บทนำ โรคสมองขาดเลือดที่มีอาการเนื่องจากการแข็งหลอดเลือดแดงในสมอง (symptomatic intracranial atherosclerotic disease; sICAD) เป็นโรคสมองขาดเลือดที่พบได้บ่อยมากเป็นอันดับที่ 2 ของผู้ป่วยโรคสมองขาดเลือดในคนไทย และมีอัตราการเป็นซ้ำที่สูง ประมาณ ร้อยละ 15 ในปีแรก เมื่อเทียบกับกลุ่มอื่นที่ประมาณ ร้อยละ 5% ในปีแรก ทำให้งานวิจัยและแนวทางการรักษาในระดับชาติและนานาชาติ จึงแนะนำให้ใช้ยาต้านเกล็ดเลือดสองชนิดควบคู่กันระยะสั้น (short term dual antiplatelet; DAPT) โดยเฉพาะสูตรที่มียาโคพิโดเกรล (clopidogrel) เป็นส่วนผสม เพื่อป้องกันการเกิดโรคหลอดเลือดสมองซ้ำต่อไป อย่างไรก็ตามยา clopidogrel นั้นจำเป็นที่จะต้องเปลี่ยนแปลงโดยผ่านเอนไซม์ไซโตโครมพีสี่ห้าศูนย์ เอนไซม์ซิปสองซีสิบเก้า (CYP2C19) เพื่อให้ยาอยู่ในสภาพออกฤทธิ์และป้องกันโรคต่อไป จากการศึกษาก่อนหน้าพบว่าความชุกของประชากรที่มีการเมตาบอลิสมยาไม่ดี (poor metabolizers; PM) ของเอนไซม์ CYP2C19 ในประชากรไทยมีสัดส่วนจากมากแตกต่างจากประชากรชาติตะวันตก อย่างไรก็ตามจากการศึกษาในอดีตเกี่ยวกับของการเปลี่ยนแปลงทางเภสัชพันธุศาสตร์ของเอนไซม์ CYP2C19 ต่อผลการรักษาทางคลินิกในผู้ป่วยโรคสมองขาดเลือดที่มีอาการเนื่องจากการแข็งหลอดเลือดแดงในสมอง ยังมีอยู่อย่างจำกัดและผลไม่สอดคล้องกันในหลายงานวิจัย วัตถุประสงค์ วัตถุประสงค์หลักของงานวิจัย (primary objective) คือศึกษาความชุกของประชากรที่มีการเมตาบอลิสมยาไม่ดีของเอนไซม์ CYP2C19 ผู้ป่วยโรคสมองขาดเลือดที่มีอาการเนื่องจากการแข็งหลอดเลือดแดงในสมองในโรงพยาบาลจุฬาลงกรณ์ วัตถุประสงค์รองของงานวิจัย (secondary objectives) คือเพื่อศึกษาผลของการเสียการทำงานของ CYP2C19 allele (LOF allele status) ต่ออุบัติการณ์การเกิดโรคหลอดเลือดสมองตีบซ้ำ (recurrent stroke) การเกิดภาวะแทรกซ้อนทางหัวใจที่รุนแรง (major adverse cardiac event; MACE) การเกิดเลือดออก (bleeding event) ที่ 90 วันหลังเกิดโรคสมองขาดเลือดที่มีอาการเนื่องจากการแข็งหลอดเลือดแดงในสมอง วิธีการทำวิจัย เก็บข้อมูลผู้ป่วยจากการศึกษาไปข้างหน้าเชิงพรรณนา (ongoing prospective study) ตั้งแต่เดือนกรกฎาคม พ.ศ. 2565 โดยเก็บผู้ป่วยอายุมากกว่าหรือเท่ากับ 40 ปี ที่มีอาการ อาการแสดง ผลตรวจทางรังสีวิทยาที่เข้าได้กับโรคสมองขาดเลือดที่มีอาการเนื่องจากการแข็งหลอดเลือดแดงในสมอง (หลอดเลือดตีบ 50-99%) และมีผลตรวจทางรังสีวิทยามีผลการตรวจหลอดเลือดสมองทางรังสีวิทยายืนยัน โดยผู้วิจัยจะได้เก็บข้อมูลทางคลินิก ข้อมูลทางยา การตรวจทางห้องปฏิบัติการ การตรวจทางรังสีวิทยา จากการทบทวนข้อมูลในเวชระเบียนระหว่างมานอนโรงพยาบาลและที่ตรวจติดตาม …

