Hypertension is a major cause of cardiovascular disease and deaths worldwide especially in low- and middle-income countries. Despite the availability of safe, well-tolerated, and cost-effective blood pressure (BP)-lowering therapies, <14% of adults with hypertension have BP controlled to a systolic/diastolic BP <140/90 mm Hg. We report new hypertension treatment guidelines, developed in accordance with the World Health Organization Handbook for Guideline Development. Overviews of reviews of the evidence were conducted and summary tables were developed according to the Grading of Recommendations, Assessment, Development, and Evaluations approach. In these guidelines, the World Health Organization provides the most current and relevant evidence-based guidance for the pharmacological treatment of nonpregnant adults with hypertension. The recommendations pertain to adults with an accurate diagnosis of hypertension who have already received lifestyle modification counseling. The guidelines recommend BP threshold to initiate pharmacological therapy, BP treatment targets, intervals for follow-up visits, and best use of health care workers in the management of hypertension. The guidelines provide guidance for choice of monotherapy or dual therapy, treatment with single pill combination medications, and use of treatment algorithms for hypertension management. Strength of the recommendations was guided by the quality of the underlying evidence; the tradeoffs between desirable and undesirable effects; patient's values, resource considerations and cost-effectiveness; health equity; acceptability, and feasibility consideration of different treatment options. The goal of the guideline is to facilitate standard approaches to pharmacological treatment and management of hypertension which, if widely implemented, will increase the hypertension control rate world-wide.

This study aims to identify the major phytochemical constituents in Aquilaria malaccensis (Thymelaeaceae) ethanolic leaf extract (ALEX-M) and elucidate their ability to suppress nitric oxide (NO) production from a murine macrophage-like cell line (RAW 264.7) stimulated by lipopolysaccharide (LPS) and interferon-γ (IFN-γ). Dichloromethane (DCM) and ethyl acetate (EtOAc) fractions of ALEX-M were subjected to column chromatography. Eight known compounds were isolated for the first time from this species. Compounds were identified using spectroscopic techniques (IR, UV, HRESIMS, and 1D and 2D NMR). Anti-inflammatory activity of both extract and isolated compounds were investigated in vitro. The heterogeneous nature of multiple sclerosis (MS) and the unavailability of treatments addressing its intricate network and reversing the disease state is yet an area that needs to be elucidated. Liraglutide, a glucagon-like peptide-1 analogue, recently exhibited intriguing potential neuroprotective effects. The currents study investigated its potential effect against mouse model of MS and the possible underlying mechanisms. Demyelination was induced in C57Bl/6 mice by cuprizone (400 mg/kg/day p.o.) for 5 weeks. There has been a higher prevalence of developing anxiety due to frequent episodes of stress among adults in recent years. Chronic anxiety can contribute to the prevalence of elevated blood pressure and hypertension. High anxiety and stress also contribute to overactivation of the sympathetic nervous system which can be quantified by increased muscle sympathetic nerve activity (MSNA). Sympathetic overactivation can lead to vasoconstriction and loss of arterial elasticity. Anxiety, MSNA, blood pressure, and arterial stiffness are all interconnected, thus studying these relationships is crucial to understand the underlying mechanisms for prevention and treatment of cardiovascular disease (CVD). Automatic Cephalometric Landmark Detection On X-Ray Images Using Object Detection, Cheng-Ho King, Yin-Lin Wang, Chia-Ling Tsai

We propose a new deep convolutional cephalometric landmark detection framework for orthodontic treatment. Our proposed method consists of two major steps: landmark detection using a deep neural network for object detection, and landmark repair to ensure one instance per landmark class. For landmark detection, we modify the loss function of the backbone network YOLOv3 to eliminate the constrains on the bounding box and incorporate attention mechanism to improve the detection accuracy. For landmark repair, a triangle mesh is generated from the average face to eliminate superfluous instances, followed by estimation of missing landmarks from the detected ones using Laplacian Mesh. …

