Obesity and poor nutrition, individually and together, have created costly musculoskeletal disease epidemic in the United States. Processed food, with abundant "empty" calories, has contributed greatly to our dietary woes. Much of the food consumed today is packed with calories but refined to the point that essential nutrients are lacking. Even worse, processed food may have ingredients added that are detrimental to good health. Abundant research has documented a close relationship between obesity, poor diet and orthopaedic problems. BACKGROUND: Scaffold surface features are thought to be important regulators of stem cell performance and endurance in tissue engineering applications, but details about these fundamental aspects of stem cell biology remain largely unclear.

METHODOLOGY AND FINDINGS: In the present study, smooth clinical-grade lactide-coglyolic acid 85:15 (PLGA) scaffolds were carved as membranes and treated with NMP (N-metil-pyrrolidone) to create controlled subtractive pits or microcavities. Scanning electron and confocal microscopy revealed that the NMP-treated membranes contained: (i) large microcavities of 80-120 microm in diameter and 40-100 microm in depth, which we termed primary; and (ii) smaller microcavities of 10-20 microm in diameter …

Provenance becomes a critical requirement for healthcare IT infrastructures, especially when pervasive biomedical sensors act as a source of raw medical streams for large-scale, automated clinical decision support systems. The haloarchaeal phototaxis receptor sensory rhodopsin I (SRI) in complex with its transducer HtrI delivers an attractant signal from excitation with an orange photon and a repellent signal from a second near-UV photon excitation. Using a proteoliposome system with purified SRI in complex with its transducer HtrI, we identified by site-directed fluorescence labeling a site (Ser(155)) on SRI that is conformationally active in signal relay to HtrI. Using site-directed spin labeling of Ser(155)Cys with a nitroxide side chain, we detected a change in conformation following one-photon excitation such that the spin probe exhibits a splitting of the outer hyperfine extrema …

Hamstring strain is common in athletes, and both diagnosis and surgical treatment of this injury are becoming more common. Nonsurgical treatment of complete ruptures has resulted in complications such as muscle weakness and sciatic neuralgia. Surgical treatment recently has been advocated to repair the complete rupture of the hamstring tendons from the ischial tuberosity. Surgical repair involves a transverse incision in the gluteal crease, protection of the sciatic nerve, mobilization of the ruptured tendons, and repair to the ischial tuberosity with the use of suture anchors. The database of incident forms relating to anaesthesia services in an institutional risk management programme were reviewed for 2003-2005, the aim being to identify any recurring patterns. Incidents were prospectively categorised as relating to attitude/behaviour, communication breakdown, delay in service, or were related to care, cost, environment, equipment, security, administrative process, quality of service or miscellaneous. The total number of anaesthesia-related incidents reported during the period was 287, which related to 0.44% of the total number of anaesthetics administered during the time period. This paper presents an exploratory study of alcohol and drug use in two rural communities in Plateau State, Nigeria. The aim is to raise awareness of the rural alcohol and drug problem. The paper examines the patterns of alcohol consumption and drug use, and their perceived functions for substance use among rural farmers in Nigeria. The study shows the common use of marijuana and alcohol in addition to prescription drugs. There is also evidence of multiple or combinational drug use. Although fewer women at all ages are undergoing mastectomy, for those elderly women who undergo mastectomy, reconstruction options should be offered. Success Rate And Postoperative Complications Of Phacoemulsification In 25 Cataractous Eyes : A Retrospective Study In 20 Dogs (2003-2005), Pasakorn Brikshavana

Phacoemulsification was performed on 25 eyes of 20 dogs (7 females and 13 males) with a mean patient age ± SD at the time of surgery of 8.12 ± 2.76 years (range 4-14.5 years). The medical records were reviewed for surgical outcome and postoperative complications. Success rate was calculated by determining the number of successful eyes divided by the total number of the eyes in the studied group. The results showed that restoration of functional vision was noted in 24/25 eyes at the end of the study. The success rate of this study was 96%. The most frequently observed complication …

OBJECTIVE:

To investigate the frequency of irritable bowel syndrome (IBS) and health seeking behaviour in patients belonging to different ethnic groups and their squatter settlements in two cities of Pakistan.

METHODS:

Questionnaire based on Rome II criteria for the diagnosis of IBS was sent to 1167 persons living in, Karachi and Bahawalpur and their squatter settlements in Baloch Colony and Tibba Badar Sher respectively. About 90% (1048) completed the questionnaire.