Through the capacity to mediate Th1-biased immune responses and the induction of neutralizing antibodies, the mRNA-lipid nanoparticle (LNP) platform could represent an innovative approach for shorter, safer, and more effective treatment of house dust mite (HDM) allergy. Our previous study showed that nucleoside-modified mRNA encoding recombinant forms of HDM allergens Der p 1 and Der p 2 (ProDer p 1 and Der p 2 K96A) and formulated with ionizable LNP, induced Th1-biased immune responses and potent specific blocking IgG responses, while being non-reactogenic in sensitized mice. Older adults over 65 will double from 52.4 million in 2018 to 94.7 million in 2060. Among this aging group, falls affect one out of four older adults and are a critical economic concern for our American society, with yearly costs of $50 billion. In older adults, falling can result in negative consequences such as fear of falling, injuries, mobility restrictions, and loss of independence. California has one of the highest costs per fall ($4 billion annually) and a high percentage of adults who have fallen (30.6%) compared to the nation. Introduction: Developmental synaptopathies are neurodevelopmental disorders caused by genetic mutations disrupting the development and function of neuronal synapses.
Methods: We administered the validated Social Responsiveness Scale, Second Edition (SRS-2) to investigate the phenotypic presentation of social-behavioral impairments for the developmental synaptopathy-SYNGAP1-related Intellectual Disability (SYNGAP1-ID) (n = 32) compared with a phenotypically similar disorder Phelan-McDermid syndrome (PMD) (n = 27) and healthy controls (n = 43). A short form SRS-2 analysis (n = 85) was also conducted.

Many neurodegenerative diseases, including Huntington’s disease (HD) and Alzheimer’s disease (AD), occur due to an accumulation of aggregation-prone proteins, which results in neuronal death. Studies in animal and cell models show that reducing the levels of these proteins mitigates disease phenotypes. We previously reported a small molecule, NCT-504, which reduces cellular levels of mutant huntingtin (mHTT) in patient fibroblasts as well as mouse striatal and cortical neurons from an HdhQ111 mutant mouse. Here, we show that NCT-504 has a broader potential, and in addition reduces levels of Tau, a protein associated with Alzheimer’s disease, as well as other tauopathies. We …

Introduction: Among US men, oropharyngeal cancer (cancer of the back of the mouth and throat) is the 8th most common cancer. If detected early, human papillomavirus (HPV)-16-associated oropharyngeal cancer has a high 5-year survival rate. Risk factors such as high numbers of oral sex partners, disparities in smoking and drinking, and low rates of HPV vaccination may put gay and bisexual men at even higher risk for oropharyngeal cancer.

