Purpose of review

While excess adiposity is a known risk factor for incident heart failure ( HF), once the condition is established, observational data suggest that increased body mass index (BMI) may confer a survival advantage. This paradox has emphasized the underlying roles of cardiorespiratory fitness (CRF), and body composition, particularly lean mass (LM), in influencing clinical outcomes.

Recent findings

In this review, we explore the multifaceted nature of the obesity paradox in HF, with a focus on emerging anti-obesity incretin-mimetic therapies, such as glucagon-like peptide-1 receptor agonists (GLP-1 RAs) and dual GLP-1/glucose-dependent insulinotropic polypeptide (GIP) receptor agonists. Objective:

To determine whether gestational vitamin D status modulates the effect of pre-pregnancy obesity on gestational diabetes mellitus (GDM) risk while stratifying by maternal age.

Design:

Birth cohort.

Setting:

A major maternity hospital in Kuwait.

Participants:

Pregnant women in their second/third trimester of gestation were enrolled. Pre-pregnancy body mass index (BMI, kg/m²) was categorized as under/normal weight (< 25.0), overweight (25.0 to < 30.0), and obesity (≥30.0). Gestational 25-hydroxyvitamin D concentrations were categorized as deficiency (< 50 nmol/L) or insufficiency/sufficiency (≥50 nmol/L). GDM status was ascertained according to international guidelines. Adjusted odds ratios (aOR) and 95% confidence intervals (CI) were estimated using logistic regression.

Results:

Data from 957 pregnant women were analyzed, with GDM affecting 166 (17.4%) pregnancies. Pre-pregnancy obesity and gestational vitamin D deficiency was ascertained in 275 (28.7%) and 533 (55.7%) pregnant women, respectively. To examine physical activity (PA) among US military servicemembers (SM) with overweight and obesity (OW/OB) and if they meet Physical Activity Guidelines (PAG). To assess differences in PA by sex and associations between PA and body composition.

PA duration and intensity were objectively collected via accelerometers and self-reported daily average via self-report International Physical Activity Questionnaire (IPAQ) among US active-duty SM (n=113, 58% female, Mean BMI = 33.3 ± 3.9 kg/m2). The U.S. social welfare system includes programs aiming to increase access to nutritious food to reduce nutrition insecurity. However, studies suggest that immigrants continue to face disproportionate risks of nutrition insecurity and may face barriers to enrollment due to legal status. This study, using secondary data from the 2023 California Health Interview Survey, examines the associations between food stamp participation, food insecurity, and clinical diagnosis of diabetes among adult U.S.-born, naturalized, and non-citizen adults in California. Fermented soybean products (FSPs) contain soybean-derived substrates, fermentation-associated microorganisms, and bioactive compounds that may interact with the human gut microbiome and host physiology. However, despite the growing interest in FSPs, longitudinal human studies specifically evaluating traditional Korean-FSPs (K-FSPs), particularly among United States-based populations, remain limited. The denial, concealment, or omission of disordered eating behaviors has received limited attention in the eating disorder literature, partly due to the lack of reliable and valid measures. This study re-examined the factor structure of the Denial of Disordered Eating Behaviors Scale (DDEBS-12) and tested measurement invariance across gender identity, racial identity, and levels of disordered eating in a non-clinical undergraduate sample (N = 3285). Confirmatory factor analyses did not support the original unidimensional structure. Instead, a seven-item model assessing denial of dietary restriction demonstrated the best fit and was renamed the DDEBS-restriction. The revised scale showed good internal consistency …

This study aimed to examine the differences in the proportion of CED based on macronutrient intake and related factors. The study used a quantitative cross-sectional design with secondary data involving 124 pregnant women in selected areas served by community health centers in Bogor City and Regency. Although dietary counseling is essential in managing obesity, maintaining a calorie deficit and preventing weight regain remain difficult. Functional foods like high-protein, high-fiber meal replacements may enhance adherence and support weight loss. This randomized controlled trial evaluated the subjective outcomes of a weight loss program incorporating a protein- and fiber-enriched meal replacement among 73 overweight women aged 20–30 years. The gut-brain axis relies on nervous, immune, and endocrine interactions. Gut dysbiosis, an imbalance within the gut microbiome, disrupts gut-brain communication, impairing gut barrier integrity, inflammation regulation, and hormonal and neurotransmitter production. This study sought to develop and evaluate a new instrument to measure the capability, opportunity, and motivation for healthy eating behaviors (COM-HE) among community-dwelling older adults.

A Pilot Study Exploring Vitamin D, Mental Health, And Gastrointestinal Health In Humans, Brianna C. Williams

Mental disorders are experienced by nearly one billion people worldwide. Depression is one of the most common. Treatments exist, yet a devastating 75% of individuals suffering from depression do not receive adequate treatment. An emerging treatment strategy includes exploring the connection between mental health and nutrition, which includes the areas of gut health and nutrient deficiencies. Vitamin D is obtained through food, supplements, and sunlight. Many individuals are considered deficient (25[OH]D concentrations < 12 ng/mL) or insufficient (25[OH]D concentrations < 20 ng/mL). Purpose: This study aimed to explore mental and gastrointestinal (GI) health using surveys and lipopolysaccharide binding protein (LBP) concentrations, a biomarker of GI health, in individuals considered 25(OH)D deficient (DEF) vs not deficient (N-DEF). Methods: This cross-sectional study recruited physically active participants aged 18-45 years who underwent three evaluations in one visit. Health was assessed using surveys to evaluate mental health, GI symptoms, and vitamin D intake. Serum from fasting blood samples was used with an enzyme-linked immunosorbent assay to determine 25(OH)D and LBP concentrations. Unpaired t-tests were used to compare DEF vs N-DEF and significance was set at p < 0.05. Results: Thirty-two participants (mean age 25.3 ± 6.3 years) with mean and body fat percentage of 24.0% ± 10.7%, were placed into DEF (n=16) and N-DEF (n=16) categories. Interestingly, five of the 16 participants in DEF (31%) showed some level of depression (scores ranging from mild to severe) and only two of the 16 participants in N-DEF iv (13%) scored positively in the depression category (scores ranging from mild to moderate). Additionally, the participant with the lowest 25(OH)D concentration (5.42 ng/mL) was the only participant to meet the “severe” depression category criteria, with a score of 24 compared to other participants who scored within 0-16. Also, the only participant who scored “severe” in the stress category, was in the bottom 20% of 25(OH)D concentrations. As for GI health, the difference between average GI scores and average serum LBP concentrations between the groups were not statistically significant. Conclusion: There were no significant differences between DEF and N-DEF in perceived depression (p = 0.16), anxiety (p = 1.0), stress (p = 0.96), GI symptoms (P values ranged from 0.27 to 1.00), nor in LBP concentrations (p = 0.77). Kid Eats is a free app for iPad encouraging children and parents to cook together at home using fruits and vegetables. A preliminary study with parents and children showed children enjoyed using the app and that it provided useful information to support children’s involvement in the kitchen, including guidance around safety concerns. Every family in the study voluntarily completed at least one of the recipes with their child at least once following the in-person activity. Parents reported that their children actively asked to cook the recipes from the app and felt the app helped increase their time spent cooking with …

This qualitative study explores the lived experiences of 27 participants enrolled in Diabetes Solutions, a telehealth-based diabetes self-management education and support (DSMES/T) program offered by the Mississippi Diabetes Network. Using a phenomenological approach and structured interviews, the study identifies four major themes: structural benefits of the program, knowledge gained, common changes by participants, and quality of life outcomes.
