การป้องกันการติดเชื้อที่ตำแหน่งผ่าตัดมีความสำคัญต่อความปลอดภัยของผู้ป่วยที่เข้ารับการรักษาในโรงพยาบาลด้วยการผ่าตัด การดำเนินการป้องกันการติดเชื้อที่ตำแหน่งผ่าตัดให้มีประสิทธิภาพเป็นความท้าทายของบุคลากรทางการแพทย์ ซึ่งต้องร่วมมือกันแบบสหสาขา พยาบาลในฐานะที่ทำงานใกล้ชิดผู้ป่วยมากที่สุด มีบทบาทในการประสานความร่วมมือระหว่างผู้ป่วยและทีมบุคลากรทางการแพทย์จำเป็นต้องมีความรู้ ความเข้าใจในประเด็นสำคัญและทันสมัยที่เกี่ยวข้องกับการติดเชื้อที่ตำแหน่งผ่าตัดได้แก่ ประเภทของการติดเชื้อที่ตำแหน่งผ่าตัด ปัจจัยที่เกี่ยวข้องกับการติดเชื้อที่ตำแหน่งผ่าตัด ทั้งปัจจัยด้านผู้ป่วย เชื้อก่อโรค และสิ่งแวดล้อม และการป้องกันการติดเชื้อที่ตำแหน่งผ่าตัดทั้งในระยะก่อนผ่าตัดระยะผ่าตัด และระยะหลังผ่าตัด เพื่อช่วยให้ผู้ป่วยปลอดภัยและลดโอกาสเสี่ยงต่อการติดเชื้อที่ตำแหน่งผ่าตัด

วัตถุประสงค์ : เพื่อศึกษาพฤติกรรมทางเพศและปัจจัยทำ.นายพฤติกรรมทางเพศของนักเรียนประถมศึกษาตอนปลาย\n\nแบบแผนการวิจัย : การวิจัยแบบบรรยายเชิงทำนาย\n\nวิธีดำเนินการวิจัย : กลุ่มตัวอย่าง คือ นักเรียนชั้นประถมศึกษาชั้นปีที่ 6 จำนวน 306 คน จ.จันทบุรี รวบรวมข้อมูลโดยใช้แบบสอบถาม การเข้าถึงสื่อที่ยั่วยุความรู้สึกทางเพศ การรับรู้ความรุนแรงของโรคเอดส์และการตั้งครรภ์ พฤติกรรมเสี่ยงทางเพศของเพื่อน ความสะดวกใจในการสื่อสารเรื่องเพศกับผู้ปกครอง การควบคุมกำกับของผู้ปกครอง การรับรู้ความสามารถในการปฏิเสธพฤติกรรม และพฤติกรรมทางเพศ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และสมการถดถอยพหุคูณแบบเชิงชั้น\n\nผลการวิจัย : กลุ่มตัวอย่างมีประสบการณ์ทางเพศร้อยละ 4.5 (เพศชายร้อยละ 10.6 และเพศหญิงร้อยละ 0.5) ปัจจัยทำ.นายพฤติกรรมทางเพศโดยควบคุมปัจจัยด้านเพศ พบว่า การรับรู้การมีพฤติกรรมเสี่ยงทางเพศของเพื่อน (. = .328) การเข้าถึงสื่อที่ยั่วยุความรู้สึกทางเพศ (. = .242) การรับรู้ความรุนแรงของโรคเอดส์และการตั้งครรภ์ (. = -.157) และการรับรู้ความสามารถในการปฏิเสธพฤติกรรมที่นำไปสู่การมีเพศสัมพันธ์ (. = -.120) สามารถร่วมกันทำนายพฤติกรรมทางเพศได้ร้อยละ 30.6 อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (R2 = .306, p < .05)\n\nสรุป : การส่งเสริมให้เด็กวัยเรียนมีพฤติกรรมทางเพศที่เหมาะสม ควรเน้นการปรับเปลี่ยนการรับรู้เกี่ยวกับพฤติกรรมเสี่ยงทางเพศ ส่งเสริมการเรียนรู้ผลกระทบจากการมีพฤติกรรมเสี่ยงทางเพศ การป้องกันการใช้สื่อยั่วยุความรู้สึกทางเพศ และการฝึกทักษะการปฏิเสธพฤติกรรมที่นำไปสู่การมีเพศสัมพันธ์

