Center of Excellence in Rural Health Workforce Reports

Family Resource and Youth Services Center (FRYSC) coordinators help at-risk students and their families obtain access to the necessary resources to overcome obstacles that affect their ability to learn. Although mandated to provide certain services, each FRYSC coordinator is also able to identify the unique needs of their student population and tailor some services specifically to the children and families in their school community. The current study looks at the educational levels, experience on the job and feelings of preparedness for their positions. Additionally, FRYSC coordinators were asked to provide insight as to which factors influence their job success.

Breast cancer is the most common cancer among women in the United States with over 60% of cases diagnosed as early stage disease. For those women without prohibiting clinical or cosmetic concerns, a choice between breast-conserving surgery and mastectomy can be made. Either choice confers equivalent survival. The decision-making process also involves consideration of recurrence risk as well as management of the unaffected, contralateral breast for both future surveillance and risk reduction. Folate is required for metabolic processes and neural development. Insuring its adequate levels for pregnant women through supplementation of grain-based foods with synthetic folic acid (FA) in order to prevent neural tube defects has been an ongoing public health initiative. However, because women are advised to take multivitamins containing FA before and throughout pregnancy, the supplementation together with natural dietary folates has led to a demographic with high and rising serum levels of unmetabolized FA. BACKGROUND: Multiple treatment options, combined with disease heterogeneity, have created nursing challenges in the management of adverse events (AEs) during antilymphoma therapy. Testing has revealed that less than half of participating nurses correctly graded peripheral neuropathy and neutropenia related to antilymphoma regimens.

OBJECTIVES: This article identifies nursing challenges in the management of AEs associated with therapy for lymphomas and describes how strategies in critical thinking can help meet those challenges.

The purpose of this proposal is to link the gold standard power test on land (Wingate test) to an in-water power test. There is a lack of information on power in the water and would be beneficial in tracking swimmers progress throughout a season and between seasons. Power curves of individuals on the Wingate test will be compared to those of In-water tests to see weather this water test is valid. The experiment will follow IRB rules and regulation and the staff will adhere to the safety conditions set forth by the IRB.

This honors thesis explores the complex interplay between health status, healthcare, and tribal sovereignty among native communities in the United States. These relationships are explored through analyzing the paradoxical and condescending nature of the Federal Trust Responsibility in relation to government-organized healthcare programs for natives. In establishing this relationship, the thesis goes on to illustrate how native communities have effectively fought to regain sovereignty through reclaiming autonomy of their healthcare systems through the use of the 1975 Indian Self-Determination and Education Assistance Act. Au Québec, les statistiques de la Commission des normes, de l’équité, de la santé et de la sécurité du travail (CNESST) indiquent que les déclarations des lésions professionnelles par les jeunes travailleurs ont beaucoup diminué dans la dernière décennie. Certains jeunes demeurent cependant plus vulnérables aux lésions professionnelles, dont ceux qui éprouvent des difficultés d’apprentissage ou d’adaptation. Les systèmes de chauffage, de ventilation et de climatisation de l’air (CVCA) permettent le maintien d’une bonne qualité de l’air dans différents types d’édifices. Toutefois, lorsque ces systèmes ne sont pas bien entretenus, ils peuvent devenir des sources d’émission de polluants dans l’air ambiant. Actuellement, les critères de déclenchement du nettoyage des conduits de ventilation sont souvent basés sur les dépôts de poussières. Au cours des dernières années, la conscientisation écologique des populations a stimulé de façon importante l’intérêt qu’elles portent à la protection de l’environnement. Des comportements favorisant le développement durable et permettant la préservation à long terme de l’environnement comme le compostage sont maintenant répandus. De par sa nature, le compostage nécessite l’action des microorganismes. À chaque phase de compostage correspondent des populations de microorganismes caractéristiques et, par conséquent, des émissions de bioaérosols qui leur sont propres. Le technicien ambulancier paramédical (TAP) a comme fonction principale de prodiguer les soins préhospitaliers d’urgence tout en assurant, de façon sécuritaire, le transport du bénéficiaire vers un centre hospitalier. Fondamentalement, son objectif est de réduire au minimum la mortalité et la morbidité des bénéficiaires en minimisant le temps de réponse. The relationship between disordered eating, body dissatisfaction, and religion is complex. Previous research demonstrated that religious body affirmations are beneficial in reducing body dissatisfaction among undergraduate females. Because previous research used affirmations which were simultaneously religious in nature and related to the body, it is difficult to determine if the religiousness of the affirmation is the beneficial characteristic. The current study investigated which type of affirmation had the strongest effect on disordered eating attitudes and body dissatisfaction. This study was designed to identify the financial burdens associated with purchasing medical foods and the impact this has on the dietary management of adults with PKU. PKU is a genetic disorder in which the body cannot metabolize the amino acid, phenylalanine, which occurs in all foods with protein. Dietary treatment of foods low in protein along with medical supplementation is required for the treatment of PKU. If PKU is left untreated this can lead to severe neurological damage. Popular media often features stories about 'helicopter parenting' and asserts that this style is detrimental to youngsters (c.f., Fingerman, Cheng, Wesselmann, Zarit, Furstenberg, & Birditt, 2012; LeMoyne & Buchanan, 2011). However, few studies have examined the phenomenon empirically (Fingerman et al., 2012), particularly in a college population. The few existing studies of 'helicopter parenting' have evaluated different facets of this parenting style (e.g., intrusive parenting, inappropriate contact with school personnel), which makes comparing research results difficult. In this study, stressors, burdens, and strains were analyzed in relation to caregiver demographics and the following variables: caregiving relationship quality, caregiving roles, communication, and resources (time and finances). The aim of this study was to evaluate whether volumetric changes in persistent periapical lesions can be detected in follow-ups six months to five years after apicoectomy using cone-beam computed tomography. Patients with a previous treatment history of apicoectomy and for whom a pre-surgical CBCT scan was taken between November 2010 and December 2015 were invited to participate in the study. A post-surgical CBCT image of the treated tooth was obtained at the recall visit. Objective: The purpose was to compare orthodontists’ and parents’ perception of orthodontic treatment outcomes in the anterior-posterior (AP) dimension. Assessment of treatment time and compliance were also investigated.