Introduction: To further understand the effects of L-arginine on both its ability to enhance lactate clearance and increase overall blood flow before, during, and after exhaustive anaerobic exercise.
Methods: Twelve healthy male subjects between the ages of 18-25 first completed an initial visit where baseline data was recorded. Subjects completed three additional visits, in which they ingested either a placebo, two-gram or four-gram dose of L-arginine. Multidirectional instability (MDI) of the shoulder is characterized by generalized shoulder capsular laxity and symptomatic shoulder instability in more than one direction with one direction of instability as inferior. Generalized ligamentous laxity and specifically shoulder laxity, has been associated with female athletes. While males are at a higher risk of shoulder instability due to a number of extrinsic risk factors including participation in higher risk contact/collision activities, females are particularly susceptible to MDI due to their association with increased joint laxity. Significance: Visual anticipation is vital for performance in several domains such as driving, military, and high-speed interceptive sports, as it enables performers to handle severe time constraints. There has been little investigation into individual differences of anticipation skill transfer within a domain, which can guide training of anticipation to improve performance.

Chronic nonspecific low back pain (CNSLBP) is a complex and multifaceted problem. The following Perspective Piece tries to help make sense of this complexity by describing a model for the development and maintenance of persistent low back pain that integrates modifiable factors across the biopsychosocial spectrum. The Fit-for-Purpose model (FFPM) posits the view that chronic nonspecific low back pain represents a state in which the person in pain holds strong and relatively intransient internal models of an immutably damaged, fragile, and unhealthy back, and information that supports these models is more available and trustworthy than information that counters them. วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ มารดาวัยรุ่นอายุ 15-19 ปี ที่มารับบริการฝากครรภ์และคลอดบุตรที่โรงพยาบาลชุมชนเพื่อรับส่งต่อผู้ป่วย ในจังหวัดชลบุรี จำนวน 50 คน สุ่มเข้ากลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 25 คน กลุ่มทดลองได้รับโปรแกรมการส่งเสริมความมีคุณค่าในตนเองในการปฏิบัติบทบาทการเป็นมารดาในระยะตั้งครรภ์ 32-40 สัปดาห์ จนถึง 4-6 สัปดาห์หลังคลอด กลุ่มควบคุมได้รับการพยาบาลตามปกติ เก็บรวบรวมข้อมูลโดยใช้แบบประเมิน 4 ชุด ดังนี้ 1) แบบสอบถามข้อมูลทั่วไป 2) แบบวัดภาวะซึมเศร้า CES-D 3) แบบวัดความรู้สึกมีคุณค่าในตนเอง 4) แบบวัดความสำเร็จในการปฏิบัติบทบาทการเป็นมารดา โดยแบบประเมินชุดที่ 2,3 และ 4 มีค่าความเที่ยงสัมประสิทธิ์อัลฟ่าครอนบาค เท่ากับ .807, .870 และ .949 ตามลำดับ วิเคราะห์ข้อมูลทั่วไปโดยใช้สถิติบรรยาย และทดสอบค่าเฉลี่ยโดยใช้สถิติทดสอบที
ผลการวิจัย: ค่าเฉลี่ยของคะแนนภาวะซึมเศร้าของมารดาวัยรุ่นหลังได้รับโปรแกรมการส่งเสริมความมีคุณค่าในตนเองในการปฏิบัติบทบาทการเป็นมารดา ต่ำกว่าก่อนได้รับโปรแกรม อย่างมีนัยสำคัญทางสถิติที่ p < .01 และค่าเฉลี่ยของคะแนนภาวะซึมเศร้าของมารดาวัยรุ่นกลุ่มที่ได้รับโปรแกรมการส่งเสริมความมีคุณค่าในตนเองในการปฏิบัติบทบาทการเป็นมารดา ต่ำกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ

โรคติดเชื้อไวรัสโคโรนา 2019 (Coronavirus disease 2019 หรือ COVID-19) เป็นโรคติดเชื้อ ที่มีการแพร่ระบาดอยู่ในปัจจุบัน ทั่วโลกพบอุบัติการณ์การติดเชื้อและมีผู้เสียชีวิตจำนวนมาก เนื่องจากในระยะแรกผู้ติดเชื้อไม่แสดงอาการชัดเจนจึงไม่มารักษาที่โรงพยาบาล ทำให้มีการแพร่กระจายเชื้อได้ง่ายส่งผลให้อัตราการเสียชีวิตของผู้ป่วยเพิ่มมากขึ้น ดังนั้น พยาบาลซึ่งเป็นด่านหน้าในการดูแลผู้ป่วยควรมีความรู้ความเข้าใจเกี่ยวกับการคัดกรองโรค อาการแสดง การแพร่กระจายเชื้อ และบทบาทพยาบาลในระบบการแพทย์ทางไกล โดยใช้กระบวนการพยาบาลนำความรู้ภาคทฤษฎีและทักษะการพยาบาลมาประยุกต์ใช้ในสถานการณ์การแพร่ระบาดทั้งการเตรียมพร้อม การตอบสนอง และการปรับปรุงฟื้นฟูซ่อมแซม เพื่อประเมินอาการและช่วยป้องกันการแพร่กระจายเชื้อไปยังผู้รับบริการท่านอื่นและเจ้าหน้าที่ได้