วัตถุประสงค์: เพื่อศึกษาการสื่อสารเรื่องเพศและพฤติกรรมการใช้ถุงยางอนามัยของกลุ่มเยาวชนชายรักชาย และความสัมพันธ์ระหว่างการสื่อสารเรื่องเพศกับพฤติกรรมการใช้ถุงยางอนามัยในกลุ่มเยาวชนชายรักชาย ทั้งที่เป็นคู่นอนประจำและคู่นอนชั่วคราว\n\nแบบแผนการวิจัย: การศึกษาแบบบรรยายเชิงความสัมพันธ์\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ เยาวชนชายที่มีเพศสัมพันธ์กับชาย (MSM) อายุระหว่าง 15-24 ปี จำนวน 196 คน คัดเลือกโดยใช้เทคนิคการบอกต่อจากกลุ่มเพื่อน เก็บรวบรวมข้อมูลโดยใช้แบบวัดพฤติกรรมการใช้ถุงยางอนามัย และแบบวัดการสื่อสารเรื่องเพศที่ประกอบด้วยความถี่ในการสื่อสารฯ และความสะดวกใจในการสื่อสารฯ มีค่าสัมประสิทธิ์อัลฟ่าครอนบาคของแบบวัดการสื่อสารเรื่องเพศ เท่ากับ .80 ด้านความถี่ในการสื่อสารฯ เท่ากับ .83 และความสะดวกใจในการสื่อสารฯ เท่ากับ .85 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและการทดสอบสหสัมพันธ์ของเพียร์สัน\n\nผลการวิจัย: กลุ่มตัวอย่างร้อยละ 51 ใช้ถุงยางอนามัยทุกครั้งกับคู่นอนชั่วคราว และร้อยละ 35 กับคู่นอนประจำส่วนใหญ่มีการสื่อสารเรื่องเพศกับคู่นอนประจำสูงกว่ากับคู่นอนชั่วคราว และการใช้ถุงยางอนามัยเป็นหัวข้อที่มีการสื่อสารกับคู่นอนมากที่สุด การสื่อสารเรื่องเพศกับคู่นอนมีความสัมพันธ์เชิงบวกกับพฤติกรรมการใช้ถุงยางอนามัยในช่วงสามเดือนที่ผ่านมาอย่างมีนัยสำคัญทางสถิติ ทั้งในคู่นอนประจำและคู่นอนชั่วคราว (r = .344, p <0.001 และ r=.197, p <0.039)สรุป: การป้องกันโรคเอดส์ในกลุ่มเยาวชนชายรักชายควรเน้นการสร้างแรงจูงใจและส่งเสริมการเริ่มการสื่อสารเรื่องเพศและต่อรองการใช้ถุงยางอนามัยอย่าง สมํ่าเสมอกับคู่นอนทุกประเภทให้เป็นเรื่องปกติ

