Frequency Of Limited English Proficient (Lep) Patient Visits And Language Resources: An Assessment Of Rural Oregon's Health Centers And Working With Lep Communities, Gabriel I. Lehrburger
University Honors Theses
Persons with limited English proficiency (LEP) can face barriers when seeking health care. Rural communities often have reduced access to some health resources. However, little is known about how rural communities meet the language needs of their LEP populations. A cross-sectional survey and interview were designed and sent to managerial staff in rural health centers around the state of Oregon. During the months of January – March 2020, survey and interview data were collected to assess the relationship between frequency of LEP patient visits and available language resources in rural Oregon. The study also investigated staff perceptions when working with …

Expansion Of Medication Therapy Management Services To Rural Sites For Patients With An Employer-Sponsored Health Plan, Taylor Thooft Pharmd
Theses and Dissertations
Previous literature supports that pharmaceutical care provider in the ambulatory care setting can improve clinical, economic, and humanistic outcomes for recipients, and when utilized in employer-provided health plans can lead to reduced spending on healthcare costs by employers. A large health system in central Minnesota offers an Employee Medication Therapy Management (MTM) benefit that offers further financial incentives to meet with an MTM Pharmacist for a comprehensive review of one’s medications. Currently, this program is underutilized due to both lack of awareness of the program and geographic for employees in the wester region of the health system. This project sought …

The goal of this experiential report is to outline the adoption of a family-centered Kids Don’t Float approach. We conducted a critical synthesis of information to reflect the expansion of the Kids Don’t Float program into a more family-centered approach. The critical synthesis provided insights into why we should adopt this approach, how it was implemented, and how it influenced drowning incidents compared to the previously used child-centered approach. The adoption of a family-centered approach may contribute to reducing drowning incidents by targeting parents, providing safety information to families, and promoting parental modelling of life jackets. Program evaluators and water …

Innate immune activation is a major contributor to Alzheimer’s Disease (AD) pathophysiology, although the mechanisms involved are poorly understood. Chemokine C-C motif ligand (CCL) 2 is produced by neurons and glial cells and is upregulated in the AD brain. Transgene expression of CCL2 in mouse models of amyloidosis produces microglia-induced amyloid β oligomerization, a strong indication of the role of these activation pathways in the amyloidogenic processes of AD. We have previously shown that CCL2 polarizes microglia in wild type mice. However, how CCL2 signaling contributes to tau pathogenesis remains unknown. To address this question, CCL2 was delivered via recombinant …

To date current therapies of glioblastoma multiforme (GBM) are largely ineffective. The induction of apoptosis by an unresolvable unfolded protein response (UPR) represents a potential new therapeutic strategy. Here we tested 12ADT, a sarcoendoplasmic reticulum Ca2+ ATPase (SERCA) inhibitor, on a panel of unselected patient-derived neurosphere-forming cells and found that GBM cells can be distinguished into "responder" and "non-responder". By RNASeq analysis we found that the non-responder phenotype is significantly linked with the expression of UPR genes, and in particular ERN1 (IRE1) and ATF4. We also identified two additional genes selectively overexpressed among non-responders, IGFBP3 and IGFBP5. CRISPR-mediated deletion of …
Increasing Access To Medicare Annual Wellness Visits In Primary Care; Utilizing Registered Nurses As Part Of An Interdisciplinary Team Based Approach, Iyo Kubota
Doctor of Nursing Practice (DNP) Projects
Background: In order to provide evidence-based preventive care to the aging population, Medicare began providing free annual wellness visits (AWVs) to its beneficiaries in 2011. The AWVs are a great opportunity for primary care providers to develop personalized preventive care as well as health promotion. However, many primary care practices especially those with limited resources struggle to incorporate AWVs into their busy primary care practice. Centers for Medicare and Medicaid Services allows other providers than physicians, NPs and PAs to perform those visits including registered nurses, health educators, and registered dietitians.
Methods: A pilot project was conducted with 12 patients …

The Impact Of An Opioid Use Disorder Immersive Education Experience On Preprofessional Health Care Students’ Development Of Hope And Hopeful Communication (Dissertation), Kerri Green Ms, Edd
Department of Education
Currently, the prescription opioid and heroin overdose epidemic is the worst public health crisis in Pennsylvania and addiction problems within the Lehigh Valley far outpace the number of health professionals trained to provide care in the field of Addiction Medicine. Additionally, negative attitudes towards individuals with substance use disorder are prevalent among health care providers. Unfortunately, this trend may continue as preprofessional health care curricula across many disciplines still lack the social, emotional, and behavioral competencies to treat patients with substance use disorder. Lehigh Valley Health Network has been an integral community-based partner in addressing the multi-faceted disease of Opioid …

