Background: Primary care services, as recommended by the National Health Security Office (NHSO), are crucial for Thai community pharmacists. Training needs for the services and a professional development or training program however remained unexplored. Objectives: To analyze community pharmacists' training needs and develop professional development programs to meet their needs. Methods: A mixed-methods study with two phases was conducted during 2018-2019. การศึกษาแบบเก็บข้อมูลย้อนหลังนี้มีวัตถุประสงค์เพื่อสร้างแบบจำลองทางเภสัชจลนศาสตร์และหาปัจจัยที่มีผลต่อค่าพารามิเตอร์ทางเภสัชจลนศาสตร์ของยาแวนโคมัยซินในผู้ป่วยเด็กชาวไทย ค่าความเข้มข้นของ ยาแวนโคมัยซินในซีรั่มจานวน 348 ตัวอย่าง จากผู้ป่วยเด็กอายุระหว่าง 1 เดือน ถึง 18 ปี ที่เข้ารับการรักษา ณ โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย ระหว่างปี พ.ศ. 2555-2560 จานวน 212 คน วิเคราะห์ข้อมูลเภสัช จลนศาสตร์ประชากรด้วยโปรแกรม NONMEM® โดยวิธี first-order conditional estimation with interaction ลักษณะทางเภสัชศาสตร์ของยาแวนโคมัยซินอธิบายได้ด้วยแบบจำลองชนิดหนึ่งห้องที่มีการกำจัดยา แบบปฏิกิริยาอันดับหนึ่ง ค่าเฉลี่ยอัตราการกำจัดยาแวนโคมัยซิน (CL) มีค่า 0.13 ลิตร/ชั่วโมง/กิโลกรัม และ ค่าเฉลี่ยปริมาตรการกระจายยา (Vd) มีค่า 0.88 ลิตร/กิโลกรัม ปัจจัยที่มีผลต่อค่าพารามิเตอร์ทางเภสัชศาสตร์ ได้แก่ น้ำหนัก (กิโลกรัม) และอัตราการกรองของไต (มิลลิลิตร/นาที/1.73 ตารางเมตร) ดังแสดงในสมการ CL (ลิตร/ชั่วโมง) = 1.66*(น้ำหนัก/14)[superscript 0.75]*(อัตราการกรองของไต/108.9) และ V (ลิตร) = 12.7*(น้ำหนัก/14) ค่า ความผันแปรระหว่างบุคคลของ CL เท่ากับร้อยละ 34.8 ค่าความผันแปรระหว่างบุคคลของ V เท่ากับร้อยละ 39.6 และแบบจำลองสุดท้ายถูกตรวจสอบความสมเหตุสมผลด้วยวิธี bootstrap และ visual predictive check (VPC) ค่าพารามิเตอร์จากแบบจำลองสุดท้ายมีค่าใกล้เคียงกับค่าพารามิเตอร์ที่ได้จากการทำ bootstrap 1,000 ครั้ง และอยู่ในช่วงความเชื่อมั่นที่ร้อยละ 95 ผลการจำลองข้อมูลด้วยวิธี VPC 1,000 ครั้ง พบค่าความเข้มข้นของ ยาแวนโคมัยซินร้อยละ 6 ที่อยู่นอกช่วงความเชื่อมั่นที่ร้อยละ 90 แสดงถึงความเหมาะสมของแบบจำลองดังกล่าว ในการอธิบายเภสัชจลนศาสตร์ของยาแวนโคมัยซิน