วัตถุประสงค์: เพื่อศึกษาปัจจัยทำนายพฤติกรรมป้องกันโรคหลอดเลือดหัวใจของกำลังพลกองทัพบก\n\nแบบแผนการวิจัย: การวิจัยแบบบรรยายเชิงทำนาย\n\nวิธีดำเนินการวิจัย : กลมุ่ ตัวอย่าง คือ กำลังพลกองทัพบก ในเขตกรุงเทพมหานคร จำนวนทั้งสิ้น 184 คน คัดเลือกโดยการสุ่มอย่างง่าย จากคลินิกโรคเบาหวานและโรคความดันโลหิตสูง โรงพยาบาลพระมงกุฎเกล้า กรมทหารราบที่ 11 รักษาพระองค์ และกองพลทหารม้าที่ 2 รักษาพระองค์ เครื่องมือที่ใช้ในการวิจัย ประกอบด้วยแบบสอบถาม 7 ส่วน ได้แก่ 1) ข้อมูลส่วนบุคคล 2) แบบประเมินความเสี่ยงต่อการเกิดโรคหลอดเลือดหัวใจ 3) แบบสอบถามความรู้เกี่ยวกับโรคหลอดเลือดหัวใจ 4) แบบสอบถามทัศนคติต่อพฤติกรรมป้องกันโรคหลอดเลือดหัวใจ 5) แบบสอบถามการสนับสนุนทางสังคม 6) แบบสอบถามความเพียงพอของทรัพยากรทางสุขภาพ และ 7) แบบสอบถามพฤติกรรมป้องกันโรคหลอดเลือดหัวใจ โดยผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน มีค่าดัชนีความตรงเชิงเนื้อหาเท่ากับ .81, .80, .93, 1.00 และ .95 ตามลำดับ มีค่าสัมประสิทธิ์แอลฟ่าครอนบาค เท่ากับ .76, .80, .76 และ .79 ตามลำดับ โดยแบบสอบถามด้านความรู้เกี่ยวกับโรคหลอดเลือดหัวใจมีค่า KR-20 เท่ากับ .76 วิเคราะห์ข้อมูลโดยใช้สถิติวิเคราะห์ถดถอยพหุคูณแบบขั้นตอน\n\nผลการวิจัย: 1) กำ.ลังพลกองทัพบกมีพฤติกรรมป้องกันโรคหลอดเลือดหัวใจอยู่ในระดับเหมาะสม (x = 73.39, S.D. = 10.52) 2) ความเพียงพอของทรัพยากรทางสุขภาพ (Beta = .316) ทัศนคติต่อพฤติกรรมป้องกันโรคหลอดเลือดหัวใจ (Beta = .279) การสนับสนุนทางสังคม (Beta = .219) การทำงานเป็นกะ (Beta = .150) และความรู้เกี่ยวกับโรคหลอดเลือดหัวใจ (Beta = .144) สามารถร่วมกันทำนายพฤติกรรมป้องกันโรคหลอดเลือดหัวใจของกำลังพลกองทัพบกได้ร้อยละ 33.2 อย่างมีระดับนัยสำคัญทางสถิติที่ระดับ .05 สำหรับปัจจัยด้านสถานภาพในกองทัพบก และความเสี่ยงในการเกิดโรคหลอดเลือดหัวใจไม่สามารถทำนายพฤติกรรมป้องกันโรคหลอดเลือดหัวใจในกำลังพลกองทัพบกได้\n\nสรุป: …

