Background
Vaginal stones may form in the setting of mesh exposure with urinary incontinence. This report serves to help understand the presentation, evaluation, and management of vaginal urinary stones.
Case
A 68-year-old female presented with a vaginal calculus. She had a history of anterior and posterior polypropylene mesh placement for prolapse 7 years earlier and urinary incontinence. The stone was identified on a portion of exposed mesh and removed in office. Pathology confirmed urinary etiology. The exposed mesh resolved with topical estrogen. Cystourethroscopy excluded urinary fistula and bladder mesh erosion.

When identified, a vaginal calculus should be removed and …

Although significant advances have been made in the biologic understanding of graft-versus-host disease (GVHD) and its treatment options, GVHD remains the single most challenging obstacle to the success of allogeneic hematopoietic cell transplantation (HCT) due to high risk of disabling morbidity and mortality. Extracorporeal photopheresis (ECP) has promising effects in controlling steroid-refractory GVHD, both acute and chronic, and it has been studied extensively. Its putative immunomodulatory mechanisms, while not immunosuppressive, position ECP as an attractive treatment strategy for GVHD patients who are already receiving global immunosuppression. However, ECP is relatively underutilized due in part to limited access and time commitment. …

The V600 BRAF mutation has been described as a key mutation in the pathogenesis of classical hairy cell leukemia (c-HCL) cases without expression of a mutant immunoglobulin heavy chain (IgHV). Here we present a rare case of c-HCL with neither V600 BRAF mutation nor the aforementioned IgHV variant successfully treated with cladribine and review the current literature on its use in women of childbearing age/pregnancy.

The number of Registered Nurses (RNs) in the United States (U.S.) is roughly three times that of physicians and surgeons, making RNs a critically important component of the U.S. healthcare system. RN Burnout – defined as the feeling of exhaustion from working long hours without rest – is a real concern, having been reported in many hospitals. The purpose of this research is to examine the causes and consequences of Burnout Syndrome among RNs in U.S. hospitals, in order to identify solutions to this problem. The purpose of my study is to examine the molecular evolution of the HEXA gene and its implications to Tay-Sachs disease. Also, this study intends to compare and contrast the mutations that occur within the various populations at high-risk of developing this disease. My central concept is population genetics in which populations are compared among each other to identify patterns or possible causes of a particular phenomenon. Past and present, community gardening has been used as a response to poverty, a way to supplement healthy foods, and a mechanism for community involvement. For thirteen refugee families living at the Housing Authority of Bowling Green, community gardening has become a way to recreate their traditional cult ure. Based on data collected through interviews during the summer of 2016, these refugee s identified gardening as a buffer against food insecurity, providing access to both healthy a nd culturally appropriate foods. In 2010, after voluntary medical male circumcision (VMMC) had been shown to be one of the most cost-effective strategies for preventing HIV infections, South Africa initiated a VMMC program with an ambitious target of performing 4.3 million circumcisions by 2016. However, because of a gap in knowledge concerning the overall cost of scaling up services, the South African National Department of Health requested that Project SOAR—Supporting Operational AIDS Research—conduct a private-sector costing of providing VMMC services in South Africa. Airborne surveillance operations present challenging environments for tactical operators and for the technologies that support these activities. Information from multiple sources is currently presented on 2D displays, but the influx of data hasmade it difficult to represent this informationusing traditional technologies. A body illusion, commonly known in the form of the “Rubber Hand Illusion”, is an illusion wherein visual inputs on an inanimate object and simultaneous tactile inputs on a part of the body lead to a situation where the inanimate object is identified as the body part. Situation awareness (SA) has been investigated in the aviation industry for decades and recently has become more prominent in other industries. For example, healthcare has experienced tremendous growth in SA research and training. Despite agreement among researchers that SA is important for performance and safety in complex domains, less agreement exists for defining and measuring SA. Certain industries (e.g., aviation, healthcare, process control operations) often have specific methods on how to approach SA. These approaches of introducing employees and students to SA in a specific context may inadvertently limit their full appreciation and understanding of this construct.

Research showsthat a high percentage of weather-related General Aviation (GA) accidents can be attributed to pilots flying into Instrument Meteorological Conditions (IMC) without experience or appropriate certifications to safely operate beyond Visual Flight Rules (VFR). To makesafety-critical decisions, pilots oftenuse weather indication delivered on screens of portable electronic devices. This information often is obsolete with a latency up to20 minutes. Early flying skills have been reported to strongly influence future flying skills. Few published studies have evaluated the comprehensive relationship among all flying training skills. This study evaluated three causal path models for flying skills that influenced flying performance rated by instructor pilots in five training phases. A covariance structure analysis showed that asequential model with a connection between the first and last phases was optimumfor representing flying training performance and skills. The flying skill acquired in the first training phase in a primary propeller aircraft influenced the last training skill in a fighter jet directly as well as indirectly. …

The concept of scalable autonomy with a guided access to lower automation levelsallows human UAV pilots to select the level of autonomy and enables the onboard automation to understand the pilot’s intent and offer support. To evaluate this concept, we conducted flightand simulationexperiments with German military personnel performing reconnaissance missions with a small UAV. We compared threeconfigurations. The high autonomy configuration completely prevents access to low-level functions. The naive approach configuration allows unrestricted access to all lower-level functions. For decades, spatial auditory displays have been considered to be a promising technology to help fight pilot disorientation and loss of SA. Inherently heads-up, these displays can provide time-critical spatial information to pilots about navigational targets, air and runway traffic, wingman location, and even the attitude of one’s aircraft without placing additional demands on the already over-tasked visual system. Unfortunately, currently-fielded auditory displays often suffer from poor spatial fidelity, particularly in elevation, due to their use of a one-size-fits-all (i.e., non-personalized) head-related transfer function (HRTF), the set offilters responsible for creating the spatial impression. The total consumption of fresh and processed vegetables has exceeded 123 kg per capita in the United States and 118 kg per capita in the European Union (http://www.helgilibrary.com/ indicators/index/vegetable‐consumption‐percapita) in 2009. Changes in lifestyles and consumption trends have prompted the sustained growth of fresh‐cut or minimally processed vegetables that are fresh and ready‐to‐eat (RTE). In 2011, for example, total US fresh‐cut produce sales through both food service and retail channels were estimated to surpass $27 billion (Cook et al., 2012).

Electronic Medical Records (EMRs) have shown benefit for clinical, organizational, and societal outcomes(1). In South Africa, the desire for effective record keeping will continue to rise with increasing trauma and infectious disease rates. Recognizing this, the Western Cape government in 1999 signed a tender to rollout Clinicom EMR, an online storage system for paper records, to all hospitals within the province(2).
In 2012, Khayelitsha Hospital (KH) was opened with the Clinicom EMR installed. In 2013, KH received the ministerial award for clinical excellence as a result of the effective EMR rollout. Due to a high incidence of violence around Khayelitsha, …

Background: To examine the interaction between social income inequality, social integration, and health status among internal migrants (IMs) who migrate between regions in China.
Methods: We used the data from the 2014 Internal Migrant Dynamic Monitoring Survey in China, which sampled 15,999 IMs in eight cities in China. The Gini coefficient at the city level was calculated to measure social income inequality and was categorized into low (0.2 < Gini <= 0.3), medium (0.3 < Gini <= 0.4), high (0.4 < x < = 0.5), and very high (Gini > 0.5). Health status was measured based upon self-reported health, subjective well-being, and perceptions of stress and mental health.