สาร Oxyresveratrol เป็นสารสำคัญในแก่นของต้นมะหาด โดยได้มีการศึกษาถึงฤทธิ์ทางเภสัชวิทยาของสาร Oxyresveratrol ในหลายด้าน แต่อย่างไรก็ตามสาร Oxyresveratrol เป็นสารที่มีค่าชีวปริมาณออกฤทธิ์ต่ำจึงเป็นอุปสรรคที่สำคัญในการนำมาพัฒนาเป็นยารับประทาน ดังนั้นจุดประสงค์ในการศึกษานี้ เพื่อศึกษาเภสัชจลนศาสตร์ของสาร Oxyresveratrol ที่เปลี่ยนไปเมื่อให้สาร Piperine เป็นสารเพิ่มชีวปริมาณออกฤทธิ์ในหนูแรท ซึ่งขนาดที่ให้แบ่งเป็นสาร Oxyresveratrol เดี่ยวขนาด 10 mg/kg หรือ สาร Oxyresveratrol ขนาด 10 mg/kg ให้ร่วมกับสาร Piperine 1 mg/kg โดยการบริหารยาทางหลอดเลือดดำ และ สาร Oxyresveratrol เดี่ยวขนาด 100 mg/kg หรือ สาร Oxyresveratrol ขนาด 100 mg/kg ให้ร่วมกับสาร Piperine 10 mg/kg โดยการบริหารยาทางปากจากนั้นเก็บตัวอย่างเลือด, อวัยวะ, ปัสสาวะ และ อุจจาระ ตามเวลาที่กำหนดนำไปวิเคราะห์ด้วยเครื่อง LC-MS/MS เพื่อวิเคราะห์ข้อมูลทางเภสัชจลนศาสตร์ จากผลการศึกษาพบว่าสาร Piperine สามารถเพิ่มค่าชีวปริมาณออกฤทธิ์ของสาร Oxyresveratrol ขึ้นประมาณ 2 เท่า รวมถึงสามารถช่วยเพิ่มค่าความเข้มข้นของสาร Oxyresveratrol สูงสุดในพลาสมาโดยการบริหารทางปากและ เมื่อคำนวณอัตราส่วนปริมาณยาในอวัยวะต่อพลาสมามีค่าประมาณ 10-100 เท่า ซึ่งสอดคล้องกับค่าปริมาณการกระจายยาที่สูง ประมาณ 50 L/kg ในกระบวนการเปลี่ยนแปลงสภาพของสารเมื่อได้รับร่วมกับสาร Piperine มีค่า Conversion ratio ลดลงในทั้งสองรูปแบบการบริหาร โดยสรุปสาร Piperine สามารถปรับปรุงค่าทางเภสัชจลนศาสตร์ของสาร Oxyresveratrol ได้ทั้งในทั้งสองรูปแบบการบริหารยา การศึกษาทางเภสัชจลนศาสตร์นี้จึงสามารถนำมาเป็นข้อมูลในการพัฒนาสาร Oxyresveratrol เพื่อใช้ประโยชน์ต่อไปได้

ผู้ป่วยมะเร็งเต้านมที่มีการแสดงออกของตัวรับเอสโตรเจนในระยะลุกลามร้อยละ 40 สามารถกลับเป็นมะเร็งซ้ำเนื่องจากมีการดื้อยา tamoxifen แพทย์จึงเปลี่ยนไปใช้ยาต้านฮอร์โมนกลุ่มอื่น แต่ยังเกิดการดื้อยาและมีผลข้างเคียงที่รุนแรง ดังนั้นการนำสารจากธรรมชาติมาพัฒนาเป็นยาที่มีฤทธิ์ยับยั้งการเจริญของเซลล์มะเร็งจึงเป็นแนวทางที่อาจลดปัญหาดื้อยาได้ สารสกัดจากต้นจันทน์หอม (Mansonia Gagei Drumm) ยับยั้งการเจริญของเซลล์มะเร็งได้หลายชนิด โดย mansonone G (MG) สารสำคัญที่แยกจากสารสกัดสามารถแย่งจับกับตัวรับเอสโตรเจนได้ ในการศึกษานี้จึงทำการทดสอบฤทธิ์อนุพันธ์ MG คือ Ethoxy mansonone G (EMG) สารกึ่งสังเคราะห์ในการยับยั้งการเจริญ การลุกลามและการยับยั้งการเจริญแบบไร้การยึดเกาะ พบว่าเมื่อเพิ่มความเข้มข้นและระยะเวลาในการทดสอบ EMG สามารถยับยั้งการเจริญในเซลล์มะเร็งเต้านมชนิดที่ไม่ดื้อยา (MCF-7) และเซลล์มะเร็งเต้านมที่ดื้อต่อยาต้านฮอร์โมน (MCF-7/LCC2, MCF-7/LCC9) อย่างมีนัยสำคัญทางสถิติและมีความเป็นพิษน้อยกว่ายาเคมีบำบัดในเซลล์ไฟโปรบลาสต์ และพบว่ากลไกการออกฤทธิ์ของ EMG ในการยับยั้งการเจริญของเซลล์ MCF-7 ส่วนหนึ่งผ่านการลดการแสดงออกของยีนที่เป็นเป้าหมายของตัวรับเอสโตรเจนและสาร EMG ยังสามารถลดการแสดงออกของยีนที่เกี่ยวกับการดื้อต่อยา tamoxifen ใน MCF-7/LCC2 และ MCF-7/LCC9 นอกจากนี้ EMG ยังยับยั้งการลุกลามของ MCF-7/LCC2 และ MCF-7/LCC9 โดยลดการแสดงออกของ MMP-9 และ uPA ทั้งยังสามารถลดการเจริญแบบไร้การยึดเกาะผ่านการลดจำนวน ขนาดของ colony ใน MCF-7/LCC2 และ MCF-7/LCC9 สรุปว่าสาร EMG มีคุณสมบัติในการต้านมะเร็งเต้านมชนิดไม่ดื้อยาและชนิดที่ดื้อต่อยาต้านฮอร์โมน ต้านการลุกลาม รวมไปถึงลดการเจริญแบบไร้การยึดเกาะของ MCF-7/LCC2 และ MCF-7/LCC9 ซึ่งอาจนำไปพัฒนาเป็นยาชนิดใหม่ในการรักษาผู้ป่วยมะเร็งเต้านมที่ดื้อต่อยาต้านฮอร์โมน

