BACKGROUND: Intravitreal vascular endothelial growth factor (VEGF) inhibitors stabilize vision in a majority of patients with neovascular age-related macular degeneration (AMD) and can improve vision in almost 40% of patients. However, some individuals who respond to anti-VEGF treatment still lose vision due to the formation of geographic atrophy (GA). While optical coherence tomography is often the primary imaging modality used, fluorescein angiography (FA) can provide useful information on GA development after choroidal neovascularization (CNV) regression.

Randomized Comparison of Two Internet-Supported Methods of Natural Family Planning

Both Internet provided NFP methods were effective with correct use but the electronic hormonal monitoring method was statistically better with a typical use of only 7 unintended pregnancies per 100 women over 12 months of use compared with 19 with the traditional cervical mucus monitoring method.

Satisfaction of using the hormonal method increased over time but motivation decreased, i.e., they had a desire to achieve a pregnancy.

A high discontinuation rate contributed to the limitation of the study.

However, this Internet based method of providing NFP with professional support is efficient and effective for select motivated couples.

Background: Phthalates are common plasticizers present in medical-grade plastics and other everyday products. They can also act as endocrine-disrupting chemicals and have been linked to the rise in metabolic disorders. However, the effect of phthalates on cardiac metabolism remains largely unknown.

Objectives: We examined the effect of di(2-ethylhexyl)phthalate (DEHP) on the metabolic profile of cardiomyocytes because alterations in metabolic processes can lead to cell dysfunction.

Objectives: Mobile phone technology may be useful in helping to guide medical decisions for lacerations. We examined whether emergency department (ED) provider opinions on which lacerations require repair differed using mobile phone–generated images compared with in-person evaluations.

Subjects and Methods: Patients presenting to an urban ED for initial and follow-up laceration care were prospectively enrolled. Patients took four mobile phone pictures of their laceration and provided a medical history. Cases were reviewed by ED providers who assessed image quality and made a recommendation about whether the laceration needed repair. The same provider then assessed the patient in-person. Purpose:Guided by Friedemann's framework, the purpose of this study was to examine the dimensions of new family caregivers’ emotional strain in caring for persons with stroke. Method: Seventy-three caregivers who were new to that role participated in an interview every 2 weeks for a year as part of a NIH project. Of these caregivers, 36 participants were randomly assigned and had access to a Web-based intervention and its e-mail discussion. In this secondary data analysis, 2,148 e-mail discussion messages plus 2,455 narrative interview entries were used to examine dimensions of caregivers’ emotional strain. Mitochondrial dysfunction characterized by depolarization of mitochondrial membranes and the initiation of mitochondrial-mediated apoptosis are pathological responses to hypoxia-ischemia (HI) in the neonatal brain. Carnitine metabolism directly supports mitochondrial metabolism by shuttling long chain fatty acids across the inner mitochondrial membrane for beta-oxidation. Our previous studies have shown that HI disrupts carnitine homeostasis in neonatal rats and that L-carnitine can be neuroprotective. Objective To investigate the prevalence and risk factors, both prosthesis and microbiological, associated with different types of denture stomatitis in Thai patients. Materials and methods Thai patients with upper removable denture (n = 137) were evaluated for the prevalence of and risk factors associated with denture stomatitis by questionnaire, oral and dental prosthesis examination. Palatal mucosa and denture fitting surfaces were swabbed for yeast carriage investigation. วัตถุประสงค์ เพื่อประเมินค่ากําลังแรงยึดแบบผลักของเดือยฟันคอมโพสิตเสริมเส้นใยเมื่อยึดด้วยเซลฟ์แอดฮีซีฟเรซินซีเมนต์ในผนังคลองรากฟันแต่ละตําแหน่งที่มีการปรับสภาพด้วยวิธีแตกต่างกัน วัสดุและวิธีการ ฟันกรามน้อยล่างรากเดียวจํานวน 40 ปี แบ่งเป็น 4 กลุ่ม กลุ่มละ 10 โดยวิธีสุ่ม ตัดส่วนตัวฟัน รักษาคลองรากฟันและเตรียมพื้นที่สําหรับเดือยฟันไฟบริเคลียร์ เบอร์ 3 ปรับสภาพผนังคลองรากฟันดังนี้ กลุ่มที่ 1 ไม่มีการปรับสภาพผนังคลองรากฟัน กลุ่มที่ 2-4 ปรับสภาพผนังคลองรากฟันด้วยกรดฟอสฟอริกความเข้มข้นร้อยละ 37 นาน 5 วินาที (กลุ่มที่ 2) อีดีทีเอความเข้มข้นร้อยละ 17 นาน 1 นาที (กลุ่มที่ 3) และอีดีทีเอความเข้มข้น ร้อยละ 17 นาน 1 นาที ตามด้วยโซเดียมไฮโปคลอไรท์ความเข้มข้นร้อยละ 2.5 นาน 15 วินาที (กลุ่มที่ 4) จาก นั้นยึดเดือยฟันด้วยเซลฟ์แอดฮิซิฟเรซินซีเมนต์รีไลเอ็กซ์ยูนิเซม (RelyXTM Unicem) ตัดฟันทุกซี่เป็น 6 ชิ้น แต่ละชิ้นหนา 1 +0.05 มิลลิเมตร ทดสอบค่ากําลังแรงยึดแบบผลักด้วยเครื่องทดสอบสากล (Instron) ความเร็ว ของหัวกด 0.5 มิลลิเมตรต่อนาที นําข้อมูลมาวิเคราะห์ความแปรปรวนสองทางและเปรียบเทียบเชิงซ้อนชนิด ทุกีย์ ที่ระดับนัยสําคัญ p < 0.05 และวิเคราะห์ลักษณะการแตกที่เกิดขึ้นของทุกชิ้นทดสอบด้วยกล้องจุลทรรศน์ชนิดสเตอริโอ ผลการศึกษา ค่ากําลังแรงยึดแบบผลักของกลุ่มที่ 2 มีค่าสูงที่สุด โดยค่ากําลังแรงยึดของกลุ่มที่ 3 มีค่าสูงกว่ากลุ่มที่ 1 และ 4 อย่างมีนัยสําคัญ แต่กลุ่มที่ 1 และ 4 มีค่าไม่แตกต่างกัน (p>0.05) นอกจากนี้ตําแหน่งรากฟันไม่ส่งผล ต่อค่ากําลังแรงยึดแบบผลัก (p>0.05) สรุป การปรับสภาพผนังคลองรากฟันด้วยกรดฟอสฟอริกความเข้มข้นร้อยละ 37 นาน 5 วินาทีหรืออีดีทีเอความ เข้มข้นร้อยละ 17 นาน 1 นาที สามารถเพิ่มค่ากําลังแรงยึดแบบผลักของเซลฟ์แอดฮีซีฟเรซินซีเมนต์ (ว ทันต จุฬาฯ 2555;35:167-78)

