Diabetic retinopathy (DR) is a prevalent cause of vision impairment and blindness among adults in the United States. Early diagnosis of DR through dilated eye examinations can reduce the risk of vision impairment or loss. Background: Tono-Pen® is a regularly used device. A tip cover is needed to prevent damage to the transducer tip and patient-to-patient contamination. Ocufilm® (OF), latex disposable tip cover, is costly and able to cause allergic reactions. Objective: To evaluate repeatability, reproducibility, and agreement of intraocular pressure measurement with Tono-Pen® using OF and polyethylene wrap tip cover (PE) in human eyes. Cepharanthine (CEP) is a biscoclaurine alkaloid, isolated from the roots of Stephania cepharanthine Hayata that possesses potent anticancer properties against several types of cancer such as hepatocellular carcinoma, colorectal cancer, leukemia and ovarian cancer. However, there is still limited information regarding to its effects on non-small cell lung cancer cells. Menthol is widely used in tobacco products. This study compared the effects of menthol on human bronchial epithelium using submerged cultures, a VITROCELL® cloud chamber that provides air liquid interface (ALI) exposure without solvents or heating, and a Cultex ALI system that delivers aerosol equivalent to that inhaled during vaping. In submerged culture, menthol significantly increased calcium influx and mitochondrial reactive oxygen species (ROS) via the TRPM8 receptor, responses that were inhibited by a TRPM8 antagonist. การศึกษานี้เป็นการศึกษาทางคลินิกแบบแบ่งส่วนช่องปากซึ่งมีการจัดกลุ่มด้วยวิธีสุ่มและมีกลุ่มควบคุม มีวัตถุประสงค์เพื่อเปรียบเทียบการลุกลามของรอยผุด้านประชิดระยะแรกเริ่มในฟันหลังแท้ที่ระยะเวลา 6 เดือนภายหลังการทาฟลูออไรด์วาร์นิชและคาวิตี้วาร์นิช ในอาสาสมัครจำนวน 30 คนที่มีรอยผุด้านประชิดระยะแรกเริ่มที่ระดับความลึกไม่เกินรอยต่อเนื้อฟัน-เคลือบฟันอย่างน้อย 1 คู่ที่อยู่คนละจตุภาค ซึ่งไม่เป็นรูทางคลินิกและมีการดำเนินโรค อาสาสมัครได้รับการถ่ายภาพรังสีดิจิทัลชนิดกัดปีกที่ระยะเริ่มต้น รอยผุดังกล่าวได้รับการจัดกลุ่มโดยการสุ่มเพื่อทาฟลูออไรด์วาร์นิช หรือคาวิตี้วาร์นิช (กลุ่มควบคุม) อาสาสมัครได้รับการถ่ายภาพรังสีดิจิทัลชนิดกัดปีกอีกครั้งที่ระยะ 6 เดือน เพื่อติดตามการลุกลามของรอยผุ โดยใช้วิธีการวัดผล 2 วิธี ได้แก่ การวัดความหนาแน่นแร่ธาตุของรอยผุจากภาพรังสีซ้อนทับชนิดดิจิทัลระหว่างภาพรังสีที่ระยะเริ่มต้นกับที่ระยะ 6 เดือน และการวัดความลึกรอยผุโดยตรงจากภาพรังสีดิจิทัลชนิดกัดปีกที่ระยะเริ่มต้นและที่ระยะ 6 เดือนด้วยโปรแกรม