High-dose methotrexate (HD MTX) is critical for treating pediatric malignancies such as acute lymphoblastic leukemia and neuro-carcinoma. However, its significant toxicity due to drug accumulation poses substantial risks. This retrospective study assesses the impact of demographic factors on MTX toxicity and clearance in pediatric oncology patients. Patient records from Saint Mary Hospital were analyzed, focusing on two MTX administration protocols: a 24-hour infusion followed by alkaline hydration and a 4-hour infusion followed by alkaline hydration. We hypothesize that factors such as age, body surface area (BSA), and body mass index (BMI) are associated with MTX clearance and toxicity. The study …

วัตถุประสงค์: เพื่อหาปัจจัยนำ (Predisposing factors) ที่สัมพันธ์กับการเกิดภาวะกรดคั่งในเลือดจากสารคีโตน (Diabetic ketoacidosis; DKA) ในผู้ป่วยชาวไทยโรคเบาหวานชนิดที่ 2 ที่ใช้ยาในกลุ่ม Sodium glucose co-transporter 2 inhibitors (SGLT-2 inhibitors) วิธีการศึกษา: งานวิจัยนี้เป็นการศึกษาจากผลไปหาเหตุแบบเก็บข้อมูลย้อนหลังในหลายศูนย์ รวบรวมข้อมูลผู้ป่วยโรคเบาหวานชนิดที่ 2 ที่ได้รับยา SGLT-2 inhibitors ณ โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทยและโรงพยาบาลราชวิถี ช่วง 1 ตุลาคม 2557 ถึง 30 เมษายน 2567 ผลการศึกษา: กลุ่มตัวอย่างจำนวน 310 ราย แบ่งเป็นกลุ่มศึกษาคือผู้ป่วยที่เกิดภาวะ DKA 62 ราย (คิดเป็นความชุกของการเกิดภาวะ DKA ณ ช่วงเวลาที่ศึกษาเท่ากับร้อยละ 1.37) และกลุ่มควบคุมคือผู้ป่วยที่ไม่เกิดภาวะ DKA 248 ราย ส่วนปัจจัยนำที่มีความสัมพันธ์อย่างมีนัยสำคัญทางสถิติกับการเกิดภาวะ DKA ได้แก่ การติดเชื้อ (Adjusted OR = 28.31, P < 0.001) การปรับลดขนาดยาอินซูลินภายใน 1 เดือน (Adjusted OR = 5.58, P = < 0.001) และการมีประวัติภาวะ DKA ภายใน 1 ปี (Adjusted OR = 8.76, P = 0.038) สรุปผล: ปัจจัยที่มีผลต่อการเกิดภาวะ DKA ในผู้ป่วยโรคเบาหวานชนิดที่ 2 ที่ใช้ยา SGLT-2 inhibitors ได้แก่ การติดเชื้อ การปรับลดขนาดอินซูลินภายใน 1 เดือน และการมีประวัติภาวะ DKA ภายใน 1 ปี ผู้ป่วยที่มีปัจจัยดังกล่าวจึงควรได้รับการดูแลติดตามอย่างใกล้ชิด เพื่อช่วยลดความเสี่ยงในการเกิดภาวะ DKA และเพิ่มความปลอดภัยจากการใช้ยา SGLT-2 inhibitors

N-glycosylation is a crucial post-translational modification that significantly affects immune recognition. The plant-based platform, especially Nicotiana benthamiana, offers scalable and cost-effective production of therapeutic proteins with glycosylation patterns similar to mammalian cells. However, plant-specific N-glycans, such as α(1-3)-fucose and β(1-2)-xylose (XF), have the potential to trigger allergic reactions. This study investigates how various N-glycan structures on the RBD-Fc fusion protein influence the immune response in mice. The RBD-Fc fusion protein was used for vaccine formulation, produced in both wild-type (WT) and glycoengineered (∆XF) N. benthamiana plants. Pancreatic cancer is one of the deadliest cancers to treat. The primary cause for the drug resistance is the dense extracellular matrix (ECM) that surrounds the tumor and restricts blood supply. Gemcitabine (GEM) and paclitaxel (PTX) are drugs that are clinically used in the treatment of pancreatic cancer. GEM, however, is rapidly metabolized by cytidine deaminase (CDA) and eliminated from the body, and paclitaxel is a substrate for P- glycoprotein (P-gp) efflux receptors. Introduction: Phosphodiesterase 7 (PDE7) is a high-affinity cyclic AMP (cAMP)- specific PDE that is expressed in immune and proinflammatory cells. In this work, we explore the possibility that selective small molecule inhibitors of this