Clostridium difficile (C. difficile) is known as the most important causative agent of antibiotic-associated pseudomembranous colitis in human. According to recent studies, food animals, especially poultry, are known as carriers for toxin harboring C. difficile. Since ostrich farming is a growing industry in Iran, the characterization and genotyping of C. difficile isolates in ostriches are very important for public health. A total of 100 fecal samples were collected from 10 ostrich farms located in the north-east of Khorasan Razavi province, Iran. The samples were cultured anaerobically. Effects Of Centrifugation And Removal Of Seminal Plasma On Motility Of Fresh Boar Sperm, Olivia Bury, Victoria Mcrae, Jose Len, Kate Plush, Roy N. Kirkwood

Extended semen from 6 boars was centrifuged and then gently resuspended (CR), had the supernatant decanted to remove residual seminal plasma and resuspended in new extender (CD), or served as non-treated controls. Measures of sperm motility were assessed immediately after centrifugation and then every 24 h until 96 h using a computer-assisted sperm analyser. Total and progressive sperm motilities were reduced (P<0.05) by 48 h in CD. Total motility was not affected in CR sperm but progressive motility was reduced (P<0.05) at 72 h. Changes with time were very similar for VSL, VAP, VCL and BCF. Control values remained relatively stable but VCL, VSL, VAP and ALH were lower within minutes of centrifugation and decanting of CD sperm. Both CR and CD treatment values for all variables were lower (P<0.05) from 3 d. These data indicate that centrifugation can negatively affect boar sperm motility and that even the small amount of residual seminal plasma in fresh-extended boar semen has a protective role. Sperm age-associated reduced motility can be countered with caffeine.

Seventy-eight unhealthy and forty-two apparently healthy looking freshwater aquarium fish belonging to twenty-six different species were collected from local aquarium stores in different cities and were investigated for the presence of mycobacteria in north-western Iran by culture and molecular methods. Using the culture method, Mycobacterium sp. was isolated from 31.6% of the fish (38 cases), while it was obtained in 30.8% (37 cases) by polymerase chain reaction (PCR) assay for a 439 bp fragment of the heat shock protein 65kD gene (hsp65). A 14-year-old neutered female mixed breed dog weighing 12 kg was admitted at the in-patient unit of The Small Animal Teaching Hospital, Chulalongkorn University in October 2017 with clinical signs of depressed, anorexia, weakness and continuous vomiting. However, no sign of abdominal pain was found. History taking showed that the dog had gait disorientation due to central nervous system disorder. The dog also had murmur heart sound graded IV/VI without sign of cardiac disease since June 2017. This paper considers the denial of health insurance benefits based on a participant's high-risk behaviors such as self-inflicted injuries. In many instances, healthcare benefits can be denied if patients are injured while engaging in criminal activities, from a self-infliction, and from injuries relating to the consumption of alcohol. With increases in healthcare expenditures and government regulation, the necessity for benefit reductions is directed at individuals engaging in high-risk behaviors. The belief is that high-risk behaviors can be modified by individuals. Unfortunately, modification of behaviors may not be available to everyone.

Objective: To evaluate the efficacy and safety of ixekizumab alone or with concomitant conventional disease-modifying antirheumatic drugs (cDMARDs) versus placebo in patients with active psoriatic arthritis (PsA) as part of a SPIRIT-P1 subgroup analysis (NCT01695239). Methods: Patients were stratified by cDMARD use (concomitant cDMARDs use (including methotrexate) or none (past or naive use)) and randomly assigned to treatment groups (ixekizumab 80 mg every 4 weeks (IXEQ4W) or every 2 weeks (IXEQ2W) or placebo). The pseudomyogenic hemangioendothelioma (PMH) is a low-grade malignant vascular neoplasm of different tissue planes including skin and soft tissue. Primary tumors in the skeletal muscle and bone have also been diagnosed. The PMH was introduced into the WHO classification of tumors of soft tissue and bone in 2013. This is the first description of oral involvement. A 21-year-old female presented with a 2-month old swelling of her gingiva. The swelling appeared red in color and was soft in consistency. Antibodies against programmed death (PD) pathway are revolutionizing cancer immunotherapy. Currently five antibodies against PD-1/PD-L1 have been approved. The clinical use of these antibodies is rapidly expanding. Incorporation of PD antibodies into chemotherapy regimens is in active clinical investigations. The combination of pembrolizumab with carboplatin and pemetrexed has been approved for the first line therapy of metastatic non-squamous non-small cell lung cancer. Combination of PD-1/PD-L1 antibodies with small molecule inhibitors such as tyrosine kinase inhibitors and IDO inhibitors are in active clinical trials. This review summarized recent development in clinical trials of PD-1 and PD-L1 antibodies for cancer immunotherapy.