Image-Pro Plus พบว่าค่าเฉลี่ยความหนาแน่นแร่ธาตุระหว่างรอยผุด้านประชิดระยะแรกเริ่มกลุ่มที่ได้รับการทาฟลูออไรด์วาร์นิช (125.38 ± 6.9) และคาวิตี้วาร์นิช (132.63 ± 8.59) มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p=0.001) เมื่อใช้สถิติ paired-T test ตรงข้ามกับค่าเฉลี่ยผลต่างความลึกรอยผุกลุ่มที่ได้รับการทาฟลูออไรด์วาร์นิช (-0.0078 ± 0.13) และคาวิตี้วาร์นิช (-0.0227 ± 0.11) ที่ไม่พบว่ามีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p=0.525) เมื่อใช้สถิติ paired-T test ดังนั้นจึงยังไม่สามารถสรุปผลของฟลูออไรด์วาร์นิชต่อการลุกลามของรอยผุด้านประชิดระยะแรกเริ่มในฟันหลังแท้ได้ และควรมีการศึกษาเพิ่มเติมในระยะยาวต่อไป

วัตถุประสงค์ เพื่อศึกษาปริมาตรของสมองที่ขาดเลือดโดยเอกซเรย์คอมพิวเตอร์ด้วยโปรแกรมอัตโนมัติสามารถใช้ทำนายการเกิดเลือดออกในสมองร่วมกับอาการทางระบบประสาทที่เลวลงหลังให้ยาละลายลิ่มเลือดทางหลอดเลือดดำในผู้ป่วยโรคสมองขาดเลือดเฉียบพลันภายใน 7 วันได้หรือไม่ วิธีการวิจัย การศึกษาย้อนหลังในผู้ป่วยโรคสมองขาดเลือดเฉียบพลันในโรงพยาบาลจุฬาลงกรณ์ตั้งแต่ เดือนมกราคม พ.ศ.2561 ถึงเดือนมิถุนายน พ.ศ.2563 ที่ได้รับการรักษาด้วยยาละลายลิ่มเลือดทางหลอดเลือดดำและได้รับการตรวจเอกซเรย์คอมพิวเตอร์สมอง perfusion CT ภายหลังการได้รับยาละลายลิ่มเลือดทางหลอดเลือดดำและติดตามเอกซเรย์คอมพิวเตอร์สมองภายหลังการได้รับยาละลายลิ่มเลือดทางหลอดเลือดดำภายใน 36 ชั่วโมง ผลการศึกษา มีผู้ป่วยโรคสมองขาดเลือดเฉียบพลันที่ได้รับการรักษาด้วยยาละลายลิ่มเลือดทางหลอดเลือดดำและมีเลือดออกในสมองจำนวน 33 ราย (19.07%) โดยมีผู้ป่วยที่มีเลือดออกในสมองแต่ไม่มีอาการ 28 ราย (16.2%) และ ผู้ป่วยที่มีเลือดออกในสมองที่มีอาการทางระบบประสาทที่เลวลง 5 ราย (2.9%) โดยปริมาตรสมองที่ขาดเลือดที่ใช้ทำนายการเกิดเลือดออกในสมองคือ มากกว่าหรือเท่ากับ 3.5 มิลลิลิตร มีความไว 75.8% และความจำเพาะ 82.1% สรุปการศึกษา ปริมาตรของสมองที่ขาดเลือดโดยเอกซเรย์คอมพิวเตอร์ด้วยโปรแกรมอัตโนมัติสามารถทำนายการเกิดเลือดออกในสมองได้ แต่ไม่สามารถทำนายการเกิดเลือดออกในสมองที่มีอาการทางระบบประสาทที่เลวลงได้

ที่มา: การไหลของอากาศถูกจำกัดในช่วงหายใจออก คือภาวะที่ผู้ป่วยไม่สามารถเพิ่มอัตราการไหลของอากาศช่วงหายใจออกได้ แม้จะมีความแตกต่างของระดับความดันภายในหลอดลม ซึ่งสามารถพบได้ในผู้ป่วยที่มีภาวะการหายใจล้มเหลวหลังการหย่าเครื่องช่วยหายใจได้ถึง 60% อย่างไรก็ตาม ปัจจุบันยังไม่มีข้อแนะนำหรือแนวทางการประเมินเพื่อหย่าเครื่องช่วยหายใจในผู้ป่วยกลุ่มนี้อย่างชัดเจน วัตถุประสงค์ของการศึกษา: เพื่อเปรียบเทียบอัตราการถอดท่อช่วยหายใจได้สำเร็จ และสามารถถอดท่อช่วยหายใจได้เป็นเวลาอย่างน้อย 72ชั่วโมง ในผู้ป่วยที่มีการไหลของอากาศถูกจำกัดในช่วงหายใจออก ระหว่างวิธีการทดสอบการหายใจด้วยแรงดันบวก (SBT with Pressure Support