Modification Of Anterior Guidance In Patient With Wear In Mandibular Anterior Teeth: A Case Report, Mathuros Korkerdsup, Chalermpol Leevailoj

This case report describes how to establish the favorable anterior guidance in an anterior tooth wear patient. The anterior tooth wear is present at lower anterior teeth with an improper contour upper anterior fixed partial dental prosthesis (FPDP) 12-21. The FPDP was removed and replaced with a provisional cantilever FPDP. The favorable anterior guidance was established using a custom incisal guide table. Final restoration was placed. Periodic follow up evaluation is necessary.

วัตถุประสงค์ เพื่อศึกษาเปรียบเทียบผลของความต้านทานการสึกกร่อนของรอยโรคจุดขาวจําลองกับรอยโรคจุด ขาวจําลองที่ถูกแทรกซึมโดยวัสดุเรซินไอคอน เมื่อสัมผัสกับเครื่องดื่มที่มีฤทธิ์เป็นกรด วัสดุและวิธีการ เตรียมชิ้นตัวอย่างจากฟันกรามน้อยของมนุษย์ที่ถูกถอนจํานวน 80 ซี่ สุ่มฟันเป็น 4 กลุ่มทดลอง ดังนี้ 1) แช่เครื่องดื่มน้ําอัดลมโคล่า 2) แช่เครื่องดื่มน้ําส้มคั้น 3) แช่เครื่องดื่มเกลือแร่ หรือ 4) แช่น้ําปราศจาก ประจุ เตรียมชิ้นฟันตัวอย่างโดยตัดตัวฟันในแนวดิ่งให้ได้ชิ้นตัวอย่างด้านใกล้กลางและด้านไกลกลางเป็น 2 ส่วนเท่า ๆ กัน นําชิ้นตัวอย่างมาฝังลงในเรซิ่นหล่อใสให้ด้านแก้มอยู่ใกล้ผิวด้านนอกของเรซิ่นหล่อใส ขัดเรซิน ออกจนผิวเคลือบฟันด้านแก้มโผล่จากเรซินหล่อใส และมีพื้นที่ประมาณ 1 X 1 ตารางมิลลิเมตร นําชิ้นตัวอย่าง มาแช่ในสารละลายเพื่อสร้างรอยโรคจุดขาวจําลองของผิวเคลือบฟัน โดยให้ชิ้นที่หนึ่งที่มาจากฟันซี่เดียวกัน ใช้วัสดุเรซินจากผลิตภัณฑ์ไอคอน ส่วนชิ้นที่เหลือไม่ใช้วัสดุใด และแช่ในเครื่องดื่มแต่ละชนิดดังที่กล่าวมา โดยแช่ เครื่องดื่มก่อน 5 วินาที ตามด้วยแช่น้ําลายเทียม 5 วินาที สลับกันไปรวมทั้งหมด 10 รอบ ทําซ้ําแบบเดิมอีกสอง รอบทุก ๆ 6 ชั่วโมง วัดค่าปริมาตรและความลึกเฉลี่ยที่เกิดขึ้นจากการสึกกร่อนของรอยโรคจําลองโดยใช้เครื่องวัด ความหยาบพื้นผิวโปรไฟโลมิเตอร์ ทําเช่นเดิมในเครื่องดื่มแต่ละชนิด นําค่าปริมาตรและความลึกเฉลี่ยที่ได้มา ทดสอบด้วยสถิติการวิเคราะห์ความแปรปรวนสองทาง และสถิติการวิเคราะห์ความแปรปรวนทางเดียวร่วมกับสถิติ การเปรียบเทียบเชิงซ้อนแบบแทมเฮนที่ระดับความเชื่อมั่นร้อยละ 95 ผลการทดลอง กลุ่มที่ใช้ผลิตภัณฑ์เรซินไอคอนที่แช่เครื่องดื่มที่มีฤทธิ์เป็นกรด มีค่าเฉลี่ยของปริมาตร