วัตถุประสงค์: เพื่อศึกษาผลของโปรแกรมการจัดการตนเองของบุคคลและครอบครัวต่อระดับฮีโมโกลบินเอวันซีของผู้สูงอายุโรคเบาหวานชนิดที่ 2\n\nแบบแผนการวิจัย: การวิจัยแบบกึ่งทดลอง\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ที่มีอายุ 60 ปีบริบูรณ์ขึ้นไป ที่ได้รับการวินิจฉัยว่าเป็นโรคเบาหวานชนิดที่ 2 ที่มีระดับ HbA1c > 7.5% ที่มารับการรักษาที่คลินิกเบาหวาน-ธัยรอยด์ และระบบฮอร์โมน แผนกผู้ป่วยนอก โรงพยาบาลจุฬาลงกรณ์ แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 22 คน กลุ่มควบคุมได้รับการพยาบาลตามปกติ กลุ่มทดลองได้รับโปรแกรมการจัดการตนเองของบุคคลและครอบครัว เป็นระยะเวลา 8 สัปดาห์ จัดให้ทั้ง 2 กลุ่ม มีคุณสมบัติใกล้เคียงกันในเรื่อง เพศ อายุ ระดับการศึกษา ระยะเวลาที่เป็นโรค และระดับ HbA1c เครื่องมือที่ใช้ในการทดลอง คือ โปรแกรมการจัดการตนเองของบุคคลและครอบครัวตามแนวคิดของ Ryan and Sawin เก็บรวบรวมข้อมูลโดยแบบบันทึกข้อมูลส่วนบุคคล วิเคราะห์ข้อมูลโดยใช้ค่าเฉลี่ย ร้อยละ ส่วนเบี่ยงเบนมาตรฐาน สถิติการทดสอบที (t-test)\n\nผลการวิจัย: \n1) ค่าเฉลี่ยระดับ HbA1c ของผู้สูงอายุโรคเบาหวานชนิดที่ 2 หลังได้รับโปรแกรมการจัดการตนเองของบุคคลและครอบครัวมีค่าต่ำกว่าก่อนได้รับโปรแกรม อย่างมีนัยสำคัญทางสถิติที่ระดับ .05\n2) ค่าเฉลี่ยระดับ HbA1c ของผู้สูงอายุโรคเบาหวานชนิดที่ 2 ของกลุ่มที่ได้รับโปรแกรมการจัดการตนเองของบุคคลและครอบครัวมีค่าต่ำกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05\n\nสรุป: โปรแกรมการจัดการตนเองของบุคคลและครอบครัว ทำให้ผู้สูงอายุมีการจัดการตนเองที่เหมาะสมอย่างสม่ำเสมอและต่อเนื่อง ส่งผลให้สามารถลดระดับ HbA1c ได้และลดได้ดีกว่าการพยาบาลตามปกติ

วัตถุประสงค์: เพื่อพัฒนารูปแบบการอบรมการช่วยฟื้นคืนชีพขั้นสูงสำหรับพยาบาลวิชาชีพ และเพื่อประเมินรูปแบบการอบรมการช่วยฟื้นคืนชีพขั้นสูงสำหรับพยาบาลวิชาชีพ งานผู้ป่วยหนัก โรงพยาบาลอุดรธานี\n\nรูปแบบการวิจัย: การวิจัยเพื่อพัฒนา (Research and development)\n\nวิธีดำเนินการวิจัย: ระยะที่ 1 ศึกษาสภาพปัญหาและความต้องการเกี่ยวกับรูปแบบการอบรมการช่วยฟื้นคืนชีพขั้นสูงสำหรับพยาบาลวิชาชีพ ระยะที่ 2 พัฒนารูปแบบการอบรมการช่วยฟื้นคืนชีพขั้นสูงสำหรับพยาบาลวิชาชีพ ระยะที่ 3 ประเมินประสิทธิภาพรูปแบบการอบรมการช่วยฟื้นคืนชีพขั้นสูงสำหรับพยาบาลวิชาชีพ ด้วยการประเมินความรู้เกี่ยวกับการช่วยฟื้นคืนชีพขั้นสูง การประเมินทักษะการปฏิบัติการช่วยฟื้นคืนชีพขั้นสูงในสถานการณ์จำลอง และประเมินอัตราการช่วยฟื้นคืนชีพขั้นสูงสำเร็จและอัตราการเกิดภาวะแทรกซ้อนจากการช่วยฟื้นคืนชีพขั้นสูงในหอผู้ป่วยที่กลุ่มตัวอย่างปฏิบัติงาน ภายหลังนำรูปแบบการอบรมการช่วยฟื้นคืนชีพขั้นสูงสำหรับพยาบาลวิชาชีพ ไปใช้แล้ว 3 เดือน \n\nผลการวิจัย:\n\n1. รูปแบบการอบรมการช่วยฟื้นคืนชีพขั้นสูงสำหรับพยาบาลวิชาชีพงานผู้ป่วยหนักมีดังนี้\n 1.1 จัดอบรมการช่วยฟื้นคืนชีพขั้นสูงซ้ำทุก 3 เดือน ครั้งละ 2 วัน โดยวันแรก อบรมทฤษฎีจำนวน 4 ชั่วโมง อบรมภาคปฏิบัติ 3 ชั่วโมง โดยฝึกทักษะการอ่านและแปลผลคลื่นไฟฟ้าหัวใจในสถานการณ์จำลอง วันที่ 2 ฝึกทักษะการปฏิบัติการช่วยฟื้นคืนชีพขั้นสูงในสถานการณ์จำลอง เป็นเวลา 7 ชั่วโมง\n 1.2 จัดทำ Pre- Arrest Signs และ Guideline ไปติดทุกหอผู้ป่วย และคัดเลือกครู ก. ประจำหอผู้ป่วย ทำหน้าที่เป็นผู้สอนและประเมินการปฏิบัติการช่วยฟื้นคืนชีพขั้นสูง ณ หอผู้ป่วยที่สังกัด\n2. ผลการประเมินประสิทธิภาพรูปแบบการอบรมการช่วยฟื้นคืนชีพขั้นสูงสำหรับพยาบาลวิชาชีพ\n 2.1. พบว่า คะแนนเฉลี่ยความรู้เกี่ยวกับการช่วยฟื้นคืนชีพขั้นสูงภายหลังอบรมครั้งที่ 1 มากกว่าก่อนการอบรม และมากกว่าระยะหลังการอบรมครั้งที่ 2(3 เดือน) อย่างมีนัยสำคัญทางสถิติ ส่วนค่าเฉลี่ยคะแนนทักษะการปฏิบัติการช่วยฟื้นคืนชีพขั้นสูงในสถานการณ์จำลอง หลังการอบรมครั้งที่ 1 สูงกว่าหลังการอบรมครั้งที่ 2 (3 เดือน) อย่างมีนัยสำคัญทางสถิติ\n 2.2 อัตราการช่วยฟื้นคืนชีพขั้นสูงสำเร็จและอัตราการเกิดภาวะแทรกซ้อนจากการช่วยฟื้นคืนชีพขั้นสูงในหอผู้ป่วยที่กลุ่มตัวอย่างปฏิบัติงาน พบว่า จำนวนผู้ป่วยที่มีภาวะหัวใจหยุดเต้นจำนวน 16 ราย สามารถช่วยฟื้นคืนชีพขั้นสูงสำเร็จ จำนวน 14 ราย คิดเป็นร้อยละ 88.89 อัตราการเกิดภาวะแทรกซ้อนด้วยอาการ Facture ribs, Hemothorax/Pneumothorax หรือ Rupture Heart เท่ากับ …

