Acute Myocardial Infarction In Systemic Sclerosis Patients: A Case Series, Chris T. Derk, Sergio A. Jimenez

To characterize the clinical manifestations of patients with systemic sclerosis who develop a myocardial infarction (MI), a retrospective review of the medical records of all patients who were admitted to our institution between 1982 and 2002 and had the dual diagnosis of systemic sclerosis and an acute MI was done. From 1,009 systemic sclerosis hospital admissions, 11 (1.09%) were for an acute MI. Three of these patients had normal coronaries, and instead of wall motion abnormalities, left ventricular hypertrophy was the predominant finding of an echocardiography. In the United States, the lack of a centralized, national public health system has meant that “public health” is the primary responsibility of state and local governments. Many states have established various types of intrastate regional structures to deliver the range of on-going, occasional, and/or episodic services that characterize the world of public health. Purpose

The purpose of this article is to expand the understanding of self-management support by describing factors that contribute to implementing a comprehensive self-management program in primary care.

Methods

Four rural health centers in medically underserved areas participated in a study to document the implementation of a self-management program. This program consisted of a social marketing plan and decision-making tools to guide patients in making self-management behavior changes. The stages of change constructs of the transtheoretical model were used to design the social marketing plan. Key informant interviews were conducted at 6-month and 9-month intervals to document the implementation process. …

Concept maps are visual depictions of information, generally in diagram form, that are used to enhance and document learning (Novak, 1990; Whiteley, 2005). As a learning tool, concept maps have been used for many years in the fields of education and nursing (Novak, 1990; Schuster, 2002). The methods and uses of concept mapping have evolved in the areas of academic education, clinical education, and research since the 1970s (All & Havens, 1997; Novak, 1990; Schuster, 2002; Whiteley, 2005). This article discusses the use of concept mapping in occupational therapy education.

Rapid accurate detection is a prerequisite for the successful control of meticillin-resistant Staphylococcus aureus (MRSA). The IDI-MRSA real-time polymerase chain reaction (PCR) assay was designed to provide rapid results from nasal specimens collected in Stuart’s liquid transport medium. Taurolidine is an experimental antibacterial and antiendotoxic compound whose clinical utility as an antitumor agent is being investigated in human clinical trials. Taurolidine in aqueous solution exists in equilibrium with taurultam. Taurultam is subsequently transformed to taurinamide. The pharmacokinetic profiles of these metabolites are not well established. In this study, 18 healthy volunteers were administered 5.0 g of taurolidine in 250 mL of 5% polyvinylpyrrolidone in water over 2, 1, or 0.5 hours by intravenous infusion in a parallel-group design. All subjects noted discomfort at the infusion site, although there were no serious adverse events. t(max) generally occurred at the …

PURPOSE: To compare salivary characteristics in 3 and 4 year-old Head Start children with and without visible caries.

METHODS: This case-control study was performed on 84 healthy 3 and 4 year-old Head Start children. After consent and inclusion criteria were met, each child was screened and assigned to one of two test groups based on their caries status. Group 1 consisted of children with no visible evidence of caries, although non-visible caries may have been present. This study addresses the effects of emotional valence on selective processing of body-shape related words in young women who suffer from anorexia, bulimia, or obesity, and non-clinical controls. Research in the area of emotional Stroop interference has indicated that persons with eating disorders, particularly anorectics, show elevated latencies in naming colors of words related to food or body-shape, in contrast to either neutral words, or to non-clinical control groups. This study attempted to examine the relationship between Fibromyalgia Syndrome (FMS) pain, religious coping, and depression. The Fibromyalgia Impact Questionnaire (FIQ), Short-Form McGill Pain Questionnaire (SF-MPQ), RCOPE, and the Center for Epidemiologic Studies Depression Scale (CES-D) were used to assess these constructs in 50 participants. The sample was primarily middle aged and female, which is typical of the FMS population. This study used multiple regression to make assumptions about the causal progression of the variables. Study findings show that religious coping does not appear to significantly mediate the strong relationship between FMS pain and depression. This research served to uncover …

HIV/AIDS rates have continued to rise in Eastern Europe where adolescents are particularly vulnerable. The theory of reasoned action (TRA) has successfully been applied to understanding adolescent sexual risk behavior and provides a theoretical framework for the prevention of HIV/AIDS. According to TRA, attitudes and subjective norms predict behaviors via the mediating role of intentions, and in order to have an impact on behavior, changes must occur at the attitude and subjective norm level. PURPOSE: To compare the effect of traditional phosphoric acid etching with Er,Cr:YSGG laser enamel etching on the shear bond strength of sealant material.

METHODS: Extracted healthy third molars were mounted in acrylic exposing the flattest surface available. Three groups of twenty teeth were used to evaluate traditional acid etching with 37% phosphoric acid as a control group as well as two different laser settings. The sealant material was attached using specialized jigs and then subjected to shear bond strength testing utilizing the ultradent method (MTS machine). Statistical methods employed were the Kruskal-Wallis rank test and the Mann-Whitney U test.

