Anesthesia For The Patient With A Recently Diagnosed Concussion: Think About The Brain!, Mohammed R. Rasouli, Michelle Kavin, Stephen Stache, Michael E. Mahla, Eric S. Schwenk
Department of Anesthesiology Faculty Papers
Some patients require emergent, urgent, or elective surgery in the time period immediately following diagnosis of concussion. However, changes in brain homeostatic mechanisms following a concussion and concern for secondary brain injury can complicate the decision as to whether or not a surgery should proceed or be postponed. Given the paucity of available evidence, further evaluation of the use of anesthesia in a patient with concussion is warranted. This article summarizes what is currently known about the relevant pathophysiology of concussion, intraoperative anesthesia considerations, and effects of anesthesia on concussion outcomes in an attempt to help providers understand the risks …
Urinary Bladder Metastasis From Breast Cancer: A Rare Cause Of Hematuria, Samuel Gitau, Allan Njau, Sitna Mwanzi
Urinary Bladder Metastasis From Breast Cancer: A Rare Cause Of Hematuria, Samuel Gitau, Allan Njau, Sitna Mwanzi
Imaging & Diagnostic Radiology, East Africa
Breast cancer is the most common cancer in women globally as well as in Kenya. The most common sites of metastases reported include the bones, liver and lung. Metastasis to the urinary bladder is relatively uncommon with only a few case reports in literature. It can therefore be easily overlooked as a cause of hematuria in these patients. We describe a rare case of a patient with breast cancer who presented with urinary bladder metastasis as a late complication of her illness.

Leveraging Eelectronic Health Records To Develop Cardiovascular Risk Profiles For Primary Prevention With Youth Living With Hiv In Multidisciplinary Adolescent Hiv Care Settings, Sitaji Gurung
Dissertations and Theses
Background: HIV infection has shown to be associated with increased risk for, and earlier occurrence of cardiovascular disease (CVD). Emerging evidence suggests that detectable viral load (VL) and CD4 ≤ 200 contribute substantially to increased risk of CVD. Cigarette smoking is prevalent in at least 40% of those with HIV, and significantly more common compared to the general population. As many as 33% of adolescents are overweight, and this excess weight leads to lipid abnormalities, and increases in metabolic syndrome. With the newer formulation of antiretroviral therapy (ART) agents available as a component of single tablet regimens (STR), it is …

Dendritic cells are an important link between innate and adaptive immune response. The role of dendritic cells in bone homeostasis, however, is not understood. Osteoporosis medications that inhibit osteoclasts have been associated with osteonecrosis, a condition limited to the jawbone, thus called medication-related osteonecrosis of the jaw. We propose that disruption of the local immune response renders the oral microenvironment conducive to osteonecrosis. We tested whether zoledronate (Zol) treatment impaired dendritic cell (DC) functions and increased bacterial load in alveolar bone in vivo and whether DC inhibition alone predisposed the animals to osteonecrosis. We also analyzed the role of Zol …

United States Department of Agriculture Wildlife Services: Staff Publications
Impacts of large-scale oil spills on avian species are far-reaching.While media attention often focuses on lethal impacts, sub-lethal effects and the impacts of rehabilitation receive less attention. The objective of our study was to characterize effects of moderate external oiling and subsequent rehabilitation on feather structure and thermoregulation in gulls. We captured 30 wild ring-billed gulls (Larus delawarensis) and randomly assigned each individual to an experimental group: 1) controls, 2) rehabilitated birds (externally oiled, rehabilitated by washing), or 3) oiled birds (externally oiled, not rehabilitated). We externally oiled birds with weathered MC252 Deepwater Horizon oil (water for controls) …