วัตถุประสงค์: เพื่อศึกษาคุณภาพการตายของผู้ป่วยมะเร็งระยะสุดท้ายตามการรับรู้ของผู้ดูแล และปัจจัยที่สัมพันธ์กับคุณภาพการตายของผู้ป่วยมะเร็งระยะสุดท้ายตามการรับรู้ของผู้ดูแล\n\nแบบแผนการวิจัย: แบบบรรยายเชิงความสัมพันธ์\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ดูแลหลักในครอบครัวซึ่งดูแลผู้ป่วยมะเร็งระยะสุดท้ายที่เคยเข้าพักรักษาแบบผู้ป่วยใน ณ โรงพยาบาลระดับตติยภูมิในเขตกรุงเทพและปริมณฑล 2 แห่ง และเสียชีวิตแล้วมานาน 2-6 เดือน จำ.นวน 110 คน คัดเลือกตามสะดวก เครื่องมือวิจัยประกอบด้วย 1) แบบบันทึกข้อมูลส่วนบุคคล 2) แบบประเมินคุณภาพการตาย 3) แบบประเมินระดับความทุกข์ทรมาน 4) แบบประเมินสัมพันธภาพของบุคคลในครอบครัว 5) แบบประเมินการสื่อสารของทีมสุขภาพ และ 6) แบบประเมินความผาสุกทางจิตวิญญาณ โดยชุดที่ 2, 4, 5 และ 6 มีค่าสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ .86, .86, .82 และ .98 ตามลำดับ ชุดที่ 3 มีค่าความเที่ยงแบบทดสอบซํ้า เท่ากับ 0.99 วิเคราะห์ข้อมูลโดยหาความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และค่าสัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน\n\nผลการวิจัย : 1) คุณภาพการตายของผู้ป่วยมะเร็งระยะสุดท้ายตามการรับรู้ของผู้ดูแลอยู่ในระดับดี (x=6.95, SD = 0.40) 2) ความทุกข์ทรมานมีความสัมพันธ์ทางลบกับคุณภาพการตายของผู้ป่วยมะเร็งระยะสุดท้าย ตามการรับรู้ของผู้ดูแลอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r = -.56; p<.05) 3) สัมพันธภาพของบุคคลในครอบครัว การสื่อสารของทีมสุขภาพ และความผาสุกทางจิตวิญญาณ มีความสัมพันธ์ทางบวกกับคุณภาพการตายของผู้ป่วยมะเร็งระยะสุดท้ายตามการรับรู้ของผู้ดูแลอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r = .38, .42, และ .43 ตามลำ.ดับ p<.05)\n\nสรุป: การบรรเทาความทุกข์ทรมานจากอาการต่างๆ ของโรค การส่งเสริมสัมพันธภาพอันดีในครอบครัว การเพิ่มคุณภาพการสื่อสารของทีมสุขภาพ และการส่งเสริมความผาสุกทางจิตวิญญาณของผู้ป่วยมะเร็ง ระยะสุดท้าย มีความเชื่อมโยงกับคุณภาพการตายที่ดีของผู้ป่วยมะเร็งระยะสุดท้ายตามการรับรู้ของผู้ดูแล

วัตถุประสงค์: เพื่อศึกษาพฤติกรรมป้องกันภาวะความดันโลหิตสูง และปัจจัยทำนายพฤติกรรมป้องกันภาวะความดันโลหิตสูงในสตรีผู้ใช้แรงงานที่มีภาวะนํ้าหนักเกิน\n\nแบบแผนงานวิจัย: การศึกษาแบบบรรยายเชิงทำนาย\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ สตรีผู้ใช้แรงงานที่ปฏิบัติงานในโรงงานอุตสาหกรรม อายุระหว่าง 15-59 ปี ที่มีภาวะนํ้าหนักเกินเกณฑ์มาตรฐาน จำ.นวน 183 คน ที่คัดเลือกตามสะดวก เครื่องมือที่ใช้ในการวิจัย คือ แบบสอบถามข้อมูลส่วนบุคคล การรับรู้โอกาสเสี่ยง การรับรู้ความรุนแรง การรับรู้ประโยชน์ การรับรู้อุปสรรค ปัจจัยกระตุ้นการปฏิบัติ การรับรู้ความสามารถ และพฤติกรรมป้องกันภาวะความดันโลหิตสูง ผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิจำ.นวน 5 คน ได้ค่าดัชนีความตรงเท่ากับ .88,.84, .93, .89, .90, .96 และ .92 ตามลำ.ดับ มีค่าสัมประสิทธิ์อัลฟาของครอนบาคเท่ากับ .76, .85, .87, .73, .83, .80 และ .77 ตามลำ.ดับ วิเคราะห์ข้อมูลโดยคำ.นวณค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน ค่าสัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน และค่าสัมประสิทธิ์ถดถอยพหุคูณแบบขั้นตอน\n\nผลการวิจัย: 1) พฤติกรรมป้องกันภาวะความดันโลหิตสูงในสตรีผู้ใช้แรงงานที่มีภาวะนํ้าหนักเกิน โดยรวมอยู่ในระดับดี ( = 61.98, S.D. = 6.96) 2) ปัจจัยกระตุ้นการปฏิบัติเพื่อป้องกันโรค การรับรู้ต่ออุปสรรคของการปฏิบัติเพื่อป้องกันโรค การรับรู้ความรุนแรงของโรค และการรับรู้โอกาสเสี่ยงของการเกิดโรค สามารถร่วมกันทำนายพฤติกรรมป้องกันภาวะความดันโลหิตสูงในสตรีผู้ใช้แรงงานที่มีภาวะนํ้าหนักเกิน ได้ร้อยละ 33.8 อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ส่วนปัจจัยด้านอายุ ลักษณะการทำงาน การรับรู้ประโยชน์ และการรับรู้ความสามารถของตนเอง ไม่สามารถทำ.นายพฤติกรรมป้องกันภาวะความดันโลหิตสูงได้\n\nสรุป: ควรส่งเสริมให้สตรีผู้ใช้แรงงานท่มี ีภาวะน้าํ หนักเกินมีการปฏิบัติพฤติกรรมป้องกันภาวะความดันโลหิตสูงด้วยการลดการรับรู้อุปสรรคต่อการปฏิบัติพฤติกรรม ส่งเสริมการรับรู้ความรุนแรงและโอกาสเสี่ยงต่อการเกิดโรค โดยให้ครอบครัว เพื่อนร่วมงาน และบุคลากรทางสาธารณสุขเข้ามามีส่วนร่วมในการกระตุ้นการปฏิบัติพฤติกรรมเพื่อป้องกันภาวะความดันโลหิตสูง