ยาฟิแนสเทอไรด์ (finasteride) เป็นยาในกลุ่ม 5-alpha reductase inhibitor ใช้รักษาโรคผมร่วงทางพันธุกรรมในเพศชาย โดยยาฟิแนสเทอไรด์อาจทำให้เกิดผลข้างเคียงหลายประการได้แก่ การเสื่อมสมรรถภาพทางเพศ ปริมาณน้ำอสุจิลดลง ดังนั้นควรมีการถูกใช้ในรูปแบบยาทาหนังศีรษะเพื่อลดผลข้างเคียงดังกล่าว โดยการศึกษานี้มีวัตถุประสงค์เพื่อศึกษาระดับยาฟิแนสเทอไรด์ในเลือดและในชั้นผิวหนัง หลังการให้ยาความแรง 0.5% ด้วยการทาผิวหนังเพียงครั้งเดียว (single dose) และแบบทาผิวหนังหลายครั้ง (repeated dose) บนพื้นที่ 4 ตารางเซนติเมตรของหนูแรทเพศผู้สายพันธุ์ Sprage-Dawley โดยมีการเก็บพลาสมาและผิวหนังภายใน 24 ชั่วโมง หลังการทายาครั้งสุดท้ายและสกัดยาออกมาเพื่อตรวจวัดระดับยาฟิแนสเทอไรด์ด้วยเครื่อง LC-MS/MS ผลการศึกษาแสดงให้เห็นว่า ระดับยาฟิแนสเทอไรด์ในพลาสมาหลังการให้ยาด้วยการทาผิวหนังเพียงครั้งเดียวมีแนวโน้มการดูดซึมเข้ากระแสเลือดเพิ่มขึ้นตามปริมาตรยาที่ทา โดยเริ่มตรวจพบระดับยาที่เวลา 4 ชั่วโมง หลังทายาในปริมาตร 300 ไมโครลิตร เพียงครั้งเดียว แต่ไม่พบระดับยาในพลาสมาของกลุ่มที่ได้รับยาปริมาตร 75 หรือ 150 ไมโครลิตร นอกจากนี้การทายาซ้ำวันละ 1 ครั้งและทายาซ้ำวันละ 2 ครั้ง ติดต่อกัน 7 วัน ไม่มีนัยสำคัญต่อระดับยาในพลาสมา ซึ่งเห็นได้ว่ายาฟิแนสเทอไรด์ถูกดูดซึมเข้ากระแสเลือดได้น้อยหลังการให้ยาด้วยการทาผิวหนัง สำหรับอัตราส่วนระหว่างระดับยาในผิวหนังและระดับยาในพลาสมาที่เวลา 24 ชั่วโมง หลังได้รับยาทางปากเพียงครั้งเดียวมีค่าประมาณ 2.6 เท่า โดยพบอัตราส่วนระหว่างระดับยาในผิวหนังบริเวณที่ไม่สัมผัสยา (control sites) และระดับยาในพลาสมาเกินกว่า 2.6 เท่า ในหนูแรทกลุ่มที่ได้รับยาทางผิวหนังทุกกลุ่ม ทั้งนี้ค่าที่ตรวจพบระดับยาในพลาสมามีความแปรปรวนสูงมาก อย่างไรก็ตามผลการศึกษานี้แสดงให้เห็นว่า ยาฟิแนสเทอไรด์สามารถสะสมอยู่ในผิวหนังชั้นหนังแท้ (dermis) และถูกดูดซึมเข้าสู่กระแสเลือดได้น้อยหลังให้ยาด้วยการทาผิวหนัง ทั้งนี้จำนวนครั้งการทาทั้งการทาซ้ำวันละ 1 ครั้งและการทาซ้ำวันละ 2 ครั้งอาจไม่มีผลอย่างมีนัยสำคัญต่อการดูดซึมยาเข้าสู่กระแสเลือด

Gamma-Hydroxybutyrate (GHB) is an endogenous short-chain fatty acid formed from Gamma-aminobutyric acid (GABA). Clinically, GHB is marketed in the United States as Xyrem to treat narcolepsy with cataplexy and in Europe for the treatment of alcohol withdrawal and narcolepsy. However, the illicit use and abuse of GHB occurs due to its sedative/hypnotic and euphoric effects. Monocarboxylate transporters (MCTs and SMCTs) are integral membrane proteins that control the bidirectional transport of endogenous substrates including lactate, acetate and pyruvate. Introduction: Prescription drug monitoring programs (PDMPs) are one strategy established to curb the prescription opioid abuse epidemic. Prescriber use mandates has emerged as a promising practice to increase PDMPs impact on prescription opioid abuse; however, evidence of its effectiveness has not yet been established. Kentucky was the first state to implement comprehensive prescriber use mandates in July 2012.

