The main aim of this study was to investigate the effects of Portulaca oleracea extract on tumor formation in colon cancer stem cells and chemotherapy sensitivity. In addition, this study analyzed the genetic changes within the Notch signal transduction pathway associated with the effects of the extracts. Serum-free cultures of colon cancer cells (HT-29) and HT-29 stem cells were treated with the chemotherapeutic drug 5-FU to assess sensitivity. Injections of the stem cells were also given to BALB/c mice to confirm tumor growth and characteristics. Skeletal co-morbidities in type 1 diabetes include an increased risk for fracture and delayed fracture healing, which are intertwined with disease duration and the presence of other diabetic complications. As such, chronic hyperglycemia is undoubtedly a major contributor to these outcomes, despite standard insulin-replacement therapy. การวิเคราะห์องค์ประกอบสมรรถนะพยาบาลวิชาชีพ หน่วยวิกฤตหลังผ่าตัดหัวใจและทรวงอก โรงพยาบาลระดับตติยภูมิ, กัญญารัตน์ ทะริยะ

การวิจัยนี้ มีวัตถุประสงค์เพื่อศึกษาองค์ประกอบและตัวแปรที่อธิบายสมรรถนะพยาบาลวิชาชีพ หน่วยวิกฤตหลังผ่าตัดหัวใจและทรวงอก โรงพยาบาลระดับตติยภูมิ กลุ่มตัวอย่างเป็นพยาบาลวิชาชีพที่ปฏิบัติงานในหน่วยวิกฤตหลังผ่าตัดหัวใจและทรวงอก โรงพยาบาลระดับตติยภูมิที่มีประสบการณ์การทำงานตั้งแต่ 1 ปีขึ้นไป จำนวน 276 คน ที่ได้จากการสุ่มตัวอย่างโดยวิธีการสุ่มอย่างง่าย เครื่องมือที่ใช้ในการเก็บข้อมูลเป็นแบบสอบถามที่ผู้วิจัยสร้างขึ้นมี 2 ขั้นตอน คือ 1) การทบทวนวรรณกรรม 2) การสัมภาษณ์ผู้เชี่ยวชาญที่มีประสบการณ์การทำงานในหน่วยวิกฤตหลังผ่าตัดหัวใจและทรวงอก โรงพยาบาลระดับตติยภูมิ ซึ่งผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิและหาความเที่ยงของเครื่องมือ โดยใช้สูตรสัมประสิทธิ์อัลฟาครอนบาค ได้ค่าความเที่ยงเท่ากับ .979 วิเคราะห์ข้อมูลโดยการหาค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และค่าสัมประสิทธิ์สหสัมพันธ์ของเพียร์สันโปรดักโมเมนต์ วิเคราะห์องค์ประกอบโดยการสกัดองค์ประกอบหลัก (Principle component) หมุนแกนองค์ประกอบแบบออโธโกนอล (Orthogonal) ด้วยวิธีแวริแมกซ์ (Varimax) ผลการวิจัยพบว่า องค์ประกอบสมรรถนะพยาบาลวิชาชีพ หน่วยวิกฤตหลังผ่าตัดหัวใจและทรวงอก โรงพยาบาลระดับตติยภูมิ มีทั้งหมด 7 องค์ประกอบ บรรยายด้วย 55 ตัวแปร มีค่าความแปรปรวนรวมคิดเป็นร้อยละ 79.963 ได้แก่ 1) องค์ประกอบที่ 1 สมรรถนะด้านการปฏิบัติการช่วยฟื้นคืนชีพขั้นสูงในผู้ป่วยวิกฤตหลังผ่าตัดหัวใจและทรวงอก ประกอบด้วย 14 ตัวแปร สามารถอธิบายความแปรปรวนได้มากที่สุด คิดเป็นร้อยละ 18.468 2) องค์ประกอบที่ 2 สมรรถนะด้านการส่งเสริมการพัฒนาคุณภาพและวิจัยทางการพยาบาล ประกอบด้วย 8 ตัวแปร สามารถอธิบายความแปรปรวนได้ ร้อยละ 12.736 3) องค์ประกอบที่ 3 สมรรถนะด้านการประเมินและเฝ้าระวังอาการผิดปกติในผู้ป่วยวิกฤตหลังผ่าตัดหัวใจและทรวงอก ประกอบด้วย 8 ตัวแปร สามารถอธิบายความแปรปรวนได้ ร้อยละ 11.919 4) องค์ประกอบที่ 4 สมรรถนะด้านการปฏิบัติการพยาบาลเฉพาะทางในผู้ป่วยวิกฤตหลังผ่าตัดหัวใจและทรวงอก ประกอบด้วย 8 ตัวแปร สามารถอธิบายความแปรปรวนได้ ร้อยละ 10.336 5) องค์ประกอบที่ 5 สมรรถนะด้านจริยธรรมและจรรยาบรรณวิชาชีพการพยาบาลผู้ป่วยวิกฤตหลังผ่าตัดหัวใจและทรวงอก ประกอบด้วย 5 ตัวแปร สามารถอธิบายความแปรปรวนได้ ร้อยละ 9.718 6) …