enzyme family could provide a novel approach to alleviate the inflammation that is associated with many inflammatory diseases. Methods: A series of novel substituted 4-hydrazinoquinazoline derivatives and fused triazoloquinazolines were designed, synthesized, and evaluated in vitro for their PDE7A inhibition activities, in comparison with Theophylline, a non-selective PDE inhibitor, and BRL50481, a selective PDE7A inhibitor. Neuropathic pain is a critical source of comorbidity following spinal cord injury (SCI) that can be exacerbated by immune-mediated pathologies in the central and peripheral nervous systems. In this article, we investigate whether drug-free, biodegradable, poly(lactide- co -glycolide) (PLG) nanoparticle treatment mitigates the development of post-SCI neuropathic pain in female mice. Our results show that acute treatment with PLG nanoparticles following thoracic SCI significantly reduces tactile and cold hypersensitivity scores in a durable fashion. Nanoparticles primarily reduce peripheral immune-mediated mechanisms of neuropathic pain, including neuropathic pain-associated gene transcript frequency, transient receptor potential ankyrin 1 nociceptor expression, and MCP-1 (CCL2) chemokine …

Drug shortages, quality defects and challenges regarding innovation within the biopharmaceutical industry pose significant risks to patient safety and organisational performance. These problems persist despite the many scientific, technological and economic advances over the past several years in a well-funded, high-revenue industry operated by highly qualified personnel. Evidence suggests that the industry has yet to fully realise the potential for building organisational capabilities to enable learning from mistakes to enhance performance outcomes. With growing resistance to vaccinations-premised, for instance, on misplaced fears of side effects or religious objections-it takes little effort to imagine that a state might act to prohibit some or all uses of a particular vaccine licensed by the federal government. Indeed, one year ago, legislators in a couple of states toyed with the idea of banning the primary vaccines against Covid-19, and, twenty years ago, half a dozen states effectively barred certain uses of one flu vaccine formulation. On July 9, 2021, President Joseph R. Biden signed Executive Order 14036 (“Promoting Competition in the American Economy”), which directed the U.S. Food and Drug Administration (FDA) and the U.S. Patent and Trademark Office (USPTO) to collaborate on new approaches to increasing competition and lowering prices in the pharmaceutical marketplace. In response, the USPTO outlined several new initiatives, among them an intent to improve the robustness and reliability of issued patents.

Background: Heart disease remains a leading cause of death in the country, with significant ethnic disparities. Filipino Americans (FAs), the third-largest Asian subgroup in U.S., face an elevated burden of dyslipidemia, a primary modifiable risk factor for heart disease. Objectives: This community-based study addresses critical knowledge gaps by characterizing the cardiometabolic and genetic factors contributing to dyslipidemia among FAs and implications for personalized cardiovascular drug therapies, particularly statin pharmacogenomics (PGx). Despite newfound attention to how artificial intelligence (AI) may accelerate pharmaceutical development, federal