Introduction: Improved environments in the U.S. have resulted in taller, larger, and heavier children and adults compared to past generations. Studies of other skeletal-dental dimensions have shown increases across generations, and our perception is that teeth now are forming faster. The purpose of this study is to test for a secular trend towards faster tempos of tooth mineralization in a sample of U.S. white adolescents over the past quarter century (1980-85 to 2005-10).

The cranberry (species: Vaccinium macrocarpon, family: Ericaceae) is a polyphenolicrich berry fruit native to North America and exhibits numerous beneficial properties for human health, such as inhibition of human cancer cell line proliferation and prevention of urinary tract colonization by bacterial pathogens. Orthodontic correction of tooth size versus arch size discrepancy (TSASD) can be achieved with two distinct methods of treatment. The first involves the extraction of teeth to gain the space needed for tooth alignment. The second relies on arch expansion to gain the space needed for correction. In recent decades, arguments for and against the extraction of teeth for orthodontic purposes have become increasingly important, particularly in regards to oropharyngeal airway size. People with dementia are hospitalized for a variety of reasons. The combination of dementia with additional health conditions creates a unique challenge to caregivers in acute care settings. There is a dearth of information available to provide guidance to the nursing staff caring for these patients. This integrated review of the literature examined the experiences of hospitalization from the perspective of the older adult with dementia, the family caregiver, and the patient care staff. Results showed a limited body of literature that addressed hospital experiences of people with dementia and those of family and professional caregivers. Additionally, few studies addressing …

Background: Home care of a patient with a tracheostomy after surgery for head and neck cancer requires the caregiver to be proficient with new equipment and required skills. The responsibility of managing an artificial airway, may lead to an increase in caregiver anxiety. Omi, also known as HtrA2, is a mammalian pro-apoptotic mitochondrial protein and a member of the HtrA (high temperature requirement A) family of serine proteases. Omi promotes the caspase-dependent apoptotic pathway through cleavage of IAPs (inhibitor of apoptosis proteins); this cleavage inactivates IAPs and facilitates caspase activity. Omi's proteolytic activity is necessary and essential for its pro-apoptotic function. This study is aimed to further understand the role of Omi in the cytoplasm by using the yeast two-hybrid system to identify novel Omi interactors/substrates. Pulmonary bulla in children represent interesting entities. Mostly congenital bronchopulmonary foregut malformations and acquired cysts like pneumatocoeles have also been described. We present a case of a 12 1/2 years old girl with acute onset respiratory distress symptoms harbouring a huge pulmonary cyst exhibiting mass effects, resulting in mediastinal deviation. Following initial workup, cyst excision was carried out which revealed presence of fungal hyphae that was susceptible to Fluconazole therapy post operatively. The case points out how a pulmonary cyst can present in older children with symptoms of respiratory distress and mass effects.

Background—This study uses the Framingham Risk Score (FRS) for 10-year cardiovascular disease (CVD) to evaluate differences between Mexican American immigrants and the U.S.-born population. Methods and Results—Data from the Cameron County Hispanic Cohort (N=1,559). Average total risk scores were generated by age group for each gender. Regression analysis was conducted adjusting for covariates and interaction effects. Both women and men in the CCHC sample who were long-term immigrant residents (mean FRS scores women 4.2 with p<.001 vs. men 4.0 with p<.001) or born in the U.S. (mean FRS scores women 4.6 with p<.001 vs. men 3.3 with p<.001) had significantly higher risk scores than immigrants who had only been in this country for less than 10 years. The interaction model indicates that differences between immigrant and native-born Mexican Americans are most greatly felt at lowest levels of socioeconomic status for men in the CCHC. Conclusions—This study suggests that in terms of immigrant advantage in CVD risk, on whom, where, and how the comparisons are being made have important implications for the degree of difference observed.

Recently, researchers have debated two views on the connection between lifestyle and health. In the first, health-related lifestyles including tobacco and alcohol use, diet, and physical activity are seen as primary influences on health. In the second, social stratification is the dominant influence with lifestyles simply markers of social status. Neither approach leads to interventions that can reverse the world's most serious health problems. This article proposes that corporate practices are a dominant influence on the lifestyles that shape patterns of health and disease. Modifying business practices that promote unhealthy lifestyles is a promising strategy for improving population health. Corporations …

The purpose of this study was to understand the adjustment families undergo when a child is diagnosed with deafness as well as their adjustment to the child hearing after a cochlear implant is in place. The participants of the study were four hearing adults between the ages of 18-28 who have a younger sibling with a cochlear implant. The qualitative study used reports from four participants to reflect on how hearing loss and subsequent cochlear implantation impacted the family’s lives. Contrary to expecting findings, the themes did not change and all four reported nurturing and personal growth as key themes.

STUDY OBJECTIVE: To investigate whether the phase of menstrual cycle influences the incidence of postoperative nausea and vomiting (PONV) in women undergoing general anesthesia for elective laparoscopic gynecological surgery.
DESIGN: Prospective, observational, blinded study.
SETTING: General hospital, Postanesthesia Care Unit, and gynecologic floor room.
PATIENTS: 111 ASA physical status 1 and 2 women, aged 18 to 53 years.