ที่มา: การติดสีของสารมิวซินในชั้นหนังแท้เป็นองค์ประกอบหนึ่งที่ใช้ช่วยในการวินิจฉัยโรคผื่นผิวหนังลูปัสอีริย์ทีมาโตซัส ชนิดดิสคอยด์ และในการะบวนเกิดโรค สารมิวซินในชั้นหนังแท้เป็นสิ่งที่ทำให้ผื่นผิวหนังอักเสบมากขึ้น อย่างไรก็ตามยังไม่เคยมีการศึกษาว่าการติดสีของสารมิวซินเป็นปัจจัยบอกการเกิดโรคลูปัสอีริย์ทีมาโตซัสทั่วร่างในผู้ป่วยมาก่อน
วัตถุประสงค์: ต้องการศึกษาความสามารถในการพยากรณ์การเกิดโรคลูปัสอีริย์ทีมาโตซัสทั่วร่าง (เอสแอลอี) ในผู้ป่วยโรคผื่นผิวหนังลูปัสอีริย์ทีมาโตซัส ชนิดดิสคอยด์ โดยใช้ระดับการติดสีของสารมิวซินในชั้นหนังแท้เป็นงานวิจัยแบบ exploratory study เพื่อมุ่งหวังงานวิจัยต่อยอด
วิธีการศึกษา: การวิจัย ณ จุดเวลาใดเวลาหนึ่ง โดยการสืบค้นผู้ป่วยที่วินิจฉัยว่าเป็นโรคผื่นผิวหนังลูปัสอีริย์ทีมาโตซัส ชนิดดิสคอยด์จากการผลพยาธิวิทยา ตั้งแต่ปี 2554-2560 จำนวน 77 คน ณ โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย นำชิ้นเนื้อมาย้อมสี H&E, PAS, Alcian blue เพื่อยืนยันการวินิจฉัย และประเมินการติดสีมิวซินจากชั้น papillary dermis, supficial reticular dermis, deep reticular dermis โดยให้คะแนน 0-3 ในแต่ละชั้น (mucin score) หลังจากนั้นเก็บข้อมูลการวินิจฉัยโรคลูปัสอีริย์ทีมาโตซัสทั่วร่างและผลทางห้องปฏิบัติการของผู้ป่วยจากเวชระเบียน
ผลการศึกษา: : พบผู้ป่วยโรคผื่นผิวหนังลูปัสอีริย์ทีมาโตซัส ชนิดดิสคอยด์อย่างเดียว จำนวน 49 คน ผู้ป่วยโรคผื่นผิวหนังลูปัสอีริย์ทีมาโตซัส ชนิดดิสคอยด์ที่พัฒนาเป็นโรคลูปัสอีริย์ทีมาโตซัสทั่วร่าง จำนวน 8 คน ผู้ป่วยโรคลูปัสอีริย์ทีมาโตซัสทั่วร่างที่มีผื่นผิวหนังลูปัสอีริย์ทีมาโตซัสชนิดดิสคอยด์ภายหลัง จำนวน 5 คน และผู้ป่วยโรคลูปัสอีริย์ทีมาโตซัสทั่วร่างที่มีผื่นผิวหนังลูปัสอีริย์ทีมาโตซัสชนิดดิสคอยด์ พร้อมกันขณะวินิจฉัย จำนวน 15 คน พบว่าค่าเฉลี่ยของการติดสีสารมิวซิน [OR=0.97 (95%CI0.49-1.92)] ค่าสูงสุดของการติดสีสารมิวซิน [OR=0.98 (95%CI0.6-1.58)] การติดสีของสารมิวซินแต่ละชั้นหนังแท้ [Papillary dermis : OR=0.7 (95%0.31-1.57), Superficial reticular dermis: OR=1.06 (95%CI0.62-1.85), Deep reticular dermis: OR=1.04 (95%CI0.64-1.66)] และการพบการติดสีของสารมิวซิน [OR=1.01 (95%CI0.31-3.32] ในชิ้นเนื้อของผู้ป่วยผื่นผิวหนังลูปัสอีริย์ทีมาโตซัสชนิดดิสคอยด์ ไม่สัมพันธ์กับการเป็นโรคโรคลูปัสอีริย์ทีมาโตซัสทั่วร่าง โดยปรับปัจจัยเรื่องของเพศและการกระจายของผื่น นอกจากนี้ยังพบว่าค่าเฉลี่ยของการติดสีสารมิวซินมีความแตกต่างอย่างมีนัยสำคัญในผู้ป่วยโรคผื่นผิวหนังลูปัสอีริย์ทีมาโตซัสชนิดดิสคอยด์ที่สูบบุหรี่และไม่สูบบุหรี่ …