และความ ลึกในการสึกกร่อนของเคลือบฟันมากกว่ากลุ่มที่ไม่ใช้ผลิตภัณฑ์เรซินไอคอนทุกกลุ่ม และพบว่ากลุ่มที่แช่ในเครื่องดื่ม โคลามีค่าเฉลี่ยของปริมาตรและความลึกในการสึกกร่อนมากที่สุด จากการทดสอบทางสถิติพบว่า การใช้ผลิตภัณฑ์เรซินไอคอนและชนิดของเครื่องดื่มมีผลต่อค่าเฉลี่ยของปริมาตรและความลึกในการสึกกร่อนอย่างมี นัยสําคัญทางสถิติ (p < 0.0001) และยังเป็นปัจจัยร่วมทางสถิติ (p < 0.0001) สรุป การใช้ผลิตภัณฑ์เรซินไอคอนแทรกซึมเข้าไปในรอยโรคจุดขาวจําลองไม่สามารถต้านทานการสึกกร่อนจาก เครื่องดื่มที่มีฤทธิ์เป็นกรดได้ (ว ทันต จุฬาฯ 2555;35:179-88)

วัตถุประสงค์ เพื่อศึกษาค่ากําลังแรงดึงไดอะเมทรัลและความทนแรงอัดของวัสดุอุดฟันชั่วคราวชนิดซิงค์ออกไซด์- แคลเซียมซัลเฟตที่พัฒนาขึ้นใหม่ วัสดุและวิธีการ สังเคราะห์โคพอลิเมอร์ระหว่างพอลิเมทิลเมทาคริเลตและพอลิสไตรีนด้วยกระบวนการ พอลิเมอไรเซชันแบบดิฟเฟอเรนเชียลไมโครอิมัลชันเพื่อใช้เป็นวัสดุเสริมแรงให้กับวัสดุอุดฟันชั่วคราว โดยปรับ เปลี่ยนอัตราส่วนระหว่างพอลิเมทิลเมทาคริเลตและพอลิสไตรีนเป็น 1:1 112 113 และ 14 จากนั้นนํามาผสมกับ ซิงค์ออกไซด์ แคลเซียมซัลเฟตและวัสดุอื่น ๆ ซึ่งจะได้วัสดุอุดชั่วคราวที่พัฒนาขึ้นใหม่ทั้งหมด 4 สูตร และแบ่งเป็น 4 กลุ่มเพื่อใช้ในการศึกษา โดยใช้เควิต เป็นกลุ่มควบคุมเตรียมชิ้นตัวอย่างกลุ่มละ 15 ชิ้น เพื่อหาค่ากําลังแรงดึง ไดอะเมทรัลที่ดัดแปลงจากดาร์เวลล์ และเตรียมชิ้นตัวอย่างกลุ่มละ 30 ชิ้น เพื่อหาค่าความทนแรงอัดตามวิธีที่ ดัดแปลงจากมาตรฐานสากล ไอเอสโอ 3107/2004 วิเคราะห์ผลโดยใช้สถิติการวิเคราะห์ความแปรปรวนทางเดียว ที่ระดับนัยสําคัญทางสถิติ 0.05 ผลการศึกษา วัสดุอุดฟันชั่วคราวชนิดซิงค์ออกไซด์ แคลเซียมซัลเฟตที่เสริมแรงด้วยโคพอลิเมอร์ของพอลิเมทิล เมทาคริเลตและพอลิสไตรีนในอัตราส่วนต่าง ๆ ให้ค่าเฉลี่ยกําลังแรงดึงและความทนแรงอัดที่แตกต่างกันทุกสูตร เมื่อปริมาณของพอลิสไตรีนเพิ่มขึ้น ค่าเฉลี่ยกําลังแรงดึงและความทนแรงอัดมีค่าลดลง โดยสูตรที่ใช้อัตราส่วนของ พอลิเมทิลเมทาคริเลตต่อพอลิสไตรีนเท่ากับ 1:1 ให้ค่าเฉลี่ยของกําลังแรงดึงไดอะเมทรัลและค่าเฉลี่ยความทน แรงอัดมากที่สุด และยังมีค่ามากกว่าเควิตอย่างมีนัยสําคัญทางสถิติ (p<0.05) สรุป วัสดุอุดฟันชั่วคราวที่พัฒนาขึ้นใหม่ที่เสริมแรงด้วยโคพอลิเมอร์ของพอลิเมทิลเมทาคริเลตและพอลิสไตรีน ในอัตราส่วน 1:1 มีกําลังแรงดึงไดอะเมทรัลและความทนแรงอัดมากกว่าเควิตอย่างมีนัยสําคัญทางสถิติ (ว ทันต จุฬาฯ 2555;35:157-66)

