การประเมินการเกิดฟันผุก่อนปรากฏลักษณะทางคลินิก หรือก่อนฟันผุเดิมเปลี่ยนแปลงขนาดและความรุนแรงนั้นเป็นมาตรการหนึ่งที่จะช่วยให้บุคลากรทางทันตสาธารณสุขสามารถจําแนกผู้ป่วยที่มีความเสี่ยงต่อการเกิดโรคฟันผุที่แตกต่างกันซึ่งต้องการการเฝ้าระวัง และการวางแผนติดตามการเกิดโรคที่แตกต่างกัน และสามารถจัดสรรทรัพยากรที่มีอยู่อย่างจํากัดได้อย่างเหมาะสม วัตถุประสงค์ของการทบทวนวรรณกรรมนี้เพื่อชี้ให้เห็นความสําคัญและประโยชน์ของการประเมินความเสี่ยงต่อการเกิดโรคฟันผุและการพิจารณาเลือกปัจจัยด้านต่าง ๆ เพื่อทํานายการเกิดฟันผุในอนาคตของเด็กได้ โรคฟันผุเกิดจากหลายปัจจัย ประกอบด้วย ปัจจัยทั่วไป ได้แก่ สถานภาพทางเศรษฐกิจและสังคม โรคประจําตัวและยาที่มีผลต่อสุขภาพช่องปาก และปัจจัยเฉพาะ ได้แก่ ตัวฟัน สิ่งแวดล้อมในช่องปาก พฤติกรรม และเชื้อแบคทีเรีย การพิจารณาเลือกรูปแบบหรือแบบจําลองในการทํานายความเสี่ยงต่อการเกิดโรคฟันผุควร พิจารณาถึง กลุ่มอายุ ความชุกของการเกิดโรค ลักษณะของการผุ และงบประมาณที่มี ซึ่งปัจจัยฟันผุในอดีต ปัจจัยอนามัยช่องปาก และพฤติกรรมการแปรงฟันจะเป็นตัวทํานายความเสี่ยงของการเกิดฟันผุในอนาคตได้ดีที่สุดส่วนการใช้ชุดทดสอบเชื้อจะเหมาะในการทํานายฟันที่ผุเริ่มแรกในเด็ก แต่จะไม่เหมาะในการทํานายฟันผุที่ลุกลาม ไปแล้วซึ่งควรใช้ประกอบการพิจารณาร่วมกับปัจจัยอื่น ๆ (ว ทันต จุฬาฯ 2557;37:97-112)

วัตถุประสงค์ เพื่อเปรียบเทียบความรู้และทัศนคติต่อการใช้และการดื้อยาปฏิชีวนะในทางทันตกรรมของอาจารย์ทันตแพทย์และนิสิตทันตแพทย์ชั้นปีที่ 4 วัสดุและวิธีการ โดยให้อาจารย์ทันตแพทย์และนิสิตทันตแพทย์ชั้นปีที่ 4 ตอบแบบสอบถามเกี่ยวกับความรู้และทัศนคติที่มีต่อการใช้ยาปฏิชีวนะเพื่อรักษาการติดเชื้อในช่องปากและการดื้อยาโดยที่แบบสอบถามไม่มีการระบุข้อมูลของผู้ตอบและนําข้อมูลมาวิเคราะห์โดยใช้สถิติเชิงพรรณนาและสถิติเชิงวิเคราะห์ ผลการศึกษา จํานวนแบบสอบถามที่นํามาวิเคราะห์ข้อมูลมี 117 ชุด จากจํานวนที่แจกทั้งหมด 200 ชุด (ร้อยละ 59) พบว่าร้อยละ 70 ของผู้ตอบแบบสอบถามทั้งสองกลุ่มคิดว่าปัญหาการดื้อยาเป็นปัญหาที่สําคัญของประเทศและ ร้อยละ 74 ของผู้ตอบแบบสอบถามทั้งหมดมีความเห็นว่าการใช้ยาปฏิชีวนะอย่างแพร่หลายและการใช้ยาอย่างไม่ เหมาะสมเป็นสาเหตุสําคัญที่ทําให้เกิดการดื้อยา และร้อยละ 93 ของผู้ตอบแบบสอบถามทั้งหมดมีความเห็น ว่าการกําหนดแนวทางในการสั่งจ่ายยาจะเป็นวิธีที่มีประสิทธิภาพมากที่สุดในการลดปัญหาการดื้อยา สําหรับการใช้ยาปฏิชีวนะเพื่อการป้องกันการติดเชื้อของเยื่อหุ้มหัวใจ พบว่าทั้งกลุ่มอาจารย์ทันตแพทย์และกลุ่มนิสิต ทันตแพทย์มีความเห็นไม่ต่างกัน (ร้อยละ 86 และร้อยละ 98 ตามลําดับ) แต่สําหรับในกรณีของการป้องกันการ ติดเชื้อในผู้ป่วยที่ได้รับการฉายแสงที่ศีรษะและลําคอ ทั้งสองกลุ่มมีความเห็นต่างกันอย่างมีนัยสําคัญทางสถิติ (p < 0.05) คือร้อยละ 36 ของกลุ่มอาจารย์และร้อยละ 60 ของกลุ่มนิสิตที่สั่งจ่ายยาปฏิชีวนะ ทั้งนี้โดยรวมพบว่า ผู้ตอบแบบสอบถามทั้งหมดมีความรู้ในการสั่งจ่ายยาปฏิชีวนะในระดับที่เหมาะสม แต่ก็ยังคงมีแนวโน้มของการสั่งจ่ายยาที่มากเกินในบางกรณี ซึ่งอาจเป็นสาเหตุของการเกิดผลข้างเคียงจากการใช้ยาและเกิดการดื้อยาได้เช่นกัน สรุป ผู้ตอบแบบสอบถามส่วนใหญ่มีความเห็นว่าปัญหาการดื้อยาปฏิชีวนะเป็นปัญหาที่สําคัญในระดับประเทศแต่มีความคิดเห็นที่หลากหลายเกี่ยวกับปัญหาการดื้อยารวมทั้งสาเหตุและวิธีการแก้ไข การจัดให้มีแนวทางการสั่ง จ่ายยาและจัดให้มีการสอนในระดับหลังปริญญาอาจเป็นวิธีที่จะช่วยลดปัญหาการดื้อยา (ว ทันต จุฬาฯ 2557;37:83-96)