อาการเคลื่อนไหวลำบากตอนนอน (nocturnal hypokinesia) เป็นอาการที่ทำให้เกิดผลแทรกซ้อนตามมา ได้ หลากหลาย ตั้งแต่ความรุนแรงของอาการน้อย เช่นอาการ หรือเสียชีวิตฉับพลันขณะนอน ซึ่งอาการเคลื่อนไหวลำบากขณะนอน เป็นปัญหาที่ไม่ค่อยได้รับความสนใจจากแพทย์โดยทั่วไป และตรวจพบได้ยากในการตรวจที่ห้องตรวจผู้ป่วยนอกปกติ จุดประสงค์ของการศึกษานี้ เพื่อการพัฒนาอุปกรณ์ในการวัดการเคลื่อนไหวตอนนอน ชนิดที่ใช้ติดกับตัวผู้ป่วย เพื่อใช้ประเมินอาการที่บ้าน และเปรียบเทียบการเคลื่อนไหวตอนนอน ของผู้ป่วยพาร์กินสัน กับคู่สามีหรือภรรยาของผู้ป่วยที่ไม่ได้เป็นโรคพาร์กินสัน ร่วมกับการหาปัจจัยที่ส่งผลต่อการเกิดอาการเคลื่อนไหวลำบากตอนนอน (nocturnal hypokinesia) ในผู้ป่วยพาร์กินสัน การดำเนินการศึกษาวิจัย เริ่มจากการพัฒนาชุดวัดการเคลื่อนไหวตอนนอน ชนิดติดกับตัวผู้ป่วย ซึ่งประกอบด้วย accelerometer และ gyroscope 3 แนวแกน ที่มีขนาดเล็ก เพื่อติดที่บริเวณลำตัวและแขนขาของผู้ป่วย ผลการศึกษา พบว่าในผู้ป่วยพาร์กินสัน 27 ราย และคู่สามีภรรยาที่ไม่ได้เป็นโรคพาร์กินสัน มีอายุเฉลี่ย น้ำหนัก ไม่แตกต่างกัน และผู้ป่วยมีการดำเนินโรค ส่วนใหญ่อยู่ในช่วงระดับกลาง มีอาการทั้งอาการเคลื่อนไหวผิดปกติ และอาการที่ไม่เกี่ยวกับการเคลื่อนไหว และมีการตอบสนองต่อการรักษาโดยยาทดแทนโดปามีน ไม่สม่ำเสมอ จากผลการประเมินด้วยชุดวัดการเคลื่อนไหวตอนนอน พบว่า ผู้ป่วยพลิกตัว ความเร็วและความเร่งเฉลี่ยในการพลิกตัว น้อยกว่าคู่สามีหรือภรรยา (P < 0.001) และมุมที่เฉลี่ยในการพลิกตัว น้อยกว่าคู่สามีหรือภรรยา (P= 0.003) และพบความสัมพันธ์ระหว่าง อาการละเมอผิดปกติ (RBD) กับจำนวนการพลิกตัวในช่วงครึ่งหลังของการนอน การศึกษานี้ได้แสดงให้เห็นถึงประสิทธิภาพของชุดวัดการเคลื่อนไหวตอนนอน ที่พัฒนาขึ้นร่วมกับ แสดงให้เห็นการประเมินอาการตอนกลางคืน เป็นค่าที่วัด เปรียบเทียบได้ และพบอาการเคลื่อนไหวลำบากในผู้ป่วยพาร์กินสัน เพื่อประโยชน์ในการปรับยาเพื่อครอบคลุมอาการตอนกลางคืนต่อไป