The primary objectives for this experiment were to analyze the neuroanatomical correlates of autobiographical, episodic and semantic memory, use a different paradigm to promote episodic memory retrieval, and employ a different analysis technique to understand retrieval processes. Autobiographical and episodic memories are personal memories from the past. Autobiographical is more general (e.g. a street name of a house growing up) and episodic is more specific to time (e.g. 13th birthday party that took place on a street). PURPOSE: To compare the ability of newer adhesive resin cements and traditional glass ionomer cements to prevent microleakage under stainless steel crowns on extracted permanent third molars.

METHODS: All teeth were hand prepared and stainless steel crowns were cemented according to manufacturers' instructions. The specimens were thermocycled for 500 cycles and immersed in 2% methylene blue dye solution for 8 hours. Each tooth was imbedded in resin and cut into I mm thick sections by a slow speed diamond saw. The amount of microleakage was measured in microns under a traveling microscope. The study was aimed at determining if cultural beliefs are salient factors contributing to the spread of HIV/AIDS among African born adolescents aged 15 to 19 years living in southern California. We used a cross-sectional design and collected data in two phases. The first phase was qualitative and data were collected from 25 participants in focus groups. The second phase utilized a questionnaire which was developed from the results of the focus group, using the theory of planned behavior. Long-term care can be a depressing subject. Most of us tend not to think about it. However, we cannot long avoid it as the 76 million baby boomers begin reaching older ages in a few short years. According to projections, in 40 years, those aged 65 to 84 (numbering 31.6 million in 2005) will more than double, and those 85 plus (about 5.1 million in 2005), who are more at risk of dependency, will more than triple (U.S. Census Bureau, 2004 and 2006). Surfactant protein A (SP-A) is an important lung innate immune protein that kills microbial pathogens by opsonization and membrane permeabilization. We investigated the basis of SP-A-mediated pulmonary clearance of Pseudomonas aeruginosa using genetically-engineered SP-A mice and a library of signature-tagged P. aeruginosa mutants. A mutant with an insertion into flgE, the gene that encodes flagellar hook protein, was preferentially cleared by the SP-A(+/+) mice, but survived in the SP-A(-/-) mice. Opsonization by SP-A did not play a role in flgE clearance. However, exposure to SP-A directly permeabilized and killed the flgE mutant, but not the wild-type parental strain. P. aeruginosa …

This study reports on what we believe are novel mechanism(s) of the vascular protective action of adiponectin. We used intravital microscopy to measure leukocyte-endothelium interactions in adiponectin-deficient (Ad^–/–) mice and found that adiponectin deficiency was associated with a 2-fold increase in leukocyte rolling and a 5-fold increase in leukocyte adhesion in the microcirculation. Measurement of endothelial NO (eNO) revealed that adiponectin deficiency drastically reduced levels of eNO in the vascular wall. Immunohistochemistry demonstrated increased expression of E-selectin and VCAM-1 in the vascular endothelium of Ad^–/– mice. Most reports dealing with vaccines against botulinum toxin have focused on the injection route of administration. This is unfortunate, because a mucosal vaccine is likely to be more efficacious for patients and pose fewer risks to health care workers and to the environment. Therefore, efforts were made to generate a mucosal vaccine that provides protection against the botulinum serotypes that typically cause human illness (serotypes A, B, and E). Sixty male Spraque-Dawley rats were used in this study. The hearts were isolated and placed on a constant-pressure Langendorff apparatus, and perfused with only Krebs-Henseleit buffer (control), atenolol, atenolol plus prazosin, and atenolol plus salbutamol. After that the left anterior descending artery was occluded for 8 minutes and reperfused for 30 minutes to induce cardiac arrhythmias. The left ventricular pressure and coronary flow rate were monitored, and ECGs were recorded to evaluate cardiac arrhythmias. The administration of atenolol (β1-adrenergic antagonist) combined with prazosin (α1-adrenergic antagonist) prolonged RR intervals whereas that of atenolol combined with salbutamol (β2-adrenergic antagonist) did not prolong …

The current research details interviews with 50 victims and 50 defendants who participated in a specialized criminal domestic violence court in Lexington County, South Carolina. These victims and defendants indicated satisfaction with their court experiences, thought the process allowed them to voice their views, felt they were treated with respect, and were generally satisfied with the outcome of their cases. Court observations and interviews with court personnel confirmed that this court has successfully incorporated victims and defendants into the decision-making process while also providing a fair system to address the issue of violence against women.

The Clinical Focus Program was designed to prepare senior nursing students for the transition to practice in the hospital setting. Through a series of 1:1 precepted clinical experiences, the students developed a broad base of clinical competencies and self-confidence in their nursing skills. Hospitals experienced reduced costs related to orientation and recruitment of new graduate nurses. The program also was valuable in building a network of collegiality and scholarship between education and practice environments.