RESULTS:

The overall frequency of IBS was 14% with a mean age of 30 +/- 12.5 years and range of 16-85 years. Of the IBS positive patients, 56% (82/146) were …

The tail-withdrawal circuit of Aplysia provides a useful model system for investigating synaptic dynamics. Sensory neurons within the circuit manifest several forms of synaptic plasticity. Here, we developed a model of the circuit and investigated the ways in which depression (DEP) and potentiation (POT) contributed to information processing. DEP limited the amount of motor neuron activity that could be elicited by the monosynaptic pathway alone. POT within the monosynaptic pathway did not compensate for DEP. There was, however, a synergistic interaction between POT and the polysynaptic pathway. This synergism extended the dynamic range of the network, and the interplay between …

Ceftobiprole (BAL9141) is an investigational cephalosporin with broad in vitro activity against gram-positive cocci, including enterococci. Ceftobiprole MICs were determined for 93 isolates of Enterococcus faecalis (including 16 beta-lactamase [Bla] producers and 17 vancomycin-resistant isolates) by an agar dilution method following the Clinical and Laboratory Standards Institute recommendations. Ceftobiprole MICs were also determined with a high inoculum concentration (10(7) CFU/ml) for a subset of five Bla producers belonging to different previously characterized clones by a broth dilution method. OBJECTIVES: To evaluate cytotoxicity and bactericidal effects of chloramine-T.

METHODS: In vitro study of various concentrations and exposure times to preparations containing human fibroblasts or 1.5 x 10⁸ colony forming units per milliliter (CFU/mL) of 3 gram-positive bacteria-Staphylococcus aureus, methicillin-resistant S aureus, and vancomycin-resistant Enterococcus faecalis-and 2 gram-negative bacteria-Escherichia coli and Pseudomonas aeruginosa-with and without fetal bovine serum present.

MAIN OUTCOME MEASURES: Percentage reduction of bacterial growth and percentage of viable fibroblasts 48 hours after exposure.

Although mammography is the gold standard for screening for breast cancer, 10 to 15% of lesions are missed, and this is higher with dense breasts. Alternative diagnostic modalities are being developed. One of these, breast specific gamma imaging (BSGI), which is not effected by dense tissue, utilizes the increased metabolic activity of tumors to localize and distinguish benign from malignant processes in the breast. The test can be used for screening and diagnostic purposes.

A massive retroperitoneal hematoma caused by a retroperitoneal ectopic pregnancy is managed successfully utilizing multidisciplinary cooperation and transfusion of blood products. The authors report an unusual case of a retroperitoneal ectopic pregnancy presenting as a massive retroperitoneal hematoma in a hemodynamically unstable patient.

Background and Purpose. Literature supports the importance of the environment on promoting or compromising health. However, little is known about how the semi-fixed environment affects client perceptions and recovery from a debilitating condition. The purpose of this study was to investigate beliefs about control within the semi-fixed environment and client perceptions of the rehabilitation process.

Phacofragmentation and aspiration technique without intraocular lens implantation was reported in 26 mature cataractous dogs. Dogs included in this study were 13 Poodles, 6 crossbred dogs, 4 Cocker spaniels, 1 Shih-tzu, 1 West Highland white terrier and 1 Miniature pinscher. All dogs were clinically blinded from bilateral mature cataracts with positive dazzle reflex prior to the surgery. 24 dogs (92.31%) regained vision after the surgery which remained until the end of the study. This study was to conduct a surveillance of Mycoplasma gallisepticum (MG) infection in mixed Thai native chickens. Samples were submitted from 30 mixed Thai native chicken flocks, 15 birds per flock, aged between 1 and 4 months without MG vaccination in the area of Nakornpathom province during the September 2005 - October 2006 period. Each bird was bled for MG serology by a serum plate agglutination (SPA) test and enzyme linked immunosorbent assay (ELISA) test kits and swabbed for MG antigen detection by polymerase chain reaction (PCR) technique. Significant changes in respiratory disease have occurred over the last 20 years. There are two primary reasons for this. In the first instance, there has been the occurrence of two major worldwide plagues-PRRS and PCV2-associated diseases. The second reason has been the considerable advances in vaccination, which have become available for some of the respiratory diseases, in particular Mycoplasma hyopneumoniae. Vaccines have also become available for porcine reproductive and respiratory syndrome virus (PRRS), influenza viruses (outside UK) and probably in the near future against porcine circovirus type 2 (PCV2).

BACKGROUND: We recently demonstrated that the ubiquitous Enterococcus faecalis ebp (endocarditis- and biofilm-associated pilus) operon is important for biofilm formation and experimental endocarditis. Here, we assess its role in murine urinary tract infection (UTI) by use of wild-type E. faecalis OG1RF and its nonpiliated, ebpA allelic replacement mutant (TX5475).

METHODS: OG1RF and TX5475 were administered transurethrally either at an ~1 : 1 ratio (competition assay) or individually (monoinfection). Kidney pairs and urinary bladders were cultured 48 h after infection. These strains were also tested in a peritonitis model.

Strains of Enterococcus faecium express a cell wall-anchored protein, Acm, which mediates adherence to collagen. Here, we (i) identify the minimal and high-affinity binding subsegments of Acm and (ii) show that anti-Acm immunoglobulin Gs (IgGs) purified against these subsegments reduced E. faecium TX2535 strain collagen adherence up to 73 and 50%, respectively, significantly more than the total IgGs against the full-length Acm A domain (28%) (P < 0.0001). Blocking Acm adherence with functional subsegment-specific antibodies raises the possibility of their use as therapeutic or prophylactic agents.
