The breast is an important organ system for the adolescent female and occasionally for the adolescent male as well. This chapter reviews basic principles of breast concerns and disorders including congenital anomalies, asymmetric breast development, underdeveloped breasts, virginal hypertrophy, breast masses, mastaglia, galactorrhea, gynecomastia in males, and others. Clinicians should be attuned to issues and concerns for this important structure in adolescents.

High rates of unprotected sexual behavior in adolescents result in millions of sexually transmitted diseases (STDs) in the world. This paper reviews factors inducing high STD rates, specific STDs, and their management based on 2010 US Centers for Disease and Prevention (CDC) STD guidelines. Clinicians should screen all their sexually active adolescent patients for STDs and provide preventive education as well as treatment measures.

Over 14 million adolescents in the world annually give birth and nearly 800.000 adolescents in the United States become pregnant each year. This paper considers concepts of teen pregnancy including risks to the mother as well as offspring, the adolescent father, issues related to abortion, and concepts of prevention of unwanted pregnancy in youth. Adolescent pregnancy is a global phenomenon affecting all societies and cultures.

This paper reviews basic concepts of menstrual disorders in adolescents beginning with an overview of menstrual physiology followed by consideration of various abnormal menstrual patterns: amenorrhea (primary and secondary), dysfunctional uterine bleeding, dysmenorrhea (primary and secondary), and premenstrual syndrome.

Hawthorn {Crataegus monogyna) is a herb that has been used medicinally since ancient times. In modern medicine it is known for its cardiotonic properties and is used as a cardiovascular protective, hypotensive and hypocholesterolemic agent. In recent years supercritical fluid extraction (SFE) has become more popular as an extraction method for botanical material as it is an environmentally-friendly method that can produce toxic-free extracts. A retrospective review of 21 patients that underwent bone screw removal from the elbow was studied in relation to the type of metal, duration of implantation, and the location of the screws about the elbow. Screw failure during extraction was the dependent variable. Five of 21 patients experienced hardware failure during extraction. Fourteen patients had titanium alloy implants. In four cases, titanium screws broke during extraction. Compared to stainless steel, titanium screw failure during removal was not statistically significant (P = 0.61). Screw removal 12 months after surgery was more likely to result in broken, retained screws in general …

Human TP53 gene is characterised by a polymorphism at codon 72 leading to an Arginine-to-Proline (R/P) substitution. The two resulting p53 isoforms have a different subcellular localisation after stress (more nuclear or more mitochondrial for the P or R isoform, respectively). p53P72 variant is more efficient than p53R72 in inducing the expression of genes involved in nuclear DNA repair. Since p53 is involved also in mitochondrial DNA (mtDNA) maintenance, we wondered whether these p53 isoforms are associated with different accumulation of mtDNA damage. The goal of the study was to determine the association between diabetes and inflammation in clinically diagnosed diabetes patients. We hypothesized that low-grade inflammation in diabetes is associated with the level of glucose control. Using a cross-sectional design we compared pro and anti-inflammatory cytokines in a community recruited cohort of 367 Mexican Americans with type 2-diabetes having a wide range blood glucose levels. Cytokines (IL-6, TNF-α, IL-1β, IL-8) and adipokines (adiponectin, resistin and leptin) were measured using multiplex ELISA. Carotid bodies (CB) are paired, oxygen-sensing organs located in the bifurcation of the carotid artery that act as peripheral chemoreceptors in the detection of hypoxic, hypercapnic and acidotic levels in the arterial blood. CBs respond to these fluctuations in blood gases by initiating firing of the carotid sinus nerve. This ultimately results in the appropriate ventilatory change to restore blood gases to their physiological levels. Studies have shown that the hypoxic response of the carotid body in juvenile mammals is low, but as maturation occurs this response is strengthened and clearly exhibited in adults. DNA mismatch repair (MMR) is a critical genome-maintenance system. It ensures genome stability by correcting mismatches generated during DNA replication, suppressing homologous recombination, and inducing apoptosis in response to severe DNA damage. As a result, defects in MMR lead to genome-wide mutations and susceptibility to both hereditary and sporadic cancer syndromes. The hallmark of cancer cells defective in MMR is their ability to display frequent instability in simple repetitive DNA sequences, a phenomenon called microsatellite instability (MSI). However, only ~70% of the MSI-positive tumors have identifiable MMR gene mutations, indicating that additional factor(s) are responsible for the MSI phenotype in …