Structural brain changes in aging are known to occur even in the absence of dementia, but the magnitudes and regions involved vary between studies. To further characterize these changes, we analyzed paired MRI images acquired with identical protocols and scanner over a median 5.8-year interval. The normal study group comprised 78 elders (25M 53F, baseline age range 70-78 years) who underwent an annual standardized expert assessment of cognition and health and who maintained normal cognition for the duration of the study. We found a longitudinal grey matter (GM) loss rate of 2.56 ± 0.07 ml/year (0.20 ± 0.04%/year) and a …

Drowning continues to be a major cause of death for children, especially among minority groups. USA Swimming commissioned studies in 2008 and 2010 to look at swimming ability or inability, an associated drowning variable. The 2010 study showed alarming high percentages regarding insufficient swimming skill for many demographic groups. The current study’s purpose was to provide further analysis of variables which emerged from 2010 study and how these variables are affecting all US populations. Focus group interviews accomplished in three regionally diverse US cities asked participants (3=fathers; 12=mothers) about their personal history with swimming, perceptions concerning swimming in general, and …

Even with an efficient combination of antiretroviral therapy (ART), which significantly decreases viral load in human immunodeficiency virus type 1 (HIV-1)-positive individuals, the occurrence of HIV-1-associated neurocognitive disorders (HAND) still exists. Microglia have been shown to have a significant role in HIV-1 replication in the brain and in subsequent HAND pathogenesis. However, due to the limited ability of ART drugs to cross the blood–brain barrier (BBB) after systemic administration, in addition to efflux transporter expression on microglia, the efficacy of ART drugs for viral suppression in microglia is suboptimal. Previously, we developed novel poly (lactic-co-glycolic acid) (PLGA)-based elvitegravir nanoparticles (PLGA-EVG …

Designing Peptides To Bind Kappa B Dna And Mimic Nf-Kappa B Protein Complexes, Nicholas A. Markowich
Designing Peptides To Bind Kappa B Dna And Mimic Nf-Kappa B Protein Complexes, Nicholas A. Markowich
Honors College Theses
Transcription factors are important proteins that regulate gene expression and protein synthesis. Transcription factors can either boost the gene’s transcription rate by helping RNA polymerase activate transcription or restrict it by interfering with RNA polymerase, thereby repressing transcription. Nuclear factor-kappa-B (NF-κB) transcription factors are a family of proteins that control the synthesis of proteins involved in many cellular processes such as inflammatory and immune responses, cell growth, and apoptosis. However, the overexpression and activation of these transcription factors is linked to deadly conditions such as cancer and neurodegenerative diseases which currently have few safe cures. The goal of this research …

Department of Urology Faculty Papers
Urologic and gynecologic surgeons are the top utilizers of robotic surgery; however, non-obstetrical robotic-assisted laparoscopic surgery (RALS) in pregnant patients is infrequent. A systematic literature review was performed to ascertain the frequency, indication and complications of RALS in pregnancy. Results showed thirty-eight pregnancies from eleven publications between 2008-2020. Five cases were for urologic indication and thirty-three for gynecologic indication. Minimal surgical alterations were required. Although no adverse maternal-fetal outcomes were reported, there are not enough cases published to determine safety. This review demonstrates the feasibility of RALS for the pregnant population in the hands of competent robotic surgeons.

Master of Occupational Therapy Student Critically Appraised Topics
This project aims to appraise evidence of the effectiveness of infant massage for increasing social-emotional bonding. In our research, we focused on two studies considered Level I evidence and three studies considered Level II evidence. Study designs include quasi-experimental and randomized controlled trials. Strong evidence supports that infant massage increases social-emotional bonding and attachment. Based on the evidence, implementation varied from once per day for 10 minutes to 2-3 times per day for 15 minutes. It is recommended to use infant massage in an acute care setting and in the home to increase social-emotional bonding and attachment.

This study reported the efficacy of short-course corticosteroids and convalescent plasma (CP) transfusion in treating five patients with severe COVID-19 disease. Five adults (mean±standard deviation age: 70±9 years) with laboratory-confirmed COVID-19 and severe hypoxemia (PaO2/FiO2 [P/F]<100) requiring invasive mechanical ventilation received five days methylprednisolone (40 mg intravenously every 12 hours) and subsequent CP transfusion (250–400 ml). Compared with the baseline, P/F ratios increased by 46% and by 28% after short course corticosteroids and CP transfusion, respectively. Four patients survived. Short-course corticosteroids and CP transfusion may improve hypoxemia in patients with severe COVID-19 disease.