Nanoparticle Brain Delivery: A Guide To Verification Methods, Robert A. Yokel
Pharmaceutical Sciences Faculty Publications
Many reports conclude nanoparticle (NP) brain entry based on bulk brain analysis. Bulk brain includes blood, cerebrospinal fluid and blood vessels within the brain contributing to the blood-brain and blood-cerebrospinal fluid barriers. Considering the brain as neurons, glia and their extracellular space (brain parenchyma), most studies did not show brain parenchymal NP entry. Blood-brain and blood-cerebrospinal fluid barriers anatomy and function are reviewed. Methods demonstrating brain parenchymal NP entry are presented. Results demonstrating bulk brain versus brain parenchymal entry are classified. Studies are reviewed, critiqued and classified to illustrate results demonstrating bulk brain versus parenchymal entry. Brain, blood and peripheral …

Ventricular arrhythmias are an important cause of morbidity and mortality and come in a variety of forms, from single premature ventricular complexes to sustained ventricular tachycardia and fibrillation. Rapid developments have taken place over the past decade in our understanding of these arrhythmias and in our ability to diagnose and treat them. The field of catheter ablation has progressed with the development of new methods and tools, and with the publication of large clinical trials. Therefore, global cardiac electrophysiology professional societies undertook to outline recommendations and best practices for these procedures in a document that will update and replace the …

We tested whether a multisectoral household agricultural and finance intervention increased the dietary intake and improved the nutritional status of HIV-affected children. Two hospitals in rural Kenya were randomly assigned to be either the intervention or the control arm. The intervention comprised a human-powered water pump, microfinance loan for farm commodities, and training in sustainable farming practices and financial management. In each arm, 100 children (0-59 mo of age) were enrolled from households with HIV-infected adults 18-49 y old. Children were assessed beginning in April 2012 and every 3 mo for 1 y for dietary intake and anthropometry. Children in …

The health care field has integrated advances into digital technology at an accelerating pace to improve health behavior, health care delivery, and cost-effectiveness of care. The realm of behavioral science has embraced this evolution of digital health, allowing for an exciting roadmap for advancing care by addressing the many challenges to the field via technological innovations. Digital therapeutics offer the potential to extend the reach of effective interventions at reduced cost and patient burden and to increase the potency of existing interventions. Intervention models have included the use of digital tools as supplements to standard care models, as tools that …
Investigating The Modulatory Effect Of Expectations On Memory Retrieval During Sentence Comprehension, Luca Campanelli
Investigating The Modulatory Effect Of Expectations On Memory Retrieval During Sentence Comprehension, Luca Campanelli
Dissertations, Theses, and Capstone Projects
Memory retrieval and probabilistic expectations are recognized factors in sentence comprehension that capture two different critical aspects of processing difficulty: the cost of retrieving and integrating previously processed elements with the new input words and the cost of incorrect predictions about upcoming words or structures in a sentence. Although these two factors have independently received substantial support from the extant literature, how they interact remains poorly understood. This study investigated memory retrieval, more specifically retrieval interference, and lexical-semantic expectations in three reading experiments, pitting these factors against each other to observe how they interact. Overall, results showed that retrieving and …
Direct And Indirect Treatment Effects In Multilingual People With Aphasia, Aviva Lerman
Direct And Indirect Treatment Effects In Multilingual People With Aphasia, Aviva Lerman
Dissertations, Theses, and Capstone Projects
Background: Successfully assessing and treating aphasia in multilingual people requires a detailed understanding of the mechanisms underlying language processing in the multilingual population, and the potential impairments to those mechanisms. The balance between spreading activation of language processes via treatment and controlling interference of competing items within the lexico-semantic networks appears to be a key factor in determining whether treatment effects generalise within and across languages in multilingual people with aphasia (Kiran, Sandberg, Gray, Ascenso, & Kester, 2013).
This balance can be exploited through treatment, which, if carefully chosen, should maximise potential within- and cross-language generalisation. One treatment that has …

For example, dopamine (DA) neurons, whose degeneration causes motor deficits characteristic of Parkinson’s Diseases (PD), communicate with all their targets by DA but only a selective subset of their targets using GABA, Glutamate, and the secreted cell signaling protein Sonic Hedgehog (Shh). It is unknown whether Levo-dopamine (L-Dopa) induced dyskinesia (LIDs), a severely debilitating side effect of DA supplementation in PD, might appear because DA neuron targets are exposed to high DA- but low Shh- signaling …