ATP-binding cassette (ABC) transporters are transmembrane proteins that transport a wide variety of substrates across intra and extra-cellular membranes. A few examples of endo and xenobiotic substrates are metabolic products, lipids, sterols, and drugs. An important function of ABC transporters involved in export is to prevent intracellular the buildup of toxic products. Several ABC transporters have also been associated with drug resistance upon treatment with chemotherapeutic agents. P-glycoprotein (P-GP) and the multidrug resistant (MRP) transporters of the ABC C family are examples of transporters that confer chemo-resistance.

All human chromosomes end with protective structures called telomeres, which consist of thousands of double-stranded TTAGGG repeats and end in a 3’ guanine-rich overhang. These structures shorten normally during each round of replication, and extremely short telomeres along with telomere dysfunction are thought to contribute to the development of aging and cancer. Although many proteins have roles in telomere maintenance, WRN, which is a 3’ to 5’ helicase that is deficient in the premature aging disorder Werner’s syndrome, has been proposed to play multiple roles at telomeres. Abstract
Adults spend a significant portion of their lives at their workplace. Thus the workplace provides an important venue for public health interventions aimed at prevention and wellness. Best practices for workplace wellness programs are continually being refined. Ironically, health care workers are often less healthy than other professionals and face significant barriers to participating in workplace wellness programs. This paper analyzes the experience of a large urban health care system as it instituted a workplace wellness program and offers recommendations to increase participation in healthcare workplace wellness program and create an overall healthier work environment for healthcare workers.

A microfluidic device that is able to perform dielectric spectroscopy is developed. The device consists of a measurement chamber that is 250 μm thick and 750 μm radius. Around 1000 cells fit inside the chamber assuming average quantities for cell radius and volume fraction. This number is about 1000 folds lower than the capacity of conventional fixtures. A T-cell leukemia cell line Jurkat is tested using the microfluidic device. Measurements of deionized water and salt solutions are utilized to determine parasitic effects and geometric capacitance of the device. Introduction: This article demonstrates the utility of the system dynamics approach to model and simulate US demand for ambulatory health care service both for the general population and for specific cohort subpopulations over the 5-year period, from 2003 to 2008. A system dynamics approach that is shown to meaningfully project demand for services has implications for health resource planning and for generating knowledge that is critical to assessing interventions.

A key factor in the lack of improvement in prognosis of oral squamous cell carcinoma (OSCC) lesions over the years is the fact that a significant proportion are not diagnosed or treated until they reach an advanced stage. A molecular marker for malignant transformation in innocent looking oral lesions and a monitor for the aggressiveness of malignant lesions might be of help. Aim: This study was carried out to evaluate whether OT Equator Profi le attachment or GPS attachment is less destructive to crestal bone around implants and to bone in distal aspect of the ridge in implant retained mandibular overdenture cases. Materials and Methods: Following two stage surgical protocol twelve completely edentulous patients received two implants placed bilaterally in the canine region (24 implants) to retain mandibular overdentures. Background
There are limited data on the prevalence and associated factors of truancy in southern Africa. Yet truancy should attract the attention of public health professionals, educators and policy makers as it may be associated with adolescent problem behaviours. The objectives of the study were to estimate the prevalence and determine correlates of school truancy among pupils in Zambia.
Findings
We used data collected in 2004 in the Zambia Global School-based Health Survey. Logistic regression analysis was conducted to identify factors associated with truancy. A total of 2257 pupils participated in the survey of whom 53.9% were male. Overall 58.8% …

BACKGROUND: Clinical inertia, provider failure to appropriately intensify treatment, is a major contributor to uncontrolled blood pressure (BP). Some clinical inertia may result from physician uncertainty over the patient’s usual BP, adherence, or value of continuing efforts to control BP through lifestyle changes.
OBJECTIVE: To test the hypothesis that providing physicians with uncertainty reduction tools, including 24-h ambulatory BP monitoring, electronic bottle cap monitoring, and lifestyle assessment and counseling, will lead to improved BP control.
DESIGN: Cluster randomized trial with five intervention clinics (IC) and five usual care clinics (UCC).