Obesity has become a major health problem with increasing prevalence and is related to multiple medical consequences, such as increased risk for diabetes, heart disease, arthritis, and cancer (Hasler et al., 2004). Due to the limited availability of effective treatment of weight problems, it is necessary to identify potential risk factors for obesity. One factor that has received increased attention is chronic partial sleep deprivation (<7 hours of sleep a night). The decrease in average sleep duration over the past three to four decades has occurred simultaneously with the increase in the prevalence of obesity. Chronic partial sleep deprivation could be a possible risk factor for obesity, but current understanding of the processes linking sleep deprivation to obesity is incomplete. The research will focus on examining the possible pathways through which partial sleep deprivation contributes to the development of obesity: alterations in metabolic pathways and waking behavior, including eating behavior and physical inactivity. The hypothesis behind the proposed study is that increasing sleep duration in obese individuals who experience partial sleep deprivation will lead to a decreased BMI. Research addressing the possible pathways and targeting the amount of sleep will benefit individuals in preventing and/or treating obesity.

Although women, young people and refugees are vulnerable to sexual and gender-based violence (SGBV) worldwide, little evidence exists concerning SGBV against refugees in Europe. Using community-based participatory research, 223 in-depth interviews were conducted with refugees, asylum seekers and undocumented migrants in Belgium and the Netherlands. Responses were analysed using framework analysis. The majority of the respondents were either personally victimised or knew of a close peer being victimised since their arrival in the European Union. A total of 332 experiences of SGBV were reported, mostly afflicted on them by (ex-)partners or asylum professionals. More than half of the reported violent …

Two areas of research interest in the design and use of lower limb prosthetic devices are how foot performance varies with design and how prosthesis alignment affects gait. The current study addressed both of these areas through the use of the JR3 triaxial transducer mounted at the base of the socket. The transducer is an innovative form of instrumentation that measures forces and moments at the end of the socket. Preceptors are vital to the success of new graduate registered nurses in their transition to practice as they assist the new nurse in developing skills, competency, and confidence. Most residency programs may include training for preceptors but there may not be evidence that preceptor preparation makes a difference in the retention of nurse residents or the relationship to favorable evaluations of preceptors from the residents. This study evaluated and compared the effects of preceptor training on competency levels of the resident and therefore retention.

Patients with end-stage renal disease (ESRD) who receive hemodialysis often fail to follow a prescribed diet and fluid regimen, which undermines the effectiveness of care and leads to unpredictable disease progression and greater likelihood of complications. Non-adherence to dietary and fluid limitations compromises the outcomes of patients receiving hemodialysis. Noncompliance can lead to detrimental long-term outcomes including deterioration of the cardiovascular system, heart failure, hypertension, and pulmonary edema as well as short-term problems such as edema, itching, bone pain, and breathlessness.

Premature birth has risen steadily over the last decade resulting in an increased number of infants needing neonatal intensive care. The demand for neonatal intensive care has lead to many improvements in the delivery of such care with an increase in the survival of the smallest and most premature infants. However, most of these infants are being discharged from neonatal intensive care units (NICU) with severe extrauterine growth restriction (EUGR).

The Centers for Disease Control and Prevention's National Center for Health Statistics (2010) report that nearly one-third of babies were born by Cesarean section (CS) in 2007. Of interest, six states, including Nevada, experienced increases of more than 70% in the last 10 years (CDC, 2007). Based on the increased rate of CS deliveries, the National Institutes of Health (NIH) convened a consensus panel in 2010, which urged the medical community to reduce barriers to women who want to try a vaginal birth after Cesarean delivery (VBAC) in the hope this would safely decrease the total CS rate. The goal of this research project was to produce a model of the effects of drug dependence on general self-rated health. Due to power issues, two additional models, one for cocaine and one for heroin, were required. The models used data from the 2005-2009 National Survey on Drug Use and Health. The result of this effort was a ranking of the effects of drug dependence on general health for individuals and for the study population. The model controlled for infectious, chronic and mental illness as well as sociodemographic variables. Significantly increased odds ratios were found for alcohol, marijuana, analgesics, and …

Commercial air travel is amongst the safest forms of transportation yet the environment within the aircraft cabin to which the passenger is exposed may have adverse physiological and psychological effects on passengers, especially for those with pre-existing medical issues. A comprehensive, systematic review and analysis of current literature was performed to develop evidenced-based guidelines for evaluation of passenger fitness to fly for integration into primary care practice. Modern experience sampling methods attempt to understand the phenomenology of inner experience through a variety of methods, including questionnaires, diaries, and verbalization of ongoing cognitive processes. The goal of such studies is to minimize potential roadblocks to accessing inner experience, such as retrospective recall, memory failures, and bias. The current project focuses on one such method, Descriptive Experience Sampling (DES). DES is an idiographic, exploratory, and ecologically valid method of studying inner experience. Standard treatment protocols for high-grade gliomas, such as glioblastoma multiforme (GBM), are highly ineffective due to their inability to eradicate infiltrating tumor cells. Improvements in overall survival are likely to be realized only with the development of more effective localized therapies capable of eradicating tumor cells in the surgical resection margin and beyond. Photochemical internalization (PCI) is a localized light-based therapeutic modality that enhances the efficacy of therapeutic macromolecules including chemotherapeutic agents such as bleomycin. A number of studies have shown that this photodynamic therapy (PDT) - based modality may prove effective in the treatment of high-grade gliomas.
