that the neural network model yielded the highest accuracy, comparable precision, and recall scores to the other models. The proprioceptive loss accompanied by lower limb amputation can impair function and mobility. We explore a simple, mechanical skin-stretch array configured to generate superficial tissue behaviour that might occur with movement about an intact joint. Four adhesive pads attached around the circumference of the lower leg were connected via cords to a remote “foot” mounted on a ball joint attached to the underside of a fracture boot, such that “foot” reorientation would result in skin stretch. Limb loss poses a significant challenge to mobility and the performance of daily activities. Currently, nearly half of the amputees using powered prosthetics abandon their device due to inadequate limb coordination and movement control abilities. In the healthy system, passive and neural dynamics facilitate the synchronized activation of muscles necessary for coordinated movement. However, in the amputee, the system is disrupted. The purpose of this capstone project is to build a collapsible prosthetic shower leg that can provide a transtibial amputee with reliable support while showering. Phil Tamoush, an 83-year old transtibial amputee, was the inspiration for this work. Phil’s constant travel for his arbitration work leaves him without the option to bring along his usual shower leg due to the additional space it takes up in his travel bag. The traditional treatment protocol for young children with congenital or acquired amputations at or proximal to the knee prescribes a prosthesis without a working knee joint, based in part on the assumption that a child learning to walk cannot properly utilize a passively flexing prosthetic knee component. A 3D printed hand and arm prosthetic was created from the idea of adding bionic elements while keeping the cost low. It was designed based on existing models, desired functions, and materials available. A tilt sensor keeps the hand level, two motors move the wrist in two different directions, a limit switch signals the fingers to open and close, and another motor helps open and close the fingers. A regenerative prosthesis is modeled and simulated. The simulation is optimized with the biogeography-based optimization algorithm to obtain desired knee angle tracking with a reference knee trajectory and to regenerate energy. The American Academy of Orthotists and Prosthetists has long recognized the valuable synergy between research and education, and the mutual importance of each in advancing the field. Consequently, a focal point of an Academy grant initiative funded by the U.S. Department of Education has been a systematic analysis of research capacity and advanced education. A grant project in 2004 convened a series of meetings and produced the Advanced Education and Research Training Initiative (AERTI) report.