รูปแบบการวิจัย: การวิจัยแบบผสานวิธี
วิธีดำเนินการวิจัย: ประชากรเชิงปริมาณ คือ เครือข่ายสุขภาพชุมชนทั้งหมด จำนวน 209 ราย และผู้ให้ข้อมูลเชิงคุณภาพ คือ การสนทนากลุ่มเครือข่ายสุขภาพชุมชนที่ผ่านการตอบแบบสอบถาม จำนวน 10 ราย โดยการสุ่มแบบเจาะจง เครื่องมือการวิจัย ได้แก่ แบบทดสอบความรู้และแบบสอบถาม มีค่าความเชื่อมั่น ระหว่าง 0.87-0.90 วิเคราะห์ข้อมูลด้วยค่าร้อยละ ค่าเฉลี่ย ค่าเบี่ยงเบนมาตรฐาน สหสัมพันธ์แบบเพียร์สัน และการวิเคราะห์เชิงเนื้อหา
ผลการวิจัย: 1) การวิจัยเชิงปริมาณ พบว่า ความรู้ และทัศนคติ มีความสัมพันธ์ระดับต่ำ และการจัดการปัญหาการดูแลผู้ป่วยมีความสัมพันธ์ระดับสูง กับพฤติกรรมการดูแลผู้ป่วยโรคความดันโลหิตสูง อย่างมีนัยสำคัญทางสถิติที่ระดับ 0.01 และ 2) การวิจัยเชิงคุณภาพ พบว่า เครือข่ายสุขภาพชุมชน มีความรู้ ความเข้าใจเกี่ยวกับโรคความดันโลหิตสูง แนวทางการส่งเสริมสุขภาพ การป้องกันภาวะแทรกซ้อน มีทัศนคติ ที่ดีในการดูแลผู้ป่วย มีทักษะการดูแลสุขภาพและการปฐมพยาบาลภาวะฉุกเฉินเบื้องต้น การติดต่อ ประสานงาน การมีส่วนร่วมออกแบบกิจกรรมดูแลสุขภาพ และการติดตามเยี่ยมบ้าน
สรุป: ความรู้ ทัศนคติ การจัดการปัญหาการดูแลผู้ป่วย มีความสัมพันธ์ทางบวกกับพฤติกรรมการดูแลผู้ป่วย ดังนั้นพื้นที่ควรพัฒนารูปแบบการดูแลผู้ป่วยเน้นการมีส่วนร่วมเชิงประจักษ์