Background: Mannitol has been used in the past for the prevention of cisplatin-induced nephrotoxicity. Studies on its efficacy have conflicting results. An educational newsletter was designed for local oncologists on the conflicting data of mannitol use in preventing cisplatin-induced nephrotoxicity. Purpose: The purpose of this study was to determine whether a pharmacist-created newsletter intervention led to changes in the mannitol prescribing practices of local oncologists. Methods: A newsletter describing the paucity of evidence to support mannitol use to prevent cisplatin-induced nephrotoxicity was distributed via e-mail to local oncologists in October 2010. Obesity, elevated blood pressure and dyslipidemia are highly prevalent in Hispanic children. Compared to their non-Hispanic White peers, Hispanic children experience higher prevalence of obesity and hypertension. The Hispanic population in Tennessee has been growing, with about a tenth of newborn babies being Hispanic. This study aimed to: 1. Examine the influence of sociodemographic factors on Hispanic children’s cardiometabolic risk; 2. Assess the relationship between food group intake and cardiometabolic risk in Hispanic children; and 3. Abstract
OBJECTIVE:
To observe changes in surgeons' practice of antibiotic usage in patients with acute cholecystitis before and after the implementation of Tokyo Guidelines.
METHODS:
This retrospective, descriptive study was conducted at the Aga Khan University Hospital, Karachi, and comprised the medical records of all patients with the diagnosis of acute calculus cholecystitis who presented in 2009 and those who presented in 2014 after the implementation of Tokyo Guidelines. The major variables included patients' demographics, antibiotics used and surgical outcomes. SPSS 19 was used for data analysis.

