People with persistent mental illness experience more chronic disease and co-morbidities than the general population, impacting their quality of life and increasing the cost of health care. In spite of the increased need for primary care services, people with mental illness encounter barriers to health care including lack of access to care, and a shortage of both primary care and psychiatric care providers. Acetylcholine (ACh) release by starburst amacrine cells (SACs) has been implicated in the production of retinal waves during early retinal development. Inhibitory gamma-aminobutyric acid (GABA) release by SACs is known to be involved in directional selectivity in the mature retina. However, the role of SAC release of ACh in the adult developed mammalian retina is not entirely understood. Some evidence suggests a neuroprotective effect on retinal ganglion cells (RGCs) acting through alpha7 nicotinic acetylcholine receptors (nAChRs) present in RGCs. If ACh released by SACs is neuroprotective to RGCs, it would follow that this cholinergic transmission might be compromised in glaucoma …

Objective: To study the degree to which individuals in different trajectories of cigarette smoking from adolescence to the early forties are similar or different in terms of lack of preventive health behaviors (e.g., underuse of preventive health services, unhealthy eating habits) in early midlife. Methods: Participants came from a community-based random sample of residents in two upstate New York counties (N = 548). Data were collected from adolescence to early midlife (mean age = 43 years, standard deviation [SD] = 2.8) at seven time points. Using growth mixture modeling, we statistically identified the number of smoking trajectories. …

Background: ELISAs are known to have a higher diagnostic sensitivity than the agar gel immunodiffusion (AGID) when employed for serological diagnosis of equine infectious anaemia (EIA). For this purpose, an “in-house” and five commercial ELISAs available in Italy were assessed by the National Reference Centre for EIA for their analytic specificity (Sp); precocity, defined as capability of detecting first antibodies produced during a new infection; precision based on repeatability and reproducibility, estimated from the coefficient of variation (CV); accuracy, estimated from multiple K and relative Sp and sensitivity (Se). Background: Attention deficit hyperactivity disorder (ADHD) is a common neuropsychiatric disorder among the children. The burden of ADHD or its associated factors in Uganda are not known. The objective of this study was to determine the prevalence and the associated factors of ADHD among children attending the neurology and psychiatry clinics at Mulago National Referral Hospital. Methods: Using the disruptive behavior scale (45 items), we investigated the presence of ADHD symptoms among children attending Mulago Hospital. Hospitals need to analyze all patient care delivery methods, and especially those designated for vulnerable populations. The Deaf community is a particularly unique and often overlooked vulnerable population because of the additional communication barriers that they possess. The goal for this research is to look at the current best practices for communication and treatment in hospitals from the perspective of the Deaf community. Introduction: Based on a 2014 newborn education needs assessment, 39.6% of surveyed pediatric hospitalists expressed interest in receiving more education on newborn medications, and 94% were interested in computer-based learning modules as the method of learning. Thus, this module was designed to serve as a self-study tool or as a tool for small-group teaching.
Methods: Initially designed for pediatric hospitalists, the module can be used for any learner interested in newborn care as it is meant to provide practical applications to the bedside clinician. Background: This study seeks to quantify variation in healthcare utilization and per capita costs using system-defined geographic regions based on enrollee residence within the Military Health System (MHS).
Methods: Data for fiscal years 2007 – 2010 were obtained from the Military Health System under a data sharing agreement with the Defense Health Agency (DHA). DHA manages all aspects of the Department of Defense Military Health System, including TRICARE. Adjusted rates were calculated for per capita costs and for two procedures with high interest to the MHS- back surgery and Cesarean sections for TRICARE Prime and Plus enrollees. Seeds of Carica papaya L. (Caricaceae) are a promising source of investigation for a novel anthelmintic to treat equine strongyle infestation. Strongyles are small redworms that attach to the intestinal wall in the equine. They can cause colic, poor hair coat, poor body condition, and, in severe cases, death. The objective of this study was to determine if benzyl isothiocyanate (BITC) and papain, two compounds found in papaya seeds, could be used to prevent migration of third stage cyathostome larvae (L3) in vitro using larval migration inhibition assays. High levels of bilirubin in infants can cause kernicterus, which includes basal ganglia damage and dystonia. Stem cell transplantation may be an effective treatment for this disease. In this study, we transplanted human neural progenitor cells differentiated toward propriospinal interneurons into the striatum of 20-day-old spontaneously jaundiced (jj) Gunn rats and nonjaundiced (Nj) littermates. Using immunohistochemical methods, we found that grafted cells survived and grew fibers in jj and Nj brains 3 weeks after transplantation. Grafted cells had a higher survival rate in jj than in Nj brains, suggesting that slightly elevated bilirubin may protect graft survival due to its …

Spreading depolarization (SD) is a propagating wave of neuronal activation occurring in the central nervous system. SD is accompanied by depression of synaptic activity and complex blood flow changes. Better understanding the mechanisms underlying these phenomena may be important to elucidate the pathophysiology of human disease, and to develop therapeutic interventions. SD contributes significantly to the morbidity and mortality of thromboembolic and ischemic stroke, subarachnoid hemorrhage, and traumatic brain injury. Educators who teach in a clinical setting, primarily in healthcare fields, while having knowledge of the subject, having skills in what is taught, and having experience in the field, often have little-to-no training in teaching. This results in instructional practices being based upon intuition and experience. Background Taenia solium, a parasite that affects humans and pigs, is the leading cause of preventable epilepsy in the developing world. Geographic hotspots of pigs testing positive for serologic markers of T. solium exposure have been observed surrounding the locations of human tapeworm carriers. This clustered pattern of seropositivity in endemic areas formed the basis for geographically targeted control interventions, which have been effective at reducing transmission. In this study, we further explore the spatial relationship between human tapeworm carriers and infected pigs using necroscopic examination as a quantitative goldstandard diagnostic to detect viable T. solium cyst infection in pigs. …

Stressful experiences potently activate kappa opioid receptors (κORs). κORs in the ventral tegmental area regulate multiple aspects of dopaminergic and non-dopaminergic cell function. Here we show that at GABAergic synapses on rat VTA dopamine neurons, a single exposure to a brief cold-water swim stress induces prolonged activation of κORs. This is mediated by activation of the receptor during the stressor followed by a persistent, ligand-independent constitutive activation of the κOR itself. This lasting change in function is not seen at κORs at neighboring excitatory synapses, suggesting distinct time courses and mechanisms of regulation of different subsets of κORs. We also …

Pre-adolescent female mentorship through team membership over the course of four years is examined for the purpose of this Doctoral of Nursing Practice - Family Nurse Practitioner project. Dr. Jean Watson theory of Human Caring is used as a catalyst for the project. A literature review was performed to help guided the project and bring to surface opportunities for further examination. A Doctor of Nursing Practice - Family Nurse Practitioner student mentored the eleven participants. The Essentials of Doctoral Education for Advance Practice nursing were utilized in the project.