BACKGROUND: The aim of this study was to evaluate the rate of periocular allergic skin reactions to topical neomycin, polymyxin B, and dexamethasone (NPD) ophthalmic ointment.

METHODS: A consecutive patient prospective study was performed. A total of 522 patients who had a procedure involving incision of the periocular skin with subsequent postoperative application of NPD ophthalmic ointment were included. Patients were evaluated for signs of allergy at 1 week postoperatively or prior if the patient had complaints. The schwannoma-like pleomorphic adenoma is a rare histopathological variant of the pleomorphic adenoma. Five previous reports with seven cases exist in English language literature. These tumors present in the parotid gland most commonly. Intraparotid schwannomas of the facial nerve and schwannomas with glandular differentiation have also been reported. A 60-year-old male presented with an asymptomatic swelling over the left angle of the mandible. The swelling had been present for about 12 years with a recent increase in size. Ten consensual sex workers who were currently working in Nevada were qualitatively interviewed in an effort to explore their perceptions of the adult industry. Interviews specifically focused on their experience disclosing their profession to others and the potential effects that it has on their personal relationships, and access to effective treatment. The Moustakas method of phenomenological inquiry was used to analyze data from ten participants who self-identified as direct sex workers. Filipinos in the US are 3.4 million, yet the main causes of death among this primarily immigrant population have not been well characterized nor compared with the mortality experience of their counterparts in the Philippines. Age-adjusted mortality rates were computed for the main causes of death for three populations: Filipinos living in the Philippines (FPHs), Filipinos living in California (FCAs), and non-Hispanic whites in California (WCAs). Propionic acid (PPA) is a ubiquitous short-chain fatty acid which is a fermentation product of the enteric microbiome and present or added to many foods. While PPA has beneficial effects, it is also associated with human disorders, including autism spectrum disorders (ASDs). We previously demonstrated that PPA modulates mitochondrial dysfunction differentially in subsets of lymphoblastoid cell lines (LCLs) derived from patients with ASD. We investigated the nationwide trends in admissions for STEMI, utilization of percutaneous coronary intervention (PCI), and in-hospital outcomes in patients with the three most common cancer diagnoses (lung, breast, and colon) compared to patients without cancer. Myelodysplasia (MDS) /myeloproliferative neoplasm (MPN) overlap syndrome has been described since the 2001 WHO classification as disorders that have both proliferative and dysplastic changes simultaneously. Specific disorders include chronic myelomonocytic leukemia (CMML), juvenile myelomonocytic leukemia (JMML), BCR-ABL negative atypical chronic myeloid leukemia (aCML) and unclassifiable MDS/MPN (MPN/MDS-U). Recurrent gene mutations in these conditions have been described. Among them, SETBP1 mutations have been identified in up to 32% of aCML, 24% of JMML, 18% of CMML and 10% of MDS/MPN-U patients. This research explores current approaches to managing pain control in patients undergoing operative fixation of lower extremity fractures. As opioid use in the U.S. and abroad increases, alternative methods of treating and preventing opioid requirements are being examined. Patients with lower extremity fractures are susceptible to nonunion when anti-inflammatories are consumed, so alternative approaches are warranted. One such alternative, a peri-operative regional nerve block (PNB), is accomplished by injecting local anesthetic adjacent to one or more peripheral nerves supplying the area of the fracture to decrease pain (Fowler, Symons, Sabato, & Myles, 2008).