Background : Radiation-induced xerostomia is the most common complication in patients with nasopharyngeal carcinoma (NPC) after radiation therapy (RT).Objective : To correlate salivary dysfunction as assessed by clinical xerostomia grade with change in computed tomography (CT) density of the salivary glands on contrast-enhanced CT in patients with NPC treated with RT.Methods : Seventeen patients with NPC, who had contrast-enhanced CT before and after radiation therapy (70 - 75 Gy) were assessed retrospectively. เหตุผลของการทำวิจัย : ปัญหาหลอดเลือดเทียมตีบตันเฉียบพลัน (acute shunt occlusion orblockage) ในผู้ป่วยเด็กโรคหัวใจพิการแต่กำเนิดชนิดเขียว ที่เข้ารับการผ่าตัด modified Blalock-Taussig shunt (MBTS) เป็นภาวะแทรกซ้อนที่สำคัญ ซึ่งอาจนำไปสู่การเสียชีวิตของผู้ป่วยได้วัตถุประสงค์ : เพื่อศึกษาหาปัจจัยที่มีผลต่อการเกิดภาวะหลอดเลือดเทียมตีบตันเฉียบพลัน อันอาจเป็นประโยชน์ในการช่วยวินิจฉัยและวางแผนป้องกันภาวะหลอดเลือดเทียมตีบตันเฉียบพลัน ซึ่งอาจช่วยลดภาวะแทรกซ้อนที่จะเกิดตามมาได้วิธีดำเนินการวิจัย : เก็บรวบรวมข้อมูลของผู้ป่วยเด็กโรคหัวใจพิการแต่กำเนิดชนิดเขียวอายุ 0 – 3 เดือนที่เข้ารับการผ่าตัด MBTS ในโรงพยาบาลจุฬาลงกรณ์ช่วงระหว่างวันที่ 1 มกราคม พ.ศ.2545 - 31 ธันวาคม พ.ศ. 2553จำนวน 123 ราย โดยแบ่งเป็นข้อมูลทั่วไป ข้อมูลก่อนและหลังการผ่าตัดข้อมูลการผ่าตัด แล้วนำมาวิเคราะห์ทางสถิติ เพื่อหาความสัมพันธ์ต่อการเกิดภาวะหลอดเลือดเทียมตีบตันเฉียบพลันผลการวิจัย : ผู้ป่วยจำนวนทั้งหมด 123 ราย อายุเฉลี่ย 28 วัน น้ำหนักตัวเฉลี่ย 3.17กิโลกรัม ขนาดของหลอดเลือดเทียมที่ใช้อยู่ระหว่าง 2.5 - 5.0 มิลลิเมตรพบอุบัติการณ์การเกิดภาวะหลอดเลือดเทียมตีบตันเฉียบพลันจำนวน22 ราย (17.9%) โดยพบว่าปัจจัยที่มีความเกี่ยวข้องกับภาวะหลอดเลือดเทียมตีบตันเฉียบพลันอย่างมีนัยสำคัญทางสถิติ ได้แก่ น้ำหนักตัวของผู้ป่วย, ขนาดเส้นผ่านศูนย์กลางของ subclavian artery, ขนาดของหลอดเลือดเทียมที่ใช้, และการให้ PGE1 เป็นระยะเวลานานในผู้ป่วยที่มีpatent ductus arteriosus (PDA) ก่อนการทำผ่าตัด (P value = 0.007,0.00, 0.042 และ 0.001 ตามลำดับ) การเปลี่ยนแปลงของความเข้มข้นของออกซิเจน (SpO2) ที่ลดลง 4 ชั่วโมงหลังผ่าตัดเป็นสิ่งที่บ่งชี้อย่างหนึ่งว่าอาจเริ่มมีภาวะหลอดเลือดเทียมตีบตันเกิดขึ้น (P value = 0.012)ผู้ป่วยที่มีภาวะหลอดเลือดเทียมตีบตันเฉียบพลันจำนวน 22 ราย ได้รับการรักษาโดยการทำ cardiac catheterization with interventionเพียงอย่างเดียว 13 ราย, ทำ cardiac catheterization …