Background
Renal cell carcinoma (RCC) is the most common type of kidney cancer. Patients diagnosed with metastatic RCC (mRCC) have shorter overall survival compared to those diagnosed at earlier stages. Several targeted therapies, which cost from $7,000 - $16,000 per month have been approved since 2005 to treat mRCC. In addition, there is a growing interest in the use of cytoreductive nephrectomy (CN) with targeted therapies among mRCC patients. Introduction: Warfarin is the most commonly prescribed drug for stroke prevention among Atrial Fibrillation (AF) patients, especially in older adult populations, but medication nonadherence reduces its effectiveness in clinical practice. Group Based Trajectory Models (GBTM) have been used to identify distinct patterns of adherence behavior related to various medications and understand the patient characteristics associated with each trajectory. Secreted proteins play important roles in many cellular functions and molecular processes. Because secreted proteins potentially enter the blood stream, they can serve as valuable measures of health and disease useful for disease diagnosis and prognosis, therapeutic target identification, and patient stratification in personalized medicine. Consequently, significant interest exists in secreted protein analysis within complex biospecimens, particularly blood but significant bioanalytical challenges including the wide protein dynamic range >10 orders of magnitude remain. The organic cation transporters (OCTs) play a critical role in the absorption, distribution and elimination of many drugs, hormones, herbal medicines, and environmental toxins. Given the broad substrate specificity of OCTs, they fall victim to the high susceptibility for contributing to harmful drug-drug interactions. Further defining how human (h)OCTs mechanistically bind to its broad array of substrates will provide significant insight to the understanding and prediction of drug-drug interactions in polypharmacy patients and the advancement of future rational drug design for therapeutics targeting OCTs. Human immunodeficiency (HIV) infection can result in neurocognitive deficits in about one-half of infected individuals. Despite excellent systemic effectiveness, restricted antiretroviral penetration across the blood-brain barrier (BBB) is a major limitation in fighting HIV infection within the central nervous system (CNS). Drug abuse exacerbates cognitive impairment and pathologic CNS changes in HIV-infected individuals. This work investigates the effects of the HIV-1 protein, Tat, and drugs of abuse on factors affecting drug penetration into the brain.

Glycosaminoglycans are heterogeneous polysaccharides that mediate important biological functions. There has been considerable interest in deciphering the precise GAG sequences that are responsible for protein interactions. In fact, several GAG oligosaccharides have been discovered to date as targeting proteins with higher level of specificity. Yet, it has been difficult to develop GAG oligosaccharides as drugs. One of the key reasons for this state of art is that GAG synthesis is extremely challenging and is highly structure-specific. Thus, much of the biology and pharmacology of GAG remains unknown and unexploited to date.

Intracellular polymerization of deoxygenated sickle hemoglobin (Hb S) remains the principal cause of the pathophysiology associated with sickle cell disease (SCD). Naturally occurring and synthetic allosteric effectors of hemoglobin (AEH) have been investigated as potential therapeutic agents for the treatment of SCD. Several aromatic aldehydes, including vanillin, have been studied previously as AEHs for their antisickling activities. Specifically, these compounds form Schiff- base adduct with Hb to stabilize the oxygenated (R) state, increase Hb affinity for O2 and concomitantly prevent the hypoxia-induced primary pathophysiology of Hb S polymerization and RBC sickling, in turn, ameliorating several of the cascading secondary adverse …

Using a semi-PBPK modeling/quantitative meta-analysis approach, this project investigated what factors affect pulmonary and systemic exposures of Budesonide (BUD), Tobramycin (TOB), and Ciprofloxacin (CIP) after inhalation:
Three structurally different pulmonary disposition models were developed for each drug, including pulmonary absorption (all three), excretion (TOB and CIP) and sequestration (TOB) in a peripheral and central lung compartment. Systemic disposition parameters were estimated using available human mean plasma (cp(t)) and sputum (cs(t)) concentration profiles after IV administration, and GI absorption parameters were estimated from these profiles after oral administration.