Material and Methods: Parallel surveys for orthodontists (n=1000) and parents (n=750) displayed occlusions from 3 mm Class III (Cl III:3) to 3 mm Class II. Participants rated occlusal relationships on a 100 mm VAS from least to most acceptable (0-100).

Results: 233 orthodontists (23%) and 243 parents (32%) responded. Orthodontists (mean=93.9, 25.9) and parents (mean=80.7, 40.9) rated Class I (Cl I) occlusion most and Cl III:3 least acceptable. No significant difference was …

There is a chronic lack of donor hearts to meet the need for heart transplant both in the US and worldwide. Further, the use of available hearts is limited by the short period between collection and implantation during which the heart can be safely preserved ex vivo. Using mid-thermic Langendorff machine perfusion, we have been able to preserve the metabolic function of a healthy heart for up to 8 hours, twice the limit for current static cold storage. A Comparison Of Moderate Oral Sedation Drug Regimens For Pediatric Dental Treatment: A Pilot Study, Ojas A. Parikh

Purpose: Compare moderate oral sedation of pediatric patients using Hydroxyzine and Meperidine with either Diazepam or Midazolam in management of pediatric dental patients.

Methods: Randomized, double-blind, crossover pilot study of patients 3 to 7 years of age requiring two sedation visits. Frankl and Houpt behavior scores recorded at injection time, initiation of treatment and 100% oxygen at end of treatment. Postoperative phone call surveys conducted within eight hours and within 24 hours of discharge. Wilcoxon Signed-Rank tests, Fisher’s Exact Chi-squared test and 0.10 significance level.

The overall objectives of this research project were i) to develop and evaluate methods of characterizing nasal spray products using realistic nasal airway models as more clinically relevant in vitro tools and ii) to develop and evaluate a novel high-efficiency antibiotic nanoparticle dry powder formulation and delivery device. Scrub typhus is a potentially fatal infection that threatens one billion persons in the Asia-Pacific region and is caused by the obligate intracellular bacterium, Orientia tsutsugamushi. How this organism facilitates its intracellular survival and pathogenesis is poorly understood. Intracellular bacterial pathogens utilize the Type 1 (T1SS) or Type 4 secretion system (T4SS) to translocate ankyrin repeat-containing proteins (Anks) into the host cell to modulate host cell processes. The O. tsutsugamushi genome encodes one of the largest known bacterial Ank libraries as well as Type 1 and Type 4 secretion systems (T1SS and T4SS), which are expressed during infection. In …

Extensive research has been undertaken on the examination of tissue biopsies using vibrational spectroscopic techniques. However, fewer studies have focused on less invasive and commonly acquired blood samples. Cigarette smoking causes cardiovascular disease and is associated with poor kidney function in individuals with diabetes mellitus and primary kidney diseases. However, the association of smoking on patients with atherosclerotic renal artery stenosis has not been studied. The current study utilized data from the Cardiovascular Outcomes in Renal Atherosclerotic Lesions (CORAL, NCT00081731) clinical trial to evaluate the effects of smoking on the risk of cardio-renal events and kidney function in this population. INTRODUCTION. Decreased expression of highly immunogenic cancer-testis antigens (CTA) might help tumor to achieve low immunogenicity, escape immune surveillance and grow unimpeded. Our aim was to evaluate CTA expression in tumor and normal tissues and to investigate possible means of improving the immune response in a murine prostate cancer (CaP) model by using the combination of epigenetic modifier 5-azacitidine (5-AzaC) and immunomodulator lenalidomide. The purpose of this review is to discuss the indications for operative and nonoperative management in patients with blunt liver injuries.

Recent Findings

Over the past several years, research has begun to show that nonoperative management in blunt liver injuries is feasible and has favorable outcomes over immediate operations in patients who are hemodynamically stable. This includes high-grade injuries who were previously taken to the operating room, in the absence of peritoneal signs or instability, for washout and packing. Objective: To review the history of the innovation of damage control (DC) for management of trauma patients.

Background: DC is an important development in trauma care that provides a valuable case study in surgical innovation.

Methods: We searched bibliographic databases (1950-2015), conference abstracts (2009-2013), Web sites, textbooks, and bibliographies for articles relating to trauma DC. The innovation of DC was then classified according to the Innovation, Development, Exploration, Assessment, and Long-term study model of surgical innovation.

Background

The objective of this systematic review and meta-analysis was to determine the effect of REBOA, compared to resuscitative thoracotomy, on mortality and among non-compressible torso hemorrhage trauma patients.

Methods

Relevant articles were identified by a literature search in MEDLINE and EMBASE. We included studies involving trauma patients suffering non-compressible torso hemorrhage. Studies were eligible if they evaluated REBOA and compared it to resuscitative thoracotomy. Two investigators independently assessed articles for inclusion and exclusion criteria and selected studies for final analysis. We conducted meta-analysis using random effect models.