วัตถุประสงค์: เพื่อศึกษาผลของโปรแกรมการให้ความรู้ต่อพฤติกรรมการป้องกันภาวะไตเสื่อมของแม่บ้าน ทำความสะอาด มหาวิทยาลัยอุบลราชธานี
รูปแบบการวิจัย: การวิจัยกึ่งทดลอง
วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ แม่บ้านทำความสะอาด มหาวิทยาลัยอุบลราชธานี จำนวน 60 ราย จัดกลุ่มตัวอย่างด้วยการจับคู่พิจารณาจากอายุและการมีโรคประจำตัว แบ่งเป็นกลุ่มควบคุม 30 ราย ได้รับการพยาบาลตามปกติ และกลุ่มทดลอง 30 ราย ได้รับโปรแกรมการให้ความรู้ ประกอบด้วย การให้ความรู้เรื่องโรคไตเรื้อรัง โมเดลไตจำลองป้องกันไตเสื่อม และคู่มือการดูแลตนเอง เพื่อป้องกันไตเสื่อม เก็บข้อมูลโดยใช้ แบบสอบถามพฤติกรรมการป้องกันภาวะไตเสื่อม โดยผ่านการตรวจสอบความตรงเชิงเนื้อหาจากผู้ทรงคุณวุฒิ 3 ท่าน มีค่าดัชนีความตรงตามเนื้อหาเท่ากับ .90 และค่าความเที่ยงสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ .84 วิเคราะห์ ข้อมูลโดยใช้สถิติบรรยาย และทดสอบค่าเฉลี่ยโดยใช้สถิติทดสอบที
ผลการศึกษา: คะแนนเฉลี่ยพฤติกรรมการป้องกันภาวะไตเสื่อมในกลุ่มทดลองที่ได้รับโปรแกรมการให้ความรู้สูงกว่า ก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติ (p<.05) และพบว่าคะแนนเฉลี่ยพฤติกรรมการป้องกันภาวะไตเสื่อม ในกลุ่มทดลองที่ได้รับโปรแกรมการให้ความรู้สูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p< .05)
สรุป: โปรแกรมการให้ความรู้ช่วยส่งเสริมให้แม่บ้านทำความสะอาด มหาวิทยาลัยอุบลราชธานี มีพฤติกรรมการป้องกันภาวะไตเสื่อมที่ดีขึ้น ควรนำโปรแกรมนี้ไปเผยแพร่กับบุคลากรกลุ่มอื่น และศึกษาวิจัยในบุคคลที่มีความเสี่ยงต่อการเกิดโรคไตเรื้อรังต่อไป

รูปแบบการวิจัย: การวิจัยแบบผสานวิธี
วิธีดำเนินการวิจัย: การวิจัยเชิงปริมาณกึ่งทดลอง จำนวน 30 คน และผู้ให้ข้อมูลเชิงคุณภาพ คือ เกษตรกรที่เข้าร่วมกิจกรรมโปรแกรม จำนวน 17 คน เครื่องมือการวิจัย ได้แก่ โปรแกรมความรอบรู้ด้านสุขภาพ แบบสอบถามความรอบรู้ด้านสุขภาพ และชุดตรวจเลือดโคลีนเอสเตอเรส มีค่าความเชื่อมั่นระหว่าง 0.82-0.96 การวิเคราะห์ข้อมูล ได้แก่ ค่าร้อยละ ค่าเฉลี่ย ค่าเบี่ยงเบนมาตรฐาน Pair t-test และการวิเคราะห์เชิงเนื้อหา
ผลการศึกษา: เกษตรกรมีคะแนนเฉลี่ยความรอบรู้ด้านสุขภาพ พฤติกรรมป้องกันสารเคมีกำจัดศัตรูพืช และค่าเฉลี่ยโคลีนเอสเตอเรสในเลือดดีกว่าก่อนการทดลอง อย่างมีนัยสำคัญทางสถิติ ที่ระดับ 0.01 โดยพบว่า เกษตรกรมีความรู้ สามารถค้นหา สอบถามผู้รู้ผ่านระบบออนไลน์ จัดการปัญหาและอุปสรรค เข้าถึงข้อมูลและบริการสุขภาพ และมีพฤติกรรมป้องกันสารเคมีกำจัดศัตรูพืชที่ถูกต้องและเหมาะสม ส่งผลให้ระดับเอนไซม์โคลีนเอสเตอเรสในเลือดลดลง
สรุป: ผลของโปรแกรมความรอบรู้ด้านสุขภาพลดระดับเอนไซม์โคลีนเอสเตอเรสในเลือด ดังนั้น ควรนำโปรแกรมไปประยุกต์ใช้กับเกษตรกรเพื่อลดหรือชะลอการเกิดภาวะแทรกซ้อน