Ventilation) และวิธีการทดสอบการหายใจด้วยการให้ออกซิเจนผ่านท่อวงจรรูปตัวที (SBT with T-piece) วิธีศึกษา: การศึกษานี้เป็นการศึกษาแบบสุ่มที่มีกลุ่มควบคุม และไม่ด้อยกว่า โดยศึกษาในผู้ป่วยที่มีภาวะการหายใจล้มเหลวซึ่งได้รับการช่วยหายใจผ่านทางท่อช่วยหายใจเป็นเวลาอย่างน้อย 24 ชั่วโมง และเข้าได้กับเกณฑ์ความพร้อมเพื่อการหย่าเครื่องช่วยหายใจ ทำการสุ่มด้วยระบบคอมพิวเตอร์เพื่อแบ่งผู้ป่วยออกเป็น 2 กลุ่ม ได้แก่ กลุ่มที่ได้การทดสอบการหายใจด้วยแรงดันบวกในระยะเวลา 30 นาที และกลุ่มที่ได้การทดสอบการหายใจด้วยออกซิเจนผ่านท่อวงจรรูปตัวทีในระยะเวลา 30 นาที วัตถุประสงค์หลักของการศึกษานี้ คืออัตราการถอดท่อช่วยหายใจได้สำเร็จ และสามารถถอดท่อช่วยหายใจได้เป็นเวลาอย่างน้อย 72 ชั่วโมง และวัตถุประสงค์รองของการศึกษา ได้แก่ อัตราของภาวะการหายใจล้มเหลวหลังการถอดท่อช่วยหายใจในช่วง 7 วัน, ระยะเวลาที่ต้องมีการใส่ท่อช่วยหายใจซ้ำหลังการหย่าเครื่องช่วยหายใจและถอดท่อช่วยหายใจ, อัตราการผ่านขั้นตอนการทดสอบการหายใจ เป็นต้น ผลการศึกษา: ผู้ป่วยที่เข้าสู่การศึกษาทั้งสิ้น 99 ราย เป็นเพศชาย 50.5% มีค่ามัธยฐานของอายุที่ 70 [23] ปี และมีสาเหตุที่นำมาสู่ภาวะการหายใจล้มเหลวจากระบบทางเดินหายใจเป็นส่วนใหญ่ (68.7%) เมื่อวิเคราะห์ข้อมูลทางสถิติแล้ว พบว่าอัตราการหย่าเครื่องช่วยหายใจและถอดท่อช่วยหายใจได้สำเร็จเป็นเวลาอย่างน้อย 72 ชั่วโมงของกลุ่มที่ได้การทดสอบการหายใจด้วยแรงดันบวก ไม่ด้อยกว่ากลุ่มที่ได้การทดสอบการหายใจด้วยออกซิเจนผ่านท่อวงจรรูปตัวที คิดเป็น 82% และ 81.63% ตามลำดับ (95%CI -0.148 to 0.156, p=0.0475) รวมถึงอัตราการผ่านขั้นตอนการทดสอบการหายใจ (96% และ 93.9% ตามลำดับ; 95%CI -0.065 to 0.108, p<0.001) พบว่าไม่ด้อยกว่าด้วยเช่นกัน ส่วนอัตราของภาวะการหายใจล้มเหลวหลังการถอดท่อช่วยหายใจในช่วง 7 วันของกลุ่มที่ได้การทดสอบการหายใจด้วยแรงดันบวก และกลุ่มที่ได้การทดสอบการหายใจด้วยออกซิเจนผ่านท่อวงจรรูปตัวที พบว่าไม่สามารถสรุปผลได้ (22.9% และ 15.2% ตามลำดับ; 95%CI -0.081 to 0.235, p=0.369) นอกจากนี้ยังไม่มีความแตกต่างทางสถิติของระยะเวลาที่ต้องมีการใส่ท่อช่วยหายใจซ้ำหลังการหย่าเครื่องช่วยหายใจและถอดท่อช่วยหายใจอีกด้วย (55 [95.5] ชั่วโมง และ 25.33 [48] ชั่วโมงตามลำดับ, p=0.683) สรุป: อัตราการหย่าเครื่องช่วยหายใจและถอดท่อช่วยหายใจได้สำเร็จ ด้วยวิธีการทดสอบการหายใจด้วยแรงดันบวก ไม่ด้อยกว่าการทดสอบการหายใจด้วยออกซิเจนผ่านท่อวงจรรูปตัวทีในผู้ป่วยที่มีการไหลของอากาศถูกจำกัดในช่วงหายใจออก และการศึกษานี้ยังเป็นการศึกษาแรกที่แสดงถึงความไม่แตกต่างของผลการทดสอบการหายใจระหว่าง 2 วิธีดังกล่าว ในแง่ของอัตราการหย่าเครื่องช่วยหายใจและถอดท่อช่วยหายใจสำเร็จในผู้ป่วยที่มีภาวะการไหลของอากาศถูกจำกัดในช่วงหายใจออกนี้