This photovoice study examined the hypothesis that photos and narratives produced by older adults would facilitate the co-construction of a shared narrative about the food access issues in a rural county in West Virginia. This qualitative synthesis explores a public health narrative and events in the US that may have contributed to the prominence of a one-size-fits all communications frame and public health tool that excludes the significance of social status. An analysis of past events is used to carry out a textual analysis of two communications framing studies. One central narrative is found to be more effective than other potential themes on equality, social conditions, and health inequities. The 2010 USDA Dietary Guidelines for Americans (DGA) recommends a diet largely composed of fruit and vegetables. Consuming a diet high in fruit and vegetables and low in refined carbohydrates and saturated fat may reduce an individual’s risk for type 2 diabetes, nonalcoholic fatty liver disease, low-grade chronic inflammation, and metabolic syndrome (MetS). Introduction Cardiovascular diseases are the leading cause of morbidity and mortality among individuals with diabetes. Despite the beneficial effects of antidiabetic drugs (ADDs) in terms of lowering haemoglobin A1c, several ADDs have been shown to increase the risk of cardiovascular events. Given the high prevalence of cardiovascular disease among individuals with diabetes, it is important to weigh the benefits of ADDs against their cardiovascular safety. Conduct a systematic review of previous systematic reviews with meta-analysis to determine the effects of exercise (aerobic, strength or both) on cancer-related-fatigue (CRF) in adults with any type of cancer.

Methods

Systematic reviews with meta-analyses of previous randomized controlled trials published through July of 2016 were included by searching six electronic databases and cross-referencing. Dual-selection and data abstraction were conducted. Methodological quality was assessed using the Assessment of Multiple Systematic Reviews (AMSTAR) instrument. Overweight and obesity is a major public health concern that includes associations with the development of cardiovascular disease (CVD) risk factors during childhood and adolescence as well as premature mortality in adults. Objective: Headache is the most common type of pain reported by people with schizophrenia. This study aimed to establish prevalence, characteristics and management of these headache.

Method: One-hundred participants with schizophrenia/schizoaffective disorder completed a reliable and valid headache questionnaire. Two clinicians independently classified each headache as migraine (MH), tension-type (TTH), cervicogenic (CGH) or other (OH).

Results: The twelve-month prevalence of headache (57%) was higher than the general population (46%) with no evidence of a relationship between psychiatric clinical characteristics and presence of headache. Prevalence of CGH (5%) and MH (18%) was comparable to the general population. TTH …

Eyetracking is commonly used to investigate attentional bias. Although some studies have investigated the internal consistency of eyetracking, data are scarce on the test–retest reliability and agreement of eyetracking to investigate attentional bias. This study reports the test–retest reliability, measurement error, and internal consistency of 12 commonly used outcome measures thought to reflect the different components of attentional bias: overall attention, early attention, and late attention.