This article is a case report of a patient who lost left permanent maxillary lateral incisor, canine and left permanent maxillary first premolar and alveolar bone due to the enucleation of eosinophilic granuloma. Objective: To describe experiences of uncertainty and management strategies for staff working with families in a hospital waiting room. Setting: A 288-bed nonprofit community hospital in a Midwestern city. Methods: Data were collected during individual, semi structured interviews with 3 volunteers, 3 technical staff members, and 1 circulating nurse (n=7), and during 40 hours of observation in a surgical waiting room. BACKGROUND: Burn injury results in a chronic inflammatory, hypermetabolic, and hypercatabolic state persisting long after initial injury and wound healing. Macrophage Autophagy In Immunity To Cryptococcus Neoformans And Candida Albicans, Magdia De Jesus

Autophagy is used by eukaryotes in bulk cellular material recycling and in immunity to intracellular pathogens. We evaluated the role of macrophage autophagy in the response to Cryptococcus neoformans and Candida albicans, two important opportunistic fungal pathogens. The autophagosome marker LC3 (microtubule-associated protein 1 light chain 3 alpha) was present in most macrophage vacuoles containing C. albicans. In contrast, LC3 was found in only a few vacuoles containing C. neoformans previously opsonized with antibody but never after complement-mediated phagocytosis. Bilateral brachial plexus blocks and regional anesthesia in trauma patients are rarely performed due to potential complications when using these techniques. We illustrate a case in which bilateral infraclavicular nerve blocks were placed as part of a multimodal approach to pain management in a trauma patient. We discuss potential hazards, important considerations, and rationale for attempting this procedure. Ultimately, performing bilateral brachial plexus nerve blocks in trauma patients is a viable option when choosing pain management techniques.

Objective:

Pulse oximetry has been recognized as a promising screening tool for critical congenital heart disease (CCHD). The aim of this research was to study the feasibility of implementation in a community hospital setting.

Study Design:

Meetings were conducted to determine an implementation plan. Pulse oximetry was performed on the right hand and foot after 24h of age. Newborns with a saturation 95% or a 3% difference were considered to have a positive screen. Screening barriers, screening time and ability to effectively screen all eligible newborns were noted.

Objective: To determine the factors causing complications in unilateral total hip replacement.
Study Design: Analytical study.
Place and Duration of Study: The Aga Khan University Hospital, Karachi, between 2000 and 2010.
Methodology: During the study period, 199 patients underwent elective unilateral total hip replacement at the Aga Khan University Hospital. Patients were divided into two groups on the basis of postoperative complications within 30 days of surgery. Significant factors at 5% significance level on univariate analysis were further analyzed by multivariate logistic regression.

Objective: To propose alternative models of training for doctors treating patients with stones, and to identify their relative value, as such doctors are trained through urology programmes which sometimes cannot be expanded to meet the need, are short of teachers, too comprehensive and lengthy. This review explores new pathways for training to provide competence in the care of patients with stones.

OBJECTIVES: To evaluate the current status of ureteroscopic lithotripsy (UL) for treating renal calculi of >2 cm, as advances in flexible ureteroscope design, accessory instrumentation and lithotrites have revolutionised the treatment of urinary calculi. Objective: Although bipolar disorder has high heritability, the onset occurs during several decades of life, suggesting that social and environmental factors may have considerable influence on disease onset. This study examined the association between the age of onset and sunlight at the location of onset.

Method: Data were obtained from 2414 patients with a diagnosis of bipolar I disorder, according to DSM-IV criteria. Data were collected at 24 sites in 13 countries spanning latitudes 6.3 to 63.4 degrees from the equator, including data from both hemispheres.