วัตถุประสงค์ เพื่อศึกษาการเปลี่ยนแปลงองค์ประกอบของร่างกายในผู้ป่วยโรคอ้วนหลังการผ่าตัดกระเพาะเพื่อลดน้ำหนัก ที่ระยะเวลา 12 เดือน และเปรียบเทียบกับกลุ่มผู้ที่ไม่เคยได้รับการผ่าตัดกระเพาะเพื่อลดน้ำหนักมาก่อน ที่มี อายุ เพศ และค่าดัชนีมวลกายใกล้เคียงกัน วิธีการวิจัย เป็นการวิจัยโดยการสังเกตเชิงวิเคราะห์โดยใช้ข้อมูลผู้ป่วยโรคอ้วนที่ได้รับการผ่าตัดกระเพาะเพื่อลดน้ำหนักด้วยวิธี Laparoscopic Roux-en-Y gastric bypass(LRYGB) หรือ Laparoscopic sleeve gastrectomy(LSG) ในโรงพยาบาลจุฬาลงกรณ์ ช่วงปี พ.ศ.2558-2562 ที่หลังผ่าตัดที่ 12 เดือน มีค่าดัชนีมวลกาย น้อยกว่า 30 กิโลกรัม/เมตร2 กลุ่มควบคุมคือกลุ่มผู้ที่ไม่เคยได้รับการผ่าตัดกระเพาะที่มี อายุ เพศ และค่าดัชนีมวลกายใกล้เคียงกัน และไม่มีโรคประจำตัว วิเคราะห์องค์ประกอบของร่างกายด้วยเครื่อง Inbody 770 ที่คลินิกอายุรกรรมโรคอ้วน โรงพยาบาลจุฬาลงกรณ์ ผลการศึกษา ผู้เข้าร่วมวิจัยทั้งหมด 60 คน แบ่งเป็นกลุ่มผู้ป่วยโรคอ้วนที่ได้รับการผ่าตัดกระเพาะเพื่อลดน้ำหนัก 30 คน กลุ่มควบคุม 30 คน จากการศึกษาพบว่าเมื่อเปรียบเทียบกับกลุ่มควบคุม กลุ่มผู้ป่วยมีค่าสัดส่วนเอวต่อสะโพก (waist-hip ratio)(0.83 และ 0.9, P-value <0.001) ร้อยละของไขมันในร่างกาย (percentage of body fat)(ร้อยละ 30.6 และ ร้อยละ 35.9, P-value 0.001) มวลไขมันบริเวณลำตัว (trunk fat mass)(10.3 กิโลกรัม และ 12.4 กิโลกรัม , P-value 0.04) และ มวลรวมกล้ามเนื้อและอวัยวะบริเวณแขนขา (appendicular lean mass)(9 กิโลกรัม และ 16.9 กิโลกรัม, P-value <0.001) น้อยกว่ากลุ่มควบคุม โดยมีค่ามวลรวมกล้ามเนื้อและอวัยวะทั้งร่างกาย (soft lean mass)(47.7 กิโลกรัม และ 39.9 กิโลกรัม, P-value 0.001) มวลรวมกล้ามเนื้อและอวัยวะบริเวณลำตัว (trunk lean mass)(21.2 กิโลกรัม และ 19 กิโลกรัม, P-value 0.02) มวลกล้ามเนื้อลาย (skeletal muscle mass)(27.5 กิโลกรัม และ 23 กิโลกรัม, P-value 0.003) และ มวลกายไร้ไขมัน (fat free mass)(51.1 กิโลกรัม และ 42.3 กิโลกรัม, P-value 0.001) มากกว่ากลุ่มควบคุมอย่างมีนัยสำคัญ และพบว่าหลังผ่าตัดกระเพาะเพื่อลดน้ำหนัก ที่ระยะเวลา 6 เดือน และ 12 เดือน ค่าตัวแปรองค์ประกอบของร่างกายลดลงทั้งหมด โดยเห็นผลลดลงมากสุดที่ระยะเวลา 12 เดือนหลังเข้ารับผ่าตัด สรุปการศึกษา แสดงให้เห็นว่า ในกลุ่มผู้ป่วยโรคอ้วนที่ได้รับการผ่าตัดกระเพาะเพื่อลดน้ำหนัก แม้ค่าตัวแปรองค์ประกอบของร่างกาย เช่น มวลไขมัน มวลกายไร้ไขมัน มวลกล้ามเนื้อ ลดลงทั้งหมดในช่วงตลอดระยะเวลา12 เดือน หลังเข้ารับการผ่าตัด แต่ มวลกล้ามเนื้อ และมวลกายไร้ไขมันในกลุ่มผู้ป่วยนั้น ยังคงสูงกว่าในกลุ่มควบคุมที่อายุ เพศ และค่าดัชนีมวลกายใกล้เคียงกัน