regulators may find that current statutes are ambiguous or silent about their applicability to AI-based drug design and manufacturing methods. This poses a serious problem in the era of Loper Bright and the Major Questions Doctrine. This research focuses on the development of a prototype of a continuous molten salt carbonization and activation process to convert plastic waste into porous carbon by improving the immersion of zinc chloride molten at 350°C under atmospheric conditions. The plastic wastes used in the study include polypropylene (PP) from plastic cups, polylactic acid (PLA) from plastic glasses, and polyethylene terephthalate (PET) from plastic bottles with a ZnCl2 to plastic waste ratio of 10:1 by mass. Background: Drug importation is critical for ensuring access to essential medicines in Thailand. However, the current system faces significant challenges, including outdated regulations, inefficient processes, and limited use of modern technology. Conventional bottom-up synthesis of carbon quantum dots (CQDs) often requires extended reaction times, high energy input, and specialized equipment, limiting scalability and sustainability. In this study, we present an eco-friendly and energy-efficient method for synthesizing CQDs using H₂CO₃ as a carbon precursor at just 72 °C for 1 hour—representing one of the lowest reported synthesis temperatures and shortest reaction times using simple apparatus. Graphene oxide vacancies act as catalytic and nucleation sites, promoting the formation of CQDs under these mild conditions. The COVID-19 pandemic has prompted the Thai government to explore herbal medicines grounded in Traditional Thai Medicine (TTM) knowledge and review literature. Highlighted herbs formula, including Ya-Kao formula and propolis extract, have been recommended as supportive treatments for non-symptomatic COVID-19 patients. Additionally, Cannabis sativa (C. sativa) has gained recognition for its pharmacological properties, such as analgesic effects, pain relief, and anti-SARS-CoV-2 activity. One formula, Ya-Kao, traditionally used for fever relief, is documented in TTM textbook. The second formula is propolis extract (PE) was combined with C. sativa extract (CS). Introduction: The global burden of hematologic neoplasms is rising. Despite advances in therapy, relapse, resistance, and limited options persist. Bispecific antibodies (BsAbs) offer a novel approach by targeting multiple antigens. This systematic review assesses their efficacy and safety. Methods: We conducted this systematic review following the PRISMA 2020 guidelines. Articles published up to 1 February 2025 were searched across PubMed, Scopus, Web of Science, and Embase. The clinical trials, focusing on SAEs, grade ≥ 3 AEs, hematologic and non-hematologic toxicities, and efficacy outcomes (ORR, CR, OS, and PFS), were included. Results were summarized qualitatively. Cannabidiol (CBD), a major phytocannabinoid present in Cannabis sativa, possesses multiple effective pharmacological activities, such as neuroprotective, cardioprotective and anti-inflammatory properties. However, its hydrophobic nature and rapid degradation mechanisms limit its clinical usefulness. Encapsulation of lipophilic compounds inside modified polymers with hydrophilic properties can improve the aqueous solubility and stability of drugs in specific environments. Therefore, this study focused on self-assembled poly N-isopropyl acrylamide (pNIPAM) grafted hyaluronic acid (HA) nanogels for encapsulating CBD. วัตถุประสงค์: เพื่อประเมินคุณภาพชีวิตด้านสุขภาพ คุณภาพชีวิตด้านการใช้ยา และความเต็มใจที่จะจ่าย รวมถึงศึกษาปัจจัยที่มีความสัมพันธ์กับคุณภาพชีวิตด้านสุขภาพ คุณภาพชีวิตด้านการใช้ยา และความเต็มใจที่จะจ่ายในผู้ป่วยโรคเพมฟิกัสและผู้ป่วยโรคบูลลัสเพมฟิกอยด์ วิธีการศึกษา: การศึกษาวิจัยเชิงวิเคราะห์แบบภาคตัดขวางในผู้ป่วยโรคเพมฟิกัสและผู้ป่วยโรคบูลลัสเพมฟิกอยด์ที่มีอายุ 18-90 ปีที่เข้ารับการรักษาในสถาบันโรคผิวหนัง ระหว่างเดือนมกราคมถึงพฤษภาคม 2568 จำนวน 178 ราย โดยใช้แบบสอบถามคุณภาพชีวิตด้านสุขภาพ EQ-5D-5L และ Dermatology Life Quality Index (DLQI) แบบสอบถามคุณภาพชีวิตด้านการใช้ยา Patient-Reported Outcomes Measure of Pharmaceutical Therapy (PROMPT) และแบบสอบถามความเต็มใจที่จะจ่าย ผลการศึกษา: ผู้ป่วยส่วนใหญ่มีคุณภาพชีวิตด้านสุขภาพและด้านการใช้ยาในระดับดี โดยมิติที่มีผู้ป่วยรายงานปัญหามากที่สุดในแบบสอบถาม EQ-5D-5L คือ มิติอาการเจ็บปวดหรือไม่สบายตัว ขณะที่มิติอาการและความรู้สึกในแบบสอบถาม DLQI เป็นมิติที่ได้รับผลกระทบมากที่สุด สำหรับแบบสอบถาม PROMPT พบว่ามิติผลกระทบทางด้านจิตใจของการใช้ยามีคะแนนต่ำที่สุด ปัจจัยที่มีความสัมพันธ์กับคุณภาพชีวิตด้านสุขภาพ ได้แก่ อายุ ระดับการศึกษา ระดับความรุนแรงของโรค และจำนวนโรคร่วม ส่วนเพศ อายุ ระดับการศึกษา และสภาวะโรคปัจจุบันสัมพันธ์กับคุณภาพชีวิตด้านการใช้ยา ขณะที่รายได้ต่อเดือนและระดับการศึกษามีความสัมพันธ์กับมูลค่าความเต็มใจที่จะจ่ายอย่างมีนัยสำคัญทางสถิติ สรุปผล: ผู้ป่วยโรคเพมฟิกัสและผู้ป่วยโรคบูลลัสเพมฟิกอยด์ได้รับผลกระทบต่อคุณภาพชีวิตในหลายมิติ ทั้งจากตัวโรค การรักษา และการใช้ยา ข้อมูลที่ได้สามารถใช้ประโยชน์ในการวางแผนการรักษา การให้คำปรึกษาด้านยา และสนับสนุนการกำหนดนโยบายเพื่อเพิ่มการเข้าถึงยาที่จำเป็นให้แก่ผู้ป่วยมากยิ่งขึ้น

Cancer remains the leading cause of global mortality, with conventional chemotherapy drugs often limited by poor selectivity, side effects, and drug resistance. As a promising alternative and adjuvant, curcumin offers potent anticancer properties, but its low bioavailability highlights the need for targeted delivery systems to enhance its therapeutic potential. This study designs a targeted delivery system for curcumin using hyaluronic acid-grafted pNIPAM (HA-g-pNIPAM) modified liposomes, characterized both experimentally and computationally. วัตถุประสงค์: เพื่อพัฒนารายการยาที่มีแนวโน้มไม่เหมาะสมสำหรับพระภิกษุอาพาธสูงอายุ (PIM-BHIKKHU) โดยใช้เทคนิคเดลฟาย และเปรียบเทียบความชุกของการใช้ยาที่มีแนวโน้มไม่เหมาะสมซึ่งประเมินโดยเกณฑ์ที่พัฒนาขึ้นกับ Beers ปี 2023 และ STOPP version 3 ปี 2023 วิธีการศึกษา: ระยะที่ 1 การศึกษาเชิงคุณภาพเพื่อพัฒนารายการ PIM-BHIKKHU โดยใช้เทคนิคเดลฟายกับผู้เชี่ยวชาญรวม 18 คน ประกอบด้วยแพทย์และเภสัชกร ระยะที่ 2 การศึกษาเชิงพรรณนาแบบภาคตัดขวางย้อนหลัง โดยทบทวนรายการยาของพระภิกษุอาพาธที่มีอายุ 65 ปีขึ้นไป และรับการรักษาแบบผู้ป่วยนอก โดยมีรายการยาที่ได้รับร่วมกันอย่างน้อย 5 รายการ จากโรงพยาบาลสงฆ์ ผลการศึกษา: จากกระบวนการเดลฟาย 2 รอบในระยะที่ 1 ได้ข้อยุติเกณฑ์ PIM-BHIKKHU จำนวน 89 รายการยา/กลุ่มยา ในระยะที่ 2 จากพระภิกษุอาพาธสูงอายุ 225 รูป พบความชุกของการใช้ยาที่มีแนวโน้มไม่เหมาะสมตามเกณฑ์ Beers ปี 2023, STOPP version 3 ปี 2023 และ PIM-BHIKKHU ปี 2024 เท่ากับร้อยละ 60.0, 45.33 และ 86.7 ตามลำดับ โดยมีความแตกต่างอย่างมีนัยสำคัญทางสถิติ (p < 0.05) ทั้งนี้ ความชุกของการใช้ PIMs สูงกว่าในกลุ่มอายุ ≥75 ปีเมื่อเทียบกับกลุ่มอายุ 65–74 ปีในทุกเกณฑ์ สรุป: รายการยาที่มีแนวโน้มไม่เหมาะสมที่พัฒนาขึ้น มีความจำเพาะกับบริบทของพระภิกษุอาพาธสูงอายุนิกายเถรวาทในประเทศไทย ทั้งนี้ ควรมีการศึกษาเพิ่มเติมเพื่อประเมินประสิทธิผลทางคลินิกเมื่อมีการใช้ในในเวชปฏิบัติจริง