The articulation of Portuguese nasalized vowels poses some articulatory problems accompanied by negative acoustic effects for the performance of Brazilian art songs. The main objective was to find strategies that permit the singer to conciliate an idiomatic pronunciation of these vowels with a well-balanced resonance, a desirable quality in classical singing. Bacillus anthracis secretes three secretary proteins; lethal factor (LF), protective antigen (PA) and edema factor (EF). The LF has ability to check proliferation of mammary tumors, chiefly depending on mitogen activated protein kinase (MAPK) signaling pathway. Evaluation of therapeutic potential of recombinant LF (rLF), recombinant PA (rPA) and lethal toxin (rLF + rPA = LeTx) on the primary mammary ductal carcinoma cells revealed significant (p < 0.01) reduction in proliferation of tumor cells with mean inhibition indices of 28.0 ±1.37% and 19.6 ± 1.47% respectively. However, treatment with rPA alone had no significant anti-proliferative effect as evident by low mean inhibition index of 3.4 ± 3.87%. The higher inhibition index observed for rLF alone as compared to LeTx is contrary to the existing knowledge on LF, which explains the requirement of PA dependent endocytosis for its enzymatic activity. Therefore, the plausible existence of PA independent mode of action of LF including direct receptor mediated endocytosis or modulation of signal transduction cascade via unknown means is hypothesized. Persistent developmental stuttering is generally considered to be a speech disorder characterized by repetitions, prolongations and postural fixations, and is relatively resistant to therapy. While mainstream stuttering therapy continues to rely on behavioral speech targets, recent research suggests that mirror neuron systems can be activated to temporarily induce natural sounding fluent speech in those who stutter via exposure to second speech signals. This is the first article that the researcher is aware of that compares walking speed and confidence levels with 1 cane versus 2 canes. An observational study was conducted with 30 participants who had difficulty walking. Gait speed was assessed on a 4-m course with the participants using 1 cane, then 2 canes. Of 30 participants, 28 walked faster and demonstrated improved confidence with 2 canes. Use of 2 canes with gait might be a technique that people want to consider to improve confidence or to improve walking speed, or improve both.

Clinical Question: Is there evidence to suggest that the Landing Error Scoring System (LESS) is able to detect functional changes in landing mechanics in healthy individuals after participation in an injury prevention program (IPP)? Clinical Bottom Line: In a healthy, physically active population, there is strong evidence to support the use of the LESS as an outcome measure for changes in landing mechanics after the implementation of IPPs. Clinicians should consider the LESS as an evaluative tool for measuring the efficacy of IPPs in clinical practice.

Objective: To answer the following questions: (1) Does HRQOL differ among adolescent and collegiate athletes and nonathletes? Patients with urinary stone disease commonly present to the emergency department for management of their acute pain. In addition to providing analgesia, administration of drug (medical expulsive therapy) is often prescribed to assist passage of the urinary stone. BACKGROUND: Recommendations for vitamin D supplementation for preterm infants span a wide range of doses. Response to vitamin D supplementation and impact on outcomes in preterm infants is not well understood.

OBJECTIVE: Evaluate serum 25(OH)D3 concentration changes after 4 weeks in response to two different doses of vitamin D3 supplementation in a population of premature infants and quantify the impact on NICU outcomes.

One significant need in this body of literature is for examination of evidence-based evaluation practices. This project describes implementation and results of an evaluation protocol, Primary Occupations for Work and Employment Readiness (POWER), designed to elicit comprehensive occupational profiles from participants. ADHD has been diagnosed in approximately 6.4 billion children in the U.S., and it is estimated that 66% of those afflicted have 1 or more comorbid conditions. Children with ADHD are often from socioeconomically challenged households.