การวิจัยกึ่งทดลองมีวัตถุประสงค์เพื่อศึกษาผลของการพยาบาลตามแนวคิดของนิวแมนต่อพฤติกรรมป้องกันการติดเชื้อของ มารดาเด็กโรคมะเร็งเม็ดเลือดขาวที่ได้รับยาเคมี กลุ่มตัวอย่าง คือ มารดาของผู้ป่วยเด็กโรคมะเร็งเม็ดเลือดขาว ที่ได้รับการรักษาด้วยยาเคมีบำบัด ในโรงพยาบาลรามาธิบดี คัดเลือกกลุ่มตัวอย่างแบบเจาะจง (purposive samping) จำนวน 30 คน แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลองกลุ่มละ 15 คน กลุ่มทดลองได้รับการพยาบาลตามแนวคิดของนิวแมน เพื่อป้องกันการติดเชื้อภายหลังได้รับยาเคมีบำบัด เก็บรวบรวมข้อมูลโดยใช้ แบบสอบถามพฤติกรรมป้องกันการติดเชื้อของมารดาเด็กโรคมะเร็งเม็ดเลือดขาว ที่ได้รับยาเคมีบำบัด มีค่าความตรงตามเนื้อหา 1.0 และค่าสัมประสิทธิ์ความเที่ยงของเครื่องมือ 0.82 วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนาและสถิติ Mann-Whitney U ผลการวิจัยพบว่า มารดาผู้ป่วยเด็กโรคมะเร็งเม็ดเลือดขาว ที่ได้รับการรักษาด้วยยาเคมีบำบัด กลุ่มที่ได้รับการพยาบาลตามแนวคิดของนิวแมน มีพฤติกรรมป้องกันการติดเชื้อดีกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05

The World Health Report 2010 encourages countries to reduce wastage and increase efficiency to achieve Universal Health Coverage (UHC). This research examines the efficiency of divisions (sub-provincial geographic units) in Pakistan in moving towards UHC using Data Envelop Analysis. We have used data from the Pakistan National Accounts 2011-12 and the Pakistan Social Living and Measurement Survey 2012-13 to measure per capita pooled public health spending in the divisions as inputs, and a set of UHC indicators (health service coverage and financial protection) as outputs. Phenylketonuria (PKU) is a rare genetic condition affecting the body’s ability to metabolise the amino acid phenylalanine (PHE). A build-up of PHE in the blood and brain can result in irreversible intellectual disability and developmental delays. Ireland has one of the highest prevalence rates of PKU in Europe and some of the earliest treated patients with PKU in the world. However, little is known about their lived experience with PKU. The relationship between perspective taking abilities, political centrism, and outgroup derogation is not defined at this time. While previous research has demonstrated a negative relationship between political centrism and outgroup derogation (Van Prooijen, Krouwel, Boiten, & Eendebak, 2015), the relationship between the other variables is unclear. Therefore, the current study sought to measure the relationship between (1) perspective taking abilities and political centrism, (2) perspective taking and outgroup derogation, (3) political centrism and outgroup derogation and lastly, (4) whether the relationship between perspective taking abilities and political outgroup derogation would be mediated by an individual's level of political centrism.