The present study aimed at identifying risk factors associated with periodontitis development and periodontal health disparities with emphasis on differential oral microbiota. The prevalence of periodontitis is recently rising dentate adults in the US, which presents a challenge to oral health and overall health. The risk of developing periodontitis is higher in African Americans (AAs), and Hispanic Americans (HAs) than in Caucasian Americans (CAs). Neurodegenerative Diseases (NDDs) are a group of disorders that cause progressive deficits of neuronal function. Recent evidence argues that sphingolipid metabolism is affected in a surprisingly broad set of NDDs. These include some lysosomal storage diseases (LSDs), hereditary sensory and autonomous neuropathy (HSAN), hereditary spastic paraplegia (HSP), infantile neuroaxonal dystrophy (INAD), Friedreich’s ataxia (FRDA), as well as some forms of amyotrophic lateral sclerosis (ALS) and Parkinson’s disease (PD). Human brain organoids are emerging models to study human brain development and pathology as they recapitulate the development and characteristics of major neural cell types, and enable manipulation through an in vitro system. Over the past decade, with the advent of spatial technologies, mass spectrometry imaging (MSI) has become a prominent tool for metabolic microscopy, providing label-free, non-targeted molecular and spatial distribution information of the metabolites within tissue, including lipids. Dystonia is a movement disorder characterized by involuntary co- or over-contractions of the muscles, which results in abnormal postures and movements. These symptoms arise from the pathophysiology of a brain-wide dystonia network. There is mounting evidence suggesting that the cerebellum is a central node in this network. For example, manipulations that target the cerebellum cause dystonic symptoms in mice, and cerebellar neuromodulation reduces these symptoms. Although numerous findings provide insight into dystonia pathophysiology, they also raise further questions. Dystonia is a highly prevalent movement disorder that can manifest at any time across the lifespan. An increasing number of investigations have tied this disorder to dysfunction of a broad "dystonia network" encompassing the cerebellum, thalamus, basal ganglia, and cortex. However, pinpointing how dysfunction of the various anatomic components of the network produces the wide variety of dystonia presentations across etiologies remains a difficult problem. In this review, a discussion of functional network findings in non-mendelian etiologies of dystonia is undertaken. Introduction: In carefully selected patients with medically refractory epilepsy, disconnective hemispherotomy can result in significant seizure freedom; however, incomplete disconnection can result in ongoing seizures and poses a significant challenge. Completion hemispherotomy provides an opportunity to finish the disconnection. We describe the use of magnetic resonance-guided laser interstitial thermal ablation (MRgLITT) for completion hemispherotomy.
Methods: Patients treated with completion hemispherotomy using MRgLITT at our institution were identified. Procedural and seizure outcomes were evaluated retrospectively.
Results: Five patients (3 males) underwent six MRgLITT procedures (one child treated twice) for completion hemispherotomy at a median age of 6 years (range 1.8-12.9). Two …