The objective of this study was to assess the association between previous antibiotic use, particularly long-term prophylaxis, and the occurrence of subsequent resistant infections in children with index infections due to extended-spectrum-cephalosporin-resistant Enterobacteriaceae We also investigated the concordance of the index and subsequent isolates. Extended-spectrum-cephalosporin-resistant Escherichia coli and Klebsiella spp. isolated from normally sterile sites of patients aged species, resistance determinants, and fumC-fimH (E. coli) or tonB (Klebsiella pneumoniae) type were identical to those of the index isolate. Study Design: Quasi-experimental. Objectives: To compare clinical and mechanical measures in detecting lower limb asymmetries associated with second anterior cruciate ligament (ACL) injuries. Background: Knee extension moment (KExtM) asymmetry is predictive of second ACL injury. Evaluation of clinical return to sport tests to assess and classify asymmetry is needed. Methods: While performing the drop vertical jump (DVJ), thirty healthy individuals underwent 3D motion analysis. Absolute difference in KExtM between limbs at initial contact of the DVJ (KExtM symmetry) was calculated separately for each trial and averaged. Nonalcoholic steatohepatitis (NASH) patients have elevated plasma saturated free fatty acid levels. These toxic fatty acids can induce liver cell death and our recent results demonstrated that the biliary epithelium may be susceptible to lipotoxicity. Here, we explored the molecular mechanisms of cholangiocyte lipoapoptosis in cell culture and in an animal model of NASH. Treatment of cholangiocytes with palmitate (PA) showed increased caspase 3/7 activity and increased levels of cleaved poly (ADP-ribose) polymerase and cleaved caspase 3, demonstrating cholangiocyte lipoapoptosis. Interestingly, treatment with PA significantly increased the levels of microRNA miR-34a, a pro-apoptotic microRNA known to be elevated in NASH. …

The initial transition from pre-clinical learning to clinical immersion is a significant and unique phase in a PA student’s education when students transition from spending more time learning in the classroom to experiential learning in the clinical setting. One essential component of that transition is the teaching of physical examination skills. The author introduces an instrument based on metacognitive theories of learning called WINDOW©. The instrument is evaluated using two consecutive PA student surveys employing a Mixed Methods approach.

Background: Oral squamous cell carcinoma (OSCC) is the most common malignant tumour of the oral cavity. Detection of OSCC is currently based on clinical oral examination combined with histopathological evaluation of a biopsy sample. Direct contact between saliva and the oral cancer makes measurement of salivary metalloproteinase- 9 (MMP-9) an attractive alternative.

Studies on mortality from postmenopausal breast cancer (PMBC) by education level have not shown consistent results among US women. For US Asians, often seen as a “model” minority in terms of affluence and education this relationship has never been studied despite PMBC being the most common cancer in the country.

Theories considering the etiology of psychopathy suggest that trauma exposure, specifically childhood maltreatment and sexual abuse, is related to the development of callous-unemotional traits in children and adolescents, which are precursors to psychopathic traits in adulthood. Furthermore, posttraumatic stress disorder has an opposite relationship with many of the emotional and behavioral components of the two-factor model of psychopathy. Specifically, PTSD is positively associated to IA and traits associated with it and negatively associated with FD. Opioid use disorder (OUD) among pregnant women increased substantially between 1992 and 2012 across the United States, with the greatest increases occurring in the southern states. We analyzed the 2013 Treatment Episodes Database-Admissions to determine how substances used, characteristics of treatment, and demographics of pregnant women entering treatment for opioid use disorder vary between geographical regions. Analyses were restricted to cases where women reported being pregnant at the time of admission and reported opioids as the primary substance problem leading to the treatment admission. While treatments involving radiation and chemotherapy currently exist, disease must be detected early in order for the treatments to be somewhat effective, and there is no effective treatment after metastasis occurs (2). Introduction: Adequate hydration is important element of good health. Several studies indicate that the majority of kids are hypohydrated and do not meet dietary water intake guidelines. Some scientist also suggest that good hydration might be achieved by large consumption of food that are rich in water (i.e. fruits and vegetables). However, the information of food consumption on total water intake in children is limited.

Arsenic is recognized as an environmental carcinogen, in which over 100 million individuals worldwide experience chronic exposure through contaminated drinking water. Arsenic exposure is linked to several health conditions, such as cancer, diabetes, skin lesions, immune dysfunction, and cardiovascular disease. Here we test the hypothesis that toxicants, such as arsenic, accelerate the program of senescence. Human (IMR-90) primary diploid fibroblasts were chronically exposed to arsenic at nanomolar concentrations and the impact on the expression of senescent-associated mRNAs was evaluated.