Introduction: Heterotopic ossification (HO) is the formation of lamellar bone within connective and other tissue where bone should not form and is a rare complication after burn injury. วัตถุประสงค์: เพื่อศึกษาปัจจัยทำนายพฤติกรรมการป้องกันโรคข้อเข่าเสื่อมของพยาบาลวิชาชีพ โรงพยาบาลระดับตติยภูมิ กรุงเทพมหานคร\n\nแบบแผนงานวิจัย: การวิจัยแบบความสัมพันธ์เชิงทำนาย\n\nวิธีดำเนินการวิจัย:กลุ่มตัวอย่าง คือ พยาบาลวิชาชีพซึ่งปฏิบัติงานในโรงพยาบาลระดับตติยภูมิ กรุงเทพมหานคร จำนวน 2 แห่ง ได้แก่ โรงพยาบาลเลิดสินและโรงพยาบาลสมเด็จพระปิ่นเกล้าจำนวน 145 คนคัดเลือกกลุ่มตัวอย่างด้วยการสุ่มอย่างง่าย รวบรวมข้อมูลโดยใช้แบบสอบถาม ประกอบด้วยข้อมูลส่วนบุคคล การรับรู้โอกาสเสี่ยงของการเกิดโรค การรับรู้ความรุนแรงของโรค การรับรู้ประโยชน์ของการป้องกันโรคข้อเข่าเสื่อม การรับรู้อุปสรรค การรับรู้ความสามารถของตนเองในการปฏิบัติเพื่อป้องกันโรค และพฤติกรรมการป้องกันโรคข้อเข่าเสื่อมของพยาบาลวิชาชีพ โดยผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิจำนวน 5 คนมีค่าความเที่ยงเท่ากับ .76, .73, .86, 79, 82 และ .70 วิเคราะห์ข้อมูลโดยคำนวณค่าสัมประสิทธิ์สหสัมพันธ์และค่าสัมประสิทธิ์สหสัมพันธ์พหุคูณแบบขั้นตอน\n\nผลการวิจัย:พฤติกรรมการป้องกันโรคข้อเข่าเสื่อมของพยาบาลวิชาชีพ โดยรวมอยู่ในระดับปานกลาง (=36.06, S.D.=5.41) การรับรู้ความสามารถของตนเองในการปฏิบัติเพื่อป้องกันโรค และอายุ มีความสัมพันธ์ทางบวกกับพฤติกรรมการป้องกันโรคข้อเข่าเสื่อมของพยาบาลวิชาชีพ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r =.572 และr =.200) และการรับรู้ความสามารถของตนเองในการปฏิบัติเพื่อป้องกันโรคและอายุ สามารถร่วมกันทำนายพฤติกรรมการป้องกันโรคข้อเข่าเสื่อมได้ร้อยละ 33.8 อย่างมีนัยสำคัญทางสถิติที่ระดับ .05\n\nสรุป: ควรส่งเสริมพยาบาลวิชาชีพให้มีพฤติกรรมการป้องกันโรคข้อเข่าเสื่อมด้วยการส่งเสริมการรับรู้ความสามารถของตนเองในการปฏิบัติเพื่อป้องกันโรค โดยมุ่งจัดกิจกรรมยืดเหยียดร่างกายระหว่างการปฏิบัติงานในกลุ่มพยาบาลวิชาชีพ โดยมุ่งเน้นในกลุ่มที่มีอายุมากกว่า 40 ปีขึ้นไป