This study aims to determine the correlation between nutritional adequacy, Fe content, body fat percentage, and muscle mass percentage with physical fitness in football athletes. Adequacy of nutrients (energy, protein, fat, protein and Fe) was obtained with a 2×24 hour recall. The Fe content value of Fe was measured using the cyanmethemoglobin method and sTfR was measured using the ELWASA method. The percentage of fat and muscle was obtained by using a Bioelectrical Impedance Analysis tool. This study aims to analyze body type, dietary intake, and cardiorespiratory function of college soccer players. A total of 32 players (20 players from Yogyakarta State University and 12 players from Gadjah Mada University) were involved in this study. The study was carried out in April - June 2014. Body type, or somatotype, was calculated based on anthropometric measurements (weight, height, epicondylar humerus and epicondylar femur width, arm and calf circumference, and subcutaneous fat thickness). 24-hour food recall was used to measure the athlete's dietary intake. The αVβ6 integrin, an epithelial-specific cell surface receptor absent in normal prostate and expressed during prostate cancer (PrCa) progression, is a therapeutic target in many cancers. Here, we report that transcript levels of ITGB6 (encoding the β6 integrin subunit) are significantly increased in metastatic castrate-resistant androgen receptor-negative prostate tumors compared to androgen receptor-positive prostate tumors. In addition, the αVβ6 integrin protein levels are significantly elevated in androgen receptor-negative PrCa patient derived xenografts (PDXs) compared to androgen receptor-positive PDXs. This study aimed to investigate the effect of low-impact aerobic exercise on increasing physical fitness for the elderly. This one group of pretest-posttest quasi-experimental research recruited elderly subjects from Sumber Waras Elderly Association in Yogyakarta, Indonesia. The ability to biosynthesize oxalic acid can provide beneficial functions to plants; however, uncontrolled or prolonged exposure to this strong organic acid results in multiple physiological problems. Such problems include a disruption of membrane integrity, mitochondrial function, metal chelation, and free radical formation. Recent work suggests that a CoA-dependent pathway of oxalate catabolism plays a critical role in regulating tissue oxalate concentrations in plants. Although this CoA-dependent pathway of oxalate catabolism is important, large gaps in our knowledge of the enzymes catalyzing each step remain. The aim of this study was to evaluate the efficacy and safety of antiulcer oral mucosal protectant-RADoralex® in the prevention and treatment of radiation-induced oral mucosal reactions elicited during intensity-modulated radiation therapy (IMRT). for locally advanced nasopharyngeal carcinoma (NPC). (1) Background: The genetic etiology of most patients with cerebral folate deficiency (CFD) remains poorly understood. KDM6B variants were reported to cause neurodevelopmental diseases; however, the association between KDM6B and CFD is unknown; (2) Methods: Exome sequencing (ES) was performed on 48 isolated CFD cases. The effect of KDM6B variants on KDM6B protein expression, Histone H3 lysine 27 epigenetic modification and FOLR1 expression were examined in vitro. Reminders are an important method for encouraging patients to return for follow-up visits, such as for successive doses of the human papillomavirus (HPV) vaccine. However, patients may have preferences for different types of reminders. This study examined which reminder methods parents of pediatric patients found most useful and their thoughts on how the reminders helped them to complete their children's HPV vaccine series. BACKGROUND: Clinical evidence suggests that chemotherapeutic agents are associated with neuropathy and peripheral autonomic dysfunction. However, the possible effects of neoadjuvant chemotherapy on intraoperative temperature remain poorly characterised.
METHODS: We evaluated patients who underwent a mastectomy for breast cancer between April 2016 and July 2020. Propensity scores were used to match patients who received neoadjuvant chemotherapy with those who did not, and intraoperative core temperature patterns were analysed in the matched cohort. The independent associations between vasopressor use and heart rate during general anaesthesia in the matched cohort were also analysed.

OBJECTIVES: Cancer patients have worse outcomes from the COVID-19 infection and greater need for ventilator support and elevated mortality rates than the general population. However, previous artificial intelligence (AI) studies focused on patients without cancer to develop diagnosis and severity prediction models. Little is known about how the AI models perform in cancer patients. In this study, we aim to develop a computational framework for COVID-19 diagnosis and severity prediction particularly in a cancer population and further compare it head-to-head to a general population.

Cancer metastasis is a major cause of cancer-related mortality. Strategies to reduce metastases are needed especially in lung cancer, the most common cause of cancer mortality. We previously reported increased ubiquitin-specific peptidase 18 (USP18) expression in lung and other cancers. Engineered reduction of USP18 expression repressed lung cancer growth and promoted apoptosis. This deubiquitinase (DUB) stabilized targeted proteins by removing the complex interferon-stimulated gene 15 (ISG15). This study explores if the loss of USP18 reduced lung cancer metastasis. Spinal manipulation is recommended by clinical guidelines to treat low back pain. However, our understanding of how spinal manipulation alleviates pain and knowledge of the role that viscoelastic tissue properties play in the dampening of applied spinal manipulation forces in vivo is limited. Skin malignant melanoma is one of the most aggressive skin tumors. Superficial spreading melanoma (SSM) is the most common histological type, which can originate from different body skin sites, and some patients can still accumulate regional lymph nodes and even have distant metastasis in some cases. This study used the relevant data from the monitoring, epidemiology and results database of the National Cancer Institute database to study the overall survival (OS) and cancer-specific survival (CSS) of SSM patients and established an SSM nomogram to evaluate the prognosis of patients.