Objective: To identify common attributes of Federal workers compensation cases referred to Navy physicians for medical opinions and to determine the impact of the review on the final case decision. Methods: Retrospective case study and descriptive analysis of 258 opinion letters written by physicians on referred cases from 2006 to 2010. Results: Navy physician opinions were considered in the outcome in some of the cases, and there was a statistically significant difference between the claim acceptance rate in the study population and the total population. [First paragraph] As senior nursing students in the 2011-2012 academic year, we recognized how difficult it is for students to transition from the classroom to the "real world" of nursing practice. Through the nursing curriculum, we have become extremely knowledgeable about numerous diseases and nursing care for different patient populations. However, with limited hands-on clinical experience, how are student nurses able to comprehend all of the diseases, disorders, and conditions studied in the classroom? Nanomaterials possess distinctive physicochemical properties (e.g., small sizes and high surface area-to-volume ratios) and promise a wide variety of applications, ranging from the design of high quality consumer products to effective disease diagnosis and therapy. These properties can lead to toxic effects, potentially hindering advances in nanotechnology. In this study, we have synthesized and characterized purified and stable (nonaggregation) silver nanoparticles (Ag NPs, 41.6 ± 9.1 nm in average diameter) and utilized early developing (cleavage-stage) zebrafish embryos (critical aquatic and eco- species) as in vivo model organisms to probe the diffusion and toxicity of Ag NPs. The application of the field of network science to the scientific disciplines of structural biology and biochemistry, have yielded important new insights into the nature and determinants of protein structures, function, dynamics and the folding process. Advancements in further understanding protein relationships through network science have also reshaped the way we view the connectivity of proteins in the protein universe. The canonical hierarchical classification can now be visualized for example, as a protein fold continuum. This review will survey several key advances in the expanding area of research being conducted to study protein structures and folding using network approaches.

Purpose- We compared the effects of two treatments for aphasic word retrieval impairments, errorless naming treatment (ENT) and gestural facilitation of naming (GES), within the same individuals, anticipating that the use of gesture would enhance the effect of treatment over errorless treatment alone. In addition to picture naming, we evaluated results for other outcome measures that were largely untested in earlier ENT studies.
Methods- In a single participant crossover treatment design, we examined the effects of ENT and GES in eight individuals with stroke-induced aphasia and word retrieval impairments (three semantic anomia, five phonological anomia) in counterbalanced phases across participants. …

Hypertension is a chronic multifactorial condition with high morbidity and mortality rates, currently affecting about one billion people worldwide. Currently available anti-hypertensive medications are found to be effective in reducing blood pressure (BP), but still more than 50% of those diagnosed with hypertension fail to respond to these anti-hypertensive regimens. Although hypertension has multiple etiologies, physical inactivity has been found to have strong correlation with the disease, so exercise has recently been recommended as a part of lifestyle modifications for all hypertensive patients. Hearing loss in dogs and cats following dental or ear procedures performed under anesthesia has recently been reported. The most likely causes for this acute-onset deafness were considered to be mechanical or vascular. Jaw manipulation as a possible cause of acute-onset deafness in the dog was investigated in the current study. Structures adjacent to the temporomandibular joint were of interest because changes in jaw orientation could disrupt vessels and nerves in this area. Objective: The purpose of this study was to investigate the effects of two different prophylactic behavioral swallowing exercise regimens performed by head and neck cancer/s (HNC) patients during radiation therapy with/without chemotherapy (RT/C) on swallowing physiology, function, quality-of-life (QOL) and weight outcomes at the completion of RT/C. Feeding tube (PEG) status at 3 months post-treatment was also compared.