วัตถุประสงค์: เพื่อศึกษา 1) คุณภาพชีวิตของผู้ดูแลในครอบครัว 2) ความสัมพันธ์ระหว่างลักษณะผู้ดูแลในครอบครัว ภาระผู้ดูแลและแรงสนับสนุนทางสังคมกับคุณภาพชีวิตของผู้ดูแลในครอบครัว 3) ปัจจัยทำนายคุณภาพชีวิตของผู้ดูแลในครอบครัวผู้ป่วยไตวายเรื้อรังที่ได้รับการล้างล้างไตทางช่องท้องแบบต่อเนื่อง\n\nรูปแบบการวิจัย: การศึกษาแบบบรรยายเชิงทำนาย\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ดูแลในครอบครัวผู้ป่วยไตวายเรื้อรังที่ได้รับการล้างไตทางช่องท้องแบบต่อเนื่อง มารับบริการที่แผนกล้างไตทางช่องท้อง โรงพยาบาลราชบุรี โรงพยาบาลพระจอมเกล้า และโรงพยาบาลหัวหิน โดยคัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจงตามเกณฑ์การคัดเข้า จำนวน 98 ราย เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามลักษณะผู้ดูแลในครอบครัว ภาระผู้ดูแล แรงสนับสนุนทางสังคม และคุณภาพชีวิตผู้ดูแลในครอบครัวผู้ป่วยไตวายเรื้อรังที่ได้รับการล้างไตทางช่องท้องแบบต่อเนื่อง ตรวจสอบคุณภาพความเที่ยงของเครื่องมือ ได้ค่าสัมประสิทธ์อัลฟ่าของครอนบาค เท่ากับ .91, .96 และ.95 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณาค่าสัมประสิทธ์สหสัมพันธ์เพียร์สันและสถิติถดถอยเชิงพหุคูณ\n\nผลการวิจัย: 1) ค่าเฉลี่ยคะแนนคุณภาพชีวิตโดยรวมยู่ในระดับปานกลาง (= 90.11, SD.= 13.77) 2) อายุผู้ดูแลในครอบครัวและแรงสนับสนุนทางสังคมมีความสัมพันธ์ทางลบกับคุณภาพชีวิตของผู้ดูแลในครอบครัวผู้ป่วยที่ล้างไตผ่านทางช่องท้องอย่างต่อเนื่อง อย่างมีนัยสำคัญทางสถิติ (r = -.294 และ r =-.528,p < .01) และแรงสนับสนุนทางสังคมมีความสัมพันธ์ทางบวกกับคุณภาพชีวิตของผู้ดูแลในครอบครัวผู้ป่วยไตวายเรื้อรังที่ได้รับการล้างไตทางช่องท้องแบบต่อเนื่องอย่างมีนัยสำคัญทางสถิติ (r = .441, p < .01)3) ปัจจัยที่ทำนายคุณภาพชีวิตของผู้ดูแลในครอบครัวได้อย่างมีนัยสำคัญทางสถิติ ประกอบด้วย ภาระผู้ดูแลในครอบครัว (β = -.411, p =.000) แรงสนับสนุนทางสังคม (β = .303, p =.001) และอายุผู้ดูแลในครอบครัว (β =-.168, p = .05) โดยสามารถอธิบายการเปลี่ยนแปลงระดับของคุณภาพชีวิตผู้ดูแลในครอบครัวได้ร้อยละ 41 (R2 = .409, F = 12.753, p < .001)\n\nสรุป: บุคลากรทางสาธารณสุขสามารถนำผลการวิจัยมาใช้ในการกำหนดแนวทางในการประเมินการวางแผนเพื่อส่งเสริมคุณภาพชีวิตของผู้ดูแลในครอบครัวผู้ป่วยที่ล้างไตผ่านทางช่องท้องอย่างต่อเนื่องให้ดีขึ้น

วัตถุประสงค์: เพื่อศึกษาผลของการชี้แนะต่อพฤติกรรมการจัดการตนเองและระดับน้ำตาลในเลือดของผู้เป็นเบาหวานชนิดที่ 2 ที่ฉีดอินซูลิน\n\nรูปแบบการวิจัย: การวิจัยแบบกึ่งทดลอง\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้เป็นเบาหวานชนิดที่ 2 ที่รับการรักษาโดยการฉีดอินซูลิน อายุ 30-65 ปี ที่มีระดับ HbA1C ระหว่าง 7-9% มารับบริการที่หน่วยบริการสุขภาพระดับปฐมภูมิ คลินิกเบาหวานเวชกรรมสังคม โรงพยาบาลเทพรัตน์นครราชสีมา จำนวน 50 คน คัดเลือกโดยสุ่มอย่างง่าย จัดเข้าเป็นกลุ่มทดลอง 25 คน และกลุ่มควบคุม 25 คน กลุ่มทดลองได้รับการชี้แนะ คู่มือการจัดการตนเอง วงล้อมหัศจรรย์ และรู้ทันอินซูลิน กลุ่มควบคุมได้รับบริการตามปกติ เครื่องมือที่ใช้ในการทดลอง ได้แก่ แผนการชี้แนะตามแนวคิดของ Haas จำนวน 5 ครั้ง ประกอบด้วย การชี้แนะที่คลินิก 2 ครั้ง ชี้แนะที่บ้าน 2 ครั้ง ชี้แนะทางโทรศัพท์ 1 ครั้ง รวมระยะเวลา 8 สัปดาห์ เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูลเป็นแบบสอบถามพฤติกรรมการจัดการตนเอง และแบบบันทึกผลการตรวจระดับน้ำตาลในเลือดหลังอดอาหาร (FBS) แบบสอบถามมีค่า CVI เท่ากับ 1 และค่าสัมประสิทธิ์แอลฟาครอนบาคเท่ากับ.86วิเคราะห์ข้อมูลโดยใช้ค่าเฉลี่ย ร้อยละ ส่วนเบี่ยงแบนมาตรฐาน สถิติการทดสอบที (t-test)\n\nผลการวิจัย: \n1) กลุ่มที่ได้รับการชี้แนะมีค่าเฉลี่ยผลต่างคะแนนพฤติกรรมการจัดการตนเอง มากกว่ากลุ่มที่ได้รับการดูแลตามปกติอย่างมีนัยสำคัญทางสถิติ (t = 10.67, p < .001)\n\n2) กลุ่มที่ได้รับการชี้แนะมีค่าเฉลี่ยผลต่างระดับน้ำตาลในเลือดหลังอดอาหารมากกว่ากลุ่มที่ได้รับการดูแลตามปกติอย่างมีนัยสำคัญทางสถิติ (t = -4.15, p < .001)\n\nสรุป: การชี้แนะส่งผลให้ผู้เป็นเบาหวานชนิดที่ 2 ที่ฉีดอินซูลินมีพฤติกรรมการจัดการตนเองทุกด้านดีขึ้น ส่งผลให้ระดับน้ำตาลในเลือดหลังอดอาหารลดลง