วิธีดำเนินการ: กลุ่มตัวอย่างเชิงปริมาณ คือ สตรีวัยรุ่นตั้งครรภ์แรกจำนวน 124 คน โดยการเลือกแบบเจาะจง ข้อมูลเชิงคุณภาพ คือ สตรีวัยรุ่นตั้งครรภ์แรก 12 คนที่ผ่านการตอบแบบสอบถาม โดยการสัมภาษณ์เชิงลึก สุ่มแบบลูกโซ่ และการสนทนากลุ่ม 10 คน เลือกแบบเจาะจง เครื่องมือที่ใช้ในการวิจัย คือ แบบสอบถามการรับรู้ความเชื่อด้านสุขภาพ และแบบสอบถามพฤติกรรมส่งเสริมสุขภาพ ค่าความเชื่อมั่นระหว่าง 0.79-0.90 ตามลำดับ การวิเคราะห์ข้อมูล ได้แก่ ร้อยละ ค่าเฉลี่ย ค่าเบี่ยงเบนมาตรฐาน สหสัมพันธ์แบบเพียร์สัน และการวิเคราะห์เชิงเนื้อหา
ผลการวิจัย: 1) การวิจัยเชิงปริมาณ พบว่า การรับรู้ความเสี่ยงและความรุนแรง การรับรู้ประโยชน์ และการรับรู้ความสามารถ มีความสัมพันธ์เชิงบวกกับพฤติกรรมส่งเสริมสุขภาพ อย่างมีนัยสำคัญทางสถิติ ที่ระดับ 0.01, 2) การวิจัยเชิงคุณภาพ พบว่า สตรีวัยรุ่นตั้งครรภ์มีการรับรู้ความเสี่ยงและความรุนแรง จากพฤติกรรมสุขภาพไม่เหมาะสม การรับรู้ประโยชน์ จากพฤติกรรมการบริโภคอาหารที่ถูกต้อง การดูแลตนเอง การติดตามนัดหมาย การรับรู้ความสามารถ จากการเห็นคุณค่าตนเอง การดูแลตนเอง และการแก้ไขปัญหาสุขภาพ
สรุป: ความเชื่อด้านสุขภาพมีความสัมพันธ์กับพฤติกรรมส่งเสริมสุขภาพ ดังนั้น บุคลากรทางการแพทย์ควรพัฒนาโปรแกรมการดูแลตนเองของสตรีวัยรุ่นตั้งครรภ์แรก

During the Colonial period, hair was one of the designated phenotypic markers of racial group membership used to assign or deny privilege. Privileges are still assigned to people based on their racial group membership and their proximity to Whiteness. Context:
Chronic ankle instability (CAI) is associated with residual instability, pain, decreased function, and increased disablement. Injury-related fear has been associated with CAI, although its relationship to other impairments is unclear. The Fear-Avoidance Model is a theoretical framework hypothesizing a relationship between injury-related fear, chronic pain, pain catastrophizing, and disability. It has been useful in understanding fear's influence in other musculoskeletal conditions but has yet to be studied in those with CAI.
Objective:
To explore relationships between instability, pain catastrophizing, injury-related fear, pain, ankle function, and global disability in individuals with CAI.

Context: To enhance the quality of patient care, athletic training students (ATSs) should experience a wide variety of clinical practice settings, interact with diverse patient populations, and engage with patients that have a wide variety of conditions. It is unclear in what ways, if any, ATSs have diverse opportunities during clinical experiences.

Context: To enhance the quality of patient care, it is important that athletic trainers integrate the components of the core competencies (CCs; evidence-based practice [EBP], patient-centered care [PCC], health information technology [HIT], interprofessional education and collaborative practice [IPECP], quality improvement [QI], professionalism) as a part of routine clinical practice. In what ways, if any, athletic training students (ATSs) are currently integrating CCs into patient encounters (PEs) during clinical experiences is unclear.

Context: Preventive training programs (PTPs) reduce injury risk by improving movement control. Corrective feedback is important; however, many cues at once may be too complicated for athletes.
Objective: To compare movement control and long-jump (LJ) changes in youth athletes participating in a season-long PTP, with simplified feedback, traditional feedback, or a warmup of the coaches' choosing.
Design: Cluster-randomized controlled trial.
Setting: Soccer fields.

Objective: To conduct a systematic review of existing literature on cross-education balance effects after unilateral training in the population with chronic ankle instability (CAI).
Data Sources: PubMed, SPORTDiscus, CINAHL Plus.
Study Selection: To be included in the systematic review, studies were required to have been published in English, included participants with CAI, had participants undergo a unilateral therapeutic exercise for the lower extremity, and measured balance performance of the untrained lower extremity before and after the intervention.
Data Extraction: The certainty of evidence in each included study was assessed via the Downs and Black checklist. A score of 24 to …