Concussions represent a serious public health concern due to their severe short and long-term consequences. Many adolescents participate in sports that involve the chance of injuries such as concussions. Concussive injuries can be difficult to describe because most symptoms involve non-physical indicators such as impaired cognition, altered behavior, or sleep disturbances. Symptoms can also be influenced by age, gender, and previous history of concussions. Purpose: The purpose of this study is to assess the relationship between principal priorities, written wellness policies, and school wellness policy implementation. Methods: Principal priorities of nutrition and physical activity, written wellness policy quality and degree of policy implementation were assessed in 95 schools from eight states using the principal priorities questionnaire, the Wellness School Assessment Tool (WellSAT) version 2.0, and the Wellness School Assessment Tool for Implementation (WellSAT-I), respectively. The function and advantages of maintaining such massive copies of transposable elements in the human genome are still unclear. Setting: Six rural South Dakota wellness coalitions.

OBJECTIVE: It remains unstudied whether poor sleep is involved in the etiology of gastrointestinal (GI) problems in athletes.
METHODS: Eighty-seven running and triathlon/duathlon race (>60 minutes) participants completed questionnaires to quantify the Sleep Problems Index-(SPI)-I and sleep parameters from the night before races. For GI symptoms, participants reported the severity (0-10 scale) of four upper and three lower symptoms during races. Spearman's correlations examined whether sleep measures were associated with in-race GI symptoms. Partial correlations were calculated to control for age, resting GI symptoms, and anxiety.
RESULTS: SPI-I scores correlated with in-race upper GI symptoms (rho=0.26, p=0.013). Controlling for …

The purpose of this study was to examine physical education (PE) experiences from the perspective of an adult with Bardet-Biedl syndrome. An interpretative phenomenological analysis (IPA) research approach was used, and one male (age 32) with Bardet-Biedl syndrome acted as the participant. Sources of data included a semi-structured, audiotaped telephone interview and reflective interview notes. The interview was transcribed verbatim and data were analyzed thematically using a two-step approach informed by the IPA.