ผลของโปรแกรมการจัดการตนเองของบุคคลและครอบครัวต่อระดับน้ำตาลเฉลี่ยสะสมของผู้สูงอายุโรคเบาหวานชนิดที่ 2, แสงอรุณ สุรวงค์, ทัศนา ชูวรรธนะปกรณ์

วัตถุประสงค์: เพื่อศึกษาผลของโปรแกรมการจัดการตนเองของบุคคลและครอบครัวต่อระดับฮีโมโกลบินเอวันซีของผู้สูงอายุโรคเบาหวานชนิดที่ 2\n\nแบบแผนการวิจัย: การวิจัยแบบกึ่งทดลอง\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ที่มีอายุ 60 ปีบริบูรณ์ขึ้นไป ที่ได้รับการวินิจฉัยว่าเป็นโรคเบาหวานชนิดที่ 2 ที่มีระดับ HbA1c > 7.5% ที่มารับการรักษาที่คลินิกเบาหวาน-ธัยรอยด์ และระบบฮอร์โมน แผนกผู้ป่วยนอก โรงพยาบาลจุฬาลงกรณ์ แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 22 คน กลุ่มควบคุมได้รับการพยาบาลตามปกติ กลุ่มทดลองได้รับโปรแกรมการจัดการตนเองของบุคคลและครอบครัว เป็นระยะเวลา 8 สัปดาห์ จัดให้ทั้ง 2 กลุ่ม มีคุณสมบัติใกล้เคียงกันในเรื่อง เพศ อายุ ระดับการศึกษา ระยะเวลาที่เป็นโรค และระดับ HbA1c เครื่องมือที่ใช้ในการทดลอง คือ โปรแกรมการจัดการตนเองของบุคคลและครอบครัวตามแนวคิดของ Ryan and Sawin เก็บรวบรวมข้อมูลโดยแบบบันทึกข้อมูลส่วนบุคคล วิเคราะห์ข้อมูลโดยใช้ค่าเฉลี่ย ร้อยละ ส่วนเบี่ยงเบนมาตรฐาน สถิติการทดสอบที (t-test)\n\nผลการวิจัย: \n1) ค่าเฉลี่ยระดับ HbA1c ของผู้สูงอายุโรคเบาหวานชนิดที่ 2 หลังได้รับโปรแกรมการจัดการตนเองของบุคคลและครอบครัวมีค่าต่ำกว่าก่อนได้รับโปรแกรม อย่างมีนัยสำคัญทางสถิติที่ระดับ .05\n2) ค่าเฉลี่ยระดับ HbA1c ของผู้สูงอายุโรคเบาหวานชนิดที่ 2 ของกลุ่มที่ได้รับโปรแกรมการจัดการตนเองของบุคคลและครอบครัวมีค่าต่ำกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05\n\nสรุป: โปรแกรมการจัดการตนเองของบุคคลและครอบครัว ทำให้ผู้สูงอายุมีการจัดการตนเองที่เหมาะสมอย่างสม่ำเสมอและต่อเนื่อง ส่งผลให้สามารถลดระดับ HbA1c ได้และลดได้ดีกว่าการพยาบาลตามปกติ