การตอบสนองของเนื้อเยื่อในของฟันสุนัขเมื่อปิดทับด้วยพอร์ตแลนด์ซีเมนต์สีขาวทีปรับปรุงคุณภาพเปรียบเทียบกับโปรรูทเอ็มทีเอ, กุลนันทน์ ดํารงวุฒิ, อัญชนา พานิชอัตรา, ชนินทร์ กัลล์ประวิทธ์

วัตถุประสงค์ เพื่อเปรียบเทียบการตอบสนองของเนื้อเยื่อในของฟันสุนัขต่อการตัดเนื้อเยื่อในบางส่วนแล้วปิดทับด้วยพอร์ตแลนด์ซีเมนต์ที่ปรับปรุงคุณภาพกับโปรรูปเอ็มทีเอ วัสดุและวิธีการ ทําการตัดเนื้อเยื่อในบางส่วนในฟันกรามน้อยของสุนัข 4 ตัว จํานวน 35 ปี และแบ่งเป็น 3 กลุ่ม กลุ่ม 1 ปิดด้วยโปรรูทเอ็มทีเอที่ผสมกับน้ํากลั่น (จํานวน 10 ที่) กลุ่ม 2 ปิดด้วยพอร์ตแลนด์ซีเมนต์ที่มีบิสมัต ออกไซด์ ผสมกับแคลเซียมคลอไรด์ความเข้มข้นร้อยละ 5 และเมททิลเซลลูโลสความเข้มข้นร้อยละ 1 (จํานวน 20 ) รองพื้นด้วยกลาสไอโอโนเมอร์ซีเมนต์ และบูรณะด้วยเรซิน คอมโพสิต โดยทั้งสองกลุ่มจะทดลองที่ 7 และ 70 วัน กลุ่ม 3 เป็นกลุ่มควบคุมบวก เนื้อเยื่อในที่ตัดถูกเปิดไว้เป็นเวลา 7 วัน (จํานวน 5 ปี) ทําการถอนฟัน ภายใต้การดมยาสลบ แล้วนําฟันที่ได้ไปผ่านกระบวนการตรวจลักษณะทางจุลพยาธิวิทยา เพื่อประเมินการอักเสบ และการสร้างเนื้อเยื่อแข็งของเนื้อเยื่อใน นําข้อมูลที่ได้มาวิเคราะห์ความแตกต่างทางสถิติระหว่างกลุ่มด้วยการ ทดสอบครัสตัล-วอลลิส และไคสแควร์ ที่ระดับนัยสําคัญน้อยกว่า 0.05 ผลการศึกษา ไม่พบความแตกต่างอย่างมีนัยสําคัญทางสถิติด้านการตอบสนองต่อการอักเสบและการหาย ระหว่างโปรรูทเอ็มทีเอและพอร์ตแลนด์ซีเมนต์ทั้งในระยะเวลา 7 วัน และ 70 วัน โดยกลุ่มทดลองทั้งสองไม่พบ การอักเสบของเนื้อเยื่อใน นอกจากนี้พบว่ามีการสร้างเนื้อเยื่อแข็งที่ต่อเนื่อง ด้วยลักษณะรูปร่างและความหนาที่ ไม่แตกต่างกันระหว่างกลุ่มทดลองทั้งสองอย่างไรก็ตามพบความแตกต่างอย่างมีนัยสําคัญระหว่างกลุ่มทดลอง ทั้งสองกับกลุ่มควบคุมบวก ซึ่งพบการอักเสบในระดับปานกลางถึงรุนแรง สรุป พอร์ตแลนด์ซีเมนต์ของประเทศไทยที่มีบิสมัดออกไซด์ผสมกับแคลเซียมคลอไรด์และเมททิลเซลลูโลส เมื่อนํามาใช้เป็นวัสดุปิดทับเนื้อเยื่อในสามารถคงความมีชีวิตของเนื้อเยื่อในของฟันได้โดยปราศจากการอักเสบส่งเสริมให้เกิดการหายและกระบวนการซ่อมแซม นอกจากนี้ยังสามารถกระตุ้นให้เกิดการสร้างเนื้อเยื่อแข็งที่มี ลักษณะไม่แตกต่างจากโปรรูปเอ็มทีเอ (ว ทันต จุฬาฯ 2557;37:47-58)
